Objective To observe clinical effect of alprostadil on DB artery of lower extremity scleratheroma block system patients.Methods To retrospectively analyze clinical data of lower extremity scleratheroma block system patients in our hospital,which were divided into detection group and control group.Results The bore 、blood flow and Vdias of femoral artery、popliteal artery、arteria tibialis posterior and dorsal artery of foot of detection group were better than control group,the clinical total effective rate of detection group was higher than control group (P ＜ 0.05 ),the difference was statistically significant.Conclusion The clinical curative effect of DB artery of lower extremity scleratheroma block system by alprostadil was obviously,which was worthy to be used.

Objective To investigate the mRNA espression level of Caspase-1 and Caspase-3 in the patients with systemic lupus erythematosus(SLE) and its association with lymphocyte apoptosis. Methods Reverse transcription-polymerase reaction (RT-PCR) technique was applied to semiquantitatively analyze the mRNA expression of Caspase-1 and Caspase-3 in the peripheral blood mononuclear cells (PBMC) from 34 active SLE patients and 30 healthy subjects. Results Compared with normal controls, the mean levels of Caspase-1 or Caspase-3 mRNA expression in active SLE group was significantly increased(P0.05). Conclusion There are increased expressin of Caspase-1 and Caspase-3 in the PBMC of SLE patients, which may be involved in the lymphocyte apoptosis of SLE patients.

Objective To explore the clinical effect of laparoscopy combined with choledochoscopy on choledocholithiasis.Methods Totally 134 elderly patients with choledocholithiasis were treated in our hospital from Jan 2013 to Dec 2014, who were randomly divided into observation group and control group (n=67 for each), treated with laparoscopy combined with choledochoscopy, and traditional surgery, respectively.The operation time, bleeding volume, exhaust time, in-hospital stay, complications and residual stones rate were compared between the two groups.Results The operation time was higher in observation group than in control group [(124.6±21.2) min vs.(94.7± 17.9) min, t=8.821, P＜0.001].The bleeding volume were less in observation group than in control group[(43.8±10.4) ml vs.(113.5±37.6) ml, t=14.624, P＜0.001].The exhaust time and in hospital time were decreased in observation group than in control group[(27.6 ±5.5) h vs.(43.4±8.1) h, (7.4±2.4) d vs.(10.3±2.8) d, t=13.209 and 6.437, P＜0.001 for both].The incidences of postoperative pain and other complications were lower in observation group than in control group [6.0％ vs.28.4％, 16.4％ vs.43.3％, x2=11.810and 11.547, P=0.001 for all].Conclusions The laparoscopy combined with choledochoscopy has advantages to minimize the surgical injury, reduce the bleeding volume and promote the postoperative recovery in treating choledocholithiasis in elderly patients.

Objective To investigate the effect of methylprednisolone pretreatment on cardiopulmonary bypass(CPB)-induced intestinal barrier injury in patients undergoing cardiac surgery.Methods Ninety NYHA Ⅰor Ⅱ patients,aged 30-50 yr,weighing 50-75 kg,scheduled for elective cardiac surgery with CPB,were randomly divided into 3 gnoups(n =30 each):control group without CPB(group Ⅰ),control group with CPB(group Ⅱ)and administration of methylprednisolone before CPB group(group Ⅲ).Anesthesia was induced with midszolam,fentanyl,etomidate and rocuronium and maintained with intravenous infusion of propofol and intermittent iv boluses of fentanyl and rocuronium.The patients were mechanically ventilated after tracheal intubation.In group Ⅲ,methylprednisolone 10 mg/kg was injected intravenously before operation and CPB.While in groups Ⅰ and Ⅱ,the equal volume of normal saline was injected instead.The blood samples were taken from the central vein before induetion of anesthesia(T1),before CPB(T2),at 30 min after the beginning of CPB(T3),at 30 rin afier the end of CPB(T4)and at 120 min after operation(T5)for determination of the plasma endotoxin concentration.Infection was recorded within 7 days after operation.Results The plasma endotoxin concentrations at T1 were within the normal range in all groups,without significant difference among the three gnoups(P ＞0.05).The plasma endotoxin concentration at T3-5 and incidence of postoperative infection in group Ⅲ were significantly lower than those in group Ⅱ,while higher than those in group Ⅰ(P ＜ 0.05).Conclusion Methylprednisolone pretreatment can reduce CPB-induced impairment of the intestinal harrier function in patients undergoing cardiac surgery.

