1.Video-assisted Mini-thoracotomy Lobotomy for Peripheral Lung Carcinoma:Analysis of 56 Cases

Objective To assess the value of lobectomy by video-assisted mini-thoracotomy(VAMT) for the treatment of peripheral pulmonary carcinoma.Methods From January 2004 to December 2007,56 cases of peripheral pulmonary carcinoma underwent VAMT lobectomy under general anaesthesia in our hospital.Of the patients,5 had the tumor in the upper lobe of the right lung,2 in the middle lobe of the right lung,19 in the superior lobe of the right lung,8 in the upper lobe of the left lung,and 22 in the superior lobe of the left lung.All the tumors were no more than 5 cm in diameter.All the cases were intubated with double-lumen tubes and then one-lung ventilation was performed.With the patients being placed in a lateral position,a mini-incision(6.0-8.0 cm in length) was made at the fourth or fifth intercostal space.And another 1.5-cm incision was made at the sixth intercostal space along the midaxillary line for introducing the video-thoracoscope.The resected pulmonary lobe was removed though the mini-incision.Afterwards,enlarged lymph nodes were dissected as routine.Results During the operation,no incision was prolonged in the cases.A mean of 14 lymph nodes were removed(ranged from 9 to 31).The mean operation time was(145?35) min(range,120 to 220 min),and the intraoperative hemorrhage was 100 to 450 ml [(210?48) ml].The patients were discharged from hospital in 5 to 11 days(mean,7 days).Seven of the patients developed complications including thoracic effusion in 5 and atelectasis in 2.The patients were followed up for 1 year,3 of them died during the period(1 year survival rate:94.6%).Conclusion VAMT lobectomy is feasible and safe for peripheral lung cancer.
2.Protective effects of ethyl docosahexaenoate on brain oxidative injury and edema induced by cerebral ischemic reperfusion

Objective To investigate the protective effects of ethyl docosahexaenoate on brain oxidative injury and edema induced by cerebral ischemic/reperfusion in gerbils. Methods The gerbils were subjected to both common carotid arteries occlusion. The contents of MDA and GSH, the activities of GSH-PX, CAT, SOD and ATPase, the water content and the concentrations of Na + and Ca 2+ were measured. Histopathological examination was also done. Results The pretreatment with E-DHA significantly prevented the raise of MDA level, the decline of GSH content, the activities of GSH-Px, CAT, ATPase and the increases of Ca 2+, Na + and water content. Conclusion E-DHA has protective effect on brain ischemic injury and edema, which may be due to an inhibitory action on hydroxyl radical formation and brain edema.
3.The Outcomes Measures of the Cochlear Implantation in Patients with Waardenburg Syndrome

Objective To measure the outcomes of the cochlear implantation for patients with Warrensburg syndrome in comparison with non-syndromic patients with sensorineural hearing loss. Methods 16 of 1 300 Waardenburg syndrome patients who received cochlear implants at PUMC hospital were reviewed in this papers. The authors will discuss the effects between Warrensburg syndrome and non- syndromic patients with sensorineural hearing loss by means of the Parents Evaluation of Aural-Oral Performance of Children (PEACH). Results There were no statistical difference in quiet, in noise, in overall results and in conversation on the phone between Warrensburg syndrome and non-syndromic patients. There was statistical difference between in quiet and in noise in every group. Conclusion The auditory perception and speech recognition between Warrensburg syndrome and non-syndromic patients with sensorineural hearing loss is same, and the results in quiet is better than that in noise.

6.Effects of hyperbaric oxygen on delayed encephalopathy after acute carbon monoxide poisoning

