1.Exercise-related sudden death

OBJECTIVE: To investigate and analyze the epidemiological and etiological characteristics of exercise-related sudden death, so as to provide scientific and effective preventive measures for competitive sports and the body building of the entire people.DATA SOURCES: A computer-based online search of Medline database was conducted to identify articles about exercise-related sudden death published in English between January 1982 and July 2005 by using the keywords of "exercise/sport, sudden death". Meanwhile, Chinese relevant articles at the same period were searched in VIP database with the same keywords in Chinese.STUDY SELECTION: The data were primarily checked, those about exercise-related sudden death were selected, and the repetitive studies and case report were deleted.DATA EXTRACTION: Fourteen of the 63 literatures were involved after classification.DATA SYNTHESIS: Exercise-related sudden death has low incidence and rapid progress, and it has no obvious correlation with the event and intensity of sports, it occurs more in males than in females of 30-60 years.Most of the etiological factors are sudden cardiac death and sudden cerebral death, and coronary atherosclerosis is one of the main causes for exercise-related sudden death.CONCLUSION: Exercise-related sudden death has low incidence, and there is sex characteristics, coronary atherosclerosis is an important factor for exercise-related sudden death. Studies on the epidemiology and etiology of exercise-related sudden death, as well as the preventive measures,should be reinforced.
2.The perioperative hidden blood loss in intertrochanteric fractures: a comparative study

Objective To determine the perioperative hidden blood loss (HBL) associated with different fixation methods for intertrochanteric fracture.Methods We retrospectively studied 128 patients with 31-A1 and 31-A2 intertrochanteric fractures which underwent internal fixation with dynamic hip screw (DHS,n=35),short reconstruction intramedullary hip screw (IMHS,n=55) or percutaneous compression plate (PCCP,n=38) from January 2008 to January 2012.The HBL were calculated and compared.Results The mean total hidden blood loss (THBL) was more than 7 times than the mean apparent blood loss (ABL) in all the three groups.For the THBL,there were significant statistical differences among three groups; no difference was found between IMHS and DHS group.For the phasic hidden blood loss (PHBL),there was no difference among the three phase of treatment in DHS group.But in IMHS and PCCP group,there were significant differences between the 4th-5th day and the 1st day,the 2nd-3rd day after operation; no difference was found between the Ist and the 2nd-3rd day.For the ratio of PHBL/THBL there were significant differences between the 4th-5th day and the 1st day,the 2nd-3rd day after operation; but no statistical differences were found between the 1st day and the 2nd-3rd day after operation.Conclusion It can be concluded that HBL after surgery for intertrochanic fractures is much greater than that observed intra-operatively.The IMHS and DHS cause more HBL than PCCP.Frequent post-operative measurements of haemoglobin are necessary,especially the first three days after surgery.
3.Expression and significance of alpha-smooth muscle actin in the traumatic neuroma

4.An in vitro study of rhBMP-2/ vancomycin/ calcium sulfate drug delivery system

Objective To study the efficacy and safety of in vitro local drug delivery capsules fabricated with the biodegradable material Osteoset(R)plus vancomycin and recombinant human bone morphogenetic protein-2 (rhBMP-2). Methods In vitro local drug delivery capsules were fabricated with the biodegradable material Osteoset(R) plus vancomycin and rhBMP-2.High-Performance Liquid Chromatography (HPLC) and bacterial inhibition tests were performed to detect the concentration and activity of vancomycin in the capsules.Enzyme-linked immunosorbent assay(ELISA) and alkaline phosphatase(ALP) tests were conducted to detect the concentration and activity of rhBMP-2 in the capsules. Results The capsules could release vancomycin above 55.8 μg/mL for up to 144 hours,with an activity of above 70%.They could release active thBMP-2 for up to 30 days,had no inhibition effects on the proliferation of mesenchymal stem cells(MSCs) and had high biological safety. Conclusion Since the capsules of Osteoset(R) plus vancomyein and rhBMP-2 can release high concentrations of active vancomycin and active rhBMP-2 with no inhibition of the proliferation of MSCs,they have good prospects for clinical use.
5.Management of vascular variation in free transplantation of posterior interosseous perforator flap

