1.Application of mifepristone in the treatment of perimenopausal dysfunctional uterine bleeding

Objective To explore an effective treatment for perimenopausal dysfunctional uterine bleeding.Methods 136 patients with perimenopausal dysfunctional uterine bleeding were received mifepristone treatment.Hemoglobin,thickness of the endometrium and sex hormones indicators,including progesterone (P),estrogen (E2),the corpus luteum generated hormone(LH),follicle hormone(FSH),prolactin(PRL) and testosterone(T),were detected before and after treatment.Results The treatment effective rate was 89.7%.After taking a course of treatment,89 cases decreased to restore normal menstrual cycle ;33 cases entered menopause after taking a course of treatment.After treatment,hemoglobin level was significantly increased compared with before treatment (t =3.899,P < 0.05),and endometrial thickness significantly reduced (t =5.652,P < 0.05).After treatment,the P,E2,FSH,LH levels were significantly lower than before treatment(t =5.320,4.309,3.872,3.641,all P < 0.05).T,PRL levels had no significant differences.Conclusion The mifepristone treatment for perimenopausal uterine bleeding have good clinical efficacy,less adverse reactions,and worthy of clinical application.

4.Preliminary study of apoptosis in human ovarian cell line COC1 induced by ultrasound and its mechanism

Objective:To investigate the antitumor effect of Low-Frequency Pulse Forcusd Ultrasound(LFPFU) exposure in human cell line COC1,and to clarify its probable mechanism.Methods:The growth inhibitory effect of ultrasound exposure[0.8MHz,83.6W/cm 2,pulse duration(PD) 50?s;sonication time of 10s] on COC1 cell was observed by MTT colormetric assay.Apoptosis was analyzed using flow cytometry(FCM) and terminal deoxynucleotidyl transferase-mediated dUTP-digoxin nick end labeling(TUNEL),and fluorescent microscope and electron microscope were applied to detect the morphological changes of COC1 cell.The protein expression of bcl-2 and bax affected by LFPFU were determined by FCM and immunofluorescent techniques.Results:Sonication could obviously suppress proliferation of COC1 cells surviving ultrasound exposure.The absorbance value(A 570 )of MTT decreased significantly at 12h,24h,36h and 48h after sonication as compared with control group.The hightest apoptosis rate was found to reach 23.57?4.62% by FCM,and brown-colored positive apoptotic cells were observed with TUNEL method at 6h after sonication.Typical morphological changes of apoptosis such as apoptotic body was found under fluorescent microscope and transmission electron microscope.Fluorescence index(FI) of bcl-2 decreased,and Fl of bax increased after LFPFU exposure.Conclusion:LFPFU can induce apoptosis in vitro in human ovarian cell line COC1 surviving ultrasound exposure.The bcl-2 and bax protein may be involved in the apoptotic process.

To investigate the effects of Carvedilol on neurohormone and heart function in patients with congestive heart failure(CHF) by double blinded, randomized clinical trial. 35 congestive heart failure patients were double blinded and randomly divided into 2 groups, Carvedilol group and placebo group. 15 healthy persons constitute the normal control group. We measured the plasma concentration of norepinephrine(NE), angiotensin (Ang) and plasma renin activity (PRA) , meanwhile left ventricular ejection fraction (LVEF) , left ventricular dimension and volume were observed before and after treatment. This article discusses the recent advance in techniques of hip and knee replacement Chiefly in the aspects of hip or knee prosthesis des ign, improvement of materials and surgical techniques. In total hip replacement, second-generation cementing technique, new types of metal-on-metal prosthesi s and minimally invasive technique have been applied to enhance the stability, r educe the wear rate and facilitate the functional recovery. In knee replacement, mobile bearing and high flexion are great advancements made in design. The comp uter-aided operational technique has greatly improved the replacement.
7.Effects of isoflurane on lung surfactant protein A in rats

Objective:To examine the effects of different concentrations of Isoflurane on the level of surfactant protein A(SP-A) and the expression of SP-A mRNA in the lung of rats.Methods: Thirty-two male Wistar rats were randomly divided into 4 groups: control group received 40%(volume fraction) O_2 inhalation(40% O_2,n=8);0.7% isoflurane group(n=8),1.5% isoflurane group(n=8).They and 2.0% isoflurane group(n=8).They were treated with 40%O_2 with 0.7%,1.5% and 2.0% isoflurane respectively.In each group,the rats inhaled experimental gas for 8 hours,and then were put to death immediately.Morphological changes of type Ⅱ pneumocytes were observed by transmission electron microscopy.Surfactant protein content in broncho-alveolar lavage fluid(BALF) was measured with Western Blotting.The level of intracellular SP-A was examined with immunohistochemistry(IHC).To assess the relative levels of SP-A mRNA in lung tissue,reverse transcriptase-polymerase chain reaction(RT-PCR) was used with the co-amplification of the "housekeeping" gene GAPDH as internal control.Results: Morphological changes of type Ⅱ pneumocytes were distinct in 1.5% isoflurane and 2.0% isoflurane groups.Isoflurane,after three doses,significantly reduced the SP-A content in BALF.Control group,(437 112)?25 654;0.7%Isoflurane group,355 789?28 116;1.5%Isoflurane group,238 554?31 531;(2.0%) Isoflurane group,223 632?25 710(P
8.COCHINCHINENIN—A NEW CHALCONE DIMER FROM THE CHINESE DRAGON BLOOD

