Objective To optimize the extracting process of the mixture of Feining.Methods Orthogonal design was used to optimize extracting process.The content of glycyrrhizicac,Ephedrine and Pseudoephedrin,and the extractive rate of water and n-butyl alcohol were indexes.Results The optimum water-extracting factors were 8 times of water,refluxing and extracting for 1 h,for 2 times.Conclusion The optimal extraction process is precise and repeatable,which can be used in industrial production.
2.Study on Extraction Process of Xuanfei Zhike Mixture

Objective To optimize the extraction process of Xuanfei Zhike Mixture. Methods The preparation process was investigated by L9(34) orthogonal design with content of ephedrine hydrochloride, the extraction rate of n-butanol and total extraction as indexes. The main factors influencing the extraction efficiency such as the amount of added water, the time of extraction and times of boiling were investigated. Results The optimum technical condition was as follows:8 times amount of water, extract 2 times and 60 min for each time. Conclusion The result is reliable. It can be used for mass production.
3.Quality Standard of Xiaofeng Granules

OBJECTIVE:To establish the quality standard of Xiaofeng granules.METHODS:Rehmannia glutinosa,Schizonepetae tenuifolia,Radix et Rhizoma Glycyrrhizae,Rehmanniae glutinosa in the formulation were identified by TLC,and the content of decloxizine hydrochloride in Xiaofeng granules was determined by HPLC.RESULTS:The TLC characteristics were distinctive.At the corresponding position,the sample and its reference substance showed identical color of TLC spots,and there was no interference from negative control.The linear range of decloxizine hydrochloride was 2.45~78.4 ?g?mL-1(r=0.999 9)and its average recovery was 99.11%(RSD=1.30%,n=6).CONCLUSION:The established quality standard can be used for the quality control of Xiaofeng granules.
4.Secondary hepato-biliary malignancy in patients suffering from hepatolithiasis: report of 27 cases

ObjectiveTo investigate the incidence, clinical features and preventive measurement of secondary hepato-biliary neoplasm (HBN) in patients with hepatolithiasis. MethodA clinical retrospective study was made on 27 hepatobiliary malignancy patients found among 556 consecutively admitted cases of hepatolithiasis from Jan. 1996 to Sep. 2000.Results The average incidence of hepatobiliary malignancy in patients with hepatolithiasis is 4.8%. In this series, preoperative definite diagnosis was made in 4 cases (4/27, 14.8%). Atypical liver abscess, refractory biliary system infection, progressive obstructive jaundice, CT or BUS scan abnormal findings, and epigastric masses are all clues to concurrent malignancy in patients of hepatolithiasis. ConclusionThere is high incidence of hepatobiliary malignancy in hepatolithiasis patients. Prophylactic partial hepatectomy for segments with stone and chronic infection may be an important measure to prevent ensuing malignancy.
5.Acupuncture as adjuvant treatment for diabetic foot: a systematic review and meta-analysis

Objective To evaluate the efficacy and safety of acupuncture as adjuvant treatment in the management of diabetic foot (DF).Methods We searched MEDLINE,Embase,CENTRAL,SinoMed,VIP,CNKI and WANFANG database.We also searched the bibliographies of retrieved articles and correlated proceedings and collected the literatures of DF of clinically randomized or quasi-randomized control trials of acupuncture as adjuvant treatment.After the data was extracted independently by 2 reviewers,we performed meta-analysis by using RevMan 5.1 software.Results Seven studies included 626 patients who met the inclusion criteria,and all employed clinical effects as evaluation indicator.Meta-analysis showed that acupuncture as adjuvant treatment could obviously improve the total effectiveness rate(RR =1.29,95% CI:1.20,1.38,P < 0.00001)and the recovery rate(RR =1.92,95% CI:1.60,2.30,P < 0.00001 ).No adverse reactions occured.Conclusions The limited current evidence shows that acupuncture as adjuvant treatment is safe and effective in the treatment of DF.Due to the poor quality of original studies,a prudent choice is suggested.Further studies with high quality and large samples according to CONSORT are also warrant.
6.Change of first-phase insulin secretion and its influencing factor in impaired glucose regulation individuals