Objective To discuss the method and treatment outcome of vascular variation in repairing skin defect of the hand by free transplantation of posterior interosseous perforator flap.Methods Eight cases with vascular variation were adjusted flap position,two cases were repaired by single perforator flap,three cases were repaired by cutting perforator and anastomosing perforator after changing the path,three cases were repaired by designing composition leaves flap.Results Seven flaps survived,one flap partly necrosis and healed by skin-grafting,and all of the patients were followed-up 3 to 18 months.The color,texture and thickness of the flaps were satisfactory.The movements of the fingers were satisfactory.According to the hand function evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association,outcomes were rated excellent in 1 case,good in 5 cases,fair in 2 cases.The excellent and good rate was 75%.Conclusion When finding vascular variation in repairing skin defect of the hand by free transplantation of posterior interosseous perforator flap,adjust flap position,repair by single perforator flap,cutting perforator and anastomosing perforator after changing the path,or designing composition leaves flap is a good treatment option.
6.Retransplantation of HCC patients with or without HCC recurrence

Objective To explore the efficacy of liver retransplantation for hepatocellular carcinoma (HCC) patients with or without HCC recurrence.Method 131 cases of retransplantation performed between 2003 and 2012 were analyzed retrospectively.Their first and second liver transplantations were both performed in our hospital.Diagnoses of their primary diseases before transplantations were confirmed pathologically after the first transplantation.Patients were divided into two groups in terms of benign causes and HCC.Results Sixty cases were fallen into benign disease group and 65 cases into HCC group.The proportions of main causes of retransplantation were similar between two groups.The graft survival rate of early retransplantation (retransplantation performed within 30 days after the first transplantation) and late retransplantation (retransplantation performed beyond 30 days after the first transplantation) was calculated and compared respectively due a great difference in survival rate between the two phrases.The deaths of HCC patients with HCC recurrence before retransplantation were more than those without HCC recurrence (P<0.01) and benign disease group.The 5-year cumulated survival rate was close between HCC patients without recurrence before retransplantation (51.0%) and benign disease group (51.8%).Conclusion The retransplantation after HCC recurrence has an unacceptable prognosis.The survival rate was similar between patients without HCC recurrence and patients with benign diseases.HCC patients without recurrence should not be restrained from retransplantation just for the HCC history.
7.Effects of glucocorticoids on fracture healing in rats

AIM: To explore the effects of glucocorticoids on fracture healing in a rat model of tibial fracture. METHODS: Sixty three months old female SD rats were divided into control and glucocorticoid-treated group. A glucocorticoid-induced osteoporosis model was established by intramuscular injection of prednisolone acetate (5 mg·kg~(-1)·d~(-1) for 3 weeks), in which the tibial was osteotomized by a wire saw as fracture healing model and internal fixed with a Kirschner pin. The rats were scarified at different time points after operation. The callus formation was monitored over a period of 6 weeks by histological method, bone mineral density (BMD) detection and biomechanical examination. Western blotting was used to measure the expression of type II collagen. RESULTS: A glucocorticoid-induced osteoporosis model was successfully established and conformed by BMD measurement. The formation of primary callus was observed in both groups 3 days after fracture. At 2 weeks after injury, the glucocorticoid-treated group had a lower BMD and less cartilage matrix as compared to control group. An increase in bone callus and chondrogenesis was observed at 4 to 6 weeks after fracture in glucocorticoid-treated group as compared to control group. The expression of type II collagen was delayed in glucocorticoid-treated group. Biomechanical measurement showed that the actual maximum load was increased by 35.8% in control group as compared to glucocorticoid-treated group at 6th week. CONCLUSION: These results indicate that chondrogenesis and transformation from cartilage callus to bony callus are delayed by glucocorticoids. The retardation of collagen Ⅱ production may be the reason for the inhibition of fracture healing.
8.Autologous and artificial vascular transplantation after vascular injuries to the extremities: A retrospective data study in 44 cases

