1.The relevance of molecular epidemiological study between Extraintestinal pathological escherichis coli infection in the clinic and intestinal pathogenic escherichis coli

sObjective:To investigate the epidemiological correlation of the Extraintestinal Pathogenic Escherichia coli(ExPEC)and the intestinal Pathogenic Escherichia coli.Methods:227 ExPEC strains isolated in the Department of Infectious Diseases of the First Affiliated Hospitial of Chongqing Medical University were studied to investigate their serotype by"OK"diagnostic serum.The virulence gene of the bacterium was analyzed by PCR.The homology of the intestinal Escherichia coli and ExPEC in some patients were analyzed by Pulsed field gel electrophoresis.Results:34 strains were selected in 227 ExPEC strains include 20(Enterotoxigenic escherichia coli,ETEC)strains,14(Enteropathogenic escherichia coli,EPEC)strains.The positive rate of virulence gene of ETEC were the heat labile enterotoxin gene (ltA)31.57%,heat stable enterotoxin gene(stI)47.36%and stI/ltA 21.05%.The positive rate of virulence gene of EPEC were the bundle-formingpilus gene(bfpA)66.66%,eeeA gene 60.0%and eaeA/bfpA 26.67%.Two cases of E.coli septicemia was found in patients with gut the same clone.Conclusion:Intestinal pathogenic E.coli serotype caused a relatively small proportion of the clinical ExPEC,and mainly ETEC and EPEC serotypes.ExPEC in patients with septemia,especially in serious ill patients,its intestinal translocation of E.coli may be the source of ExPEC.
2.Therapeutic effects of nicergoline on the treatment of posterior circulation ischemia vertigo

Objective To investigate the effects of nicergoline on the treatment of posterior circulation ischemia vertigo (PCIV).Methods A total of 87 patients with PCIV were randomly treated either with nicergoline (45 cases) or betahistine (42 eases).Dizziness assessment rating scale (DARS) and dizziness handicap inventory (DHI) were used to evaluate the changes of manifestations before treatment,after 7 days and 8 weeks treatment with nicergoline,meanwhile transcranial Doppler ultrasound (TCD) and brainstem auditory evoked potential (BAEP) were observed to assess the effects.Results After 7 days of treatment,the scores of DARS and DHI were decreased significantly in nicergoline group (DARS:15.6±2.1 ; DHI:38.3± 11.4) and control group (DARS:14.3±4.4; DHI:39.5±14.7) compared with the scores before treatment (DARS:24.9±7.3,22.5 ±9.7; DHI:55.8±19.7,57.5±19.3) (all P< 0.05),and the treatment efficiency was 71.1% (32/45),61.9% (26/42).After 8 weeks of treatment,the scores of DARS and DHI were also decreased significantly in nicergoline group (DARS:8.9 ±3.9; DHI:26.3± 12.8) and control group (DARS:9.9±4.7; DHI:35.8±13.1) compared with the scores before treatment (all P< 0.05),and the treatment efficiency was 93.3% (42/45),78.6% (33/42).There was significantly differences in the scores of DHI and treatment efficiency between the two groups at 8 weeks (t =2.169,P< 0.05,and x2=3.98,P< 0.05),but no differences at 7 days (both P> 0.05).In consistent with the remission of the clinical symptoms,nicergoline group significantly recovered from both low blood diffusion in the verbal-basal artery system and extended peak latency(PL) and inter peak latency(IPL) in the auditory pathway examined by TCD and BAEP individually at 8 weeks,especially in the upper part of the brainstem (P<0.05).Conclusions Nicergoline is very effective in the treatment of PCIV and could alleviate the subclinical ischemia to get long time benefits.

4.Significance of nueroelectrophysiological tests in the early diagnosis of sub-clinical neuropathay with diabetes mellitus.

To evaluate the application of nueroelectrophysiological tests in early diagnosis of sub-clinical neuropathy in diabetes mellitus (DM), The routine nerve conductive velocity (NCV), F-wave and sympathetic skin response (SSR) were detected in 27 patients with diabetes mellitus but without symptoms and signs of lesions of nerve system. Our results showed that 48.1%, 44.4%, 51.9% of the patients were found to have abnormal NCV, F-wave and SSR respectively. The abnormalities were mainly characterized by prolonged latency, reduced velocity and absence of wave-form. There were significant differences between the controls and the DM group (P < 0.05). Both the distal and proximal segments of nerves were affected and the distal lesions took place earlier than proximal ones and the changes in low extremities were more severe than those of upper extremities. F-wave can be used as a sensitive indicator for the early diagnosis of peripheral neuropathy and it can help to detect the subclincial lesions. SSR can be used for the evaluation of functional status of autonomic nerves in DM patients.
5.The expression of the RECK mRNA and MMP-9 mRNA in the breast cancer and fibroadenoma tissues

