1.ALLOGRAFT REPLACEMENT OF SEMI-ARTHROPLASTY

Benign or low-grade malignant tumor of bones neighbouring a joint can be treated by segmental resection and the use of an allograft for reconstruction. Fourteen patients have been treated in this way since 1977, with success in 12 cases and failure in 2 because of infection.Allograft was obtained from fresh cadaver and stored in deep-frozen state (-170C). Within 3 weeks to 6 months, the biological vigor of the bone tissue could be retained and its antigenicity destroyed, so that no immunological reaction took place after replacing the joint and the allograft survived.Eleven patients have been followed up from one and one-half years to five years In the 7 cases with giant-cell tumor at the proximal end of tibia, their range of movement has been attained around 90 degrees. Of the two patients with os-teogenic sarcoma, one has been followed up for one and a half years, and the other for three years. There appears no evidence of local recurrence or distant metastasis in both. The affected limbs are all capable of weight-bearing and locoma-tion.
2.RESECTION OF PRIMARY INNOMINATE NEOPLASM AND REPLANTATSON OF ARTIFICIAL HEMI-PELVIS AND TOTAL HIP JO!NT

The traditional treatment for primary malignant tumors involving the innominate bone is hemi-pelvictomy.In recent years, under the control of chemotherapy or radiation, partial or complete resection of innominate bone with lower extremities maintained has been reported.Since 1983, five patients in our hospital have undergone resection of hemi-pelves and femoral heads, and have been inserted by titanium-alloy hemi-pelves and total hip prostheses.The operations were uneventful.Among the five patients, the first and the second were diagnosed as condrosar-coma. Before and after operations, the two patients were treated with a large dosage of chemotherapy and had no reactions.During the follow-up for two years and a half. They were pain-free in affected limbs and could walk with canes.The other three patients diagnosed as giant-cell tumors were followed up from half a year to one year, and they all could walk with crutches.Based on the five cases, we think that in some very carefully selected cases, a limb-saving procedure-inserting a prosthesis after performing hemi-pelvictomy-can lead to a good function and maintain the limb length.
3.Ethical Thinking on Hospital Laboratory Information System

This paper discussed the existing problems of Information System ( LIS) from six aspects, inclu-ding:the security and the expansion of the function, protection of patients′privacy, the establishment of emergency response mechanisms, authenticity and accuracy of data and the risks for auto-approval rules, etc.The authors also analysed the necessity of its rectification from the perspective of ethics, proposed improvement programme and suggestions: strengthening the safety management systems and data, ensuring special check the confidentiality of the project, protect the patient′s privacy;setting up emergency mechanism;hospital based on the actual workload and resources to determine the LIS function module; regularly checking the test data, to avoid occupational inju-ries;continuously improving the rigour of automatic approval rules.
4.EXPERIMENTAL TREATMENT WITH "ENERGY-CONTAINING LACTATED RINGER'S SOLUT|ON"OF TRAUMATIC SHOCK IN RABBITS

Our experiment aims to compare the effects of "energy-containing lactated Ringer's solution"(EL-RS) with those of "lactated Ringer's solution" (LRS) as an initial resuscitation fluid for the treatment of traumatic shock in rabbits. Deep shock animal models were produced in 74 adult rabbits using the method of pincers pressure and Wiggers' hemorrhagic shock method to produce crush injury of the soft tissue on the thigh and comminuted fracture of the femur. Results showed that blood pressure of the experimental group rose more rapidly than control group and the survival time of the former were longer than the later. The lowering of serum lactate and blood pH etc. showed a similar difference. It is suggested that ELRS is more efficient than LRS for the treatment of traumatic shock in rabbits.
5.Correlated Factors in Hospital Acquired Infection of Liver Cirrhosis

OBJECTIVE To study the correlated factors in acquired infection of liver cirrhosis and offer more reliable theory of treatment for clinic. METHODS The liver cirrhosis history of 302 cases was retrospectively analyzed and reviewed. RESULTS From them 43 cases acquired infection during hospitalization, with 61 case-times of infection, the rate of hospital acquired infection was 14.2%, the rate of case-times of infection was 20.20%. Nineteen cases were died and the mortality rate was 44.19%, Among the 259 non-infected patients only 5 cases were died, the mortality rate was 1.93%, the difference was significant (P
6.Changes in peripheral blood lymphocyte subsets in children with active tuberculosis

