1.Study On Optimal Extraction Technology for Volatile Oil in Radix Angelicae Pubescentis

Objective To study the optimal extraction process of volatile oil in Radix Angelicae Pubescentis.Methods L9(34) orthogonal design was carried out for the optimization of water extraction of volatile oil in Radix Angelicae Pubescentis,and the content of volatile oil was used as referece index.Results The optimal extraction process was as follows:medicinal materials were crashed to go through mesh 80~100,then were soaked in 12-fold volume of water for 10 hours,and were extracted for 11 hours.Conclusion The optimal extraction techndogy is simple and feasible with the extraction rate of volatile oil being 2.66 %.
2.Study on the therapeutic efficacy of root canal preparation using pathfile in combination of protaper

Objective To evaluate the clinical effect of the use of pathfile and protaper nickel-titanium instruments for machine in root canal preparation.Methods A total of 200 patients suffering from pulpitis and periapical that needs root canal treatment was randomly divided into two groups according to treatment order (100 patients/each group).The experimental group was used pathfile and protaper in root canal preparation,and filled with warm gutta vertical condensation.The control group was used protaper root canal preparatory standards method,and filled with warm gutta vertical condensation.The required time of two sets of root canal preparation and the number of equipment broken were recorded,and X-ray film at the preoperation intraoperation and postoperation were used to evaluate effect of preparation and root filling.Results In the root canals prepared of the two groups,no issued offset,no side wear,no step-forming,and no position changed of the apical foramen were occurred,which was consistent with preperative X-ray film.One case was occurred fracture of instrument in the experimental group and the control group,it was Slfile.Tooth position was right mandibular,second molar nearly the root apical 1/3,and upper left second molar mesial buccal root apical position.The root canal preparation time of experimental group was significantly shorter than the control group[the front teeth:(4.73 ± 1.12) min vs (6.32 ± 1.84) min,t =2.243,P <0.05; the premolars:(7.83±1.23)min vs (10.25±2.68)min,t =2.475,P <0.05; the molars (10.14 ± 1.18) min vs (15.43 ± 5.94) min,t =2.991,P < 0.01].Postoperative X-ray showed that 2 cases were occurred small amount of paste overfilled,the root canal closed tight,no less filling; and 3 cases of the control group occurred paste overfilled,no less filling too.Conclusions The use of nickel-titanium instruments pathfile and protaper for machine in root canal preparation is feasible,and improves the efficiency of root canal preparation.
3.Preparation and Transdermal Absorption in vitro of Piroxicam Nanostructured Lipid Carrier

Objective: To prepare piroxicam nanostructured lipid carrier and investigate its transdermal absorption behavior in vitro. Methods:Piroxicam nanostructured lipid carrier was prepared by a melt-emulsion ultrasonication and low temperature-solidifica-tion method. The physicochemical properties such as appearance, morphology, particle size distribution, PdI and zeta potential of pi-roxicam nanostructured lipid carrier were evaluated. The transdermal absorption in vitro was investigated using Franz diffusion cells. Results:Piroxicam nanostructured lipid carrier was clear and transparent with small spherical shape as seen under a transmission elec-tron microscope. The particle size distribution, PdI and zeta potential was (106. 4 ± 31. 6) nm, (0. 217 ± 0. 07) and ( -31. 6 ± 2. 5) mV, respectively. Piroxicam nanostructured lipid carrier had higher cumulative transdermal amount in 12 h than piroxicam solution. Conclusion:The nanostructured lipid carrier can remarkably improve piroxicam permeation into skin, which provides reference for the new dosage form for the topical use of piroxicam.
4.Study on the possibility of pulp regeneration of different irreversible pulpitis

Objective To investigate the possibility of pulp regeneration of different irreversible pulpitis with absorbable gelatin sponge as the scaffold,and dental seed cells,induced molecules and MTA closed.Methods 150 patients with irreducible pulpitis,who were removed the crown infection pulp and retained the root living pulp,according to the visiting sequence,were randomly divided into two groups.The experimental group was given absorbable gelatin sponge as the scaffold and MTA closed.The control group was given calcium hydroxide coverage.The symptoms situation and pulp situation were observed.Results 6 months after surgery,the success rate of acute pulpitis and chronic closed pulpitis in the experimental group was 76.5% and 47.0%,which were significantly higher than those of the control group (14.3 %,8.8%) (P < 0.01).In the experimental group,the success rate of chronic hyperplastic pulpitis and chronic ulcerative pulpitis was 33.0% and 35.0%,which were higher than those of the control group(9.1%,6.0%),but the differences between the two groups were not statistically significant (P > 0.05).Conclusion Pulp regeneration in vivo is possible with absorbable gelatin sponge as the scaffold and MTA,pulp regeneration of acute pulpitis and chronic closed pulpitis are more likely to induce.

