The authors discuss the technical problems in clinical evaluation of newly-developed Chinese medicines.For improvement of evaluation quality,this paper puts forward some suggestions for solving the common problems such as unitary intervention pattern,lack of corresponding treatment for the changed syndrome,subjectivity in standards of syndrome-differentiated diagnosis,ill-chosen medicine in control group,and mismanagement of medicine combination.

Objective To investigate the myocardial protective effect of isehemic post-conditioning in total repair for tetralogy of fallot,in order to improve short-term prognosis of infant patients.Methods 64 cases of TOF infant patients were randomly divided into two groups,32 cases in the control group:conventional surgery without ischemic post-conditioning,and other 32 cases in the experimental group:given three cycles of ischemic post-conditioning.Then the clinical indieators:cardiopulmonary bypass time,aortic cross-clamping time,automatic re-jump rate,the latter parallel to the off-line time,the off-line blood pressure,the offline dopamine usage,extubation time,ICU stay time,postoperative complications,and the testing laboratory indicators:the cTnI and CK-MB levels in plasma after anesthesia induction,10min after aortic cross-clamping,10min after aortic opening,ICU1h,1d after surgery,and 2d after surgery were observed.Results Two patients were died and,mortality rate was 3.1%.The remaining patients were discharged from hospital.Laboratory indicators of the control group and the experimental group had significant differences (all P＜0.05),while the clinical indicators of the two groups had no significant differences(all P＞0.05).But for the sub-group which the aortic cross-clamping time were more than 60min,the clinical indicators were significantly different(all P＜0.05).Conclusion Ischemic post-conditioning could enhance myocardial protection in total repair for tetralogy,of fallot.for the cases aortic cross-clamping time was more than 60min,their clinical meanings were more obvious.

ObjectiveTo investigate the changes of serum apolipoprotein E (ApoE) in children with infectious diseases.MethodsA total of 279 pediatric patients with infectious diseases were enrolled in this study,including 65 patients with sepsis,47 patients with bacterial meningitis,67 patients with bacterial pneumonia, 47 patients with aseptic meningitis and 53 patients with mycoplasmapneumonia. TheserumApoEcollectedfromallpatientswasdetectedby immunoturbidimetric assay (IA).The septic mouse model was established by intraperitoneal injection of group B Salmonella typhimurium.Mouse serum ApoE levels were detected by IA,and the hepatic ApoE mRNA and protein expressions of mice were detected by quantitative real-time reverse transcription-polymerase chain reaction (RT-PCR) and Western blot,respectively.Data in two groups were compared by independent-sample t test.ResultsSerum ApoE levels in patients with bacterial infections were increased dramatically,which was (59.8±23.5) mg/L in patients with sepsis (t=-5.118,P＜0.01),while no significant differences were found in patients with aseptic meningitis and myeoplasma pneumonia.Moreover,a high level of serum ApoE was detected in septic mouse model,while the hepatic ApoE mRNA and protein expressions of the mice were both decreased,with mRNA decreased 71％ at 3 hour (t=5.022,P＜0.01) and 73％ at 24 hour (t=4.181,P＜0.01).Conclusions Serum ApoE levels in bacterial infections increase dramatically,while its hepatic expression in septic mouse model is decreased,which indicates that the elevated serum ApoE level is not related to the changes of hepatic ApoE expression.

Objective To investigate the correlation between corneal neovascularization (CNV) in rat after cautery with acid and the defect in epithelium, the infiltrating of leucocytes. Methods The model of mild, moderate and severe acid burn in rat cornea was established. Corneal angiogenesis and the diameter of the epithelial defect were observed with a slit lamp microscope. The ratio of neovascularization area to that of cornea was calculated. The specimens were taken to stain with hematoxylin and eosin (HE) at different time points after cautery and the number of the leucocytes in cornea was calculated with light microscope. The relation between CNV and the diameter of epithelial defect and the infiltrating leucocytes in cornea was analyzed with Pearson's correlation. Results The healing of epithelium differed with different degree of acid burn. The more damage the eye suffered, the more time it would take to heal. Leucocytes infiltrated in cornea after cautery. Objective: To study the nutritional status and problems of Chinese elite athletes for instructing training scientifically. Method: 599 elite athletes were investigated by means of dietary survey, physical measure and biochemical detection. Results: (1) The daily average energy intake reached the adequate intake (AI) recommended for athletes, but the energy ratio both of protein and fat higher, carbohydrate lower, being 18.9%, 38.6% and 42.5% respectively. Except vitamin A and B1, B2 the intakes of other vitamins and minerals were sufficient. (2) The average BMI and body fat percentage were 23.0?3.0 and (12.1?3.2)% in men and 21.9?3.0 and (20.5?3.9)% in women respectively. (3) The average hemoglobin levels were (145.7?13.3)g/L in men and (130.6?11.8)g/L in women. The rates of anemia and iron deficiency anemia were 12.6% and 5.2% respectively, female higher than male. (4)The hyperlipidemia rate was 22.3%, including 12.6% high TG, 9.7% high TC and 1.2% high LDL, female higher than male. (5) The rates of VB1 and VB2 insuficiency were 46.2% and 32.7% respectively, both including 9.6% deficiency. Conclusion: The nutritional status of Chinese elite athletes was good, but still with anemia, vitamin insufficiency and hyperlipidemia.

