Objective To explore the curative effect of combined use of minimally invasive percutaneous nephrolithotomy (MPCNL) and extracorporeal shockwave lithotripsy (ESWL) for the treatment of solitary renal staghorn calculi in patients with renal insufficiency. Methods Eight cases of solitary renal staghorn calculi associated with renal insufficiency were treated by a combination of MPCNL and ESWL. Results Stones were completely cleared away in 5 cases, while residual stones were found in 3 cases. No blood transfusion was required and no severe surgery-related complications were encountered. After surgery variable degrees of improvement in renal functions was observed. The serum creatinine (Cr) decreased from 289?166 ?mol/L pre-operation to 155?33 ?mol/L post-operation (t=4.69, P=0.004), and the blood urea nitrogen (BUN) decreased from 15.1?7.9 mmol/L to 8.3?1.9 mmol/L (t=4.00, P=0.005). The emission computed tomography (ECT) examinations showed the glomerular filtration rate (GFR) was elevated from 48.8?12.4 ml/s before operation to 63.0?8.4 ml/s after operation (t=4.68, P=0.003). Post-renal obstruction disappeared after operation. Follow-up for 0.5~4.5 years (mean, 2.8 years) in the 8 cases revealed no obvious changes in renal functions. Conclusions Combination of MPCNL and ESWL for solitary renal staghorn calculi associated with renal insufficiency is safe and effective.

Objective To investigate the mechanism of extremely low frequency electromagnetic field (ELF-EMF) and X-ray irradiation on liver carcinoma cell lines BEL-7402. Methods Liver carcinoma cell lines BEL-7402 had been incubated with ELF-EMF (100 Hz, 0.7 mT, 30 min, 3 days) after X-ray irradiation at different doses (0, 2, 4, 6, 8,10 Gy). The cells were observed on morphologic changes with scanning electron microscope. Flow cytometry and gene microarray were used to investigate the mechanism of cell apeptosis. Results ELF-EMF plus X-ray induced apoptosis on BEL-7402 was observed under scanning electron microscope. When X-irradiation was 2, 4, 6, 8 and 10 Gy, the apoptosis ratios of combined group and only X-irradiation group were 10.0%, 14.5%, 4.3%, 5.1%, 7.1% and 0.1%, 8.1%, 0.1%, 0.4%, 2.2% (P < 0.05) on flow cytometry. The result of microarray indicated that 1465 genes were up-regulated and 1108 down*regulated in the ELF-EMF plus X-ray group in comparison with the control group. The change rates of 110 apoptosis related genes were above 2 times, which including 71 up-regulated and 39 down-regulated. Gene microarray showed that ELF-EMF and X-ray irradiation had a mainly effect on different gene of apoptosis paths (CDC25 and CHKI, ATM, p38, PTEN, p53, G1/S, Fas, G2/M, Cell Cycle, Apoptosis and Caspase). The same genes of ELF-EMF plus X-ray group were showed 13 in ELF-EMF group and 42 in X-ray group. Conclusions Apoptosis paths were significantly different between ELF-EMF and X-irradiation. ELF-EMF cooperates with X-irradiation on inducing BEL-7402 cell apoptosis.

Objective To investigate the change of CD_4~+CD_(25)~(high)CD_(127)~(low)> regulatory T cells (Trcg cells) sub-group level in peripheral blood of non-Hodgkin lymphama (NHL) patients, and to explore its clinical significance. Methods Treg cells levels in peripheral blood of lymphoma patients and normal were detected by flow cytometry, followed by statistical analysis. Results In the 65 cases of NHL patients, Treg cells in peripheral blood were (6.72±1.38) %, higher than that in the normal control group (5.65±0.68) % (P <0.05). Percentage of Treg cells are significantly different between clinical stages and normal: [P <0.05, normal control group (5.65±0.88) %, Ⅰ -Ⅱ period (6.08±1.18) %, Ⅲ-Ⅳ period (6.95±0.85) %]. The percentage of Treg cells are also different among pathological types of patients and normal [P <0.05, normal control group (5.65± 0.68) %. The percentage of Treg diffuse large B-cell lymphoma (5.83±0.95) % and other subtypes of lymphoma (7.83±1.76) %]were observed. It is not sure that Treg cells percentage among patients with different levels of lactate dehydrogenase and normal are significantly different. [P >0.05, normal control group (5.65±0.68) %, patients with normal LDH group (6.97±1.20) %, patients with lactate dehydrogenase (6.54±1.02) %]. Conclusion Treg cells induced by tumor and could inhibit the immune cells, Treg cells percentage in peripheral blood of tumor patients is higher than that the normal control group, and increased with the clinical staging, so the percentage of Treg cells may serve as a clinical indicator to evaluate tumor load.

Chinese Journal of Organ Transplantation 2014;35(4):198-201
Objective To evaluate graft regeneration and influencing factors after right lobe adult living donor liver transplantation (LDLT) using MSCT.Method Sixty-three living recipients were included in this study. We measured graft volume periodically by using MSCT and IQQA-Liver workstation. The liver regenerative ratio (LRR) of different stages of recipients after LDLT was calculated and compared, and stepwise regression analysis was done to set up the regression equation.Result Within 0.5 month after LDLT,graft volume was increased rapidly, and LRR reached maximum [(106.11 + 30.90)%],then decreased slowly. There was significant difference in LRR among 0.5,1 month and 3,6 months after LDLT (P<0.05).The following factors, including whether the graft containing middle hepatic vein or not,age and sex of recipients, had no significant influence on LRR after LDLT (P>0.05).The status of liver function of recipients preoperatively had significant influence on LRR 0.5 month after LDLT (P<0.05).There was significantly negative correlation between the graft volume and LRRs of recipients at different stages after LDLT (P < 0.05).Regression equation could be derived. Conclusion Most evident graft regeneration occurs in early stage after adult LDLT.There are many and different factors influencing graft regeneration at the different stages after LDLT.Graft volumetric measurement by using MSCT is a valid modality to evaluate graft regeneration after LDLT.
