1.Experience of Consumptive Disease Treating Analysis of the Gushi Yijing

[Objective]To explore the consumptive disease treating experience in Gushi Yijing, written by Gu Songyuan who lived in the Qing Dynasty. [Methods]By studying consumptive disease related content in the book, discussing and summing up the consumptive disease treating experience. [Results] On the basis of pointing out seven kinds of error method that doctor treating consumptive disease at that time, Gu Songyuan put forward the opinion that consumptive disease was usual y caused by the internal injury of Zang and Fu organs which resulted into Yin deficiency as the main pathogenesis. The article contained other details as fol ows: the basic symptoms of consumptive disease of the five internal organs, three main therapeutic methods on treatment of consumptive disease, the preference of nourishment combined drugs with food, habit of using some sweet and cold durgs which can clear heat, the use of flesh and blood of sentient beings and so on. So the key idea of this article focused on the method of nourishing Yin to enrich Jing and blood which could cure the consumptive disease effectively.[Conclusion]Gu Songyuan inherited predecessor ’s theory, analysed the etiology of consumptive disease of the five internal organs, pointed out Yin deficiency is the main pathogenesis, and then put forward the therapeutic method, which has some medical knowledge on modern clinic.
2.Activation of Wnt signaling pathway in the neuronal differentiation of bone marrow mesenchymal stem cells:a microarray analysis

BACKGROUND:It wil provide a new insight into the future application of bone marrow mesenchymal stem cels in the treatment of spinal cord injury and tissue engineering by studying the effect of activation of Wnt signaling pathway in the neuronal differentiation of bone marrow mesenchymal stem cels.
OBJECTIVE: To detect the expression of related genes by gene chip technology during the neuronal differentiation of bone marrow mesenchymal stem cels.
METHODS:Human bone marrow mesenchymal stem cels were isolated and purified, and passage 5 cels were obtained. GatewayTM technology was used to build lentiviral vectors that was used to transfect Wnt-1 into human bone marrow mesenchymal stem cels. Control, non-transduction and transduction groups were set in this study. Human bone marrow mesenchymal stem cels were then induced to differentiate into neurons. Cel morphology was observed under inverted phase contrast microscope. Gene chip was used to detect the regulation changes and the differential expression of related genes in the Wnt signaling pathway.
RESULTS AND CONCLUSION: Under the scanning electron microscope, the transfected cels were found to have the similar morphology of neuron-like cels. Analysis by the gene chip hybridization technique showed that 3 287 genes were up-regulated and 4 215 genes were down-regulated in the signal pathway. In the Wnt signaling pathway, genes related to the nervous system development and differentiation were up- or down-regulated. It is verified that the Wnt signal pathway is activated via Wnt-1 transduction, and the downstream genes appear to have genetic transcription so as to promote the neuronal differentiation of human bone marrow mesenchymal stem cels.
3.The diagnostic accuracy of CT angiography for intracranial aneurysms in patients with subarachnoid hemorrhage:a Meta-analysis

Objective To evaluate the value of CT Angiography (CTA)in the diagnosis of cerebral aneurysms in patients with a-cute subarachnoid hemorrhage (SAH )with Meta analysis.Methods A systematic search in PubMed,MEDLINE,EMBASE, CNKI,CBM,WANFANG databases was performed.Criteria for inclusion were established based on validity criteria for diagnostic research published by the Cochrane Methods Group on Screening and Diagnostic.The methodological quality of each study was eval-uated by QUADAS.Meta-Disc Version 1.4 software were used for heterogeneity test and the pooled weighted sensitivity,specifici-ty,positive likelihood ratio,negative likelihood ratio and 95% confidence intervals were calculated,receiver operating characteristic (SROC)and area under the curve(AUC)were obtained with random-effects models.Stata12.0 was used for the funnel plot to ana-lyze publication bias.Results Fourteen studies were included with a total of 1 332 patients and 1 149 ruptured cerebral aneurysms. The pooled weighted sensitivity and specificity of CTA with 95% confidence interval was 0.96(0.95-0.97)and 0.99(0.97-1.00) respectively.The AUC and the Q value of SROC was 0.99 and 0.958 4 respectively.Conclusion CTA has high sensitivity and spe-cificity in the diagnosis of cerebral aneurysms in patients with SAH,can be proposed as an important imaging diagnostic method.
4.Simple bracket assist closed reduction percutaneous treatment of children's supracondylar fracture

