1.Effect of low stress environment on repairing of children necrotic femoral head

[Objective]To observe the change of children necrotic femoral head repairing under low stress environment and to explore the effect of low stress environment on Perthes disease.[Method]The sick coxa was supported by Coxa Multi-functional External Fixation Shoring Fixator,which decreased femoral head stress and creates a low-stress environment for the sick coxa.Meanwhile,the affected extremity was kept abduction and internal rotation in order to increase femoral head containment.The coxa could flex and extend partly on sagittal plane.Twenty-eight patients(32 hips) were followed up with mean follow-up time of 3 years and 5 months(2~7 years).The radiological characteristics of pre-and post-operation was analyzed.[Result]At 3 months postoperatively,joint space was broadened,high density necrosis shadow became little and little,the femoral head became full,height of the femoral head epiphysis increased.If epiphyseal nucleus fragmentation vaded epiphyseal plate,the big head and pachy-neck and the subluxation of coxa would finally result.[Conclusion]Under low stress environment,child necrotic femoral head could be repaired.The prognosis is related to Catterall stage of Perthes disease.On Catterall Ⅱ and Ⅲ stage,the curative effect is fairly good.But on Catterall Ⅳ stage,the curative effect is not evident.
2.New evidences of α7nAChR participating in 100 Hz EA regulating NK cells activity

Objective On the basis of previous experiments ,we try further to seek the new evidences of α7nAChR participating in 100 Hz electroacupuncture (EA ) regulating natural killer (NK ) cells activity .Methods α7nAChR antibody and the specificα7nAChR antagonist α-bungarotoxin (α-Bgt) were used to study the effect of α7nAChR on NK cell activity in 100 Hz EA mice .Re-sults Compared with 100 Hz EA group ,100 Hz EA plus α7nAChR-Ab group showed a significant enhanced in NK cell activity (P<0 .05) .100 Hz EA treated with 1 .5 or 3 .0 μg α-Bgt groups had higher NK cell activity than 100 Hz EA group(P<0 .01) . Conclusion α7nAChR is involved in 100 Hz EA modulation for immune function ,which provide the theoretical and practical basis for further clarifying and expanding the clinical application of EA .
3.Experimental Study in Pigs Using the Home-Made Stent(Shape Memory Alloy)

Purpose:Home-made NiTi-stent was used in the study to observe the biological compatibility.Materials card Methods:Stents were implanted within the bile ducts,arteries,veins,bronchi of swine by surgery.Swine was killed according to the tie schedule to observe the patency of stents.Results:6 of 11 biliary stents were occluded completely,with only 5 stents partially patent.The degree of occlusion was related to the time period.10 of 11 femoral stents were patent,only 1 stent was occluded completely.1 of the only 1 venous stent was occluded completely,4 of 8 bronchial stants were patent.Epithlium tissues were found along the surfaces of stents.Both sides of the stents were covered by the epithelium cells partially and dominated by the proliferation of connective tissues and parenchymal cells of the organs.The constitution of proliferated tissues was related to the implanted time period of the stents.No connective tissues were found within two weeks,but obvious proliferation of connective tissues were found associated with lympheytic tissues.Conclusion:The dagree of surface covering of the home- made stent by epithelial the rate of was related to the diameters of the of and the lumceh stent implantation segment also the flow volume within the stent,and the period of stent implantation.Good results can be yielded by selecting the suitable stent and the implanting site.
4.Application of bilingual case discussion in clinical teaching of orthopedic trauma

The practice of daily case discussion in the morning using slice show has been cartied out since orthopedic trauma department of Nanfang Hospital was set up.Bilingual ( Chinese and Englisl) discussion was hold in the department before and after the operation on the cases requiring three-level or higher level operation and those of special diseases.The bilingual case discussions will greatly improve the clinical thinking and accumulate the clinical experiences for doctors.The professional English proficiency of all doctors in the department have been improved because of the bilingual case discussions and the most effective treatment for patients was provided at the same time.

Objective To investigate the cardioprotective effects of the inhibitor of JAK/STAT pathway on ischemia-reperfusion injury in cultured immature rat cardiacmyocytes.Methods The cultured immature rat cardiacmyocytes were randomly divided into normal control group,ischemia-reperfusion group,AG490 treatment group and RMP treatment group.The cell viability was measured by MTT method.The contents of lactate dehydrogenase(LDH) and malondialdehyde(MDA),the superoxide dismutase(SOD) activity and the rate of cell apoptosis were detected.Results Compared with the normal group,the cell viability decreased significantly,the contents of LDH and MDA in the supernatant of the cells increased significantly,the SOD activity decreased and the rate of apoptosis of cells increased significantly in ischemia-reperfusion group(all P
6.Monitoring and nursing of gestational diabetes patients treated with continuously subcutaneous insulin infusion