Objective To study the efficacy of hyperbaric oxygen(HO)for the delayed encephalopathy after acute carbon monoxide poisoning(DEACMP)Method One hundred and eleven patients who were diagnozed as the DEACMP from November 2000 to March 2007 in Tangdu Hospital the Fourth Military Medical University were randomly divided into two groups.Thirty-six cases were treated by onventional approach(group A),and 76 cases by HO besides conventional treatment(group B).The efficacy of HO was evaluated after 4courses of treatment. The curative effects were evaluated as(1)cured:clinical symptoms and signs fully disappeared,abnormal electroencephalogram recovered,patients were completely self-help and competent enough for routine work.(2)improved:chnical symptoms and signs partly disappeared,abnormal electroencephalogram partly recovered,patients were partial self-help and incompetent enough for routine work.(3)inefficacy:patient's condition didn't changed.Data were expressed as((x)±s)and analyzed with the chi-quare test and t-test.The statistical significance was established as P<0.05.Results In group B,62(81.58%)were in good recover,9(11.84%)improved and 5(6.94%)were inefficacy;while in group A:21(58.33%)were in good recover,5(13.89%)were improved and 10(27.78%)were inefficacy.The effciency rate in group B was significantly higher(93.42%)than that(72.22%)in group A(P<0.05),and the required time for the therapeutic effect noticed time in group B were significantly shorter(P<0.05)Conclusions HO Can improve the therapeutic effects on DEACMP
7.Prevalence of obstructive sleep apnea-hypopnea syndrome in adults aged over 60 yeaes in dongying city.

OBJECTIVE:
To survey the prevalence of obstructive sleep apnea-hypopnea syndrome(OSAHS) in adults aged over 60 years in Dongying city,and analyze the risk factors and the extent of damage to the quality of life for the elderly to provide the basis for prevention and treatment of OSAHS people.
METHOD:
One thousand subjects were derived from a random cluster sampling in seven districts of Dongying city:they were asked to answer the questions from questionnaires. According to the questionnaire scoring, 100 subjects in high-risk group were selected randomly to make polysomnography monitoring for a whole night,so that the prevalence of the disease was calculated and the related risk factors were analyzed; elderly patients diagnosed with OSAHS were asked to assess the quality of life assessment questionnaire by face to face to understand the quality of life dimensions injury.
RESULT:
The actual number of completed surveys was 934, and the efficiency was 93.4%. The estimated prevalence of OSARS in elder people defined by apnea-hypopnea index (AHI ≥ 5) was 32.5%; Multivariate analysis revealed that age smoking, family snoring,neck circumference, waist circumference, and abnormality of the upper airway were respectively independent risk factors of OSAHS,and the abnormalities of the upper airway had the most obvious impact on AHI. The damage caused by OSAHS to the quality of life for elderly people followed their daily work life, social relationships, symptoms, alertness, emotional, general health, symptoms.
CONCLUSION
The estimated prevalences of OSAHS in elder people were high. Actively promoting good habits to older people, weight loss, early detection and correct upper airway abnormalities may reduce the estimated prevalence of OSAHS. In the treatment process, the patient's physician should pay attention to their emotional and groom their psychological problems to improve the quality of life in elder people.

8.Multi-modality therapy based on surgery in advanced non-small cell lung cancer patients: Analysis of 51 cases.

BACKGROUNDAt the time of being diagnosed as non-small cell lung cancer (NSCLC), 65%-70% of the patients are at the advanced stage (stage III, IV). It is believed that the multi-modality therapy has a better efficacy than mono-therapy. The aim of this study is to evaluate the efficacy of multi-modality therapy based on surgery in advanced NSCLC.
METHODSThe clinicopathological data were retrospectively reviewed from 51 stage IIIB-IV NSCLC patients, who underwent surgery and multiple modality therapy during 1995 to 2003 in the department of Thoracic Surgery of PUMC Hospital. The survival of patients was analyzed.
RESULTSThe overall 1- and 3-year survival rate was 86.35%±5.21% and 52.96%±8.83%, respectively. The median survival time was 37.0 months.
CONCLUSIONSThe multi-modality therapy based on surgery could improve the survival of operable patients with advanced NSCLC.
9.The research of cochlear implantation with other assident particular diseases.

OBJECTIVE:
To Respectively analyze extremely SNHL patients who received cochlear implants with other particular diseases, mainly introducing pre-/intra-/postoperative clinical experiences.
METHOD:
Eight out of one thousand and three hundred patients who received cochlear implants in PUMC hospital were diagnosed with other assident particular diseases besides SNHL, which included hypopotassaemia, hemophilia, idiopathic thrombocytopenic purpura (ITP), favism, abnormal blood coagulation factor, abnormally high alkaline phosphatase. We would introduce pre-/intra-/postoperative clinical experiences by turns.
RESULT:
All of the surgery were performed without any complication, and there were no remote complications.
CONCLUSION
SNHL patients with other assident particular diseases could accept cochlear implantation with carefully preparations before and during operation.
