1.The epidemiological analysis of Urumqi pre-hospital first aid in 2011

Objective To analyze the epidemiology of the emergency medical service in Urumqi City in 2011.Methods All patients from 120-Ambulance Center of Urumqi City admitted from January 1,2011 through December 30,2011 were enrolled for prospective study.Descriptive analysis the gender,age,race,underlying diseases,the peak of calling,the time taken by ambulance and so on.Results The 120-Ambulance Center answered overall 53786 callings,the callings of effective answer were 47260,while the ineffective answers were 6526 accounting for 13.8%,and the average number of answer per day was 147.The ratio of male to female was 1.61:1 ; the number of callings from Han nationality was predominant accounting for 68.87% (37043 callings),and the number of callings from Weiwuer nationality was next to that from Han nationality.The five most common emergency problems were traffic injury,acute cardiovascular diseases,cerebro-vascular diseases,trauma and poisoning.There was no significant difference in monthly number of emergency cases during the 12 months of 2011,and the average monthly number of emergency cases was ranged in 2500-3500.The peak time of answering occurred in 10:00 am-13:00 pm、16:00 pm-18:00 pm、21:00 pm-24:00 pm; the ages of patients were dominant in the population of 20-29 years old、30-39 years old and 70-79 years old.Conclusions We should lay the emphasis on the most common diseases,the peak of answering,and the most vulnerable population,in order to come across with the theoretical and practical evidence to the relevant public health bureau for the improvement of transport mode carried out by 120-Ambulance Center in Urumqi City.
2.The Pathogenesis of Post-Stroke Depression

Post-stroke depression (PSD) is a common complication of stroke. Its patogenesis is complex, mainly including the mechanisms of neurobiotogy and social psychology. So far, there has been no uniform conclusion both at home and abroad. Now it is considered that PSD is the results of comprehensive effect of various neurobiology and social psychology, and its onset is in accordance with the bio-psycho-social medical model.
3.Effects of and Smeton on inflammatory mediator levels and therapeutic outcomes in elderly patients with chronic obstructive pulmonary disease

Objective To compare the effects of and Smeton on inflammatory mediator levels and therapeutic outcomes in elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods Ninety AECOPD patients hospitalized at our department from June 2012 to June 2013 were selected and randomly divided into the group and the Smeton group.In addition to routine therapy,such as anti-infection medication and water & electrolyte balance maintenance,for both groups,patients in the group were given a single 10 mg oral dose of,taken at night before bedtime,while patients in the Smeton group were given 2 pills of a compounding formulation containing,three times a day,for two months.Changes in plasma inflammatory mediator levels before and after treatment and therapeutic outcomes were monitored.Results Highsensitivity C-reactive protein (hs-CRP) levels decreased and interleukin-10 (IL-10) levels increased two months after treatment,compared with pretreatment levels [hs-CRP,(4.01±0.69) mg/L vs.(15.23±4.39) mg/L,and IL-10,(19.44±6.35) g/L vs.(7.86±3.24) g/L,for the group; hsCRP,(7.09±1.70) mg/L vs.(7.86±3.24) g/L,andIL-10,(11.6±3.2 g/L vs.6.7±2.8 g/L),for the Smeton group; P<0.05 or 0.01].The magnitude of increases or decreases in inflammatory mediator levels was greater in the group than in the Smeton group (P<0.05 for both),and the overall effectiveness rate was higher in the Smeton group than in the group (95.6% vs.82.2%,x2 =4.05,P <0.05).Conclusions When used for treatment of patients with AECOPD,ontelukast has more beneficial effects on inflammatory mediator levels,whereas is more effective in clinical symptom improvement.
4.Survey on characteristics of Helicobacter pylori infection in community patients in Shanghai

Objective:To investigate the characteristics of Helicobacter pylori(Hp)infection in community patients. Methods:In July 2020,Three hundred and sixty outpatients with Hp infection confirmed by 14C breath test from August 2018 to July 2019 in Pengpuxincun community health service center of Jing′an District of Shanghai were selected by simple random sampling for a questionnaire survey. The contents of the questionnaire included general information, symptoms of digestive tract, awareness of disease hazards, frequency of dining out, Hp infection in family members. A total of 335 valid questionnaires were collected with a response rate of 93.06% (335/360). Results:Among 335 cases of Hp infection, 282 cases had clinical symptoms and 53 cases had no clinical symptoms. The infection rate of family members of Hp infected patients with and without clinical symptoms was 55.67% (157/282) and 56.60% (30/53) respectively (χ2=0.016, P>0.05). Belching was the most common symptom(27.09%,175/646), followed by abdominal distension(22.45%,145/646), acid reflux (17.03%,110/646), abdominal pain(14.86%), halitosis(14.55%). There was no significant difference in the frequency of halitosis, acid regurgitation, belching, abdominal distension and abdominal pain among the three groups(χ2=13.297, P>0.05). The Hp infection rate of family members with frequent or seldom out dining was 82.40% (103/125) and 40.00% (84/210), respectively, (χ2=57.122, P<0.05). The awareness rates of Hp hazard in patients with educational levels of primary school and below, junior high school, senior high school and junior college and above were 9.43% (5/53), 51.55% (50/97), 62.50% (65/104) and 87.65% (71/81), respectively(χ2=82.447, P<0.01). There was no significant difference in the age distribution between males and females (χ2=1.155, P>0.05). Conclusion:It is necessary to strengthen the health management of Hp infected people and improve the awareness of Hp infection hazards among community residents to reduce Hp infection rate in the community.

