Objective To discuss the effect ofQiqin decoction combined with lamivudine for the patients with hepatitis B and the TCM syndrome of liver stagnation and spleen deficiency and damp heat. Methods A total of 70 patients with chronic hepatitis B were divided into two groups by random number table method, 35 patients in each group. The patients in the control group were treated with lamivudine, and the patients in the observation group used lamivudine combined withQiqin decoction. The HBV-DNA was detected by fluorescence quantitative PCR, and the levels of serum ALT and AST were detected by full automatic biochemical analyzer. The TCM syndrome score of two groups were compared, and the drug resistance in the treatment process was recorded, and the clinical effect rate was evaluated.Results After treatment, the HBV-DNA negative rate of observation group was 77.1% (27/35), and the control group was 54.3% (19/35). There were statistically significant difference of the negative rate of HBV-DNA between two groups (χ2=4.136, P=0.041). The drug resistance rate of the observation group was 11.4% (4/35), and the control group was 31.4% (11/35). There were statistically significant difference of the drug resistance rate between two groups (χ2=4.297,P=0.043). After treatment, the serum ALT (29.1 ± 13.2 U/Lvs. 121.8 ± 50.2 U/L,t=5.982), the AST (28.5 ± 10.9 U/Lvs. 98.3 ± 46.1 U/L,t=8.263), the TCM syndrome scores (4.8 ± 3.1vs. 11.5 ± 7.9, t=5.982) in the observation group were significantly lower than those in the control group (P<0.01). The total effect rate of observation group was 88.6% (31/35), and the control group was 57.2% (20/35). There was significant difference between two groups (χ2=4.248,P=0.003).Conclusions The Qiqin decoction combined with lamivudine could reduce the levels of HBV-DNA, serum ALT levels and AST levels of patients with chronic hepatitis B, and improve the clinical efficacy.

Objective To evaluate two regimes of transient intensive insulin therapy in newly diagnosed type 2 diabetic patients with severe hyperglycemia.Methods Sixty-eight newly diagnosed type 2 diabetic patients with fasting plasma glucose(FPG)≥10.0 mmol/L and glycated hemoglobin A1c (HbA1c)≥8.0％were randomized to either cantinuous subcutaneous insulin infusion(CSII)group or multiple daily injection(MDI)group.Glycemic control,daily dose of insulin,body mass index(BMI),the mean area under the insulin curve(AUC),β-cell function(HOMA-β)and insulin resistence(HOMA-IR) were compared before and after treatment.Results There were no significant differences between two groups in days reaching glycemic control and BMI.The maximal insulin dose for glycemic control in CSII group was(0.53±0.06)U·kg-1·d-1 and that in MDI group was(0.71±0.04)U·kg-1·d-1(t= 11.100,P＜0.01).After CSII and MDI treatment,HOMA-β and the mean AUC of insulin were significantly increased(P＜0.01),but there was no significant difference between two groups(P＞0.05).No significant changes in HOMA-IR were observed before and after treatment or between two groups.There were no severe hypoglycemic episodes in two groups and mild hypoglycemic episodes were 0.34％(13/3806)in CSII group and 1.09％(44/4045)in MDI group.Conclusions The excellent glycemic control can be reached by short.terra CSII and MDI treatment in newly diagnosed type 2 diabetic patients with severe hyperglycemia.Lower insulin dose and less mild hypoglycemic episodes seem to be the advantage of CSII regime than MDI regime.β-cell function can be dramatically improved in both therapy regimes.

Objective To investigate the effect of fentanyl citrate combined with psychological intervention on the treatment of elderly patients with inguinal hernia tension-free repair. Methods From May 2015 to May 2017, 110 elderly cases with inguinal hernia tension-free repair were divided into two groups, the control group and the observation group according to the different nursing intervention mode . The control group were received routine nursing intervention combined with midazolam. The observation group were given fentanyl citrate and routine nursing care combined with psychological intervention. The effect and safety in the two groups were recorded and compared. Results The curative effect in the observation group was remarkable, and the indexes during the operation were better than those in the control group. The incidence of adverse reactions was lower than that in the control group, and the nursing satisfaction was higher than that in the control group, the difference was statistically significant (P<0.05). Conclusion The effect is remarkable which Fentanyl citrate combined with routine nursing and psychological intervention were used on the treatment of elderly patients with inguinal hernia tension-free repair, the body indexes are stable, has less adverse reaction and high nursing satisfaction, is worthy of clinical application.

Objective: To observe the therapeutic effect of electroacupuncture on Jiaozheng (Extra) plus hydro-acupuncture in treating facial paralysis, and to explore a better acupuncture therapy for treating facial paralysis. Methods: One hundred and nineteen cases with peripheral facial paralysis were randomly allocated into an observation group (60 cases) and a control group (59 cases) by the visiting sequence. The cases in the observation group were treated with electroacupuncture on Jiaozheng (Extra) plus hydro-acupuncture, and the cases in the control group were treated with normal acupuncture. The treatment was conducted once a day and 7 times as a course, with 1-2 d interval. Evaluation were done after 6 courses. Results: Comparing the cure and marked efficacy rate, observation group was much better than control group, and there was statistical difference (P<0.05); after 14-day treatment, the cure and marked efficacy rate of the observation group and the control group was 78.3% and 49.2%, respectively; after 21-day treatment, the cure and marked efficacy rate of the observation group and the control group was 88.3% and 67.8%, respectively. There was significant difference between the two groups (P<0.01). Conclusion: The therapeutic effect of electroacupuncture mainly on Jiaozheng (Extra) plus hydro-acupuncture in treating facial paralysis was better and with shorter courses.

