1.Management of blood pressure in patients acute ischemic stroke

Usually the blood pressure will elevated in patients with acute ischemic stroke during the first 24 hours after the onset of symptoms, and the blood pressure levels are associated with the prognosis of patients. Regardless of trying to reduce or elevate blood pressure, the management of blood pressure in acute stroke is controversial. This article reviews the relations between the changes of blood pressure in the acute phase of ischemic stroke and the management of blood pressure and stroke prognosis.
2.Effects of electroacupuncture on disorder of intestinal motility in a rat model of irritable bowel syndrome.

Objective:To investigate the effects of electroacupuncture (EA) on intestinal motility disorder in rats with irritable bowel syndrome (IBS) in order to provide experimental evidence for improving clinical treatment to IBS with EA. Methods: IBS was induced by inserting a balloon into the rectum of male Sprague-Dawley rats from their neonatal age (8th, 21st day after birth) and applying colorectal distention stimulation by inflating air into the balloon. Peristaltic wave (PW) was used to estimate intestinal motility. Rats were divided into 4 groups (n=6): normal group, untreated group, sham EA group and EA group. Acupoints of Zusanli (ST36) and Shangjuxu (ST37) in the hind limbs bilaterally were chosen for EA and sham EA treatment. Trains of dense-sparse frequencies (100 Hz and 2 Hz alternately) and intensity of 1, 2 and 3 mA (10 min for each) were applied for 30 min with each EA treatment while inserting similar needles without electrical stimulation was done as sham EA treatment. PW recorded for 30 min in normal and untreated groups respectively was taken as control values, while PW recorded after EA or sham EA treatment for 30 min was taken as responsive values. Results: Frequency of PW was significantly increased in the untreated group as compared with that in the normal group (P<0.05), and decreased after 30-min EA treatment (P<0.05). There was no significant change in the PW of sham EA group as compared with the control group and before treatment (P>0.05). Conclusion: These results suggest that 1) there is an abnormal increase in intestinal motility indicating an intestinal motility disorder in IBS rats; 2) EA but not sham EA can relieve such intestinal motility disorder.
3.Risk factors and follow-up of diabetes patients with hypertension

Objective To analyze the prevalence and risk factors related to the diabetic patients with hypertension in the Kailuan group workers.Methods A total of 9 498 type 2 diabetic patients were followed up for 5 years,regarding myocardial infarction,stroke,and death incident.The cumulative incidence of hypertension in diabetic patients and the evaluated risk factors of hypertension were analyzed.Results The prevalence of hypertension in 9 489 subjects with diabetes was 63.3%,higher than that in subjects without diabetes(42.0%,P<0.01).Aging,smoking,obesity,high blood levels of triglycerides,total cholesterol,and low-density lipoprotein-cholesterol were the risk factors of diabetic patients with hypertension,in whom the morbidity of myocardial infarction,cerebral infarction,cerebral hemorrhage,and renal dysfunction was 3.3%,6.0%,0.9%,and 18.8%,respectively,all higher than those (2.1%,2.6%,0.3%,and 10.9%) in diabetic patients without hypertension (P<0.01).By the end of follow-up for 5 years,incidences of cerebral infarction,cerebral hemorrhage,and death in diabetic patients with hypertension were higher than those with normal blood pressure (P<0.05),but no difference in incidences of myocardial infarction between the 2 groups (P>0.05).The 5-years cumulative incidence of hypertension in the diabetic population was 40.1%.Male sex,smoking,snoring,obesity,and hypertriglyceridemia were the risk factors of hypertension in diabetic patients (P < 0.05).Conclusion The prevalence and the 5-years cumulative incidence of hypertension in diabetic patients increased significantly.Diabetic patients with hypertension may have higher risk of cerebrovascular events and kidney dysfunction.
4.Clinical observation on Yiyi Fuzi Baijiang powder for treatment of patients with ulcerative colitis accompanied by yang-deficiency of spleen and kidney: a report of 80 cases

