1.Research on expression of granzyme B in myocardium with experimental viral myocarditis

Objective To explore the role of granzyme B( GzmB) in infiltrated myocardial cells of mice with viral myocarditis( VMC) induced by coxsackievirus B3(CVB3) during the injury of myocardium and further to elucidate the pathogenesis of VMC. Methods Thirty-six BALB/c male mice, 5 to 6 weeks old, were randomly divided into two groups: the normal group, 10, and the model group, 26. On the eleventh day all mice were killed and the hearts were taken out for inspection whether there were pathological changes in the myocardium by macroscopy and microscopy. The expression of GzmB was detected in myocardium with immunohistochemistry and hemi-quantitive reverse transcript-polymerase chain reaction ( RT-PCR). Results GzmB positive cells in the mice myocardial cells infiltrated were detected by immunohistochemistry in the model group,and the number of GzmB positive cells is remarkable positive correlation to myocardial pathological score(r =0. 859, P =0.000) , however, no such cells were discovered in the normal murine hearts. Test of RT-PCR showed markedly high expression of GzmB mRNA in mice myocardium of the model group, however, no expression of GzmB mRNA in the normal group. Conclusion The expression of GzmB in the infiltrating myocardiocytes probably plays some role during the injury of myocardium, which mediated cytotoxicity effect is one of important mechanisms of myocardial injury in viral myocarditis. The expression quantity of GzmB in the infiltrating myocardiocytes is significant positive correlation to the severity of myocardial injury.
2.Clinical Management of Collection and Sterilization for Dental Handpieces

OBJECTIVE To ensure clinical application of standard procedure in the management of dental handpieces collection and sterilization.METHODS To establish the center of sterilization and supply for dental handpieces;to equip with the device for depuration and sterilization;to standardize the procedure for depuration,retrieval,pre-disposal treatment,cleaning,drying,lubrication,envelopment and sterilization.RESULTS The management of collection and sterilization for dental handpieces ensured the clinical application of standard procedure,avoided the exogenous infection and guaranteed the patients safety.CONCLUSIONS The clinical management of collection and sterilization for dental handpieces is an effective method.
3.Effects of polysaccharides from Dendrobium officinale on expression of Bax and Bcl-2 in vascular endothelial cells induced by high sugar

Aim To optimize extraction of polysaccha-rides from Dendrobium officinale ( DOP ) and investi-gate the effects of DOP on the expression of Bax and Bcl-2 in vascular endothelial cell of human umbilical vein ( HUVEC) by the induction of high sugar. Meth-ods Response surface methodology ( RSM ) was em-ployed to optimize the conditions for extraction of DOP on the basis of single factor experiment. The HUVEC cells were divided into the blank control group, the normal blood sugar group, the high sugar stimulation group and different drug concentration group plus the high sugar stimulation group. The growth condition of cells was examined by methyl thiazolyl tetrazolium ( MTT) , and RT-PCR was used to detect the expres-sion of Bcl-2 Assaciated X protein ( Bax ) and B-cell lymphoma-2 ( Bcl-2 ) in vascular endothelial cells after 24 and 48 hours. Results The optimal extraction time was 2 h and the optimal liquid-solid ratio was 15. 75∶1 ( mL/g ) , and the optimal extraction frequency was 2. 8. Under these optimized conditions, the extraction rate of polysaccharides was 25. 1%, while the experi-mental extraction rate was 24. 9%, which was highly fit with the regression model. Compared with normal sugar group, high sugar group reduced the growth of cells and enhanced the expression of Bax, and inhibi-ted the expression of Bcl-2 . DOP promoted the growth of cells in high sugar induction, inhibited the expres-sion of Bax, and enhanced the expression of Bcl-2. Conclusions The process optimized through RSM has the extraction effect, and has certain practical signifi-cance. Through the down-regulation of the expression of Bax in vascular endothelial cells and the up-regula-tion of the expression of Bcl-2 by the induction of high sugar, DOP can restrain HUVEC, which could prevent and cure diabetic angiopathy to some extent.
4.Clinical Efficacy and Effects on Hemorheology of Acupuncture Therapy Combined with Gabapentin in the Treatment of Patients with Tension Headache