Objective:To analyze the changes in T lymphocyte subsets, B lymphocytes and NK cells in children with active tuberculosis (TB) and their clinical significance.Methods:T lymphocyte subsets, B lymphocytes and NK cells in peripheral blood samples of 106 patients with acute TB (TB group) and 106 healthy children (healthy control group) were detected by flow cytometry and compared between different groups.Results:The percentages of CD3 + T, CD4 + T and NK cells as well as the CD4 +/CD8 + T cell ratio were significantly lower in the TB group than in the healthy control group ( Z=-3.783, P=0.000; Z=-5.401, P=0.000; Z=-3.434, P=0.001; Z=-2.014, P=0.044). The percentages of double negative T (DNT) and B cells in the TB group were significantly higher than those in the healthy control group ( Z=2.765, P=0.006; Z=6.880, P=0.000). No significant difference in the percentage of CD8 + T or double positive T (DPT) cells was observed between the two groups ( P>0.05). The expression of peripheral lymphocyte subsets varied in TB children of different age groups (0-<3, 3-<6, 6-<10 and 10-<16 years old). There were significant differences in CD3 + T, DNT and B cells among the four age groups ( H=10.081, P=0.018; H=14.583, P=0.002; H=8.498, P=0.037). The percentage of CD4 + T cells was significantly lower in children with extrapulmonary TB than in those with pulmonary TB ( Z=-3.068, P=0.002). No statistically significant difference in other lymphocyte subsets was found between children with extrapulmonary and pulmonary TB ( P>0.05). Conclusions:Tuberculosis could lead to immune dysfunction in children. Dynamic monitoring of the changes in peripheral lymphocyte subsets in children with TB could be conducive to better assessment of immune status and providing personalized treatment.
8.An Experimental Study of Hydroxyapatite as Bone Graft Substitute

In this study Na2HPO4-Ca(NO3)2 system was used to synthesize hydroxyapatite (HA). Its synthetical method was simple and low in cost. With the physical-chemical characteristic observation and the animal experimentit was proved that HA was pure, with pores ranging from 20 to 200 ?m in diameter.The chemical structure of HA was similar to that of inorganic part of bones.An investigation of the histological changes and osteogenetic effectiveness of HA implanted was conducted and the mechanism of HA osteogenetic effectiveness is discussed. The study will provide scientific basis for the clinical use of HA as bone graft substitute.
9.Factors influencing prognosis of thoracolumbar spinal injury combined with spinal cord injury

Objective To investigate the factors influencing prognosis in surgical treatment of thoracolumbar spinal injury combined with spinal cord injury so as to assist in the clinical treatment.Methods A retrospective study was made on 77 cases of acute thoracolumbar spinal injury combined with spinal cord injury admitted from July 2005 to April 2011.There were 66 men and 11 women,aged 14-66 years (mean,36.5 years).Neurological performance evaluated using American Spinal Injury Association (ASIA) scale was grade A in 31 cases,grade B in 11 cases,grade C in 10 cases,and grade D in 25 cases.Potential factors affecting the prognosis of spinal cord injury were identified using univariate analysis and incorporated into the Logistic regression equation to filter out the main influencing factors.Results Fifteen cases dropped out at a mean 55.5-month follow-up (range,24-96 months) and follow-up rate was 81%.Univariate analysis selected fracture or dislocation types (P < 0.01),combined injury (P < 0.05),use of glucocorticoid within 8 hours postinjury (P < 0.05),standard transport by medical professionals (P > 0.1),complete injury (P < 0.01),visiting time (P =0.055),cause of disease before operation (P < 0.05) and canal encroachment rate (P < 0.01) as predictors of neurologic recovery in spinal cord injury (P < 0.1).Logistic regression analysis revealed significant prognostic factors were canal clearance (P < 0.01) and use of steroids within 8 hours postinjury (P < 0.05).Conclusion Glucocorticoid pulse therapy within 8 hours postinjury and adequate spinal cord decompression may effectively improve the neurologic prognosis in thoracolumbar spinal injury with spinal cord injury.
10.Prevention of Sciatic Denervation Disuse Osteoporosis in Rat Tibia with Capacitively Coupled Electrical Stimulation

Sciatic neurectomy was performed on the right limb in 30 rats divided into three groups. On the first postoperative days, stainless steel capacitor plates were applied parallel to each other over the midtibial region of the right, neurectomized limb m each animal in the latter two groups, which were stimulated continuously with a 60kHz sinawave signal delivering 5-10V from peak to peak. The first group was served as controls and received no electrical stimulation. At the completion of 12d of electrical stimulation, all rats were sacrificed. A statistically significant enhancement of wet weight, dry weight, ashed weight, ultimate strength and bone density occurred in the stimulated, denervated tibiae of the experimental animals compared with the nonstimulated, denervated tibiae of the control animals. The results show that a sciatic neurectomized osteoporosis was prevented in the tibiae by a capacitively coupled electrical field.
