1.The Effects of Home Nursing on Patients with Benign Prostatic Hyperplasia

Objective To study the effects of home nursing on patients with benign prostatic hyperplasia. Methods One hundred and eighty patients with benign prostatic hyperplasia meeting our experimental conditions were selected. They were divided into experimental group and comparative group. The experimental group had 82 cases and the average age was 68.5 ?1.2 yrs, the comparative group had 98 cases and the average age was 66.8?0.8. The first prevalence survey was carried out including history of past illnesses, international prostate symptom score, quality of life assessment, customs of life, habits of diet, circumstances of medicine and statuses of psychology. The patients of experimental group received home nursing including psychology nursing, directions of medication, reformations of customs in life and diet, strength in body exercise and kinesitherapy. The patients of comparative group did not receive any nursing measures. After two, four, six months, three prevalence surveys were carried out. At the end of each time, the data were collected and statistical treatments were processed. Results The IPSS rates of improvement in experimental group were 14.6% , 31.7% , and 52.4% . The QOL rates of improvement in experimental group were 12.2% , 34.2% , 57.3% . The IPSS rates of improvement in comparative group were 4.0%, 10.0%, 18.4%. The QOL rates of improvement in comparative group were 3.1%, 13.3%, 16.3%. AIM:To study the relationship between the genetic polymorphism of CYP3A5 gene and the blood concentration/dose(C/D) ratio of cyclosporine combined with calcium channel blockers(CCBs) in renal transplant patients.METHODS:Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) analysis was applied to determine the genotype of CYP3A5 in 69 renal transplant patients.The blood concentration/dose(C/D) ratio of cyclosporine was compared in patients coadministrated of CCBs with different genetic polymorphism of CYP3A5.RESULTS:The cases of genotype of CYP3A51/1,CYP3A51/3 and CYP3A53/3 were 8,23 and 38,respectively.The mutation rate of CYP3A53 in renal transplant patients was 71.74%.The CsA C/D ratio of patients of CYP3A51/1 and CYP3A51/3 were 1.73 and 2.43 times higher than that in patients of CYP3A53/3.The CsA C/D ratio of patients(CYP3A51/1,CYP3A51/3) combined with CCBs had been obviously elevated(19.90?7.66 vs 36.01?8.18,34.33?7.87 vs 46.16?9.63 ng?mL-1 per mg?kg-1;P
3.A sensitive radioimmunoassav for morphine in a single hair

Objective The paper described a convenient and sensitive radioirnmunoassav method which can be used to measure the amount of morphine in a single hair. Method Succinvl-eggalbumin-morphine was used as immunogen to immunize New Zealand rabbits for obtaining high quality antiserum. The morphine- labeled with Iodine-125 was purified by HPLC. The radioimmunoassay method was established and the amount of morphine in a single hair was detected. Results The affinity constant of antibody is 3.25 ? 10~11L/M. The radiochemical purity of I-mophine is 95% and the specific radioactivity is 112?Ci/?g. The detection limit for morphine (3SD from zero-dose binding) is 0.01ng/ml. Five normai person's single hair and five addict's were detected . The length of these specimens is 9-24cm and the weight of them 0.7- 2. 1mg. The amount of morphine in the hair of normai person is 1. 75 ? 0.37ng/mg (x ? 5) and that of the addictos'is 471 ?204ng/mg (x ? s). Conclusion This method can be employed for accurate mea-surement of the amount of morphine in a single hair and the result can be adopted to indicate the probability of illicit-drug use.
4.Clinical analysis of cerebral angio spasm after aneurysm rupture subarachnoid hemorrhage by Fasudil

Objective To explore the clinical curative effect of cerebral angio spasm after aneurysm rupture subarachmoid hemorrhage by Fasudil, Methods The clinical data of cerebral angio spasm after aneurysm rupture subarachnoid hemorrhage were retrospectively analyzed, which was divided into detection group and control group.Results The GCS score of detection group was better than control group;the average blood flow rate of arteria cerebri media after curing in detection group the difference had statistical significance( all P <0. 05 ). The mutilation rate、rate of death of detection group were lower than control goup, the difference had statistical significance ( P < 0. 05 ).Conclusion The clinical curative effect was good of cerebral angio spasm after aneurysm rupture subarachnoid hemorrhage by Fasudil, and prognosis was good.

AIM: To determine the changes of the serum levels of tumor necrosis factor ? (TNF-?), interleukin-6 (IL-6), IL-10, C reactive protein (CRP) and D-dimer in the patients with multiple organ dysfunction syndrome (MODS) and compare the relationship between the levels of cytokines in early stage and MODS. METHODS: The serum values of TNF-?, IL-6, IL-10, CRP and D-dimer were measured in 27 patients with MODS in 1 d, 3 d and 5 d after undergoing disease, and compared with the adult peripheral blood of 15 normal controls. The levels in the first undergoing day between the lived group (n=19) and died group (n=8) were compared. Objective To study the default mode network (DMN) of mild cognitive impairment (MCI) related to leukoaraiosis (LA) with resting-state functional magnetic resonance imaging (rs-fMRI). Methods 31 LA patients (Clinical Dementia Rating of 0.5) and 27 normal controls (Clinical Dementia Rating of 0, and matched in age, gender and lever of education) were scanned with the rs-fMRI. The data was analyzed with SPM5 software, using independent component analysis. The differences between the both groups were compared with two-sample t-test. Results The DMN during resting-state of normal controls was posterior cingulate cortex/precuneus, bilateral medial frontal cortex, bilateral middle temporal gyrus, bilateral inferior parietal gyrus, angular gyrus, bilateral hippocampus. The DMN of MCI group was consistent with the normal controls, but the activation decreased in anterior cingulate cortex/left medial frontal lobe, right parahippcampus/ uncus, right inferior temporal gyrus, left deep frontal white matter/head of caudate nucleus; and increased in the left caudate nucleus/anterior cingulate cortex, left frontal lobe, and left superior temporal gyrus/inferior parietal gyrus. Conclusion Activation of resting-state functional network is disorder in LA, which may relate to cognitive impairment.
7.Study on correlation between MR spectroscopy and whole pathologic seetion in benign prostatic hyperplasia