Objective To investigate the expression level of RECK-mRNA and MMP-9-mRNA in breast cancer(BC) and fibroadenoma tissues and its significance.Methods The mRNA level of tissues of 39 cases of breast cancer and 14 cases of fibroadenoma was measured by RT-PCR technique and its correlation was analyzed.Results The mRNA level of RECK gene in breast cancer tissues was markedly lower than that in fibroadenoma tissues,while the MMP-9 level was higher in BC tissues.Conclusions RECK is a tumor suppressor gene, the expression in benign and malignant breast tissues is a negative relationship.BECK can inhibit the expression of MMP-9 and thus,inhibit the growth and metastasis of breast cancer.
6.Factors associated with joint function after plate fixation of unstable pelvic fractures

Objective To analyze the factors that influence joint function after plate fixation of unstable pelvic fractures.Methods A retrospective study was conducted to analyze the 109 patients with unstable pelvic fracture who had been treated with plate fixation from January 2007 to September 2014.They were 69 men and 40 women,23 to 72 years of age (average,46.3 years).By the Tile classification,15 cases were type B1,37 type B2,14 type B3,19 type C1,16 type C2,and 8 type C3.Their postoperative joint function was evaluated according to the Majeed criteria for functional evaluation.There were 86 cases in the good-to-excellent group (78.9%) and 23 in the fair-to-poor group (21.1%).Their data were analyzed in terms of gender,age,preoperative injury severity scale (ISS),body mass index (BMI),operation time,fracture type,reduction quality,postoperative complications,bone density,and postoperative weight bearing time.The influential factors were determined using Logistic regression analysis.Results All the 109 patients obtained a mean follow-up of 18.7 months (from 12 to 53 months).Preoperative ISS (P =0.000),fracture type (P =0.008),reduction quality (P =0.009) and postoperative complications (P =0.000) were identified as the factors influencing joint function after plate fixation of unstable pelvic fractures.Conclusion Preoperative assessment by ISS,a clear understanding of the pelvic fracture type,anatomical reduction of the fracture and efforts to reduce postoperative complications can effectively improve functional recovery of the joint after plate fixation of unstable pelvic fractures.
7.Surveillance of bacterial resistance in Tongling People′s Hospital during 2013

Objective To investigate the antimicrobial resistance of clinical isolates in Tongling People′s Hospital during 2013. Methods A total of 2 281 nonduplicate clinical isolates were collected.Kirby-Bauer disc diffusion method was employed to study the antimicrobial susceptibility.The data were analyzed with WHONET 5.6 software according to CLSI 2012 breakpoints. Results The top 5 most frequently isolated microorganisms were E.coli (479,21.0%),K.pneumoniae (360,15.8%),A. baumannii (271,11.9%),P .aeruginosa (240,10.5%),S.aureus (171,7.5%).Gram negative and gram positive microorganisms accounted for 76.5% and 23.5%,respectively.The prevalence of methicillin-resistant strains in S.aureus (MRSA)and coagulase negative Staphylococcus (MRCNS)was 38.6% and 73.1%,respectively.The resistance rates of MR strains to beta-lactams and other antimicrobial agents were much higher than those of MS strains.No staphylococcal strain was found resistant to vancomycin or teicoplanin.E.faecalis showed relatively lower resistance to penicillin,ampicillin and nitrofurantoin.E.faecium strains were more resistant than E.faecalis to most of the antibiotics tested.Approximately 50.5% of E.coli and 44.5% of Klebsiella isolates produced extended-spectrum beta-lactamases (ESBLs).The ESBLs-respectively.And 29.8% and 23.4% of the P .aeruginosa strains were resistant to imipenem and meropenem.Nearly all (94.0%)P .aeruginosa isolates were susceptible to amikacin.Conclusions There appears a trend of increasing resistance in the clinical bacterial isolates in this hospital,especially the carbapenem-resistant Enterobacteriaceae,which is of great concern.It is mandatory to take effective antibiotic policy and infection control measures.
8.Application of diffusion weighted magnetic resonance imaging in assessing the efficacy of chemoradiotherapy for uterine cervical carcinoma