Objective To observe the clinical effects of Yiyi Fuzi Baijiang powder for treatment of patients with ulcerative colitis (UC) accompanied by yang-deficiency of spleen and kidney.Methods Eighty patients with UC admitted to the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine (TCM) from January 2012 to December 2015 were enrolled, and they were divided into an integrated TCM and western medicine group (combined group) and a control group by random number table method, 40 cases in each group. The control group was treated by normal saline 100 mL with dexamethasone 10 mg enema, whilethe combined group was given Yiyi Fuzi Baijiang powder 100 mL (coix seed 30 g, aconitum carmichaeli debx 6 g, herba patriniae 15 g, radix paeoniae alba 15 g, frankincense 6 g, myrrh 6 g, glycyrrhiza uralensis 6 g) combined with dexamethasone 10 mg enema, one therapeutic course was 10 days, and after 3 courses, the clinical efficacy was observed in the two groups. The differences of erythrocyte sedimentation rate (ESR), the number of red blood cell (RBC) and white blood cell (WBC) positive patients in stool routine were compared before and after treatment, and the clinical therapeutic effects were observed in the two groups.Results After treatment, the ESR was reduced in both groups compared to that before treatment, the numbers of RBC and WBC positive patients in stool routine were decreased in both group compared to those before treatment, and the degrees of changes were more significant in combined group than those in the control group [ESR (mm/1 h): 15.00±3.07 vs. 20.55±3.87, the number of RBC positive patients: 7 cases vs. 21 cases, the number of WBC positive patients: 14 cases vs. 21 cases, allP < 0.05]. The total effective rate of combined group was significantly higher than that of the control group [92.5% (37/40) vs. 75.0% (30/40),P < 0.05].Conclusion The clinical effect of Yiyi Fuzi Baijiang powder combined with dexamethasone enema was superior to that simply using hormone enema therapy for treatment of patients with UC accompanied by yang deficiency of spleen and kidney, this combined method is simple, and its curative effect is definite.
5.An observation on clinical efficacy of paeoniae decoction add and subtract combined with mesalazine for treatment of ulcerative colitis

Objective To investigate the clinical efficacy of paeoniae decoction (PD) add and subtract or modified PD combined with mesalazine for treatment of patients with ulcerative colitis (UC).Methods A prospective study was conducted, 98 patients with UC admitted to the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from August 2015 to August 2016 were enrolled, and they were divided into an observation group and a control group by random number table, 49 cases in each group. The control group was treated with mesalazine 1 g, 3 times daily; the observation group was treated with paeoniae decoction (ingredients: paeonia 30 g, coptis chinensis 15 g, Chinese angelica 15 g, betel nut 6 g, Chinese rhubarb 6 g, baikal skullcap 6 g, cinnamon 6 g, licorice 6 g) add and subtract, on the basis of treatment of the control group ,1 dose daily, each dose was decocted 2 times and warm decoction was taken once in the morning and once in the evening; the therapeutic course in both group was consecutive 6 weeks. The contents of different inflammatory factors, interleukin (IL-1, IL-8), tumor necrosis factor-α (TNF-α), C-reactive protein (CRP)in serum were compared before and after treatment between the two groups, and the clinical curative effects and adverse reactions were observed in two groups.Results After treatment, the levels of serum IL-1, IL-8, TNF-α, CRP in two groups were significantly lower than those before treatment, and the degrees of decrease were more significant in the observation group than those in the control group [IL-1 (ng/L): 8.48±3.05 vs. 9.38±3.37, IL-8 (ng/L): 10.15±2.23 vs. 11.94±2.30, TNF-α (μg/L): 122.13±6.40 vs. 137.02±7.35, CRP (mg/L): 7.16±1.93 vs. 8.02±2.63, allP < 0.05]. After treatment, the total effective rate in observation group was significantly higher than that in control group [91.84% (45/49) vs. 77.55% (38/49),P < 0.05]; the adverse reactions of observation group and control group had no statistical significant difference [4.08% (2/49) vs. 6.12% (3/49),P > 0.05].Conclusion The clinical efficacy of add and subtract of paeoniae decoction combined with mesalazine for treatment of ulcerative colitis is prominent, and no increase of adverse reaction occurrence was seen.

