1.Preventive effects of vitamin K2 on tumor recurrence in patients with hepatocellular carcinoma after radical resection

Objective To study the preventive effects of vitamin K2 on tumor recurrence in patients with hepatocellalar carcinoma (HCC) after radical resection. Methods The clinical data of 50 patients with HCC who received radical resection from March 2006 to March 2007 in No. 181 Hospital of PLA were analyzed retrospec-tively. All the patients were divided into 2 groups according to the random number table. Twenty-six patients in vitamin K2 group were administered with menatetrenone (45 mg per day), and the rest 24 pateints were in the control group. The accumulative and tumor-free survival rates, differences between the 2 groups, multivariate factors for prognosis were analyzed by Kaplan-Meier curve, Log-rank test and Cox regression model, respectively. Results During a period of 36 month follow-up, 10 patients died and 28 had tunor recurrence. The 1-, 2-, 3-year accumulative survival rates were 96%, 92% and 83% in vitamin K2 group, and 96%, 82% and 63% in control group (χ2 = 3.61, P > 0.05). The 1-, 2-, 3-year tumor-free survival rates were 92%, 60% and 38% in vitamin K2 group, and 75%, 42% and 12% in control group, with significant difference between the 2 groups (χ2 =5.61, P <0.05). Univariate and multivariate Cox proportional hazard analysis showed that without taking menate-trenone, the preoperative level of alpha fetoprotein (AFP) ≥800 μg/L and vascular invasion were the indepen-dent risk factors for tumor recurrence. Conclusions Vitamin K2 has a suppressive effect on tumor recurrence of HCC, while patients with AFP≥800 μg/L before operation or with vascular invasion have poor prognosis.
2.3-D center of gravity mapping:a new method for assessment of FES-assisted paraplegic walking efficiency

Objective To explore a new method for assessing the walking efficiency of the paraplegic patients assisted by functional electrical stimulation (FES). Methods The measurement system based on a standard walker was developed. During FES assisted walking of the paraplegic, the real time of the upper limb support were obtained and transformed into a 3 D center of gravity (CG) motion map with a paraplegic upper body mechanical model to describe the CG motion locus. Then the FES efficiency indicated by walking balance condition was assessed objectively and quantitatively. Results In this design, the pilot study of a paraplegic patient undergoing walking training with FES showed that the force accuracy was better than 1.01%, nonlinearity was less than 0.8%, and crosstalk was less than 3.2%. Conclusion The results showed that this system may be used as 1) an evaluation index of FES assisted paraplegic walking efficiency, 2) a balance control indicator during FES assisted paraplegic walking training and 3) a feed back signal to choose an efficient FES pattern and sequence.
3.Liver transplantation for cholangiocarcinoma in 6 patients

Objective To evaluate the therapeutic effects of liver transplantation (LT) for cholangiocarcinoma(CC)and analyze the prognostic factors.Methods From December 2001 to December 2006,234 patients receiving LT for hepatic carcinoma in our institute were enrolled as a basis of comparative study for 6 CC patients undergoing LT during the same period.Results These 6 patients were followed-up from 1 to 56 months.Five patients died and one recurred.The 0.5-,1-and 2-year patient cumulative survival rates were 4/6,3/6 and 1/6,respectively.The 0.5-,1-and 2-year tumor-free survival rates were 3/6,2/6 and 1/6,respectively.The average patient or tumor-free survival time were both(14±4) months.Conclusion The prognosis of cholangioearcinoma patients after LT iS poor.
4.Setting of Noise Index in Low-dose Chest CT Examination

Purpose To explore the optimal value of noise index (NI) of adaptive statistical iterative reconstruction (ASIR) in low-dose chest examination,so as to reduce the radiation dose of patients.Materials and Methods 1200 mid-age and elderly healthy individuals were randomly divided into 4 groups according to the NI difference,with 300 in each group,group A:NI=20;group B:NI=25;group C:NI=30;group D (the normal control group):NI=14.Combined with AS IR technology,the quality of the axial image was reviewed and evaluated,and the score of each subject was given and recorded based on the quality of the image and the matching degree with the disease it indicated.Results The subjective scores of images in each group were:group A (3.53±0.43),group B (3.28±0.32),group C (3.12±0.18) and group D (5.00±0),respectively.The scores of these 4 groups were all above 3,which could be used for diagnosis.The body mass index was (24.39 ± 3.09) kg/m2,(25.49 ± 2.45) kg/m2,(25.53 ± 3.21) kg/m2 and (25.55 ± 2.28) kg/m2,respectively.The number of layers was 60.64 ± 6.38,64.42 ± 5.71,61.77 ± 6.45 and 62.32±6.15,respectively,and there was no statistically significant difference between these 4 groups (P>0.05).The effective dose (ED) was (1.66± 0.84) mSv,(1.11 ± 0.34) mSv,(0.88±0.30) mSv and (5.99± 1.37) mSv,respectively,and the differences between any two groups were all statistically significant (P<0.05).ED of group C was the lowest,85% lower than group D,47% lower than group A,and 21% lower than group B.Conclusion Using ASIR technology,setting NI=30 can ensure that the image quality meet the needs of diagnosis,and also effectively reduce the radiation dose,which is a preferable scanning scheme.
5.A meta-analysis of therapeutic effect of cyclosporine microemulsion and tacrolimus for patients undergoing liver transplantation

Objective:To evaluate the efficacy and safety of microemulsified cyclosporine(Neoral)and tacrolimus(FK506) for immunodepression after Iiver transplantation.Methods:According to the including criteria,fifteen randomized controlled trials were enrolled in this analysis.The data of trial design,characteristics of the subjects,and findings of the studies were reviewed and analyzed by RevMan 4.2.8 software.Results:The patient survival rate,graft survival rate,incidence of nephrotoxicity,and incidence of infection were not significantly different between Neoral and FK506 groups,with the relative risk and(95% CI,P)being 0.99(0.96-1.02,0.37),0.97(0.92-1.03,0.30),0.99(0.87-1.13,0.86)and 1.08(0.97-1.20, 0.16),respectively.The incidences of hypertension(1.34[1.15-1.55,0.000 1])and acute rejection(1.15[1.06-1.25,0.001]) were significantly lower in the FK506 group,with no significant difference found in the degree of acute rejection(1.00[0.92- 1.22],0.98).Interestingly,the incidence of diabetes was significantly higher in the FK506 group within 1 year after the operation,but was similar to that of the Neoral group thereafter,with relative risk(95%CI,P)being 0.72(0.62-0.83,