Objective:To investigate the clinical efficacy and effects on hemorheology of acupuncture therapy combined with gaba-pentin in the treatment of patients with tension headache. Methods:Totally 92 cases of patients with tension headache were randomly divided into the observation group (n=46) and the control group (n=46) according to the random number table. The control group was given gabapentin, while the observation group was treated with acupuncture therapy on the basis of treatment in the control group. The treatment course was 4 weeks. The clinical efficacy, changes of the headache indices, VAS score and hemorheology levels before and after the treatments in the two groups were compared, and the incidence of adverse reactions was observed as well. Results:The to-tal effective rate of the observation group (93. 48%) was significantly higher than that of the control group (73. 91%, P<0. 05). Af-ter the treatment, the headache indices in both groups significantly decreased (P<0. 05), and the decrease in the observation group was more significant than that in the control group (P<0. 05). The VAS scores in both groups significantly decreased after the treat-ment (P<0. 05), and the decrease in the observation group was more significant than that in the control group (P<0. 05). The plas-ma viscosity, hematocrit and platelet aggregation rate in both groups significantly decreased after the treatment (P<0. 05), and the decrease in the observation group was more notable than that in the control group (P<0. 05). No severe adverse reactions were shown in both groups during the treatment course. Conclusion: Acupuncture therapy combined with gabapentin in the treatment of patients with tension headache shows striking effects, which can improve hemorheology with promising safety and deserves further studies.
5.Effect of Qingxin capsule on differentiation of Th cell in the course of viral myocarditis

Objective:To investigate the changes of Th cell differentiation in the VMC and interference effect of Qingxin capsule on them.Methods:BALB/c mouses with different courses of VMC were established, after treated with Qingxin capsule, the cardiac pathological changes were observed by light microscope and transmisson electron microscope,levels of IL-2,IL-4,IL-10 and IFN-? in blood serum were detected by ELISA.Results: Whatever in acute stage or recovery stage of VMC, the myocard pathological changes of mouses were lighter after treated with Qingxin capsule;moreover, levels of IFN-? and IL-2 in blood serum of mouses with VMC decreased significantly(P0.05).Conclusion:Qingxin capsule can restore the balance of Th1 and Th2 cells through inhibiting reaction of Th1 and enhancing reaction of Th2.
6.Restoration of cardiac function in failing beagle hearts by recombinant adeno-associated viral gene transfer of sarcoplasmic reticulum Ca~(2+)-ATPase

ObjectiveTo assess the safety of intramyocardium injection of sarcoplasmic reticulum Ca2+-ATPase (SERCA2a) to rescue chronic heart failure beagles. MethodsThe heart failure beagles were assessed with myocardium enzymology, cAMP content of myocardium, myocardial oxygen consumption, arrhythmia, systemic inflammation factors, and the function of liver and kidney after SERCA2a gene transferred. ResultsThere were no increase of cAMP content, no more arrhythmia, myocardial oxygen consumption, no apparente systemic inflammation, and no injures to hepatic and renal function. ConclusionOverexpression of SERCA2a by intramyocardium injection is relatively safe.
8.Application of flipped classroom based on WeChat and TBL in basic surgery teaching

There are many problems existing in the basic teaching of traditional surgery, such as much more content, difficult operation, fewer learning hours, more passive acceptance while less active learning and so on. Flipped class based on the WeChat and team-based learning (TBL) is based on the on-line and off-line class. We can use WeChat group and subscription to issue relevant learning materials, learning tasks and test before class. Also, we can summarize and explain incisively by discussion after field operation in class, and answer and test in the way of WeChat discussion after class. Finally, the three-dimensional hybrid teaching will be condensed and sublimated by brand competition based on the basis of surgery, achieving knowledge transfer before class, knowledge internalization in class, knowledge consolida-tion after class and brand competition sublimation, in order to improve the teaching effect of surgical basis.

Objective To investigate the clinical efficacy of ginkgo leaf tablets in patients with forgotten-type mild cognitive impairment (aMCI) and its effect on soluble glycosylated terminal product receptor (sRAGE) and soluble low-density lipoprotein receptor-associated protein 1 (sLRP-1).Methods A total of 124 patients with aMCI were divided into observation group (n =62) and control group (n =62),treated by nimodipine tablets (30 mg) three times a day,ginkgo leaf tablets for 3 times a day,respectively.The changes of MoCA score,levels of sRAGE,sLRP-1 and the occurrence of adverse reactions before and after treatment were compared between the two groups.Results The scores of MoCA were improved in both groups after treatment,and the observation group improved more after treatment,and the differences were statistically significant (P < 0.0 5).There was no significant difference in plasma sRAGE and sLRP-1 levels between the two groups (P > 0.05),but the plasma sRAGE increased and the sLRP-1 level decreased after treatment in two groups (P < 0.05).Plasma sRAGE was higher in the treatment group than that in the control group,while the sLRP-1 level was lower than that in the control group (P <0.05).Conclusion Ginkgo leaf tablets have a significant clinical effect for aMCI patients,which can increase level of plasma sRAGE and decrease sLRP-1.