Objective Describes the use of homemade simple bedside bracket of children supracondylar fracture closed reduction and percutaneous pin fixed surgical methods and clinical effects.Methods 16 cases of children supracondylar fractures Gartland type Ⅱ cases,15 cases Gartland type Ⅲ,13 males and 3 females,with an average age of 6 years old,all patients used our hospital homemade simple bedside shelf assisted closed reduction,percutaneous pin fixation,and fixed with plaster immobilization.Results 16 patients were followed-up,in addition to the two cases of pin tract infection,fracture all were healed smoothly.Without iatrogenic nerve injury and internal fixation loosening,and also no volkmann contracture,and myositis ossificans etc complications.The average fracture healing time for three weeks,no bone delayed union or nonunion occurred.According to Flynn clinical function evaluation,there were excellent in 7 cases,good in 9 cases.Conclusion Useing homemade simple bedside shelf to children supracondylar fractures closed reduction and percutaneous Kirschner wire fixed can ensure the efficacy,and also simplify the surgical procedure,reduce radiation intake,which is the worthy of clinical practice.
5.Value of HASTE MR Cholangiopancreatography in the Diagnosis of the Biliary Obstructive Diseases

Objective The aim of this study was to evaluate magnetic resonance cholangiopancreatography(MRCP)using half-Fourier acquisition single-shot fast spin-echo(HASTE)sequence in the diagnosis of biliary obstructions.Methods Forty-five patients with pancreaticobiliary duct diseases underwent MRCP on 1.5 T scanner,a heavily T 2-weighted HASTE was used during a breath-hold.The source images were three-dimensional reconstructed postprocessed on workstation.The findings of MRCP images were analyzed and compared with US,CT,ERCP or PTC.Results The diagnostic accuracy of MRCP was 94%,the same as ERCP(92%),but it was superior to US and CT(?

Luminal surface of vascular endothelium is decorated with a variety of polysaccharide-protein complexes,which constitute the glycocalyx.It has been demonstrated that vascular endothelial glycocalyx plays an important role in modulation of selective permeability of vessels,mediation of the blood cell-endothelial cell interactions and the release of nitric oxide induced by fluid shear stress under physiological condition.In inflammation condition,sheding of glycocalyx due to inflammation mediator leads to its functional weakening in vessel protection.At the same time,heparan sulfate as a major constituent of vascular endothelial glycocalyx could be involved in regulating the evolution of inflammation.Heparan sulfate interacts with L-selectin to mediating leukocyte rolling,presents chemokines on luminal surfaces of endothelial cells to mediate leukocyte crawling and firm adhesion,participates in transcytosis of chemokines from tissue to luminal side of endothelial cells during inflammation.Various risk factors of atherosclerosis,as an inflammatory disease,are closely associated with vascular endothelial glycocalyx.This paper is aimed to review the role of vascular endothelial glycocalyx in inflammation and atherosclerosis.
7.Dissecting the retrohepatic inferior vena cava in right hepatectomy: an experience in 77 patients

Objective To study the technique in isolating the retrohepatic inferior vena cava in right hepatectomy.Methods Anatomical exposure of the right lateral wall of retrohepatic inferior vena cava was performed,followed by isolating its back wall.Results Part of the right liver was rotated out of the incision.The 77 patients recovered smoothly.Conclusion Dividing the right adrenal is the key to free the retrohepatic inferior vena cava.To expose the right side of the retrohepatic inferior vena cava is a standard technique to mobilize the right liver.
8.Hepatocellular carcinoma complicated by main portal vein tumor thrombus: treated by portal vein stenting,tansarterial chemoembolization and 3-dimensional conformal radiotherapy

