1.Breakdown of health resources efficiency and analysis of inter-provincial differences in China

Objective To analyze the inter-provincial utilization efficiency of health resources and master the differences between these regions so as to rationalize the allocation of such resources.Methods This paper called into play the data envelopment analysis to survey the efficiency of interprovincial health resourcas utilization,and the Malmquist productivity index method for further breakdown of the total factor productivity.Results Years 2003 to 2008 averaged an yearly growth of 6.8% for the total factor productivity of health resources in 31 provinces;the rate of technological progress was positive,while the annual rate averaged 5.2%;a slight growth of technical efficiency at the growth rate of 1.5%.Conclusion Years 2003 to 2008 saw an increase of varying extent in the total factor productivity,yet an insufficient utilization of health technology in the provinces in general.Compared to the efficiency of the central provinces,low efficiency was found in both the eastern and western regions in China.The westem regions made a faster progress in technology utilization,thus narrowing the gap of health resources among the provinces.
2.Effects of sevoflurane anesthesia on renal blood flow in dogs

Eight mongrel dogs were involved in the investigation of sevoflurane effects on renal blood flow (RBF). After intravenous anesthesia and surgical preparation, RBF, MAP, HR, CVP, CO and SV were recorded as baseline. Sevoflurane was then administered to achieve end-tidal concentrations of 1, 2, 3 and 4% in succession. Following 20 mins equilibration at each concentration, and 30 mins after the termination of inhalation, the above parameters were measured repeatly for comparing. Results showed significant dose-related depressions of RBF, MAP, HR and CO with sevoflurane. RBF didn't reduce significantly until MAP decreased to 9.3kPa, with end-tidal concentration of sevoflurane no less than 3%, it didn't recover in step with improved hemodynamic parameters after sevoflurane withdrawal. This study indicates that sevoflurane may influence little on autoregulation of RBF, its dose-related cardiac depression may be responsible for the reduction of RBF.
3.An improved association-mining research for exploring Chinese herbal property theory: based on data of the Shennong's Classic of Materia Medica.

Knowledge Discovery in Databases is gaining attention and raising new hopes for traditional Chinese medicine (TCM) researchers. It is a useful tool in understanding and deciphering TCM theories. Aiming for a better understanding of Chinese herbal property theory (CHPT), this paper performed an improved association rule learning to analyze semistructured text in the book entitled Shennong's Classic of Materia Medica. The text was firstly annotated and transformed to well-structured multidimensional data. Subsequently, an Apriori algorithm was employed for producing association rules after the sensitivity analysis of parameters. From the confirmed 120 resulting rules that described the intrinsic relationships between herbal property (qi, flavor and their combinations) and herbal efficacy, two novel fundamental principles underlying CHPT were acquired and further elucidated: (1) the many-to-one mapping of herbal efficacy to herbal property; (2) the nonrandom overlap between the related efficacy of qi and flavor. This work provided an innovative knowledge about CHPT, which would be helpful for its modern research.
4.Research on Shanghai resident satisfaction of primary healthcare after new health reform

Objective The research was to achieve Shanghai resident satisfaction of primary health service after new health reform.Methods During self-compiled questionnaire survey,the research adapted stratified random sampling to select 9 community health centers in Shanghai urban,rural and suburb areas.Results Shanghai resident satisfaction rate of new health reform was 85.2%.Satisfaction rate of basic medical security system was 85.0%.Satisfaction rate of basic medical service was 96.5%.Satisfaction rate of basic public health service was 95.1%.Age,region,job,education and income were main factors to satisfaction of primary health service.Conclusion From an overall point of view,Shanghai resident satisfaction of primary health service after new health reform was high.However,the level of basic medical security and convenience of reimbursement were not high.Medical expenses of primary health care institution had not been effectively controlled.Basic medicine could not fully meet the medication needs of primary health care institutions.
5.Analysis of voluntary blood donation among medical staff in Hangzhou medical institutions

Objective To strengthen the voluntary blood donation publicity through the analysis of the voluntary blood donation.Methods The ratio of voluntary blood donation was statistically analyzed between staffs in some Medical institutions in Hangzhou and Ordinary people.Results The ratio of voluntary blood donation in ordinary people is 1.82%,which is higher than the national data (1.05%).The ratio of medical institutions in Hangzhou is 9.54%,which is far higher than ordinary people.The data of provincial,civic and private medical institutions are 10.20%,6.11% and 15.42%,respectively.Conclusion The ratio of voluntary blood donation is different between different level hospitals,and the staffs in medical institutions are more willing to donate blood than ordinary people.
6.Analysis of prenatal diagnosis results of 206 thalassemia families