Lung adenocarcinoma is usually thought to follow a linear multistep progression schema,that is precancerous lesions progress to adenocarcinoma in situ,and progress to micro-invasive adenocarcinoma,eventually some lung adenocarcinoma happen diffusion.However,recent findings reveal that this linear progression schema might not occur in all lung adenocarcinoma subtypes,and some subtypes show a kind of non-linear progression schema.The study of these two kinds of schemas can help for early detection of lung adenocarcinoma.

[Objective]Discussion on academic ideas and clinical experience of Professor XIE Jingri in treatment of ulcerative colitis(active phase). [Method] From the disease affiliation, etiology and pathogenesis,syndrome differentiation, clinical adding and subtracting, disease aftercare to expound the academic viewpoints and clinical experience of Prpfessor XIE Jingri in treating ulcerative colitis(active phase), summarize his prescription regularity and treatment characteristics,and put an example to exemplify that.[Results]Professor XIE Jingri thinks that spleen deficiency is its fundamental pathogenesis,dampness-heat is the key factor of attack, the liver is closely related to the lung, spleen and large intestine,it could adjust the emotions and circulation of Qi, blood and body fluid.Therefore,we shuold take the method of strengthening spleen and nourishing Qi and dampness-heat clearing simultaneously as basic therapy, and pay more attention to the treatment lying in regulating the liver, use the method of regulating qi activity and activating circulation to remove blood stasis, focus on disease aftercare at the same time,the clinical therapeutic effect is satisfied.[Conclusion]The clinical experience of Professor XIE Jingri in treatment of ulcerative colitis(active phase) could effectively alleviate symptoms and reduce relapse rate,it is worth to be summarized and popularized.
7.Significance of transrectal ultrasound combined with carcinoembryonic antigen examination on accurate preoperative staging of rectal carcinoma

Objective To study the consistency of preoperative staging by transrectal ultrasonography (TRUS) combined with serum carcinoembryonic antigen (CEA) and the postoperative pathological TNM staging (PTNM).Methods 118 rectal cancer patients pathologically proven were divided into preoperative TRUS combined with CEA group (59 cases) and along TRUS group (59 cases).The consistency of preoperative stag in 2 groups and postoperative pathological stage was analyzed retrospectively.Results In TRUS combined with CEA group,the accuracy of T stage was 79.7 % (47/59),and the accuracy of N stage was 77.8 % (42/59),compared with the postoperative pTNM.While in along TRUS group,the accuracy of T stage was 86.4 % (51/59),and the accuracy of N stage was 57.7 % (30/59).The consistencies of T and N stage in TRUS combined with CEA group and postoperative pTNM were better (κ =0.685,P =0.000; κ =0.544,P =0.000).While the consistency of T stage in along TRUS and postoperative pTNM was better (κ =0.755,P =0.000),but that of N stage was poor (κ =0.154,P =0.229).Conclusion Preoperative evaluation by the TRUS combined with CEA can increase the accuracy of preoperative stage which can provide more reliable basis for decision-making and improve the rate of coincidence of operative procedures in line with forecasts.At the same time,it can provide the basis for the accurate preoperative diagnosis and individualized treatment.
8.Effect of Agonist and Inhibitor of PI3K/AKT on Inflammatory Response in Macrophages

