1.Philosophical pondering upon merging traditional Chinese medicine into systemic medicine

Traditional Chinese medicine is one of the component parts of the international health care system. It has been an inevitable trend to merge traditional Chinese medicine into systemic medicine. Along with the emergence of the concept of systemic disease, the wide dissemination of evidence-based medicine and the establishment of thought pattern of systemic medicine, the exploration emphasis of modern medical science has been transferred from microcosmic or solid structure to macrocosmic unit and system relation. The mainstream development of modern medical science is to integrate different medical systems into a complete system. Therefore, the integration of traditional Chinese medicine and Western medicine is also necessary under this condition.
2.Effects of fosinopril on apoptosis and apoptosis genes in vascular smooth muscle cells

AIM : To investigate the effects of angiotensin II (AngII) in different concentrations and different action times on the rate of apoptosis and expression of apoptosis genes in vascular smooth muscle cells and the influences of fosinopril on it. METHODS : Flow cytometer was used to measure the apoptosis rate and the expression of apoptosis genes (Fas and Bcl 2) induced by AngII in different concentrations and different action times and the influence caused by fosinopril. RESULTS BZ : The rate of apoptosis and the expression of apoptosis genes were induced and increased by AngII with the increase of concentrations and action times compared with that in the control group. The expression of Fas was increased and Bcl 2 was decreased with the increase of the concentrations and action times of AngII in fosinopril group compared with that in the AngII group. CONCLUSION : The apoptosis rate and the expression of apoptosis genes are induced and increased by angiotensin II with the increase of the concentrations and the action times. Fosinopril has the regulation effect on the apoptosis rate and the expression of apoptosis genes in vascular smooth muscle cells.
3.The different-dose simvastatin's effect on the platelet activity after the elderly patients' treatment by PCI

Objective To observe the serum level of the CD62p,CD63 which indicates the platelet activity of the elderly patients treated by different-dose simvastatin after PCI.Methods Sixty-eight patients with acute coronary syndrome post PCI were randomly divided into 20 mg/d simvastatin group(36 patients)and 40 mg/d simvastatin group(32 patients).We examined the patient's blood CD62p,CD63 levels on the day before the treatment of PCI in both groups.Then all of the patients in both groups received different-dose simvastatin treatment(20mg/d or 40 mg/d qn po);we observed the results again after four weeks in both groups,and compared the results.Results The patients' blood CD62p,CD63 levels decreased significantly after the treatment of simvastatin in both groups(P
5.Clinical features and treatment of 147 cases with severe hand, foot and mouth disease receiving mechanical ventilation

Objective To summarize the clinical features,diagnostic and treatment experience of severe hand,foot and mouth disease(HFMD) cases receiving mechanical ventilation from Jan 1 to Sep 6,2009 in our hospital and provide reference for reducing the occurrence of neurogenic pulmonary edema(NPE) and mortality. Methods 147 severe HFMD who received mechanical ventilation were analyzed by a retrospective investigation. Results 85.0% children were less than 3 years old and 100% patients had a fever. The mechanical ventilation occurred within 1 to 4 days after fever (3.06 ± 1. 02) d. Neurological complications presented as bad spirit,easy surprised, involuntary movement of the extremities and lethargy. Respiratory system complication presented as polypnea, hypepnea and irregular rhythm. Circulatory system complication presented as heart rate increase or decrease, hypertension or hypotension, piebald skin and low limb temperature.Some of the children had high white blood cell counting,glucose and lactic acid of the blood. The EV71-PCR positive rate was 34. 7% for throat swabs,and 42. 2% for anal swabs. Chest X-ray presented as increased broncho vascular shadows, exudation or nonapparent abnormality. Only three children were dead, fatality rate was 2. 0%. Conclusion The HFMD cases in this outbreak are relatively more serious,and change rapidly. It's difficult to treat as soon as NPE appeared,and it has high fatality rate. Early recognition of critical patients,timely giving mechanical ventilation,and the comprehensive treatment can reduce NPE incidence and the mortality.
6.Clinical pathological features of umbilical metastases of colorectal carcinoma