The average cited number of original articles cited by other researchers is 1.70. The cited authors are from 23 provinces and from other countries. Beijing, Hunan, and Guangdong province are in the front of most cited author's areas. The most frequently cited institute was Xiangya School of Medicine at Central South University. There are 115 citing journals.
6.Prognostic factors analysis in patients with ovarian malignant germ cell tumor treated with fertility-preserving surgery

Objective To analyse the clinicopathologic factors affecting prognosis and fertility of patients with malignant ovarian germ cell tumor (MOGCT).Methods The medical records and follow up data of 106 patients with MOGCT treated at Affiliated Tumor Hospital of Guangxi Medical University between January 1986 and December 2010.Kaplan-Meier method was used to analyse survival curves.The different prognoses between different clinicopathologic factor was evaluated by univariate analysis and log-rank test.The multivariate analysis was performed by the Cox proportional hazard regression method.Logistic regression analysis was used to evaluate the influence of different factors on the prognoses and fertility.Results The median age at primary treatment was 22 years old (range:9-61 years old),59 patients received fertility-preserving surgery,45 patients received radical surgery,only 2 cases performed biopsy; 94 patients received postoperative adjuvant chemotherapy.Median follow-up time was 56.5 months (range:2-309 months),there were 11 cases recurrences,23 cases died from cancer.Of 47 patients live without tumor,45 patients had normal menstrual.Of the 39 patients desiring pregnancy,31 cases got 33 successful pregnancies,resulting in 33 live births.There is no statistically significant difference (P >0.05) in progression free survival (PFS ; 67.6% versus 63.3 %) and overall survival (OS ; 70.0% versus 64.1%) and mortality [15% (9/59) versus 31% (14/45)] between fertility-preserving surgery patients and radical surgery patients.The univariate analysis showed that the pathological types,postoperative residual tumor size,lymph nodes and omental resection were associated with OS (P < 0.1),and postoperative residual tumor size,chemotherapy cycles,lymph nodes and omental resection were associated with PFS (P <0.1).The multivariate analysis showed only the postoperative residual tumor size was independent prognostic factor of OS (P =0.039),and postoperative residual tumor size,chemotherapy cycles,lymph nodes resection were independent prognostic factors of PFS (P < 0.05).There is no statistically significant difference in OS,PFS and fertility between fertility-preserving surgery patients treated with or without a comprehensive staging surgery (P > 0.05).Conclusions MOGCT can achieve a good prognosis after surgery combined chemotherapy.Postoperative residual tumor size is independent prognostic factor of PFS and OS.Comprehensive staging surgery could not improve prognosis.Fertility-preserving surgery plus adjuvant chemotherapy appeared to have little or no effect on prognosis and fertility.

Transpulmonary thermodilution pulse-indicated continuous cardiac output (PiCCO) monitoring only needs a central venous catheter and an artery catheter ,so is a minimally invasive ,simple ,accurate and continuous cardiac output monitor technique used at home and abroad now .It combines transpulmonary thermodilution and analysis technique of arterial pulse wave area under curve (AUC) .It helps to better evaluate ,guide and improve treatment in critically ill patients .
8.Platelet-rich plasma combined with closed reduction and hollow screw internal fixation for femoral neck fracture

Objective To explore the clinical effect of autologous platelet-rich plasma ( PRP) combined with closed reduction and hol-low screw internal fixation for femoral neck fracture .Methods Totally 200 cases of femoral neck fracture were collected from May 2010 to August 2014 in our hospital .Randomly divided them into two groups , namely the PRP group and the control group , with 100 patients in each group.The PRP group were given autologous platelet-rich plasma ( PRP) combined with closed reduction and hollow screw internal fixation , while the control group were given closed reduction and hollow screw internal fixation merely .The length of stay , time of fracture healing , wound healing state , postoperative complications rate , Harris score and functional recovery of the two group were recorded and evaluated .Re-sults The average hospitalization time and fracture healing time in the PRP group were shorter than the control group .The A-grade recovery rate of the PRP group was higher than the control group , and the postoperative femoral head necrosis and nonunion rate of the PRP group were lower than that in the control group (P<0.05).The Harris score of PRP group were 87.35 and 90.82 respectively, and the excellent rate of hip function 6 months and 12 months after operation were 86%and 90%respectively .They were both higher than the control group with statistical significance (P<0.05).Conclusion PRP combined with closed reduction and hollow screw internal fixation can significant -ly shorten the healing time , reduce postoperative complications , improve hip function and postoperative quality of life in treating femoral neck fracture, and it is of high safety and efficacy.

10.Clinical analysis of posterior vertebral pedicle nail fixed injury treatment of thoracolumbar fractures in 48 cases

Objective To investigate the injury posterior vertebral pedicle nail fixed treatment of thoracolumbar fractures of the clinical efficacy and safety .Methods Forty-eight cases of thoracolumbar fractures were treated by the posterior vertebral pedicle screw fixation ,according to cases decompressive laminectomy ,underwent intertransverse fusion treatment .Postoperative follow-up of 1 year ,preoperative ,postoperative 3 months ,after 1 years of vertebral compression rate ,Cobb Angle were compared .Results Forty-eight patients with postoperative 3 months and 1 year after injury postoperative spinal vertebral body compression rate were significantly decreased compared with preoperative (P<0 .05);After 3 months and 1 year after the Cobb Angle were markedly sig-nificant reduction than preoperative(P<0 .05) .X-ray showed all fractures were obtained good reduction ,internal fixation with no looseness or breakage ,no protruding after spinal deformity ,neural function were all in 1 -3 level recovery .Conclusion Posterior vertebral pedicle injury by nail fixed treatment of thoracolumbar fractures with fixed reliable ,reset effect is good .
