1.Surgical Treatment of Single Ventricle

Objective To review and sum up experience of surgical repair of single ventricle. Methods Since January 1973 to December 1999, 47 patients with single ventricle have been repaired. Right ventricle type was 17 patients, left ventricle type was 14, and unrecorded type was 16. Surgical procedure consisted of pulmonary banding 1, Fontan operation 23, and bidirection superior cavopulmonary anastomosis 23. Results Seven patients died. The motality was 15%. There was no death in bidirection superior cavopulmonary anastomosis patients. Conclusion For the single ventricle, early surgical intervention is required to prevent pulmonary congestion and severe cyanosis. The bidirection superior cavopulmonary anastomosis can improve single ventricle function secondary to pressure and volume load to decrease surgical mortality and to minimize the risk factors for subsequent Fontan operation.
2.The Inhibitory Effect and Mechanism of Ester Catechin Monomers-EGCG, GCG on the Growth of Human Colon Carcinoma Cell Line SW480

Objective To investigate the inhibitory effect of ester catechin monomers-EGCG, GCG on the growth of human colon carcinoma cell line SW480 and its potential mechanism. Methods MTT assay, soft-agar colony formation test, Hoechst 33258 stain and flow cytometry analysis were used to determine the inhibitory effect of theasinesin(TS), and its monomers-EGCG, GCG on SW480 growth. Results EGCG, GCG and TS significantly inhibited the growth of human colon carcinoma cell line SW480 in dose-dependent manner, and their half inhibitory concentration (IC 50 ) was 108.88, 183.21, and 83.36?g?ml -1 , respectively. 24 hours after treatment with IC 50 of EGCG, GCG and TS, the colony formation rate of SW480 cells in the experimental group was obviously lower than that in the control group. Hoechst 33258 staining showed typical apoptotic features such as cell shrinkage, nuclear condensation and cell splinter in the experimental group. A subdiploid peak before G 0/G 1 phase in the experimental group was observed by flow cytometry. Conclusion In accord with TS, EGCG and GCG could inhibit the growth of colon carcinoma cell line SW480 cells, the mechanism of which may be related to inducing cell apoptosis.
3.Application value of hand acupuncture holographic method in endoscopy

Objective To explore the Application value of hand acupuncture holographic method in endosco -py.Methods According to randomized controlled holographic method ,200 patients who underwent endoscopy were divided into the hand acupuncture holographic group (68 cases),the hand acupuncture group (65 cases) and the con-trol group(67 cases).The hand acupuncture holographic group pushed the needle sticking and made use of press and hand acupuncture combination of both methods ,and the hand acupuncture group was taken hand-hole method ,and the control group was taken normal gastroscopy operation .A questionnaire survey ( after checking the content of what dis-comfort,fear,are you willing to accept gastroscopy retest ) was taken and observed the patient's discomfort after the op-eration that nausea,vomiting,abdominal distension and pain,salivation volume,mirror smooth interpolation degree. Results The subjective feelings of the hand acupuncture holographic group were better than those of the hand acu -puncture group and the control group (all P<0.05);The adverse reactions of the hand acupuncture holographic group were lower than those of the hand acupuncture group and the control group before and during the endoscopy ( all P<0.05).Conclusion The hand acupuncture holographic therapy is a combination method of hand side of the second metacarpal holographic acupuncture massage gastroscopy which can reduce pain and fear of the patients ,and its side effects is small ,and it is inexpensive ,the risk factor is small ,and it is easy to operate easy to spread .
4.The use of enteral nutritional support in treating superior mesenteric artery syndrome

Objective:To observe the role of enteral nutritional support in treating superior mesenteric artery syndrome. Methods: Enteral nutritional support was used in two patients with superior mesenteric syndrome after percutanous endoscopic jejunostomy.Enteral nutrition was used for 69 days and 180 days.The symptoms were observed.Body weight and index of nutrition were measured.Results: After enteral nutritional support,symptoms of two patients disappeared.Body weight and fibronectin and prealnumin increased.Conclution: Enteral nutritional support is one of effective methods of treating superior mesenteric artery syndrome.
5.Clinical study of percutaneous endoscopic gastrostomy for long-term enteral nutrition in patients with cancers of head and neck