Objective To observe the effects of tetramethylpyrazine on intraoperative autotransfusion of blood quality , analysis of ligustrazine in autotransfusion of clinical value of blood quality control.Methods 68 patients undergoing elective surgery in our hospital from February 2015 to May 2016 were selected as the research object,and were randomly divided into control group and experimental group,each group of 34 cases.Two groups of patients in the intraoperative autologous blood transfusion and the control group according to conventional autologous transfusion in operation ,test group of patients with intravenous injection of ligustrazine injection in the recovery of blood before adding ligustrazine injection in the recovery process of blood.The two groups were collected before blood transfusion,observe the morphology of red blood cells, red blood cell fragments under the microscope and compared;the two groups were collected after reinfusion of blood two hours,one and five days in peripheral venous blood, the determination of superoxide dismutase (SOD), malondialdehyde (MDA), T lymphocyte subsets level of two groups were compared before and after one and five days. Results The erythrocyte deformability and erythrocyte debris of the two groups were higher than those before operation (P<0.05).The blood red blood cell deformability and the number of red blood cell debris in the blood transfusion group were significantly higher than those in the control group Significantly lower than the control group (P<0.05).The levels of SOD were significantly lower at two hours and one day after operation than those before operation, and the serum SOD was still lower than that before operation in the control group,The SOD in the experimental group was significantly higher at two hours, one and five days in the control group,MDA was lower than the control group(P<0.05).The CD3 +,CD4 +, CD4 +/CD8 +levels were significantly lower at two hours and one day after operation than those before operation (P<0.05).CD3 +,CD4 +and CD4 +/CD8 +in the control group were still lower than those in the control group at five days after operation(P<0.05),the levels of CD3 +, CD4 +, CD4 +/CD8 +were significantly higher in the experimental group than those in the control group at two hours,one and five days after treatmen (P <0.05).There was no significant difference between the two groups in the incidence of adverse reactions.Conclusion The addition of preoperative intravenous injection of ligustrazine,blood recovery process,can effectively protect the transfusion of red blood cell integrity,reduce the effect of transfusion of blood and blood antioxidant capacity in patients with T lymphocyte immune function ,to improve the quality of blood transfusion patients has important clinical value .

Objective To investigate the role of IL-6 and IL-6R in the pathogenesis of systemic lupus erythematosus (SLE) and its clinical significance. Methods RT-PCR technique was used to semiquantitatively analyze IL-6R mRNA expression in peripheral blood mononuclear cells (PBMC), and double antibody sandwich ELISA was applied to determine the serum level of IL-6 from 89 SLE patients . Results The positive IL-6R mRNA expression was found in each individuals among active SLE group (groupⅠ), inactive SLE group (groupⅡ) and control group (group Ⅲ). The mean levels of IL-6R mRNA expressions in groupⅠ,Ⅱ or Ⅲ were 0.902 ? 0.273, 0.519?0.11 and 0.573?0.24, respectively. Compared with group Ⅱand group Ⅲ, the mean level of IL-6R mRNA expression in groupⅠ was markedly increased (P = 0.00), while there was no statistical difference between group Ⅱ and group Ⅲ(P = 0.289). The mean serum levels of IL-6 in groupⅠ,Ⅱ and Ⅲ were 71.89 ? 20.02,17.96 ? 6.93, and 0.035 ? 0.035, respectively. Compared with group Ⅱand group Ⅲ, the mean level of IL-6 in groupⅠ was significantly higher than those in groupⅡ and groupⅢ (P = 0.00), while that in groupⅡwas higher than group Ⅲ (P = 0.00). Objective To detect p16 gene DNA methylation in the plasma of patients with systemic lupus erythematosus (SLE) and its significance in the disease activity and clinical manifestations. Methods Forty-five cases of SLE and 20 healthy controls were enrolled. Promoter methylation in p16 gene was measured by methylation specific PCR (MSP) in patients' plasma, and the correlation between the methylation status and clinical manifestations and routine laboratory findings were analyzed.