Phosphoinositide 3-kinase/serine-threonine kinase (PI3K/AKT)has been found playing an important role in the pathogenesis of severe acute pancreatitis (SAP)in recent years,but the underlying mechanism has not been clarified.Aims:To investigate the role of PI3K/AKT in regulating the inflammatory response in SAP by evaluating the effect of insulin-like growth factor-Ⅰ (IGF-Ⅰ)and wortmannin,the agonist and inhibitor of PI3K/AKT on Toll-like receptor 4 (TLR4)signaling pathway in macrophage cell line RAW264.7.Methods:RAW264.7 cells were treated with different concentrations of lipopolysaccharide (LPS ), IGF-Ⅰ and wortmannin, respectively, and cell viability was determined by CCK-8 assay.RAW264.7 cells were divided into blank control group (no treatment),LPS group (LPS 1 μg/mL),IGF-Ⅰ group (IGF-Ⅰ 100 ng/mL +LPS 1 μg/mL),wortmannin group (wortmannin 100 nmol/L +LPS 1 μg/mL)and IGF-Ⅰ +wortmannin group (wortmannin 100 nmol/L +IGF-Ⅰ 100 ng/mL +LPS 1 μg/mL).Protein expressions of tumor necrosis factor-α(TNF-α)and interleukin-6 (IL-6)were detected by ELISA;mRNA expressions of TLR4,myeloid differentiation factor 88 (MyD88),AKT,PI3K,p38 mitogen-activated protein kinase (p38MAPK)and nuclear factor-κB (NF-κB)were determined by real-time PCR.Results:After treated with LPS,IGF-Ⅰand wortmannin, respectively,no differences in cell viability of RAW264.7 cells were found between different concentrations groups (P>0.05).Protein expressions of TNF-αand IL-6 in LPS,IGF-Ⅰ,wortmannin and IGF-Ⅰ +wortmannin groups were significantly higher than those in blank control group (P<0.05 ).Protein expressions of TNF-αand IL-6 in wortmannin group were significantly lower than those in LPS and IGF-Ⅰ groups (P<0.05),and those in IGF-Ⅰ+wortmannin group were significantly lower than those in IGF-Ⅰ group (P<0.05).In LPS group,mRNA expressions of AKT and PI3K as well as TLR4 and its downstream molecules MyD88,p38MAPK and NF-κB were significantly higher than those in blank control group (P <0.05 ).Expressions of all above-mentioned mRNAs in IGF-Ⅰ group were further increased and significantly higher than those in LPS group (P<0.05).Expressions of all above-mentioned mRNAs in wortmannin group were significantly lower than those in LPS and IGF-Ⅰ groups (P<0.05 ),and those in IGF-Ⅰ+wortmannin group were significantly higher than those in wortmannin group (P<0.05),but significantly lower than those in IGF-Ⅰ group (P<0.05).Conclusions:PI3K/AKT might regulate TLR4 signaling pathway and its downstream molecules in macrophages, thereby affects the expressions of inflammatory cytokines and being involved in the pathogenesis of inflammatory response in SAP.
9.Differentially Expressed miRNAs in Peripheral Blood of Patients with Recurrent Colorectal Adenoma

Background:MicroRNAs ( miRNAs ) is a class of small non-coding RNAs that regulate gene expression at posttranscriptional level. They are considered to play a critical role in the tumorigenesis and development of various tumors. Aims:To identify the recurrence-specific miRNAs in peripheral blood of patients with recurrent colorectal adenoma by miRNA array for providing sensitive and specific molecular markers for recurrence prediction. Methods:Fifty colorectal adenoma patients who had undergone polypectomy by colonoscopy were re-examined from Aug. 2012 to Sep. 2013. Four recurrent and four non-recurrent patients were selected and miRNA array was used to detect the expression profiles of miRNAs in peripheral blood samples. Differentially expressed miRNAs were identified between recurrent and non-recurrent patients. Real-time PCR was applied to verify the differentially expressed miRNAs identified by microarray. Results:Seven up-regulatedmiRNAs(hp hsa-miR-548ai,hsa-miR-4446-3p,hsa-miR-139-3p,hsa-miR-27a-5p,hsa-miR-10a,hsa-miR-23a and hsa-miR-486-3p)and 4 down-regulated miRNAs( hsa-miR-124,hsa-miR-3613-5p,hsa-miR-495 and hsa-miR-485-5p)were identified by microarray between recurrent and non-recurrent patients. Among them,seven miRNAs were validated by real-time PCR as consistent with microarray. Conclusions:Some differentially expressed miRNAs may be involved in the recurrence of colorectal adenoma,which gives clues for further investigation on recurrence predictors.
10.Effect of dMLC dosimetric leaf gap in IMRT and VMAT

Objective To explore the relationship between DLG value and planning dose distribution and actual treatment dose distribution.Methods IMRT and VMAT plans were generated in Eclipse TPS for a typical whole pelvis case,separately.For IMRT plan,MLC position and actual fluence were calculated for each DLG value with the same optimal fluence.Plans with different DLGs of 0 cm and 0.3 cm were compared by several dosimetric indexes.For exploring the PTV mean dose difference between planning dose and actual IMRT or VMAT treatment,planning dose was recalculated with fixed MLC position and different DLGs.Results Dosimetric differences of PTV V50,rectum V40,bladder V40,small bowel V35,left and right femoral head Dmax were 1.49%,0.72%,0.82%,0.68%,0.02 and 0.14 Gy respectively,the average leaf pair width of MLC segments was correlated with DLG (R2 =0.996,P <0.05) and reduced with the increase of DLG.In actual treatment,3.95% and 1.5% mean dose reduction in PTV were observed while DLG increased per 0.1 cm in the typical pelvis case,in IMRT and VMAT treatment respectively.Conclusions DLG can result in the change of MLC position and the dose difference between planning and actual treatment.
