1.Application of fentanyl citrate combined with psychological intervention on the treatment of elderly patients with inguinal hernia tension-free repair

Objective To investigate the effect of fentanyl citrate combined with psychological intervention on the treatment of elderly patients with inguinal hernia tension-free repair. Methods From May 2015 to May 2017, 110 elderly cases with inguinal hernia tension-free repair were divided into two groups, the control group and the observation group according to the different nursing intervention mode . The control group were received routine nursing intervention combined with midazolam. The observation group were given fentanyl citrate and routine nursing care combined with psychological intervention. The effect and safety in the two groups were recorded and compared. Results The curative effect in the observation group was remarkable, and the indexes during the operation were better than those in the control group. The incidence of adverse reactions was lower than that in the control group, and the nursing satisfaction was higher than that in the control group, the difference was statistically significant (P<0.05). Conclusion The effect is remarkable which Fentanyl citrate combined with routine nursing and psychological intervention were used on the treatment of elderly patients with inguinal hernia tension-free repair, the body indexes are stable, has less adverse reaction and high nursing satisfaction, is worthy of clinical application.
2.Improvement of Children′s CT Radiation Dosage in Head Check through Adjusting Scanning Parameter

Objective To improve children′s radiation dosage through adjusting CT head scanning parameter. Methods During December 2006 to June 2007,292 cases were selected at random to adjust CT head scanning parameter, mainly for charge of voltage, tube current and scan time. At the same time,all images were appraised by two doctors. Results Adjustment of children′ s head scanning parameter could reduce the radiation dosage obviously, and no influence for diagnosis. Conclusion It′s no affect for diagnosis to improve children′s head CT scanning parameter, and leads reduction of CT dosage obviously.
3.Hospital Infections among Parturients An Investigation and Analysis

OBJECTIVE To investigate the risk factors of hospital infection among parturients to put forward prevention measures.METHODS The infection cases among parturients in our hospital 2005-2007 were analyzed by retrospective survey.RESULTS The first three infections sites were upper respiratory tract(29.79%);surgical incision(28.72%) and urinary tract(21.28%).The main factor for upper respiratory tract infetion was the traditional postnatal recovering modality.Surgical incision infections were mainly perineum ones.Catheterization after the caesarean operation led to urinary tract infection.CONCLUSIONS Postnatal recovery by scientific modality,aseptic operation and strict infection management are the keys to preventing obstetrical infections.
4.Preventive effect of sodium hyaluronate gel on postoperative adhesion of hysteroscopic intrauterine adhesions

Objective To investigate the Preventive effect of sodium hyaluronate gel on postoperative adhesion of hysteroscopic intrauterine adhesions. Methods 84 cases of IUA patients undergoing hysteroscopy were retrospectively selected,and divided into two groups according to the different ways of prevention and control of IUA,42 cases in each group.The control group was treated with Foley balloon catheter after operation,the study group was treated with sodium hyaluronate gel prevention on the based of control group.The clinical signs of the patients before and after treatment one,two and three months were observed,the therapeutic effects and complication were compared between two groups. Results After treatment one,two and three months,the adhesion type score and the score scale of study group, were significantly decreased,and lower than the control group,the difference was statistically significant (P<0.05).The total effective rate of the study group was 90.48%,which was significantly higher than that of the control group (83.33%),the difference was statistically significant (P<0.05).In the course of treatment,the complication rate of the two groups was 11.90% (5/42),there was no significant difference between the two groups in the incidence of complications. Conclusion Hyaluronic acid sodium gel on hysteroscopic intrauterine adhesions in patients with postoperative adhesions have a significant preventive effect,can improve the treatment efficiency and clinical signs,and with high security.
5.A clinical analysis of 30 patients with synchronous head and neck and esophageal cancer

Objective To examine the clinical characteristics,treatment,and prognosis of patients with synchronous head and neck and esophageal cancer (SHNEC).Methods The clinical data of 30 SHNEC patients treated in the Department of Radiotherapy of the First Affiliated Hospital of China Medical University from 2010 to 2014 were retrospectively reviewed.The patients were followed up and survival was calculated using the Kaplan-Meier method.Differences between survival curves were analyzed by the log-rank test,and prognostic analysis was performed using the Cox regression model.At the same and different time,the comprehensive diagnosis were 24 cases and 6 cases.21 patients were treated with radiochemotherapy, 9 patients were treated by surgery and radiotherapy±chemotherapy.Results SHNEC was frequently found in elderly men with heavy drinking and smoking habits.Hypopharyngeal cancer with middle esophageal cancer was most common among the patients;of the 30 patients,23(77%) had hypopharyngeal cancer,and 12(40%) had hypopharyngeal cancer with middle esophageal cancer.The median survival time of the patients was 20 months,and the 1-and 2-year overall survival (OS) rates were 83% and 37%,respectively.Univariate analysis showed that the stage of esophageal cancer,heavy smoking and drinking habits,and a family history of cancer were significantly associated with patient prognosis (P=0.009,0.044,0.012).In addition,the Cox analysis also demonstrated that the stage of esophageal cancer (HR=3.53,P=0.008),heavy smoking and drinking habits (HR=2.33,P=0.049),and a family history of cancer (HR=2.78,P=0.026) were independent prognostic factors.Conclusions Hypopharyngeal and middle esophageal cancer is the most common type of SHNEC,and the stage of esophageal cancer and heavy smoking and drinking habits are significantly associated with the prognosis of SHNEC patients.In addition,patients with stage Ⅰ and Ⅱ esophageal cancer can also achieve acceptable survival after receiving radiotherapy-based comprehensive treatment.