Objective To study the effects of the intensive treatment with continuously subcutaneous in-sulin infusion(CSII) in patients with gestational diabetes and the nursing during the treatment. Methods From Jan 2007 to Aug 2008, 42 patients with gestational diabetes underwent continuously subcutaneous insulin infu-sion, and a standard nursing plan of patients was made up before treatment. The efficacy of treatment and nursing were analyzed. Results After using the CSII, the blood glucose of all patients was controlled at normal level after 7.0±1.5d. 88.1% of patients were satisfied with the effect and one patient was dissatisfied with the treatment because of losing the fetus for sever bleeding caused by placenta previa. 40.5% of patients with different degrees of anxiety accepted CSII therapy after psychological care by nurses;In 9.5% of patients the infusion bump a-larmed because of needle blockage or infusion tube bending, it restarted working and ran normally once the needle replaced or bending tube unwinded. 7.1% of patients developed hypoglycemia during CSII and recovered after eating. Conclusions Intensive treatment with CSII in gestational diabetes patients can effectively control blood glucose level The effect can be improved by taking specific nursing measures.
7.Exploration on technical training courses of diabetic foot among district medical clinical staffs

Objective To survey the clinical ability of diagnosis and treatment of diabetic foot among district medical clinical staffs.Methods 55 medical staffs were questionnaired about the diabetic foot knowledge before and after a technical training course.Results (1) After training,the clinical ability of diabetes physical examination,the features of diabetic foot identification,diabetic foot prevention and treatment,diabetic foot health education and the muhidisciplinary team of diabetic foot organization improved.The medical staffs got to know how to organize a multidisciplinary team of diabetic foot.The difference was significant(P<0.05).(2)The professional category of medical personnel(OR=19.464,95% CI=1.837-206.243),the history collection and physical examination of diabetic foot (OR=34.825,95% CI=3.486-347.875)were the influent factors of antibiotic selection when diabetic foot were infected.Conclusion Targeted diabetic foot training is effective to improve the clinical diagnosis and treatment level of district medical staffs.
8.Anatomical basis of the flap based on the perforator of the first plantar metatarsal artery .

OBJECTIVETo investigate the morphological features of the perforator from the first plantar metatarsal artery, so as to provide anatomic basis for the reconstruction of soft-tissue defects of the forefoot.
METHODSThe first metatarsophalangeal joint was chosen as the landmark on 30 human cadaveric feet prefused with red latex. The following contents were observed under surgical magnifier: (1)The origin, courses,branches,distribution of the perforator of the first plantar metatarsal artery; (2)The anastomoses among the perforator of the first plantar metatarsal artery and other arteries on the medial aspect of the foot. Simulated operation was performed on one fresh specimen.
RESULTSThe perforator of the first plantar metatarsal artery passed through the space between the tendon, the abductor hallucis and the first metatarsal bone, and its entry point into the deep fascia was located (2. 3 ± 0.7 ) cm proximal to the first metatarsophalangeal joint. The perforator anastomosed with either the medial tarsal artery, the medial anterior malleolus artery or the branch of the medial plantar artery on the superior margin of the abductor hallucis, forming a longitudinal arterial chain,through which small branches were given off to the skin of the medial aspect of the foot. The perforator was( 1. 1 ± 0.2) mm in diameter and(3.2 ± 0.2) cm in length.
CONCLUSIONThe flap based on the perforator of the first plantar metatarsal artery can be harvested as an axial flap to repair the defects of soft tissue on the forefoot.

Objective To explore the molecular mechanism of BRAFV600E inducing chromosome instability in Sbcl2 and SK-MEL31 melanoma cells.Methods The endogenous Mps1 in stable Sbcl2-and SK-MEL31-B-RafV600E expression cells were depleted by siRNA approach.To test the effect of B-RafV600E on the centrosome amplification and the formation of multipolar spindles,cells at S-phase with HU-treatment were arrested and then the centrosomes and mitotic spindles structure were detected through immunofluoresence.Results The percentage of B-RafV600E expressing Sbcl2 and SK-MEL31 cells (Sbcl2-B-RafV600E and SKMEL31-B-RafV600E) with centrosome amplification and multipolar spindle was reduced from 36 % to 6 % when Mps1 was absent.Conclusion B-RafV600E leads to centrosome amplification and multipolar spindle through Mps1,thus results in chromosome instability in Sbcl2 and SK-MEL31 melanoma cells.
10.Advances in research on adaptive radiotherapy for nasopharyn-geal carcinoma

Intensity-modulated radiotherapy (IMRT) is gradually replacing conventional radiation therapy and has become the mainstream radical treatment for patients with nasopharyngeal carcinoma (NPC). IMRT can conform and increase radiation doses to tu-mor-associated regions as well as decrease exposure doses and volumes on normal organs and tissues to avoid damage on critical or-gans. Aside from system and setup errors, other factors, such as the gradual reduction of the primary NPC lesion and the decrease in vol-ume of involved neck lymph nodes as well as body weight loss and changes in the head and neck shape during IMRT, may cause devia-tions in the radiation doses and volume delivered to the NPC targets and the organs at risk. These factors may affect the accuracy of IMRT. Several researchers have attempted to correct such deviations during IMRT for NPC patients by using adaptive radiotherapy (ART). The results indicate that ART is feasible to a certain degree and can correct the deviations, including decrease in tumor volume, body weight loss, and changes in head and neck shape of NPC patients.