Objective To investigate the change of the first-phase insulin secretion in impaired glucose regulation and its influencing factors.Methods The investigation was performed in 1024 subjects who were selected by cluster sampling.The body waist circumference (WC) was examined in these subjects.Oral glucose tolerance test was performed,and fasting plasma glucose,fasting insulin as well as 30-,60-,and 120-minute plasma glucose and insulin after glucose intake were also tested.All the 1024 subjects were divided into 5 groups based on the results of oral glucose tolerance test:normal glucose tolerance group,impaired fasting glucose group,impaired glucose tolerance group,impaired fasting glucos/impaired glucose tolerance group,and diabetes mellitus group.First-phase insulin secretion index (the ratio of change in insulin to change in glucose during the first 30 minutes after glucose ingestion was calculated.Results Compared with normal glucose tolerance group (1.96 ± 1.03),the first-phase insulin secretion index significantly decreased in the impaired fasting glucose group (1.79 ±0.91) (P =0.007),the impaired glucose tolerance group (1.81 ± 0.97) (P =0.007),the impaired fasting glucose / impaired glucose tolerance group (1.59 ± 0.85) (P =0.005),and the diabetes mellitus group (1.30 ± 0.60) (P =0.004).Logistic regression analysis showed that WC was the strongest influencing factor of first-phase insulin secretion (β =0.716,P =0.000).Conclusions The firstphase insulin secretion index has already dropped in the stage of impaired glucose regulation.WC can be a useful indicator for evaluating first-phase insulin secretion.
7.Two cases of pulmonary thromboembolism associated with protein C and protein S deifciency and literature review

To explore the clinical manifestations, diagnosis and treatment of pulmonary thromboembolism associated with protein C (PC)/protein S (PS) deifciency. Two male patients 29 and 26 years old diagnosed with PC deifciency and/or PS deifciency were retrospectively analyzed and related literatures were reviewed. The most common symptoms were pain in the lower limbs with chest pain or decreased vision. Color dopper lfow imaging (CDFI) showed lower deep venous phlebothrombosis. Multislice CT angiography (CTA) revealed pulmonary embolism. The level of serum homocysteine (HCY) increased and the level of plasma PC/PS content decreased to PC 57.4%, and PS 28.9%in patient 1, while PS 33.4%in patient 2. Poor routine anticoagulant response was observed. After the diagnosis of PC/PS deficiency, vitiamin B6 and B12 anticoagulant therapy was added, and the symptoms in the patients improved significantly. Congenital thrombophilia should be taken into consideration for young patients with lower deep venous thrombosis and pulmonary embolism which occur recurrently without obvious predisposing causes before 40. Plasma PC/PS concentrations or activity help a lot in the diagnosis and treatment.
8.Determination of T Lymphocytes and Trace Elements in Spleen from Rats Infected with Toxoplasma gondii

Objective To determine the level of five trace elements(Fe 2+ ?Cu 2+ ?Zn 2+ ?Ca 2+ ?Mg 2+ )in the spleen and changes of T lymphocyte and its subtype variations in peripheral blood from the rats infected with Toxoplasma gondii . Methods Twenty rats were randomly and equally divided into two groups: control group and experiment group. Each rat in the experiment group received an ip injection of 2 ml normal saline containing 1.5?10 6 tachyzoites of T. gondii . On the 64th day after injection of T.gondii , the changes in T lymphocytes (TL) and their subgroups, the helper T lymphocytes (Th) and the suppressor T lymphocytes(Ts) in the peripheral blood of the rats with T.gondii were determined by the assay of the lymphocytes labeled with intercellular acid ? naphthyl acetate esterase. All the rats were killed and the atomic absorption method were used for detecting the level of trace elements in the spleen tissue. 9.Therapeutic Analysis of Two Kinds of Muscular Relaxation in Airway Foreign Body Removal Operation in Children