AIM To study the active constituents of Dracaena cochinchinensis (Lour.) S.C.Chen. in the commercial dragon blood. METHODS Various column chromatographies with Sephadex L-20 gel, MCI gel and silica gel were employed for the isolation and purification. The structures of compounds were elucidated by spectral analysis. RESULTS Nine chalcones were isolated from the commercial dragon's blood which was made of D.cochinchinensis (Lour.) S.C.Chen.. By means of spectral data, they were identified as 1-[5-(2,4,4′-trihydroxydihydrochalconyl)]-1-(p-hydroxyphenyl)-3-(2-methoxy-4-hydroxy-phenyl)-propane (1), 2′-methoxysocotrin-5′-ol (2), socotrin-4′-ol (3), 2-methoxy-4,4′-dihydroxydihydrochalcone (4), 2,4,4′-trihydroxy-dihydrochalcone (5), 2,4,4′-trihydroxy-6-methoxydihydrochalcone (6), 2′,4′,4-trihydroxy-chalcone (7), 2-methoxy-4,4′-dihydroxychalcone (8) and 2′-methoxy-4′,4-dihydroxychalcone (9). CONCLUSION Compound 1 is a new chalcone dimer and named as cochinchinenin. Compounds 2-9 were isolated from D.cochinchinensis (Lour.) S.C.Chen. for the first time.
9.Quantification of midazolam/1′-hydroxymidazolam and their pharmacokinetic characteristics in rat brain by microdialysis combined with LC-MS n

Aim To develop a sensitive, rapid and ac-curate LC-MSn method for determination of midazolam/1′-hydroxymidazolam and their pharmacokinetic char-acteristics in rat brain. Methods SD rats received in-travenous injection of midazolam ( 5 mg · kg-1 ) via femoral vein, a probe drug of cytochrome P450 3A. The microdialysis ( MD ) samples in situ brain were collected every 8 mins at 2. 0μl·min-1 in 2. 4 hours. Analytes in brain dialysate were quantified by the pro-posed LC-MSn method. Gradient elution was performed on an Agilent Eclipse Plus-C18 column ( 2 . 1 × 50 mm, 3. 5 μm). The mobile phase consisted of 2 mmol ·L-1 ammonium acetate and acetonitrile. The analyte was detected using electrospray ionization ( ESI ) in multiple reaction monitoring ( MRM) modes. The reac-tion selected ions were 326 . 1/291 . 1 m/z for midazo-lam, 342. 1/324. 1 m/z for 1′-hydroxymidazolam and 285. 1/154. 0 m/z for diazepam as internal standard. Result The linear ranges of midazolam and 1′-hydroxymidazolam were 0 . 78~100 and 0 . 195 ~12 . 5μg·L-1 respectively. The lower limit of quantification was 0 . 2 μg · L-1 . The RSD of intra- and inter-batch precisions was less than 7 %. The RSD of accuracy was from -1 . 34 to -8 %. Conclusion This sensi-tive and rapid LC-MSn method is suitable for determi-nation of midazolam/1′-hydroxymidazolam in rat brain dialysate. MD combined with LC-MSn method may give assistance to deep and further studies of drug metabo-lism and CYP3A enzyme in brain.
10.Analysis of prognostic relationship between absolute lymphocyte count and patients with multiple myeloma

Objective: To investigate the relationship between peripheral blood absolute lymphocyte count (ALC) and patients with multiple myeloma (MM). Methods:We obtained clinical features and follow-up data of 102 patients with MM to analyze the prog-nostic value of peripheral blood ALC. Results:Patients with ALC>1.51×109/L were designated as group 1, whereas patients with ALC<1.51×109/L were designated as group 2. Group 1 had significantly lower age as well as concentrations of lactate dehydrogenase (LDH) andβ2-MG than group 2 (P<0.05). The overall survival (OS) of group 1 was significantly higher than group 2 (P<0.05). Sex, ALB, Du-rie-Salmon stage, isotype, subgroup, and stage according to the International Staging System in group 1 were not significantly different compared with group 2 (P>0.05). Conclusion:Lower ALC is a poor prognosis factor. ALC may be an important prognostic factor of MM.
