1.CURRENT STATUS AND PROSPECT OF STUDIED ON MELATONIN AND ITS RECEPTOR

The secretion of melatonin in human is related to age, and of circadian rhythm. Our researches indicated that melatonin receptor existed in many tissues and also had a circadian rhythm. Melatonin has biological effects on genital, immunological and cardiovascular systems as well as the effects of antioxidation and antisenescence.
2.DETERMINATION OF GLUCOCORTICOID RECEPTORS IN HUMAN MONO-, POLY-MORPHONUCLEAR LEUKOCYTES, AND ITS SIGNIFICANCE

The method of glucocorticoid receptor (GR) in human peripheral mononuclear (MNL) and polymorphonucleas leukocytes (PML) was studied. The specific binding capacity of [3H] -Dex to leukocytes was estimated in 75 healthy subjects by radioligand binding method. 30 subjects were analysed by Scatchard plot to determine the maximal binding capacity (Ro) and dissociation constant (Kd), the Ro of MNL and PML were 5709 ? 901 and 6941 ?934 sites) cell respectively. Their Kd values were 4.83?1.21 and 5.78 ? 1.35 nmol/L respectively. The other 45 were investigated with the single point analysis, the specific binding capacity (Rs) of MNL and PML were 4462 ? 962 and 5483 ? 1146 sites/cell respectively. Coefficient of variation of this method in MNL and PML were 5.73?1.97 and 7.03?2.16% respectively. The results suggested that 1) no significant changes in the levels of GR in patients with Cushing's syndrome (Ro of MNL and PML were 6030?681 and 6885 ? 922 sites/cell respectively) and with Addison's disease (Ro were 4164 ? 564 and 5114 ? 658 sites/cell respectively); 2) the number of leukocytic GR was significantly lower in Shee-han's syndrome (Rs of MNL and PML were 3584?435 and 4131 ?562 sites/cell); 3) the number of GR was negatively correlated with the plasma cortisol concentration.
3.Applying Queuing Theory to Quantitative Analysis on Outpatient Billing Service in Hospital

Objective To analyze the efficiency of current outpatient billing service quantitatively.Methods The author applied mathematical model and arithmetic of the queuing theory to estimate the selective sample of gynecologic outpatient charge.The average arrival rate of patient and average charge were counted.By using the queuing theory and relevant assisted program,we got the corresponding evaluation index.Results It is reasonable for the charge of current gynecologic outpatient.Conclusion It is feasible to apply queuing theory to quantitative analysis on outpatient charge service in the hospital.

There are many options on the No.1 Military Medical Project database backup in the Hospital Information System.The DataGuard of Oracle8i is one of them.This article introduces the concepts and principles of DataGuard,and briefly describes the implementation procedure and some attention-deserved problems.DataGuard is compared with traditional method for data back-up,especially the important improvement based on Oracle7 and Standby.
5.Gene microarray analysis of peripheral blood mononuclear cells in type 2 diabetes patients with distal symmetric polyneuropathy

Objective:To screen for the differentially expressed genes in peripheral blood mononuclear cells(PBMCs) from type 2 diabetes patients with distal symmetric polyneuropathy(DSPN) by gene microarray.Methods: A 5 075 gene cDNA microarray was used to examine gene expression in PBMCs of 2 type 2 diabetes patients with DSPN(group 1),2 type 2 diabetes patients without DSPN(group 2),and 2 age-and sex-matched controls(control group).The differentially expressed genes were identified and subjected to real-time RT-PCR analysis.Results: Twenty-two genes were found to be differentially expressed,including cell metabolism and signal transduction genes,oncogenes and anti-oncogenes,DNA synthesis and repair genes,ion channel and transport protein genes,DNA-binding,transcription and transcription factor genes,and cytoskeleton genes.There were 4 genes up-regulated and 18 down-regulated.Real-time RT-PCR results of AK1 and FBXO7 were consistent with that of microarray analysis.Conclusion: Microarray expression profile of PBMC in type 2 diabetes patients with DSPN is differential.DSPN may be involved in cell metabolism,signal transduction,DNA synthesis,etc.
6.Subcutaneous insulin resistance: a new type of insulin resistance