Objective To explore the feasibility of diffusion-weighted imaging (DWI) in assessing the efficacy of chemoradiotherapy for cervical cancer.Methods Data of magnetic resonance imaging (MRI) and DWI were analyzed in 32 patients with uterine cervical cancer received conventional prior to chemoradiotherapy and after 1 and 3 months of therapy.13 cases of normal controls also had been examed by MRI and DWL DWI with b values of (0,300) s/mm2 and b values of (0,600) s/mm2 were performed in all patients.Pretreatment post-treatmentADC values were compared between the health group and patients group.Results When the b =300 s/mm2,normal cervical average ADC value was (1.72±0.31)×10-3 mm2/s,cervical cancer was (1.10±0.24)×10-3 mm2/s before treatment and was (1.61±0.23)×10-3 mm2/s after treatment.When the b =600 s/mm2,normal cervical average ADC value was (1.46±0.25)×10-3 mm2/s cervical cancer was (0.89±0.21)×10-3 mm2/s before treatment and was (1.54±0.18)×10-3 mm2/s after treatment.When b =300 s/mm2,ADC value was higher than when b =600 s/mm2.ADC values of cervical cancer was significantly lower than that of the normal cervix group,ADC values of cervical lesions after chemoradiotherapy was significantly higher than that before chemoradiotherapy (P < 0.05).In the same group with different b values,ADC value was not significant (P > 0.05).Conclusion Joint observation of DWI and ADC values could be more objective and accurate in the analysis of the disease and would help to evaluate the efficacy of chemoradiotherapy.
9.The immunoregulatory effects of bone marrow-derived mesenchymal stem cells combined with leflunomide on mice T-lymphocytes

Objective To investigate the immunoregulatory effects of bone marrow-derived mesenehy-real stem ceils (BMSCs) combined with leflunomide (LEF) on mice T-lymphocytes in vitro. Methods BMSCs from BALB/c mice were isolated and expanded. The purity of BMSCs was identified by flow cytometry (FCM). The BALB/c mice's spleen lymphocytes were isolated by using EZ-Sep<'TM> Mouse 1X. Under ConA stimulation, spleen lymphocytes were pretreated with LEF, then washed and co-cuhured with BMSCs. We set up four groups to investigate in this study: group A, spleen lymphocytes alone; group B, spleen lymphocytes with BM- SCs; group C, LEF-pretreated spleen lymphocytes alone and the group D, LEF-pretreated spleen lymphocytes with BMSCs. T-lymphocytes proliferation was assessed by MTT. FCM was used to analysis T-lymphocytes apoptosis and surface markers of CD69 and CD28. The mRNA expression of interleukin (IL)-2, IL-10 were detected by real-time RT-PCR. Results In vitro, the T-lymphocytes'values of A570 nm were significantly lower in group B and group C, compared with group A (group B vs group C vs group A, 0.578±0.042 vs 0.502± 0.040 vs 0.778±0.035, P<0.01), while the value of A<,570 nm>in group D was 0.218±0.033, which was also obviously lower than that in group B and group C (P<0.01). There were no suppressing effects on T-lympho-cytes'activation and expression of IL-2 had been observed. The proportion of apoptotic T-lymphocytes in group B and group D were (2.29±0.32)% and (4.22±0.98)%, which was significandy lower than that in group A (8.08±1.20) (P<0.01). The expression of IL-10 in group B and C were also lower than that in group A (group B vs group C vs group A, 0.098±0.039 vs 0.054±0.022 vs 1.000, P<0.01 ). Either, the expression of IL-10 in group D was 0.023±0.015, which was obviously lower than that in group B and group C (P<0.01). Conclusion BMSCs combined with LEF show synergistic immunoregulatary effects on mice's T-lymphoeytes.
10.The expression and clinical significance of Toll-like receptor 7 and 9 in peripheral blood B cells of patients with systemic lupus erythematosus

Objective The aim of this study is to examine the expressions of Toll like receptor (TLR) 7 and TLR9 in the peripheral blood B lymphocytes of SLE patients and to analyze the correlation between TLR7/9 and clinical parameters. Methods lntracellular expression of TLR7/9 in the peripheral blood CD19+Blymphocytes was analyzed in 50 SLE patients and 30 healthy controls by flow cytometry. The difference of intracellular TLR7/9 expression levels in two groups was compared. Furthermore,the correlation between TLR7/9 expression and clinical parameters such as ESR, CRP, complement 3 (C3), complement 4 (CA), the level of serum IgG, anti-double stranded DNA antibody, anti-nuclear antibodies, SLEDAI score and urine protein excretion level, were analyzed. Results Compared with healthy subjects, the proportion of B cells expressing TLR7 and TLR9 was higher among SLE patients. Positive correlation was observed between TLR7 expression levels and clinical measurement of the SLEDAI and ESR. Negative correlation was observed between TLR7 expression levels and serum C3 levels. Positive correlation was observed between TLR9 expression levels and SLEDAI scores. Negative correlation was observed between TLR9 expression levels and serum C3 levels. Conclusion TLR7 and TLR9 expression is increased in the peripheral blood B cells of SLE patients, and correlates well with clinical parameters.