[Objective] To observe the muscle relaxation clinical effect and the occurrence of intraoperative and postoperative complications,and the postoperative extubation and muscle strength recovery of Rocuronium and Succinylcholine in children during airway foreign body removal operation.[Methods] 80 cases of children (ASA I) with airway foreign body,among them,30 cases were older than 3 years old,were agreed to be on electively airway foreign body removal surgery.According to anesthesia induction,intravenous injection muscle relaxants were randomly divided into two groups (n =40):Rocuronium group (Group R) 0.9 mg/kg,Succinylcholine group (Group S) 1.5 mg/kg.According to the degree of muscle relaxation and operation time,if necessary,patients in Group R were added intravenous injection with Rocuronium 0.3 mg/kg,patients in Group S were added intravenous injection with Succinylcholine 0.5 mg/kg,the other drugs were consistent.Observation items included:the bronchoscopy conditions,the degree of muscle relaxation during operation,the numbers of intravenous injection muscle relaxant and atropine,the times of tracheal extubation,the dynamic observation about vital sign and hemodynamic intraoperative and postoperative,and the occurrence situation of complications,the incidence rate of muscle soreness in children older than 3 years old 24 h after operation.[Results] (1) There was no significant statistics difference between the two groups in the condition of bronchoscopy (P > 0.05).(2)The patients in Group R were not required to add muscle relaxants and atropine,but all patients in Group S were required to add muscle relaxants and atropine (P < 0.01).(3)The complications such as body movement,hypoxemia,restlessness during recovery period were happened in Group S,and in Group R,there were only 1 case of laryngeal spasm and restlessness during recovery period,there was statistically significant difference between the two groups (P < 0.05).(4)The muscle strength recovery of 15 min in Group S was significantly higher than the Group R (P < 0.01).(5) The time of tracheal cannula extubation was extended after the operation in Group R,there was statistically significant difference between the two groups (P < 0.05).(6)30 cases patients older than 3 years old were followed up 24 h after operation,14 cases in Group S were found with the muscle soreness of trunk and limb,but none was found in 16 cases in Group R,there was statistically significant difference between the two groups (P < 0.01).[Conclusions] The muscle relaxant effect was well during the removal of forcign body in children with Rocuronium and Succinylcholine,but the former provided a more security anesthesia condition,the muscle soreness of trunk and limb was not found in patient 24 h after operation;and the latter must continue to add drugs in operation,and the complications were found during and after the operation,the muscle soreness of trunk and limb was found in patient 24 h after operation.
10.DNA vaccines encoding hemagglutinin of novel H1N1 influenza A virus inducing neutralization antibody in mice

Objective To study the characteristics of neutralization antibody in mice induced by DNA vaccines of hemagglutinin(HA) of novel H1N1 influenza A virus(2009H1N1).Methods HA encoding plasmids of 2009H1N1 or 1918H1N1(2009HA or 1918HA)were constructed.25 μg or 200 μg dosage of 2009HA plasmids were used to immunize the mice,the total antibody of anti-20O9HA or cross-reactive antibody were assayed by ELISA using 2009HA or 1918HA protein as capture antigen,and the neutralizing antibody were assayed by two kinds of virus pseudo - particles(pp) of 2009H1N1 and 1918H1N1 .Results During of 4 to 16 weeks after boost immunization,in two groups of mice immunized with 25 μg or 200 μg dosage 2009HA plasmids,both total antibody of anti-2009HA and neutralizing antibody to 2009H1Nlpp reached the similar level(P >0.05),and there were cross-reactive antibody to 1918HA protein in two groups of mice serum,with similar titers of cross-neutralizing activity to 1918H1N1 pp(P >0.05),Conclusion A low dosage DNA vaccine encoding HA of 2009 H1 N1 virus is able to induce persistent and high level of neutralizing antibody,and may be potential valuable vaccine against the new emerging influenza virus.