Objective To design the family support scale for adolescent patients with nephritic syndrome. Methods 192 adolescent patients with nephritic syndrome were selected in the pilot investigation, and then formed the first questionnaire. The reliability and the validity of the questionnaire were evaluated by the experts. Results The questionnaire involved 15 items and its reliability and the validity were satisfied. There was a positive correlation between the family support and the self-nursing degree. Conclusion The reliability and the validity of this questionnaire are both up to the demands, it can used to study the condition of family support for adolescent patients with nephrotic syndrome.

Objective The early causes of death were analyze in 2349 patients who had undergone heart valve replacement.Methods Methods From January 1995 to December 2007,2349 patients with heart valve diseases received heart valve replacement.1109 cases were male and 1240 were female.The mean age of the patients was(41±19)years old.1962 cases had rheumatic heart valve disease,308 had congenital heart valve disease,39 had infective endocarditis,29 underwent reintervention by heart valve replacement,11 had Marfan syndrome.34 cases with coronary heart disease underwent heart valve prosthesis implantation and coronary artery bypass grafting.Mitral valve replacement(MVR)was performed in 1333 patients,aortic valve replacement(AVR)in 271,double valves replacement(DVR)in 736 and tricuspid valve replacement(TVR)in 9.There were 3075 mechanical valves and 10 bioprosthetic valves.Results From 1995 to 1999,death occurred in 16 of the 235 cases,early mortality rate was 6.81%.From 2000 to 2004,death occurred in 35 of the 1087 cases,early mortality rate was 3.22%.From 2005 to 2007,there were 29 deaths among 1027 cases,with an early mortality rate of 2.82%.Overall early mortality rate was 3.40%.The early mortality rate was 2.32%(31 in 1333 cases)in patients who underwent MVR,3.32% (9 in 271)in patients who underwent AVR,5.24%(40 in 736)in patients who underwent DVR,5.50%(7 in 127)with LVEDD≥70 mm,4.60%(14 in 304)with LVEF<0.40,2.14%(9 in 419)with NYHA class II,2.42%(37 in 1529)with NYHA class Ⅲ,and 8.48%(34 in 401)with NYHA class IV.The causes of 80 deaths were low cardiac output syndrome in 31 cases(38.8%),renal failure in 14 cases(17.5%),arrhythmia in 10 cases(12.5%),pulmonary infections in 8 cases (10.0%).cerebrovascular accidentin 5(6.3%),left ventricular rupture in 5(6.3%),multisystem and organ failure in 5(6.3%),and other cause in 2 cases(2.5%).Conclusion The causes of early death after heart valve replacement are low cardiac output syndrome,renal failure,arrhythmia,pulmonary infection,cerebrovascular accident,left ventricular rupture and multisystem and organ failure.

To investigate the expression of the subunit p65 of NF-kappaB and inhibitor kappa B alpha (IkappaBalpha) in mouse uterus during peri-implantation, thereby investigating whether transient activation of nuclear factor-kappaB (NF-kappaB) takes place during embryo implantation in mice. Immunohistochemical technique was used to examine the expression and localization of p65 in endometrium or deciduas, and Western blot analysis was employed to detect the levels of IkappaBalpha protein in mouse endometrium or deciduas. P65 protein was detected in stromal cells, epithelial cells of endometrium as well as in myometrium. Staining was predominately seen in the cytoplasm of the cells. Staining intensity for p65 was stronger in the epithelial compartment than the stromal compartment and myometrium. Staining intensity increased slightly during pregnancy, and it reached a high level on pregnancy day 5 and day 8. In contrast to p65, the level of IkappaBalpha protein was lowest on pregnancy day 5 in all groups. Our results suggested that NF-kappaB may regulate embryo implantation by its transient activation in mice.

Objective To investigate the effect of miR-145 on human lung adenocarcinaoma A549 cell proliferation and FSCN1 expression. Methods pmR-mcherry/miR-145 was constructed and transfected into A549 cell,then the expression of miR-145 and the proliferation of A549 cell were verified by QPCR and MTS assay, respectively.The situation of FSCN1 expression in A549 cell was detected by Western blot.Results pmR-mcherry/miR-145 vector was constructed successfully,and QPCR results indicated that miR-145 expressed effectively.Western blot results showed that FSCN1 was one of the targets of miR-145 in A549 cell.MTS assay results indicated that miR-145 inhibited the proliferation of A549 cell.Conclusion Overexpression of miR-145 can inhibit the proliferation of A549 cell, and FSCN1 was one of its target.