Objective To investigate the metabolic characteristics,clinic values of different tissues in benign prostatic hyperplasia by correlating MR spectroscopy(MRS)with whole pathologic sections.Methods MRS presentation was studied in 16 patients with benign prostatic hyperphsia.The peak values of citrate,choline and creatine were measured,and the mtios of(choline+creatine)/citrate(cc/c)were obtained.After suprapubic transvesical prostatectomy,the specimens of whole pathologic sections were observed.The values of cc/c of different pathologic types were compared statistically with One.way-ANOVA and t test. Results MRS results were obtained in 578 units,which included 233 glandular tissue units.130 matrical tissue units.165 mixed tissue units and 50 tissue units around urethra.The ratios of (choline+creatine)/citrate of glandular,matrical.mixed tissues and units around urethra were 0.59±0.29.1.23±0.38.0.80±0.27 and 0.68±0.33(F=3.49.P<0.05).The t tests were carried out between every two tissues.The t valUes between glandular tissues and matrical.mixed tissues were 19.60 and 7.09.P<0.05:the t values between matrical tissues and mixed tissues.units around urethra were 13.22 and 9.42,P<0.05;the t valUes between mixed tissues and units around urethra was 9.42,P<0.05.Condusions,The ratios of cc/c ofthree prostate tissues were different in MRS.The metabolic characteristic of the units around urethra Was near to glandular units.MRS may be valuable to the distinctive diagnoses of prostate diseases.

Objective To evaluate the significance of color Doppler ultrasound examination of renal blood flow combined with the detection of bone morphogenetic protein-7(BMP-7)in early diagnosis of type 2 diabetic nephropathy.Methods Blood BMP-7 level was tested in 90 patients with type 2 diabetic nephropathy and 30 controls,and parameters of renal blood flow were measured by color Doppler ultrasound examination.Blood BMP-7 level as well as resistant index(RI)of segmental renal artery(SRA)and interlobar renal artery(IRA),were compared between these two groups.Results Compared with controls,blood BMP-7 level gradually decreased with the aggravation of diabetic kidney damage(P<0.01).The peak systolic velocity(Vmax)and the end diastolic velocity(Vmin)of SRA and TRA were slowed gradually,while RI increased(P<0.01).Blood BMP-7 level was negatively correlated with IRA's and SRA's RI of IRA and SRA(r =-0.603,P<0.01;r =-0.652,P<0.01).Conclusions Color Doppler ultrasound examination of renal blood flow combined with detection of BMP-7 might play an important role in early diagnosis of type 2 diabetic nephropathy.

Objective To investigate the detection rate of abnormal cardiovascular and metabolic indicators in health check-up populations,and provide evidence for early screening and health interventions for cardiovascular and metabolic diseases.Methods A total of 5578 adults (male 3404 and female 2174) were recruited from a medical institution in Beijing in 2008 and 2010.Cardiovascular and metabolic indicators,including blood pressure,lipid profile and fasting plasma glucose,were tested at baseline and 2 years.Paired t test or paired rank sum test was used for data analysis.Results Detection rate of abnormal blood pressure in 2010 was lower than that in 2008; however,rates of abnormal total cholesterol,low-density lipoprotein cholesterol and fasting plasma glucose were higher in 2010 than in 2008 (t values were 5.378,13.885,3.210,10.401 and 4.355,respectively; all P < 0.05).High-density lipoprotein cholesterol and triglyceride showed no statistically significant difference between 2008 and 2010 (t values were 1.204 and -0.012,respectively; both P > 0.05).Conclusion We have been observing a dynamic change in abnormal cardiovascular and metabolic indicators,based on which effective interventions could be taken to reduce the risk of cardiovascular diseases.
10.The effects of gait training with a gait orthosis on the neurogenic bladder after spinal cord injury

Objective To explore the effects of gait training with an alternative gait orthosis (AGO) on the neurogenic bladders of patients with spinal cord injury (SCI). Method Twelve patients with complete SCI at the thoracic and lumbar level were trained for 8 weeks on motor and bladder function. After that, specific gait training with an AGO was given for another 8 weeks. American Spinal Injury Association ( ASIA ) sensation and motor function assessments were done before and after the gait training with the AGO, and bladder capacity, residual urine and urine flow rates were also assessed. Results Compared with pre-treatment there was no significant difference in average ASIA senzsation and motor scores after training, but the maximum rate of urine flow, average urine volume and single urination had significantly increased and residual urine volume had significantly decreased. Conclusions Therapeutic gait training with an AGO can improve bladder function after SCI.
