1.Analysis of causes of unplanned extubation in ICU patients with deep venous catheter and nursing strategies

[Objective] To analyze the reasons of unplanned extubation(UE)of deep venous catheter (DVC)and explore the prevention strategies.[Methods] The experiences and lessons of maintenance of deep venous catheter of 410 patients were retrospectively reviewed,causes of unplanned extubation in 25 cases were analyzed.[Results] Catheter obstruction,accidental pulling-off of eatheter,catheter-related infections,thrombosis formation,skin allergy and other complications were the main reasons for unplanned extubation.These complications were mainly related to patients,medical staff factors and catheter sites or other relevant situations.[Conclusions] The measures such as fully understanding and correct analysis of unplanned extubation reasons,assessment of risk factors,actively seeking effective nursing intervention measures to ensure the standardization of the nursing practice can significantly reduce the rate of unplanned extubation.
2.Clinical application of serum gastric function detection in early gastric cancer screening

Objective To investigate the clinical application value of serum gastric function detection in early gastric cancer screening.Methods Forty-two cases of gastric cancer,75 cases of benign gastric diseases and 44 individuals undergoing normal physical examination in our hospital from December 2015 to December 2015 were selected as the research subjects,performed the gastroscopic examiantion,simultaneously conducted the microscopic biopsy,and extracted blood for conducting the related indexes detection of gastric function,including pepsinogen Ⅰ (PG Ⅰ),pepsinogen Ⅱ (PG Ⅱ),PG Ⅰ/PG Ⅱ,gastrin 17 (G17) and helicobactor pylori(HP).Serum gastric function situation in the research subjects of 4 groups was studied.Results PG Ⅰ and PG Ⅰ/PG Ⅱ expressions from low to high were gastric cancer group,atrophic gastritis group,gastric ulcer group,superficial gastritis group and normal group respectively,the pairwise comparison had statistical difference (P<0.05);G17 in the normal group was lower that that in gastric ulcer group,atrophic gastritis group and gastric cancer group G17 in the gastric ulcer group was lower than that in the gastric cancer group,the differences were statistically significant (P<0.05).The HP positive rate in the normal group was lower than that in the superficial gastritis group,the superficial gastritis group was lower than the gastric ulcer group,the gastric ulcer group was lower than the atrophic gastritis group,and the atrophic gastritis group was lower the gastric cancer group,the differences had statistical significance (P<0.05).Conclusion Serum gastric function detection can better distinguish gastric benign lesions from malignant lesions,and can serve as an important indicator of early screening,which is easy to accept with low cost,for the patients with positive results,the gastroscopic examination should be conducted in order to improve the early diagnostic rate.
3.Myocardial protection by retrograde cold cardioplegic solution through right atrium in the presence of coronary artery obstruction

This experiment was done to compare the effects of antegrade and retrograde cold cardioplegic solutions on myoeardium in the presence of coronary obstruction. Twelve mongrel dogs were anesthetized with intravenous pentobarbital, and immediately after the left anterior descending of coronary artery (LAD) was tied off, all subjects were randomly allocated to receiving antegrade cardioplegic solution (4 C) through arotic root at initial dose of 20ml?kg~(-1) and supplemental dose of 10ml?kg~(-1) with perfusion pressure being 10.7kPa every 20 minutes (group n=6), or antegrade cardioplegic solution in the same way as mentioned above at initial dose of 10ml?kg~(-1) and retrograde cardioplegic solution at initial dose 10ml?kg~(-1) and supplemental dose of 10ml?kg~(-1) with perfusion pressure being 5.3kPa every 20 minutes (group Ⅱ, n=6), respectively. 4.Application of intravenous drug allocation center combined with mobile transfusion management system in intravenous infusion for outpatients

Objective To study the effect of pharmacy intravenous admixture service(PIVAS)combined with mobile transfusion management system on the intravenous infusion for outpatients.Methods Six hundred patients hospitalized from January to December 2011 were set as the control group,and another 600 patients from January to December 2012 as observation group.The control group were managed with PIVAS management mode and the observation group with PIVAS combined with mobile transfusion management system management mode.The two groups were compared in terms of transfusion reaction,transfusion errors,nurse-patient disputes, transfusion check errors,no response from patients at call and patient’s satisfaction.Results There were no significant differences between the groups in transfusion reaction and transfusion error rate( P>0.05).The rates of nurse-patient disputes,transfusion check errors,no response from patients at call and patient's satisfaction in the observation group were significantly lower than those in the control group(all P<0.05).Conclusion PIVAS combined with mobile transfusion management system can not only provides pharmaceutical care safety quality for outpatient’s infusion,but also simplify the infusion process and improve the work efficiency and patient’s satisfaction with nurses.

6.Expression and significance of Survivin in esophageal carcinoma

Objective To investigate the expression and clinical significance of Survivin in esophageal car-cinoma.Methods Survivin expression was detected by immunohistochemistry in 61 cases of esophageal squamous carcinoma tissues and 26 tumor-adjacent normal tissues.Results The positive Survivin expression rate was 60.7% (37/61)in tumor tissues.The expression of Survivin was significantly correlated with the tumor grading,infiltration, lymph node metastasis and the survival(P<0.01).Conclusion Survivin may be an important marker for tumor di-agnosis and it is of great important to the active treatment of high-risk patients and to increasing of prognosis values.
7.The plasma amino acids biological reference intervals of L-8900 amino acid analyzer