Subcutaneous insulin resistance is a condition in which glucose is insensitive to insulin from the subcutaneous tissue,but sensitive to that by intravenous injection.The mechanism of subcutaneous insulin resistance is mainly associated with skin obstruction or delayed entry of insulin into blood circulation.The treatment of the problem involves Aprotinin,anticoagulant and intramuscular injection of insulin.This paper offers an overview of the advances in the researches of subcutaneous insulin resistance.
7.Treatment strategies of reoperation of benign biliary tract diseases in the era of precision surgery

Reoperation on biliary tract diseases is a kind of complex and difficult surgery,with the high risk and recurrence rate.Benign biliary tract diseases included residual and recurrence of bile duct stone,bile duct injury and benign biliary stricture,which are still the most common causes for reoperation.The common causes,application of the precision surgery and new mode of multidisciplinary team (MDT) for the reoperation on benign biliary diseases were explored in this paper.With the techniques developments of minimal invasive and precision surgery,the new mode of MDT for the reoperation on benign biliary diseases will provide the best individual diagnosis and treatment to the patients.

10.Clinical analysis on the relationship between gynecologic malignant neoplasms and metabolic syndrome

Objective To investigate the relationship between gynecologic malignant neoplasms and metabolic syndrome (MS),in order to give a direction to the prevention and treatment of gynecologic malignant neoplasms.Methods A total of 359 cases of gynecologic malignant neoplasms (including 142cases of ovarian cancer,131 cases of endometrial cancer,86 cases of cervix cancer) were enrolled as study group,and 400 cases of gynecologic benign neoplasms were enrolled as control group.The general conditions,physical examination,fasting blood glucose,fasting insulin,homeostatic model assessment insulin resistance index (HOMA-IR),blood fat were compared between two groups.Clinical characteristics between two groups were compared.The occurrence of MS was counted.Results There was no significant difference in menarche age,onset age,gravidity and parity between two groups (P >0.05).The level of systolic pressure,diastolic pressure,body mass index,abdominal circumference,fasting blood glucose,fasting insulin,HOMA-IR,triacylglycerol in study group was higher than that in control group [(129 ± 13)mmHg (1mmHg=0.133kPa)vs.(112±14)mmHg,(87±11)mmHgvs.(75±8) mmHg,(26.7±2.8) kg/m2 vs.(22.2 ±2.1) kg/m2,(88 ±7) cm vs.(76 ±9) cm,(6.5 ±2.9) mmol/L vs.(4.7 ±0.9)mmol/L,(9.2 ± 4.7) mU/L vs.(5.2 ± 3.0) mU/L,3.9 ± 0.8 vs.3.1 ± 0.6,(3.21 ± 1.96) mmol/L vs.(1.56 ±1.22) mmol/L],and the level of high density lipoprotein cholesferol was lower than that in control group [(1.25 ± 0.51) mmol/L vs.(1.65 ± 0.47) mmol/L],and there were significant difference between two groups (P < 0.05).The occurrence of MS in study group was 37.60% (135/359),among of them,37.32% (53/142)in ovarian cancer,43.51%(57/131) in endometrial cancer,29.07% (25/86) in cervix cancer,which was higher than that in control group [12.25% (49/400)],and there was significant difference (P <0.05).Conclusions The occurrence of MS in gynecologic malignant neoplasms is higher than that in gynecologic benign neoplasms,and there is a mutual promotion and influence of the relationship between gynecologic malignant neoplasms and MS.It suggests that there is a possible role of MS in the occurrence and development of gynecologic neoplasms,gynecologist should pay attention to it and control MS actively,those measures could make important significance of prevention and treatment for gynecologic malignant neoplasms.
