1.An experimental method for endogenous pyrogen preparation from monocytes of rabbits' blood

In order to obtain monocytic endogenous pyrogen, Ficoll—Hypaque's separation technique was used in this paper. We obtained mononuclear cells from rabbits' blood in vitro and added endotoxin free saline to make suspension in the concentration of 0.65?10~6/mL. Endogenous pyrogen was produced from monocytes by stimulation of purified endotoxin of colon bacilli and incubation in saline at 37-38℃ stirred and shaked continuously for 10 hr. There is EP in collecting fluid after centrifugation and ultrafiitration.The result obtained shows:1. Employing Ficoll-Hypaque's separation technique, high concentration monocytic endogenous pyrogen can be obtained.2. Monocytic endogenous pyrogen have marked febrile effect.
2.Studies on the purification of rabbit monocyte pyrogen

Crude monocytic endogenous pyrogens from rabbits' blood were further purified by means of ultrafiltration membranes. The sterile pyrogen obtained was passed through gel filtration on a 1.5 by 100cm G-75 Sephadex column, and subje- cted to a 1.5 by 60cm ion-exchange chromatography on QAE Sephadex or subjected to isoelectric focusing. Under our experimental conditions high concentration of monocyte pyrogen could be obtained. In above described method used two pyrogen peak whose specific activity was 0.5—3.0ug, caused fever up to 0.6℃ in rabbits. In addition, isoelectric focusing column employed indicated isoeletric points of specific activity protein that induced fever were PI 5.1 and 3.6 respectively.
3.The clinical application of nonintubated video assisted thoracic surgery

Video assisted thoracic surgery(VATS) was rapidly developed in recent years.It's been widely accepted because it was minimally invasive and the patients were fast recovery with a survival rate similar to routine thoracotomy patients.However,more and more attention was paid to the side effect of general anesthesia with double cavity intubation during the surgery.Nonintubated thoracoscopic surgery for lung cancer was progressively developed.This surgery method decreases the side effect compared to general anesthesia with double cavity intubation.However,more surgical difficulty,additional surgical risk and prolonged surgery time were all inevitable become disadvantages of this new method.The less side effect of general anesthesia for patients and the more challenge for thoracic surgeon was brought simultaneous by this new method.More clinic trial should be implemented in validating the advantages and disadvantages.
4.Effects of IDH1 and IDH2 genes mutations on tumors

Isocitrate dehydrogenases(IDHs)are considered as key enzymes in the tricarboxylic acid cycle. Recurrent mutations in the IDH1 and IDH2 genes are recently found in several human cancers. Those point mutations specifically affect IDH1 and IDH2 active site arginine residues and confer a neomorphic enzyme function of directly catalyzing α-ketoglutarate(α-KG)to R-2-hydroxyglutarate(R-2-HG). R-2-HG can com-petitively inhibits α-KG-dependent enzymes and may therefore contribute to the occurrence and development of tumor. In addition,Mutation status of IDH1 and IDH2 are closely relative to the progress and prognosis of cer-tain tumor. Thus IDH1 and IDH2 are considered to be promising biomarkers for early diagnosis and prognosis and targeted therapy.
5.The cost-benefit analysis of magnetic resonance cholangiopancreatography before laparoscopic cholecystectomy in cholecystolithiasis patients unsuspected choledocholithiasis

Objective To study the cost-benefit analysis of magnetic resonance cholangiopancreatography (MRCP) before laparoscopic cholecystectomy (LC) in cholecystolithiasis patients unsuspected choledocholithiasis.Methods The clinical data of 960 cholecystolithiasis patients unsuspected choledocholithiasis were retrospectively analyzed.The patients were divided into MRCP group (665 cases) and non-MRCP group (295 cases) according to the application of MRCP or not before LC.The total hospitalization expense,missing diagnosis rate of postoperative choledocholithiasis,secondary admission rate,hospitalization time and intraoperative bile duct injury between the 2 groups were compared.Results There were no statistical differences in total hospitalization expense and hospitalization time between the 2 groups (P > 0.05).The missing diagnosis rate of postoperative choledocholithiasis and secondary admission rate in MRCP group were significantly lower than those in non-MRCP group [0.15% (1/665) and 0 vs.3.73% (11/295) and 3.73% (11/295)],there were statistical differences (P < 0.05).There was no intraoperative biliary tract injury in the 2 groups.Conclusions In terms of cost-benefit analysis,routine MRCP before LC for cholecystolithiasis patients unsuspected choledocholithiasis is benefiting.Routine MRCP before LC could effectively reduce the missing diagnosis of the postoperative choledocholithiasis and the secondary admission rate without increasing the total hospitalization expense of patients.
6.Early diagnosis and internal fixation of Lisfranc injury

