Heterotopic gastric mucosa of the upper esophagus( HGMUE)is a rare esophageal disease. It is considered as a result of immature development of esophageal mucosal epithelium in embryonic period. Most of patients have no specific symptoms. The diagnosis of HGMUE is mainly based on endoscopy and biopsy. With the development of endoscopic technique,the detection rate of HGMUE is elevated and increasing attention has been paid to this disease. This article reviewed the clinical research on HGMUE.

Objective To observe the expression of neuroglobin in brain of rat with septic shock,and study the protective effect of neuroglobin.Methods Sixty male SD rats were randomly di-vided into two groups,30 rats in each.The rats in group M were injected LPS 10 mg/kg through tail vein to establish septic shock model,the success standard of septic shock was systolic blood pressure dropping to 60% of the basic values;the control group were given equal volume of NS.The levels of rat TNF-α,IL-6 and NGB in serum at the time points of before LPS injection (T0 ),1 h (T1 ),2 h (T2 ),4 h (T3 )and 6 h(T4 )after LPS injection were measured using ELISA kits.NGB and caspase-3 protein expression in brain were determined using Western blotting.Results Compared with group C and T0 ,the TNF-α,IL-6 concentrations in serum and caspase-3 in brain were increased significantly at T1-T4 (P <0.05),NGB protein content in serum and brain at T3 and T4 were increased significantly (P <0.05).Conclusion Cell apoptosis and inflammatory response are discovered in the brain during septic shock.NGB is over-expressed in brain of rats with septic shock,which may play a role of pro-tective function.

Objective To investigate the possible role of mRNA expression of nitric oxide synthase in the abnormal intestinal motility and visceral perception in patients with diarrhea predominant IBS. Methods Anorectal manometry and rectal visceral sensory thresholds were examined in 25 patients with diarrhea predominant IBS and 15 healthy controls by means of electric barostat and perfusion catheter manometer. The quantity of NO was measured using nitrate/nitrite colorimetric assay. The nerve fibers containing NOS in mucosal muscular layer of colon was quantitatively measured with NADPH diaphorase histochemistry assay and computer image analysis. The expression of neuronal nitric oxide synthase (nNOS) mRNA was determined with fluorescence quantitative polymerase chain reaction (FQ PCR). Results Thresholds of first perception, defecation and pain in patients with IBS were significantly lower than those of controls. Objective:This study was aimed to investigate the value of osteoclast differentiation factor (ODF) and osteoclastogen-esis inhibitory factor (OCIF) detection for clinical diagnosis and assessment of patient condition in bone metastasis of lung cancer. Methods:Data from 186 lung cancer patients who were preliminary diagnosed between July 2009 and April 2012 were analyzed. Cas-es were divided into the bone metastasis group with 82 cases (group A) and the non-bone metastasis group with 104 cases (group B). Concentrations of serum ODF and OCIF in each group were detected by ELISA. Results: ODF and OCIF values of group A were (32.22±6.22) ng/L and (41.23±8.13) ng/L, respectively, which were significantly higher than the corresponding values in group B [(8.35 ±5.42) ng/L and (10.15±4.42) ng/L]. The differences between the two groups were statistically significant (P<0.01). Areas under the re-ceiver operating characteristic curves of ODF and OCIF, which are used to diagnose bone metastasis in lung cancer, were 0.91 and 0.87, respectively, manifesting good diagnostic value. The sensitivity and specificity of ODF in diagnosing lung cancer with bone metastasis were 90.38%and 86.59%, respectively, and those of OCIF were 86.54%and 84.15%, respectively. ODF increased, whereas OCIF de-creased significantly, with increasing bone metastasis. ODF and OCIF concentrations in group A and the group with newly-found bone metastasis were significantly higher than those in group B, with statistically significant differences among these groups (P<0.01). Com-pared with group A, less difference was found in the ODF and OCIF of newly-found bone metastases, without statistical significance be-tween these groups (P>0.05). Conclusion:The serum ODF and OCIF concentrations significantly increase when bone metastasis oc-curs in lung cancer patients. Hence, these variables are useful as indices for monitoring bone metastases and evaluating patient condi-tion. An extensive application prospect is proposed.

Objective To analyze the distribution and influential factors of exam results in medical students' general surgery theory course.Methods 171 medical students were selected as subjects by cluster sampling,and the distribution and influential factors of exam results were analyzed.SPSS 17.0 software was used for statistical analysis,measurement data with (-x) ± s,and normality test with Kolmogorov-Smirnov test.Those quantitative data which do not meet the normal distribution were compared with Mann-Whitney U and Kruskal-Wallis H test.Rank transformation univariate multi-factor variance of LSD (Levene test equal error variance between groups) or Tamhane method (Levene test range error variance between groups) were compared between two groups(3-4) and the influence factors of whether the grade was good was analyzed by single factor and multi factor non conditional Logistic regression model,with the test level of alpha=0.05.Results The distribution of total exam results was normal.77 score was outlier,and the scores of female students were higher than those of male students.Scores of total exam results,multiple-choice questions and essay questions in different classes were significantly different.Data from multivariate logistic regression analysis showed that male students(OR=0.212,95％CI:0.077-0.584) were unfavorable factor for good exam results,while higher scores in noun explanation (OR=12.160,95％CI:1.985-74.495),multiple-choice questions (OR=9.887,95％CI:2.997-32.617),essay questions(OR=18.323,95％CI:6.593-50.928) were favorable factors.Conclusion The cause analysis of score's outlier and sex difference should be strengthened,and the influence of examination items on score should be emphasized.