Objective To calculate the biological reference of plasma amino acid on L-8900 amino acid analyzer and to provide reference for clinical diagnosis with domestic reagents replacing original reagent .Methods By testing the original standards and the same batch of plasma (50 cases) ,we compared original reagents with domestic reagents for their performance (including resolution , repeatability ,accuracy ) .We tested the plasma free amino acids of 400 cases of healthy people using the domestic reagents to estab-lish biological reference interval of plasma amino acid ,and do correlation analysis between the amino acids level and liver function . Results (1)Domestic reagents showed high accuracy in the results of 5 consecutive detection of amino acids were high peak separa-tion and high peak retention time and high peak area .(2)Statistics derived biological reference interval of 19 amino acids and 10 kinds of amino acids had significant differences .(3)Correlation analysis showed that ALT and liver function were negatively corre-lated with threonine(P<0 .05) .GLU and valine ,isoleucine ,leucine were positively correlated(P<0 .05) .CHO and negatively cor-related with isoleucine(P<0 .05) .Conclusion Domestic agents can replace the original reagents ,on the basis ,the biological refer-ence intervals of plasma amino acids have great importance to clinical diagnosis and prognosis .
8.Protective effects of pioglitazone against lipopolysaccharides-induced neurotoxicity in cultured cortical neurons in rats

AIM To investigate whether pioglitazone can protect cortical neurons from lipopolysaccharides(LPS)-induced neurotoxicity and the mechanisms responsible for this protective effect. METHODS After 7 d cultures,cultured cortical neurons were incubated with LPS 10 mg·L~(-1) for 4-24 h with or without other drugs. In co-incubation experiments, other drugs were added to the neurons 30 min or 1 h prior to incubation with LPS. The cell viability was assessed by MTT assay. The neuronal apoptosis was quantified by scoring the percentage of cells with apoptotic nuclear morphology after Hoechst 33258 staining. The cultured cells were then fixed on the 7th day and immunocytochemically stained with phosphorylated JNK1 antibody. The protein expressions of active caspase 3 and phosphorylated JNK1 were measured by Western blot. Nitric oxide (NO) generation was measured by Griess method. RESULTS The decrease of cell viability and the increase of apoptotic cells in cultured cortical neurons were observed incubated with LPS for 24 h compared with the normal controls. The cell viability of cortical neurons was decreased from (100.0±10.9)% in the normal control group to (72.3±2.1)% in the LPS-treated group and the apoptotic cell percentages were increased from (11.5±4.2)% in the normal control group to (39.5±8.2)% in the LPS group. LPS induced the increases in phospho-JNK1, active caspase 3 expression, and NO generation. Pioglitazone 0.01, 0.1 and 1 μmol·L~(-1), respectively inhibited LPS-induced decrease in cell viability and increase of apoptotic morphology, active caspase 3 expression in cultured neurons. In LPS+pioglitazone 1 μmol·L~(-1) group, cell viability was (97.8±9.7)%, the apoptotic cells percentage was (20.6±5.0)%, NO generation (6.8±1.3)μmol·L~(-1). Furthermore, pioglitazone also inhibited LPS-induced the increase in JNK1 phosphorylation and NO generation. JNK inhibitor SP600125 5 μmol·L~(-1) significantly inhibited LPS-induced neurotoxicity, cell viability was increased from (72.3±2.1)% to (109.8±11.8)%, the apoptotic cells percentage from (39.5±8.2)% decreased to (19.1±4.8)%, NO generation from (21.1±5.0)μmol·L~(-1) decreased to(4.0±1.3)μmol·L~(-1). The PPARγ antagonist GW9662 10 μmol·L~(-1) did not reverse the effects of pioglitazone. In LPS+pioglitazone 1 μmol·L~(-1)+GW9662 10 μmol·L~(-1) group, cell viability was (90.7±6.9)%, the apoptotic cells percentage was (23.4±4.1)%, and NO concentration was (5.8±0.7)μmol·L~(-1). CONCLUSION Pioglitazone protects cortical neurons against LPS insult at least via inhibiting JNK activity and NO generation, but not PPARγ activation.
9.Diagnosis and treatment of bilateral bronchial foreign body in children.

OBJECTIVE:
To discuss the clinical characteristics of bilateral bronchial foreign body in children, increase the curative rate and reduce the complications.
METHOD:
The clinical data of 35 cases with bilateral bronchial foreign body in children were retrospectively analysed, including surgery, key surgery points, and postoperative combined therapy.
RESULT:
The foreign bodies of all cases were removed under intravenous general and tropical anesthesia without complications.
CONCLUSION
The bilateral bronchial foreign body is a serious case, the timely and effective treatment can lower the mortality rate and postoperative complications. The children lack of oxygen for a long time before and in operation should be give comprehensive therapy, for example: sedation and hyperbaric oxygen.

10.Colorimetric Detection of Copper Ions Based on Surface Modification of Silver/Platinum Cluster Nanoenzyme

A sensitive colorimetric method for the detection of copper ions (Cu2+) was developed based on the surface modification of silver/platinum nanoclusters (Ag/Pt NCs) and regulation of peroxidase-like activity. It was found that 3-mercaptopropionic acid (MPA) could inhibit the catalytic ability of Ag/Pt NCs; however, it lost the inhibition toward catalytic ability of Ag/Pt NCs after oxidized by oxygen through the catalysis of Cu2+. On the basis of this, a colorimetric method was developed for the detection of Cu2+ through measuring the colorimetric signal variation of the TMB-H2O2 reaction. This method exhibited high sensitivity and selectivity toward Cu2+ over a panel of other metal ions. The linear range was 10-100 nmol/L and the detection limit was 5.0 nmol/L (3σ). The above method was also applied to detect real water samples and spiked samples, and the results demonstrated that this method was simple with low cost.