Studies using isolated rat fundus strip preparation demonstrated that l-stephanine ( l-STP ) , l-roemerine ( 1-REM ), l-stepholidine (l-SPD), 1-tetrahydropalmatine ( l-THP ) and dl-tetrahydrode-berine ( THB ) possessed 5-HT blockade properties. 1-STP ( 16, 25 ?M ) produced competitve antagonism against 5-HT receptor on rat fundus, while higher concentration ( 50 ?M ) suggested a non-competi tive inhibition, with pA2' and2' values equal to 5.8 and 4.2 respectively. 1-REM was found to be a non-competitive antagonIst ( pD' 2 = 4.5). 1-SPD, 1-THP and THB cuased a parallel shift to the right of 5-HT concentration response curve without any significant changes in their maximum response, indicating that they were competitive antagonists of 5-HT receptor. The antagonistic potencies of 1-SPD, l-THP and THB were expressed as pA2 values: 6.2,5.2 and 5.0.
7.Technique and Clinical Applications of Spiral CT in Urinary System

Objective To probe the technique and clinical applications of spiral CT (SCT) scan in urinary system. Methods 40~80 ml angiografin was injected into the vein by high voltage injection in six cases,and 5~10 minute later, SCT scan was performed. The original data were sent to workstation, then reconstructed. 3D pictures of urinary system including renal calyx, ureter and bladder were clearly demonstrated in all cases. Results Of 6 cases, there were 3 ones with ureter stone,1 case with kidney calyx cancer, and 2 case with ureter repetitive. Conclusion CTU is a safe, convincible, convenient and fast technique. It may provide high resolving urinary tract image and will be a kind of non-injury imaging technique in diagnosing the diseases of urinary system.
8.Clinical Application of Rotational DSA in Chemoembolization for Encephalic Angioma and Liver Tumor

Objective To discuss the clinical application of rotational DSA and its advantages. Methods 35 cases undergoing cerebral angiography and hepatic arteriography were performed with standard DSA, of which, 24 ones were performed with rotational DSA. Results Standard DSA could only display partial lesion at lateral side, while rotational DSA could display the pathological structure and surrounding structure. Conclusion Rotational DSA is valuable for the diagnosis and treatment of encephalic angioma and liver tumor.

The free amino acid concentrations in serum were determined in 65 normal adults by use of the physiological fluid analysis in Hitachi 835-50 Model Amino Acid Autoanalyer. 29 specimens were determined by physiological fluid analysis and protein hydrolyzate analysis method simultaneous-ly.The results of Serine, Glycine, Valine, Isoleucine, Leucine, Tyrosine, Phenylalanine, Lysine, Tryptophan, Proline, and the molar ratio of the bran-ched-chain amino acids/aromatic amino acids were very similar. By hy-drolyzate method Aspartic acid was not detected and Threonine, Glutmic acid, Cystine, Methionine and Arginine were higher than physiological fluid analysis.The reasons for such higher values were discussed.
10.The expression of IKB kinase in the peripheral blood and its correlation with the production of interferon-α in systemic lupus erythematosus patients

Objective To investigate the mRNA expression of IKB kinase (IKK-α) and interferon-α (IFN-α) in the peripheral blood leukocytes of patients with systemic lupus erythematosus (SLE), and to explore the role of IKK-α in the production of IFN-α in SLE patients. Methods SYBR green dye I based real-time quantitative PCR was used to detect the mRNA expression levels of IKK-α and IFN-α in the peripheral blood leucocytes of SLE patients and healthy controls. Serum levels of IFN-α were measured with ELISA method. Results IKK-α mRNA expression levels in SLE patients were significantly higher than those of normal controls (P<0.05). IKK-α mRNA expression levels in SLE patients with active disease were significantly higher than patients with stable disease (P<0.01). IFN-α mRNA expression level in SLE patients was significantly lower than that of the normal controls (P<0.01). IFN-α mRNA expression levels in SLE patients with active disease were significantly higher than patients with stable disease (P<0.01). Serum levels of IFN-α in SLE patients with active disease was significantly higher than that of the normal controls and patients with stable disease (P<0.05). The anti-dsDNA antibody correlated positively, and complement C3 correlated negatively with serum concentration of IFN-α. IKK-α mRNA expression levels in SLE patients correlated positively with serum concentration of IFN-α. Conclusion IKK-α correlates positively with serum concentration of IFN-α. The IFN-α level is significantly correlated with disease activity, This suggests that IKK-α may play an important role in the pathogenesis of SLE.