Objective To analyze quantitatively the acute effect of serious air pollution on cardiovascular mortality in Taiyuan, a typical city where the air was heavy polluted in China. Methods Mortality data in 2004 were from Taiyuan mortality surveillance system, meteorological data and air pollution data were from Taiyuan Meteorological Bureau and Taiyuan Environmental Monitoring Station, respectively. Generalized Additive Model (GAM) extended Poisson regression was used to examine the relationship between air pollution and mortality in Taiyuan, and controlling with time trends, weather and the day of week. Nonparametric conditional bootstrap was used to adjust the concurvity and reduce estimation error. Results The average concentration of PM10, SO2, NO2、CO in 2004 were 173.55, 79.34, 23.03, 2 214.59 ?g/m3 respectively. The median daily count of death for cardiovascular disease in 2004 was 7/daily. Objective To assess the association of air pollution and preterm birth in Taiyuan.Methods The preterm birth data from November 2005 to January 2007 were from Taiyuan birth surveillance system,meteorological data and air pollution data were from Shanxi Meteorological Bureau and Taiyuan Environmental Monitoring Station respectively.The case-crossover analysis and conditional Logistic regression model were used to analyze the association of air pollution and preterm birth in Taiyuan,while the meteorological factors were controlled.Results The stronger effective period of PM10 was 5-day lag and 6-day lag,of SO2 was 7-day lag and 8-days moving average.When PM10,SO2 increased 50 ?g/m3,for PM10,the OR of a 5-day lag and a 6-day lag were 1.169(95%CI:1.058～1.292),1.235(95%CI:1.115～1.367)respectively,for SO2,the OR of a 7-day lag and a 8-days moving average were 1.346(95%CI:1.122～1.616),2.203(95%CI:1.517～3.201)respectively.In sensitivity analysis,the lagged effects of PM10 and the cumulative effects of SO2 were steady.Conclusion The air pollution in Taiyuan may produce an acute effect on preterm birth.

Objective To study the exposure-response relationship between particulate pollution level and hospital outpatient visits. Method From 2001 to 2005,daily outpatient visits data were from a municipal hospital in Taiyuan,meteorological data and air pollution data were from Taiyuan Meteorological Bureau and Taiyuan Environmental Monitoring Station respectively. Poisson Generalized Additive Model(GAM)) extended Poisson regression was used to examine the relationship between particulate pollution level and hospital outpatient visits,controlling with time trends,weather and the day of week. According to the shape of exposure-response relationship curve,piecewise linear regression was used for risk analysis. Results From 2001 to 2005,the average concentration of PM10 was 190.41 ?g/m3,the average number of daily hospital outpatient visits was 18.31 to the respiratory department,and 23.99 to the cardiovascular department. Exposure-response relationship analysis suggested a nonlinear positive correlation between PM10 and daily hospital outpatient visits. A larger increase in daily hospital outpatient visits was observed at lower concentration of PM10 than that at higher concentration. When PM10 increased 10 ?g/m3,respiratory outpatient visits increased 0.260 1%-0.917 8% and respiratory outpatient visits increased 0.208 3%-0.402 7%. The estimated effects of 2 or 3 days moving average of PM10 concentration were greater than that of concurrent day. The stronger effect of PM10 was found on respiratory outpatient visits,which was statistically significant. Conclusion PM10 may produce effect on respiratory outpatient visits.

Objective To probe the technique and clinical applications of spiral CT (SCT) scan in urinary system. Methods 40～80 ml angiografin was injected into the vein by high voltage injection in six cases,and 5～10 minute later, SCT scan was performed. The original data were sent to workstation, then reconstructed. 3D pictures of urinary system including renal calyx, ureter and bladder were clearly demonstrated in all cases. Results Of 6 cases, there were 3 ones with ureter stone,1 case with kidney calyx cancer, and 2 case with ureter repetitive. Conclusion CTU is a safe, convincible, convenient and fast technique. It may provide high resolving urinary tract image and will be a kind of non-injury imaging technique in diagnosing the diseases of urinary system.

Objective To analyze multislice CT (MSCT) multiphase multiphase enhancement scanning in the atypical hemangiomas. Methods During February 2013 from January 2014, chosing125 cases of patients with atypical hemangiomas in our hospital were retrospectively analyzed. Results There was a total of 240 lesions in 125 patients, multiple (2 or higher) in a total of 70 cases, 55 cases of single. A total of 170 were located in the right lobe, 70 were located in the left lobe, lesions was 0.8 to 4.5 cm in diameter, the average was (2.53±0.21) cm. A total of 205 lesions were circular, a total of 35 as lobulated or irregular. CT scan, a total of 155 were low density, 85 in equal density. A total of 120 state clearly, realm for part of the fuzzy and parts out of a total of 35, 85 was fuzzy. A total of 45 lesions density uneven, a total of 195 was lesions density uniformity. In 240 intrahepatic lesions, a total of 145 lesions present performance1, 80 lesions belong to 2 kinds of forms, 15 lesions characterized by 3 kinds of forms. Conclusion Although a few hemangiomas reinforcement is not typical, by the mutiperiodic enhanced MSCT scanning remain their respective characteristics, in identification of its pathological changes has important diagnostic value.

A 11-year-old child diagnosed as chronic tonsillitis and adenoid hypertrophy underwent adeno-tonsillectomy under general anesthesia. After surgery, patient complained with abdominal discomfort, paleness and vomiting, which presented as the old black contents. Complete blood count showed: 45.2 g/L, hemoglobin of red blood cells 2.57 x 10(12)/L, An emergency gastroscopy confirmed gastric ulcer with hemorrage. Hemorrage was controlled and complete blood count results restored to normal after supportive therapy. Gastric ulcer completely recovered two weeks after discharge.