Objective To study the clinical pathological features, treatment and prognosis of umbilical metastases of colorectal carcinoma. Methods From January 2000 to September 2010, 10 umbilical metastases of colorectal carcinoma cases were admitted. The clinical features were reviewed. Results Four radical resection of colon cancer and resection of the umbilical plexus metastases cases with chemotherapeutic intraperitoneal perfusion lived 9,11,14 and 18 months respectively, 1 ileum-transverse colon anastomosis case with chemotherapeutic intraperitoneal perfusion lived six months, 2 patients with systemic widely transfer and umbilical transfer with pure venous chemotherapy lived 3, 3.5 months respectively, 3 colon intra-operative cases with venous chemotherapy lived 5 ,5. 5 and 7 months respectively. Conclusion Surgical resection of the primary focal and periumbilical metastases can prolong survival time with adjunctive therapy.
7.Effects of Common Percutaneous Enhancers on Transdermal Permeability of Cetirizine Hydrochloride

OBJECTIVE:To study the effects of common percutaneous enhancers on transdermal permeability of cetirizine hy-drochloride(CET). METHODS:Using pH 7.4 phosphate buffer as receiving solution,accumulative permeation rate as index,mod-ified Franz diffusion cell was used to investigate the effects of common percutaneous enhancers on transdermal permeability of CET in skin of abdomen isolated from mice,such as azone (0.06%-2.5%),oleic acid (0.5%-10%),1,3-propanediol (0.5%-10%),propanetriol(0.5%-5%). RESULTS:1,3- propanediol had no improvement effect on the permeability of CET. 24 h accu-mulative permeation amount of CET in 0.06% azone,0.5% oleic acid and 1% propanetriol were the highest among same types, being 227.94,85.25 and 215.15 μg/cm2,respectively. CONCLUSIONS:Of four percutaneous enhancers,the transdermal enhanc-ing efficiencies from high to low are azone>glycerol>oleic acid>1,3-propanediol. Azone and glycerol have the conspicuous pene-tration enhancing effects.
8.Effects of rehabilitation garden on hemiplegic stroke patients

Objective To observe the effect of the rehabilitation garden on hemiplegic stroke patients. Methods 160 cases of cerebral vascular accident patients were randomized into observation group and control group. All patients were given conventional drug treatment and conventional rehabilitation training. The observation group was given the rehabilitation garden for rehabilitation training two times a day , each time 60 minutes. Results BBS, BI, FMA, HAMA and HAMD of The two groups of patients significantly improved. The improvement ofBBS and BI in observation group was better than that in control group (P < 0.05). HAMD and HAMA score inobservation group was significantly higher than that in control group (P < 0.01), the improvement in observationgroup is better than in control group (P < 0.01), the satisfaction rate in observation group is better than in controlgroup (P < 0.01), and the hospital stay period in observation group is better than in control group (P < 0.01).Conclusion The application of rehabilitation garden has a good effect on the rehabilitation effect of the hemiplegicstroke patients.
9.Mortality and burden of non-communicable diseases in China

Objective To assess the leading causes of death and disability adjusted life year (DALY) due to non-communicable disease (NCD) in China.Methods World Health Report 2004 published by the World Health Organization (WHO) was reviewed,including total deaths,deaths per 100 000,agestandardized death rate per 100 000,total DALYs,DALYs per 100 000 and age-standardized DALYs per 100 000 by cause and by member state.Diseases or injuries were assigned to three levels:communicable diseases,NCD and injuries (the first level); categories of disease or injure (the second level); specific diseases (the third level).R2.15 was used for data analysis.Results NCD causes 737.6 million deaths,141million total DALY years,627 age standardized mortality per 100 000,and 10 829 age-standardized DALYs per 100 000.NCD account for 79.4% and 70.3% total death or all-cause DALYs.Conclusions Cardiovascular disease,malignant neoplasm and respiratory disease were the leading causes of death,while neuropsychiatric disorder,cardiovascular disease and sense organ disease were the most important causes of DALYs.Among China,the United Kingdom,the United States,Canada,Japan,Korea and India,China ranked second in age-standardized mortality rate of chronic disease.DALYs of esophagus cancer and chronic obstructive pulmonary disease were 6 or 2 times of world average level.Besides,the increasing trend in the prevalence of diabetes mellitus remains impressive.