Objective To evaluate percutaneous endoscopic gastrostomy (PEG) for long-term enteral nutrition in patients with cancers of head and neck. Methods PEGs were performed for 12 patients with head and neck cancers, the indication for which included serious buccal mucositis, difficulty in swallowing, anorexia, and obstruction of the digestive tract. Before PEG and 8 weeks after PEG, serum concentrations of total protein and albumin of the patients were measured, body composition including body weight, free fat mass, and fat mass were measured by bioelectric impedance analysis (BIA), and Karnofsky performance score ( KPS) was evaluated. 6.Clinical value of detecting serum tumor markers in postoperative breast cancer patients

Objective To investigate the relationship between serum levels of CA15-3, CA-125 and CEA in postoperative breast cancer patients and clinicopathological parameters and pathogenetic condition turnover. Methods From January 2004 to December 2007, 219 breast disease cases with integrity clinical information, admitted to general surgery and oncology department of Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine, were collected. The serum concentrations of CA15-3, CA-125 and CEA were determined by chemiluminescent immunoassay(CLIA). Wilcoxon Kruskal Wallis analysis, crosstable analysis and spearman correlation analysis were used to investigate the association between the serum concentration and clinical parameters and pathogenetic condition turnover. Results The serum concentrations of CA15-3,CA-125 and CEA in metastatic breast cancer group were significantly higher than those in nonmetastatic group(P=0.000). The sensitivity of the 3 tumor markers in combination was 64.1% in metastatic breast cancer diagnosis. CA15-3 and CA-125 at the time of initial metastasis were correlated with estrogen receptor state(PCA15-3=0.039, PCA-125=0.049). The level of 3 tumor markers in multiorgan metastatic group were significantly higher than those in single organ metastastic patients(P=0.000). There were significant differences between the levels of CA15-3 and CEA in distant metastatic group and those in local metastatic group (PCA15-3=0.001,PCEA=0.022). Conclusion The high serum concentrations of CA15-3,CEA and CA-125 were useful in monitoring the metastasis. The high serum levels of CA15-3 and/or CA-125 may indicate that the patients will be more sensitive to endocrine therapy.
7.Stem cells and congenital essence of the basic theory of traditional Chinese medicine

BACKGROUND: To discuss the basic theory of traditional Chinese medicine (TCM) related to stem cell for a try to offer a new combining site +for the modernization research of the basic theory of TCM.METHODS: Comparatively analyzing the characteristic of stem cells and the essence theory in TCM.RESULTS: Stem cells have the capacity for both self-renewal and differentiation and can produce the daughter cells with the same phenotype and genotype as themselves. Stem cells have the plasticity that a stem cell may differentiate into cells other than the lineages to which it is committed, and therefore, is also termed transdifferentiation. So it is of great value for stem cell application in tissue engineering, curing tissue-damaged diseases and as the vector for transgene therapy. We compared the character of stem cells with essence theory, an elementary and important part of the basic theory of TCM, and found that stem cells are related greatly to essence,according to its two origin, congenital essence and acquired essence, and four kinds of functions of essence, which are controlling reproduction,controlling growth and development, manufacturing marrow and transforming blood; nourishing and moistening the zangfu-organs. Furthermore, stem cells especially have direct connected with the congenital essence which is stored in the kidneys. Congenital essence is the reproductive essence accepted from parents. Sperm and ovum merge to be oosperm, that is just the totipotent stem cell. The connotative meanings of congenital essence include all germ plasm and the special developing message of a genus in the oosperm. Genital stem cells are in charge of the function of controlling reproduction of congenital essence. Growth and development function is related to the proliferation and differentiation of adult stem cells controlled by gene. Manufacturing marrow function has relation with bone marrow stem cells and neural stem cells (NSC) in medullary cavity in which contains bone marrow, spinal cord and brain. Dominating bones function of the kidneys is related to mesenchymal stem cells (MSC), and hematopoietic stem cells (HSC) execute the function of producing blood completely.CONCLUSION: We consider that stem cells have a close relation with the congenital essence, and we put forward a new viewpoint in this research that stem cells have the same properties of congenital essence, and they are existence formation of congenital essence in cell level.