8.Survey on characteristics of Helicobacter pylori infection in community patients in Shanghai

Objective:To investigate the characteristics of Helicobacter pylori(Hp)infection in community patients. Methods:In July 2020,Three hundred and sixty outpatients with Hp infection confirmed by 14C breath test from August 2018 to July 2019 in Pengpuxincun community health service center of Jing′an District of Shanghai were selected by simple random sampling for a questionnaire survey. The contents of the questionnaire included general information, symptoms of digestive tract, awareness of disease hazards, frequency of dining out, Hp infection in family members. A total of 335 valid questionnaires were collected with a response rate of 93.06% (335/360). Results:Among 335 cases of Hp infection, 282 cases had clinical symptoms and 53 cases had no clinical symptoms. The infection rate of family members of Hp infected patients with and without clinical symptoms was 55.67% (157/282) and 56.60% (30/53) respectively (χ2=0.016, P>0.05). Belching was the most common symptom(27.09%,175/646), followed by abdominal distension(22.45%,145/646), acid reflux (17.03%,110/646), abdominal pain(14.86%), halitosis(14.55%). There was no significant difference in the frequency of halitosis, acid regurgitation, belching, abdominal distension and abdominal pain among the three groups(χ2=13.297, P>0.05). The Hp infection rate of family members with frequent or seldom out dining was 82.40% (103/125) and 40.00% (84/210), respectively, (χ2=57.122, P<0.05). The awareness rates of Hp hazard in patients with educational levels of primary school and below, junior high school, senior high school and junior college and above were 9.43% (5/53), 51.55% (50/97), 62.50% (65/104) and 87.65% (71/81), respectively(χ2=82.447, P<0.01). There was no significant difference in the age distribution between males and females (χ2=1.155, P>0.05). Conclusion:It is necessary to strengthen the health management of Hp infected people and improve the awareness of Hp infection hazards among community residents to reduce Hp infection rate in the community.
9.Expression of ER and PR in hyperplasia of mammary glands and breast cancer

Objective To investigate the expression of ER and PR in hyperplasia of mammary glands and breast cancer. Methods The expression of ER and PR in 68 hyperplasia of mammary glands cases and 168 breast cancer cases were quantified by immunohistochemical method. Results The expression of ER in breast cancer(30%) was significantly higher than that in hyperplasia of mammary glands(10 %)(P <0.05) and there was no significant difference in PR between them. The expression of ER in postmenopausal breast cancer was significantly higher than that in postmenopausal hyperplasia of mammary glands (P <0.05) and there was no significant difference in ER between two premenopausal groups. The expression of PR in invasive lobular carcinoma was significantly higher than that in invasive ductal carcinoma and other types (P=0.005).Conclusion The expression of ER and PR may identify the characteristics of patholobiology in breast disease.

ObjectiveTo explore change trend of Cardiac Troponin Ⅰ (cTnI) and Heart Fatty Acid-binding Protein(H-FABP) in serum during the perioperation of valve replacement.MethodsForty patients with heumatoid valvular heart disease were selected for this study,and the patients were randomly divided into two groups.Blood samples were taken from center vein,and the serum levels of cTnI and H-FABP were determined.The change of the serum levels of these two markers at different time points was recorded and compared between two groups.ResultsThere were significant differences in the concentration of cTnl in the cold crystalloid cardioplegia group ( F between group =2744.397,P <0.01 ; F interaction =125.345,P <0.01 ).There were significant differences in the concentration of cTnI in the cold blood cardioplegia group ( F between group =1056.357,P < 0.01 ; Finteraction=64.242,P < 0.01 ).There were significant differences in the concentration of H - FABP in the cold crystalloid cardioplegia group ( F between group =1775.022,P <0.01; F interaction =34.297,P <0.01 ).There were significant differences in the concentration of H -FABP in the cold blood cardioplegia group ( F between group =3064.451,P <0.01; Finteraction=60.472,P <O.01 ).ConclusionsH-FABP is suitable for early diagnosis of myocardial injury because of its myocardial specificity.There are positive correlation between the peak of H-FABP and myocardial injury.There are effective and short period of window for prediction of myocardial injury caused by H-FABP.H-FABP may be served as a good myocardial injury diagnostic markers.