Objective To retrospectively analyze the role of 3-dimensioual conformal radiotherapy (3-DCRT) after percutaneous transhepatic portal vein stenting and transcatheter arterial chemoembolization (PTPVS-TACE) in the treatment of patients with hepatocellular carcinoma (HCC) complicated by main portal vein tumor throw.bus (MPVTT).Methods Between July 2002 and July 2007,45 patients with HCC complicated by MPVTT were treated by PTPVS-TACE.Among them,3-DCRT were undertaken for MPVTT in 16 patients (group A),the other 29 patients were not treated with 3-DCRT(group B).The clinical effects,complication,stent patency rates,cumulative survival rates were evaluated among groups.The Kaplan-Meier method and log-rank test were used for survival analysis.Results No patient died during stent placement or within the preceding first 24 hours.No severe procedure-related complications were observed.The 60--,180--,360--day cumulative stent patency rates were 100.0%,62.2% and 34.6% in group A,and 58.6%,21.7% and 10.8% in group B,respectively,showing significant difference between the two groups (X2 =9.672,P <0.01).The mean patency time was(475±137) and (200±61)days,respectively.The 60--,180--,and 360--day cumulative survival rates were 93.8%,81.3% and 32.5% for group A,86.2%,13.8% and 6.9% for group B,respectively.There were significant statistical differences between the two groups(X2=9.672,14.596,P < 0.01).Conclusion Treatment with PTPVS-TACE-3-DCRT is a more effective modality than PTPVS-TACE for HCC complicated by MPVTT.
9.Efficacy evaluation of heat-sensitive moxibustion for chemotherapy symptoms of large intestine cancer.

OBJECTIVETo analyze and evaluate the clinical efficacy of heat-sensitive moxibustion for symptoms of large intestine cancer.
METHODSSixty patients with large intestine cancer were randomly divided into an observation group and a control group, 30 cases in each one. FOLFOX chemotherapy regimen was used in the two groups,and heat-sensitive moxibustion was added in the observation group. The acupoints were Zusanli(ST 36), Sanyinjiao (SP 6) Xuehai (SP 10) and Geshu (BL 17), etc. The treatment was applied once a day,five-day treatment as one course. Four courses were required. The reaction rates of uncomfortable symptoms by the Chinese version of the M. D. Anderson symptom inventory (MDASI-C) scale and clinical effects were analyzed and evaluated in the two groups.
RESULTSAfter treatment, the MDASI-C reaction rate of uncomfortable symptoms in the observation group was 50.4% which was lower than 53.3% in the control group (P < 0.05). The total effective rate of symptom improvement in the observation group was 83.3% (25/30), which was higher than 60.0% (18/30) in the control group (P < 0.05).
CONCLUSIONHeat-sensitive moxibustion can improve symptoms of chemotherapy for large intestine cancer.

10.A validated UPLC–MS/MS method for simultaneous determination of imatinib,dasatinib and nilotinib in human plasma

A sensitive, rapid, simple and economical ultra-performance liquid chromatography–tandem mass spectro-metric method (UPLC–MS/MS) was developed and validated for simultaneous determination of imatinib, dasatinib and nilotinib in human plasma using gliquidone as internal standard (IS). Liquid-liquid extraction method with ethyl acetate was used for sample pre-treatment. The separation was performed on an Xtimate Phenyl column using isocratic mobile phase consisting of A (aqueous phase: 0.15% formic acid and 0.05% ammonium acetate)and B(organic phase:acetonitrile)(A:B=40:60,v/v).The flow rate was 0.25 mL/min and the total run time was 6 min. The multiple reaction monitoring (MRM) transitions, m/z 494.5→394.5 for imatinib, 488.7→401.5 for dasatinib, 530.7→289.5 for nilotinib and 528.5→403.4 for IS, were chosen to achieve high selectivity in the simultaneous analyses. The method exhibited great improvement in sensitivity and good linearity over the concentration range of 2.6–5250.0 ng/mL for imatinib, 2.0–490.0 ng/mL for dasatinib,and 2.4–4700.0 ng/mL for nilotinib.The method showed acceptable results on sensitivity,specificity, recovery, precision, accuracy and stability tests. This UPLC–MS/MS assay was successfully used for human plasma samples analysis and no significant differences were found in imatinib steady-state trough concentra-tions among the SLC22A5?1889T>C or SLCO1B3 699G>A genotypes(P>0.05).This validated method can provide support for clinical therapeutic drug monitoring and pharmacokinetic investigations of these three tyrosine kinase inhibitors(TKIs).