Objective:To observe the effects of the vertical holding appliance (VHA) in the extraction treatment of patients with hyperdivergence.Methods:40 hyperdivergent patients orthodonticly treated by 4 first premolar extraction with(VHA group) and without VHA(control group) (n =20) respectively.Cephalometric radiographs were taken before and after orthodontic treatment and measured digitally by Winceph 8.06 software.Results:After treatment,the L6-MP,S-Go,N-Me,ANS-Me increased in the VHA group(P ＜ 0.05).The MP-SN,Y-axis,U1-PP,U6-PP,L6-MP,S-Go,N-ANS,N-Me,ANS-Me,ANS-Me/N-Me increased and the S-Go/N-Me decreased in the control group(P ＜0.05).The changes of the MP-SN,Y-axis,U6-PP,L6-MP,N-Me,ANS-Me,ANS-Me/N-Me,S-Go/N-Me were smaller in the VHA group than those in the control group (P ＜ 0.05).Conclusion:VHA is effective in the control of the height of molars in the extraction treatment of patients with hyperdivergence.

Objective To explore a method to separate and purify apolipoprotein Ⅰ from human serum conveniently and efficiently. Methods Apolipoprotein Ⅰ was separated and pufified by ultracentrifugation and affinity chromatography. Then the purified apolipoprotein Ⅰ was analyzed by SDS-polyacrylamide gel electrophoresis and agar gel double immunodiffusion test. Results The purified apolipoprotein Ⅰ was satisfactory. Conclusion This method had good reliability and was convenient and economical.

Objective To investigate the effects of different DO2-directed hemodynamic man-agement on postoperative prognosis of orthotopic liver transplantation (OLT)patients with different degrees of liver function insufficiency.Methods Seventy patients (male 48 cases,female 22 cases, aged 37-66 years,ASA grade Ⅲ or Ⅳ)scheduled non veno-venous bypass OLT were divided into two groups (n = 35 each ).The oxygen delivery (DO2 )maintained respectively between 350 ml·min-1 ·m-2 and 500 ml·min-1 ·m-2 (group control)and more than 500 ml·min-1 ·m-2 (group study).After induction of anesthesia,the left radial artery was cannulated,allowing continu-ous blood pressure monitoring as well as serial blood sampling,then the Swan-Ganz catheter was in-serted via the right internal jugular vein to continuously monitor CI and S-vO2 .After anesthesia induc-tion and before operation (T1 ),10 min before anhepatic phase (T2 ),30 min after onset of anhepatic phase (T3 ),30 min after neohepatic phase (T4 )and the end of surgical procedure (T5 ),the SaO2 , PaO2 ,Hb,CI and S-vO2 were recorded to calculate the DO2 ,VO2 and ERO2 .Postoperatively,ICU and hospital length of stay,quality of life (QOL)and 1-5 year survival rate were recorded.Results In the two groups,CI,DO2 and VO2 declined at T3 to a statistically significant degree in comparison with T1 (P <0.05).CI,DO2 and VO2 in group study significantly higher than those of group control (P <0.05).ERO2 in group study was lower than that of group control significantly (P <0.05).ICU and hospital length of stay were significantly shorten in group study in comparison with group control (P <0.05).Survival rate and QOL in group study tended to be slightly higher than those of group control until the end of study period,but there was no significant difference.Conclusion The DO2-directed hemodynamic management can improve prognosis with enhanced DO2 in OLT patients with different degrees of hepatic insufficiency.It has a positive impact on their outcome of shortening ICU stay and the hospitalization days.

This article provides a classification of method based on SVM (Support Vector Machine) for the capsule endoscopy bleeding intelligent recognition. We created a new kind of feature parameter, and the experiment result can reach 83% specificity and 94% sensitivity.

Objective To investigate the possible role of abnormal signal transduction in smooth muscle cell in the pathogenesis of diabetic gastroparesis. Methods Healthy male SD rats were randomly divided into 2 groups: normal,model group,diabetes was induced by Ⅳ of alloxan,50mg/kg. Gastric emptying was measured by method of nutritious semisolid paste. The level of cAMP in gastric smooth muscle cell was measured by radioimmunoassay. Gsαand GiαmRNA positive cells were quantitatively measured by in situ hybridization and computer image analysis system respectively. Results ( 1 ) Gastric emptying rate of the normal was significantly higher than that of model group (P < 0.05 ) ; (2)the level of cAMP of the normal was significantly lower than that of model group( P < 0.05 ) ; (3)the expression of GsαmRNA in the model group was significantly higher than that of the normal groups( P < 0.05 ) ; (4) there was no marked difference in the expression of GiαmRNA between 2 groups( P > 0.05). Conclusion Altered expression of GSαmRNA may play a key role in the pathogenesis of diabetic gastroparesis.