Objective To summarize the geographical distribution,phenotype and genotype data of 206 thalassemia families underwent prenatal diagnosis to provide information for clinical genetic counseling and avoid the birth of severe thalassemia children.MethodsTotally,206 thalassemia families were collected from Southern Medical University Nanfang Hospital from January 2008 to December 2009.Genomic DNA was extracted from peripheral blood,villus,amniotic fluid or cord blood from the couples or the fetuses.Gap-polymerase chain reaction (gap-PCR) and reverse dot blot (RDB) technology were used to detect the common α and β-thalassemia mutations.DNA sequencing was used to detect the rare mutations.Follow-up visit were done half a year after the fetuses were born. Results The 206 thalassemia families came from 12 provinces and areas across China,including Heilongjiang province.Mutations detected in α-thalassemia families included --SEA/,-α3.7/,-α4.2/,αCS α/ and αQS α/,which were all included in the testing kit. While there were 4 kinds of β-thalassemia mutations,Gγ+ (A γδβ)0,-28(A→C),CD54-58(-TATGGGCAACCCT) and CD37(G→A),could not be identified with routine testing kit. The 57 α-thalassemia families consisted of 11(19.3%) severe thalassemia,induding 8 Bart's hydrops syndrome and 3 Hb H disease,26(45.6%) heterozygote and 20(35.1%) normal infants,and the 149 β-thalassemia majors families consisted of 28 (18.8%) severe thalassemia,82(55.0%) heterozygote and 39 (26.2%) normal infants.Among the β-thalassemia heterozygotes,there was one 13-trisomy.Follow-up visit found that babies with Bart ' s hydrops syndrome (n =8),Hb H disease (n =3),β-thalassemia majors (n =28) and β thalassemia heterozygote combined with 13-trisomy(n=1) were aborted.Conclusions Thalassemia was found in some north area other than south of China,which should be paid more attention by clinicians.Gap-PCR and PCR-RDB technology are effective measures for thalassemia prenatal diagnosis in identifying major thalassemia fetuses before their birth,thus reduce the birth rate of thalassemia baby.But missed diagnosis might exist during the screening,so it is necessary to perform DNA sequencing on those patients with positive symptoms and negative common genetic diagnostic results.At the same time,prenatal diagnosis of chromosomal disorders should not be neglected for high-risk families.
7.The process and inspiration of deinstitutionalization of mental health institutions of USA and other countries

During past 50 years,has achieved a great progress and derived some profound lessons worthy of learning and studying for our country.This article introduces the deinstitutionalization processes of mental health institutions,reinstitutionalization and the inspiration for Chinese mental health reform.Introduced the paper are the reform of deinstitutionalization of mental health institutions in USA and some other countries,and the concept of such a reform,pointing out inspirations of such a reform for mental health reform in China.
8.Effects of radiofrequency ablation combined with percutaneous vertebroplasty by the injection of artificial bone for metastatic spinal tumor

Objective To explore the effects of radiofrequency ablation ( RFA) combined with percutaneous vertebroplasty ( PVP) by the injection of artificial bone for metastatic spinal tumor. Methods Fifty-eight patients with metastatic spinal tumors were divided into bone cement group 1 (n=12),bone cement group 2 (n=16), artificial bone group 1 (n=16), artificial bone group 2 (n=14). The patients in bone cement group were arranged RFA combined with PVP by injection of bone cement,and the artificial bone group were treated with RFA combined with PVP by injection of artificial bone. The vertebral body modality,fractures, and visual analogue scale ( VAS) ,spinal ODI score were compared before operation,1 week,1,3,6,9,12 months of after operation. Results There were 12 patients (42. 9%) with vertebral body deformation in bone cement group,which was more than those in artificial group (6. 7%) (χ2 =8. 4768,P=0. 0036). The VAS or DOI score decreased after operation than those pre-operation in the 4 groups (P<0. 01),but increased in bone cement group after operation of 6, 9 and 12 months than those in artificial group (P<0. 05). Conclusion The combination of RFA and PVP by injection of bone cement or artificial bone is effective to metastatic spinal tumors. The effects of PVP injection with artificial bone maintains longer duration compared to bone cement.
9.Polymorphisms of intercellular adhesion molecule-1 gene K469E and platelet endothelial cell adhesion molecule-1 gene C373G in patients with preeclampsia

Objective To investigate the distributions of genotypic and allelic frequencies of intercellular adhesion molecule-1 (ICAM-1) gene K469E and platelet endothelial cell adhesion molecule-1 (PECAM-1) gene C373G in patients with preeclampsia. Methods Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) and DNA sequencing were used for detecting ICAM-1 gene K469E and PECAM-1 gene C373G genotypes in 110 women with preeclampsia and 110 normotensive pregnant women in comparison with their clinical characteristics. Results The distributions of observed and expected genotype frequencies were consistent with Hardy-Weinberg equilibrium. No significant differences were found in the genotype and allele frequencies of ICAM-1 gene K469E between the two groups (P>0.05), but the CC and the CG genotype frequencies of PECAM-1 gene C373G were significantly different between them (P<0.05). The relative risk for preeclampsia of CG genotype was 1.959 folds of that in CC genotype carriers (OR=1.959, 95%CI:1.090-3.520, P=0.024), and this association still existed after adjustment for age, gravidity, parity and BMI in logistic regression models. The C373G allele frequencies showed no significant difference between the two groups (P>0.05). Conclusions The CG genotype of PECAM-1 gene C373G genetically predispose the carriers to preeclampsia, while ICAM-1gene K469E polymorphisms is not associated with preeclampsia.