The effect of pH on the extracellularpolysaccharide synthesis by Aureobasidium pullulans was studied by addding CaCO3and HCI. Cultivated in P2 liquid medium for 24h,pH dropped to 3. 6 because of strongly producing acid. Under this low pH environment, further fermentation for 120h,only 5. 9g/L of polysacharide was obtained. When grown in MP2 medium contaning0. 5 % CaCOs,the pH was kept above 5.0 during 144 hours,production of polysaccharide increased to 31g/L. The detailed information of effects of controlled pH on polysaccharide production showed an optimal pH value 5. 0 must be maintained through the fermentative period.
9.Aortoplasty with pulmonary autograft patch for coarctation of the aorta combined with hypoplastic aortic arch in infant

ObjectiveTo discuss the operative techniques and results of coarctation resection plus aortoplasty with pulmonary autograft patch for coarctation of the aorta combined with hypoplastic aortic arch in infant.MethodsBetween May 2007 and Dec 2009,14 cases including 9 males and 5 females with caorctation of the aorta and hypoplastic aortic arch underwent coarctation resection plus aortoplasty with pulmonary autograft patch in our hospital.The age ranged from 23 days to 17 months,with a median of 4.33 months.The mean body weight was (6.14 ±2.36) kg.All patients were diagnosed as aortic coarctation combined with VSD and hypoplastic aortic arch.The surgery was performed under deep hypothermia cardiopulmonary bypass with selective cerebral perfusion in 8 cases and circulation arrest in 6 cases.Fresh pulmonary autograft patch harvested from the main pulmonary artery was used for aortoplasty.The associated VSD was repaired in the same stage.ResultsAll patients survived except one died from circulatory failure during the perioperative period.Low cardiac output syndrome occurred in another case who was cured afterwards by correspondent treatments.No residual obstruction was detected by echocar-diography after the operation.Follow-up was carried out in 13 cases from 4 months to 3 years.Echocardiographic examination showed that the pressure gradient across the aortic arch was less than 16 mm Hg in all cases.The blood velocity at the descending aortic arch was not significantly changed during the follow-up period as compared with that of the immediate after operation.Computed tomography showed that the morphology of aortic arch was normal.The left bronchus compression was relieved obviously or totally disappeared in patients who suffered from left bronchus stenosis before operation,and no aortic aneurysm were detected in these patients.ConclusionConclusion Coarctation resection plus aortoplasty with pulmonary autograft patch is the optimal surgical method for treating coarctation of the aorta combined with hypoplastic aortic arch in infant.
10.Modified mitral valve annuloplasty for mitral valve regurgitation in children

ObjectiveTo review and summarize the experiences of the modified mitral valve annuloplasty for mitral valve regurgitation (MR) in children.MethodsOne hundred and six patients with moderate to severe MR were retrospectively studied from March 1999 to December 2009.They aged from 0.4 to 8.5 years [ mean (3.7 ± 1.8) years ]and weighted from 6.6 to 52.0 kg [ mean(10.0 ± 3.5) kg].There were 69 males and 37 females.The heart function was evaluated by echocardiography.Patients were divided into three groups according to the age: group 1, <6 months , 16 cases; group 2, from 6months to 2 years, 51 cases; and group 3, from 2 years to 8.5 years, 39 cases.Seven cases were MR combined with mitral valve stenosis. Different operations were performed according to preoperative homodynamic, heart function and anatomical structure.ResultsThe operative mortality rate was 2.8%.In 3 died patients, 2 were due to MR and 1 due to severe pulmonary hypertension.104 cases were recovered.All patients were followed up 3 months to 3 years.ConclusionModified mitral valve reconstruction is necessary for children with moderate to severe MR.It should be the first choice for MR patients associated with other cardiac malfomation.This technique could prolong the time for mitral valve replacement when needed.Moreover,this technique not only reduces the valve injury but also decreases the reoperation rate.