BACKGROUND: Vascular injuries to the extremities are frequently concomitant with vascular defects. Vascular transplantation repair can induce infection and vascular occlusion, etc.OBJECTIVE: To analyze the outcome of vascular reconstitution in 44 patients with vascular injuries of the extremities undergoing vascular transplantation.DESIGN: A retrospective case analysis.SETTING: Department of Vascular Surgery and Department of Orthopaedics of Hospital Affiliated to Jinhua College of Profession and Technology.PARTICIPANTS: Forty-four patients with vascular injuries to the extremities undergoing autologous and artificial vascular transplantation were selected at the Department of Vascular Surgery and Department of Orthopaedics of Hospital Affiliated to Jinhua College of Profession and Technology from April 1994 to October 2003. There were 29 patients with open injury and 15 patients with closed injury.METHODS: A total of 52 blood vessels were transplanted into 44 patients, including 42 blood vessels in autologous vein transplantation (35 blood vessels in great saphenous vein transplantation by end-to-end anastomosis, 5 blood vessels in small saphenous vein transplantation by end-to-end anastomosis and 2 blood vessels in superficial femoral vein and popliteal vein transplantation) and 10 blood vessels in artificial and trimming vascular transplantation by interrupted suture technique in end-to-end anastomosis.MAIN OUTCOME MEASURES: Outcomes of autologous and artificial vascular transplantation.RESULTS: Three patients received amputated extremity. Six patients developed ischemic contracture. Seven patients developed imperfect recovery of nerve function. In other patients, blood flow in the graft was satisfactory, and there was good condition of blood circulation at the distal extremities.CONCLUSION: Autologous vein is the first choice in vascular transplantation, and prosthetic material is another choice when necessary. It is important to prevent the occurrence of complication after transplantation such as vascular infection.
9.Biomechanical effect of sacroiliac screw fixation with different needling, points on extorsion unsta-hie pelvis

Objective To observe the biomechanical effect of sacroiliac screw fixation with dif-ferent needling points in treatment of extorsion pelvic instabihty so as to provide experimental basis for clinical application. Methods Ten fresh adult pelvis specimens were fixated by sacroiliac screw with needling points B (2.5 cm anterior to posteroinferior iliac spine and 40. cm posterior to greater sciatic notch), A (0.1 cm anterior to the same horizontal plane of point B) and C ( 1.0 cm posterior to the same horinzontal plane of point B). The shifting of the pubic symphysis diastasis and the sacroiliac joint crack under the same extorsion force was measured in integrity ligament group, point A group, point B group and point C group to compare the stability of different needling points. Results In confronting extor-sion force, integrity ligament group and point C group were obviously inferior to point A group and point B group (P<0.05 ) but there was no statistical difference between point B group and point A group (P > 0.05). As for the stability of the sacroiliac joint, there was statistical difference among all groups ( P < 0.05 ), while there was no statistical difference between integrity ligament group and point B group ( P > 0.05). Conclusion Point B is the most stable, safe and effective needling point.
10.miR-125b increases sensitivity of CD133+ colorectal cancer cells to cisplatin by down-regulating HAX-1 expression

AIM:To investigate the role of miR-125b in regulating the sensitivity of CD133+ colorectal cancer cells to cisplatin.METHODS:The expression of miR-125b was detected by RT-qPCR in the routine SW480 cells and CD133+ SW480 cells.Flow cytometry analysis was performed to measure the percentage of CD133+ cell population in the SW480 cell line treated with miR-125b and cisplatin.MTT assay was performed to evaluate the effect of miR-125b on the cisplatin-induced cell death in the CD133+ SW480 cells.Bioinformatics and Western blot were performed to determine whether the expression of HAX-1 was regulated by miR-125b.JC-1 staining, Annexin V staining and Western blot analysis were used to study the pathway of apoptosis in the CD133+ SW480 cells co-treated with miR-125b and cisplatin.RESULTS:The expression of miR-125b was significantly lower in the CD133+ SW480 cells than that in the routine SW480 cells and normal colonic epithelial FHC cells.Treatment with cisplatin alone increased the percentage of CD133+ SW480 cell population.However, miR-125b significantly inhibited the enrichment of CD133+ cell population induced by cisplatin.In addition, the results of MTT assay showed that the anti-tumor effect of cisplatin was significantly enhanced when the miR-125b was transfected into the CD133+ SW480 cells.The results of Western blot indicated that the HAX-1 gene was the target of miR-125b.Furthermore, the apoptosis induced by the combination of miR-125b and cisplatin was dependent on the dysfunction of mitochondrial membrane, leading to the release of cytochrome C into the cytoplasm and the subsequently activation of apoptosis in the CD133+ SW480 cells.CONCLUSION:miR-125b increased the sensitivity of CD133+ colo-rectal cancer cells to cisplatin by down-regulating the expression of HAX-1.