Objective To explore the main points of early diagnosis,the principles and the efficacy of surgical treatment of Lisfranc injury.Methods The clinical data and follow-up results of 23 cases of patients with Lisfranc injury treated from April 2004 to February 2011 (17 males,6 females,aged from 19 to 57 years old) were collected and analyzed.All patients underwent open reduction and internal fixation,among them,13cases with Kirschner wire fixation and 10 cases with screws fixation.Efficacy was evaluated in accordance with Maryland foot score.Results Twenty-one patients were followed up for 6 - 46 months with an average of (28.5 ± 2.6) months.In the last follow-up no patients has internal fixation fracture,loss of reduction and other complications.The score was 56 - 97 points with an average score of ( 86 ± 8 ) points.The results showed excellent in 6 cases,good in 10 cases and fair in 3 cases,good rate was 76.2%.Conclusion Lisfranc injury can be diagnosed by early signs and symptoms,and can be confirmed by combining with radiographic examination.Lisfranc injury should be treated with open reduction and internal fixation with Kirschner wire or screw,both of the two fixation methods can achieve good results.
7.Modified Weaver-Dunn method and comparative study of AO clavicular hook plate for acromioclavicular joint dislocatio

Objective Comparison of the modified Weaver-Dunn method with AO clavicular hook plate for acromioclavicular joint dislocation efficacy.Methods Surgical treatment of Rockwood Ⅲ to the V Type acromioelavicular joint dislocation 35 cases,depending on the surgical approach,were divided into 2 groups of the modified Weaver-Dunn method and AO clavicular hook plate.Operation time,blood loss,hospitalization costs,postoperative Xray film results,the clinical efficacy results and complications were compared between two groups.Results Modified Weaver-Dunn method Section of the longer operative time,mean intraoperative blood loss more.Shoulder pain,the incidence of postoperative modified Weaver-Dunn method group is significantly lower than the AO clavicular hook plate group,abduction activity was better than AO clavicular hook plate group.AO clavicular hook plate group prone to shoulder mobility and shoulder pain.Postoperative Karlsson assessment standard,modified Weaver-Dunn method were excellent in 10,good in 4 cases and poor in 1,excellent rate was 93% ;AO clavicular hook plate group were excellent in 8 cases,good in 7 cases and poor in 5 cases,excellent rate of 75 %.Conclusion Modified Weaver-Dunn method is used to treat complete acromioclavicular joint dislocation.Postoperative shoulder pain and shoulder mobility are in low incidence of shoulder function recovery effect.

9.Clinical Observation on Operation for Glaucoma Secondary to Senile Intumescent Cataract

Objective To study the effects of cataract extraction (ECCE) and posterior chamber intraocular lens implantation( PC-IOL) or with antiglaucoma operation in secondary glaucoma caused by senile intumescent cataract.Methods To operation of ECCE and PC-IOL、or with antiglaucoma operation was performed in 26 cases(26eyes) with acute secondary glaucomacaused by senile intumescent cataract. Follow up 3~12 months after operation. Results The corrected visualacuity of 17eyes (65.4%) was up to 0.5, 22eyes had normal IOP postoperatively.Conclusions The clinical results showed that operation of ECCE and PC-IOL or with antiglaucoma operation is very good.
10.Effect of Bupleurum Chinense injection on endogenous pyrogen fever and change of cAMP concentration in cerebrospinal fluid in rabbits

The experiments were divided into two parts. The first part: inhibitory effect of Bupleurum Chinense injection on fever was observed. The second part: while Bupleurum Chinense injection inhibited fever, the cAMP concentration in cerebrospinal fluid was observed. The experimental fever models were induced by endogenous pyrogen from monocytes of rabbits' blood. The results obtained demonstrated that the Bupleurum Chinense injection (250mg/kg) had marked inhibitory effect on the endogenous pyrogen fever and the cerebrospinal fluid cAMP levels of the experimental group was obviously lower than in control. The authors inferred that inhibitory effect of Bupleurum Chinense injection on endogenous pyrogen fever may be obtained through some-link to reduced central cAMP content.
