1.Experimental study on hydroxychloroquine raised insulin sensitivity in insulin resistance rat models

Objective:To observe the effects of hydroxychloroquine on insulin sensitivity in high-fat feeding rats.Methods:After administrating hydroxychloroquine,the glucose infusion rate (GIR) determined by euglycemic clamp was used to measure the insulin sensitivity,and determining fasting plasma glucose,lipid and insulin.Results:(1)In high-fat feeding rats,the GIR60-120 was lower and the fasting plasma glucose,insulin,lipid levels were higher than those in normal feeding rats.(2)Hydroxychloroquine,rosiglitazone and metformin increased insulin sensitivity (the value of GIR60-120 was 9.3?1.2,11.6?1.4 and 9.8?1.3 mg.kg/min,respectively) in insulin resistance (IR) models.Moreover,the fasting glucose and some plasma lipid indices were improved in the IR rats with hydroxychloroquine administration.Conclusion:It is indicated that hydroxychloroquine could raise insulin sensitivity of IR rats with high-fat feeding,which was similar to metfomin but lower than rosiglitazone.In addition,hydroxychloroquine also could improve the glucose and blood lipid of the IR rats in some extent.
2.Establishment of hyperinsulinemic euglycemic clamp technique

Objective:To establish a hyperinsulinemic euglycemic clamp technique for the study of insulin sensitivity in Chinese.Methods:A hyperinsulinemic euglycemic clamp technique was used to the study of methodology in 8 normal weight subjects with normal glucose tolerance.Results:(1)When a higher level of insulin was created during maintaining fasting euglycemia,the secretion of endogenous insulin and hepatic glucose production were completely inhibited.The secretion of counter-regulatory hormones(cortisol,growth hormone and glucagon) was not significantly stimulated..(2)During the steady-state of hyperinsulinemic euglycemic clamp,the glucose disposal rate was 9.25?0.25mg/(kg.min)Conclusion:The hyperinsulinemic euglycemic clamp technique was established successfully in Chinese.During the steady-state of exogenous insulin and glucose metabolism,glucose utilization increased.
3.Application of problem-based learning in internal medicine teaching for residency training of general medicine

To evaluate the effect of problem-based learning (PBL) in internal medicine teaching for residency training of general medicine.Fifty two residents of general medicine were divided randomly into PBL teaching and traditional teaching groups with 26 in each.The evaluation was conducted in combination of examination scores and questionnaire survey in both groups.In questionnaire survey the residents of PBL group responded positively about the use of PBL in teaching, and rated themselves as having much better competencies in problem solving and self-directed learning.The scores of case analysis examination in PBL group were higher than those in traditional teaching group ( 73.7 ± 4.1 vs.71.1 ± 4.8, P < 0.05 ).The results suggest that the application of PBL has better effect than traditional teaching in residency training of general medicine.
4.The indicative value of neutrophils/lymphocyte ratio in the progress of colorectal cancer

Objective To investigate the indicative value of Neutrophils /lymphocyte ratio in the progress of colorectal cancer .Methods Fifty one patients with colorectal cancer ,63 cases with adenomatous polyps ,75 cases with inflammatory and hyperplastic polyps ,48 case of another digestive system tumors and 21case of inflam-matory bowel diseases were enrolled in this study in the Affiliated Hefei Hospital of Anhui Medical University .We also chose sixty people as control group whom were conduct enteroscope as physical examination but didn 't find any abnormal lesions and compared the date of NLR CEA and CA 199 between six groups and different stages of colorectal cancer .The ROC curve was carried out to establish the cut off of NLR and the area under curve (AUC).Furthermore,we compared the diagnostic efficiency of the three tumor markers .Results The level of NLR in patients with colorectal cancer was obviously higher than that of adenoma group ,the polyp group ,another digestive system tumor group,inflammatory bowel disease group and normal control group (P<0.05),despite the NLR value in another digestive system tumor and inflammatory bowel disease patients was elevated slightly ,while there was no obvious differences between the another groups (P>0.05).The levels of NLR in patient with Dukes A and B stages were lower than Dukes C and D stages (P<0.05).The cut off of NLR was 2.73,AUC=0.727, suggesting an accurate diagnostic value .The diagnostic sensitivities of the three markers in order of NLR >CEA>CA199,and the specificities in order of CA199>CEA>NLR.Moreover,the diagnostic sensitivity and specificity of the combined detections of three markers were higher than either of screening respectively or two indicators of arbitrary joint .Conclusion The elevated value of NLR is corresponding to the high state of inflammation in colo-rectal cancer ,when used NLR as a potential marker for diagnosis of colorectal cancer ,the sensitivity is superior to the conventional tumor markers ,but the specificity is not high ,therefore,we should combined with the other tumor markers to improve the diagnostic accuracy .
5.Clinical significance of TrACP in diagnosis and prediction of bone metastasis in prostate cancer

Objective To study the changes of tartrate resistant acid phosphatase in the serum of patients with bone metastasis in pros-tate cancer,and to explore its clinical significance for bone metastasis diagnosis and prediction of prostate cancer. Methods Seventy-eight cases of prostate cancer and 40 cases of benign prostatic hyperplasia patients as the research object,at the same time,40 healthy young men as controls,were divided into the group of patients with prostate cancer bone metastases (group A,n=41),prostate cancer patients with no bone metastasis group (group B,n=37),benign prostatic hyperplasia patient group (group C,n=40),healthy control group (group D,n=40). Determination of serum TrACP levels in patients with the immunoassay using double antibody sandwich ELISA,combined with pathologi-cal grade,Gleason score,PSA,ALP and ALT were statistically analyzed. Results The serum TrACP concentration in patients with bone me-tastasis significantly increased,with significant difference compared with the other groups. The concentration of serum TrACP and PSA levels showed a positive correlation. The area under the ROC curve ( AUC) was higher than that in ALT,and the ROC curve of cross TrACP and PSA,suggesting that high value in the diagnosis of bone metastasis. Conclusion The detection of TrACP has directly diagnosis and predic-tive value for prostate cancer with bone metastasis,the serum TrACP content monitoring in patients with prostate cancer has valuable clinical significance in understanding the growth progression status of prostate cancer,judgment and prediction of bone metastases.

Objective To explore the diagnostic value of ultrasonography for the residual gallbladder after cholecystectomy.Methods Application of the ultrasound diagnostic apparatus in cholecystectomy patients was to be screened to find the residual gallbladder.Results Ultrasound resuts showed 18 cases of residual gallbladder,which combined stones in 12 cases(66.7% ),polyps in 1 case(5.6% ) ;Atrophic cholecystitis,acute cholecystitis,postoperative residual gallbladder probability were high,accounting for 83.0%.Conclusion Ultrasound for the preoperative prevention and postoperative diagnosis of the residual gallbladder has better value,and the cholecystectomy patients should be routinely review the ultrasound.
9.Effects of berberine on the proliferation and differentiation of HL-60 cells

Aim To study effects of berberine on the proliferation a nd differentiation of HL-60 cells. Methods Cell inhibitory effec t was determined by growthcurve method and colony formation.Cell differentiation was analyzed by cytomorphological changes, NBT reduction and cell surface antig en.Cell cycle was analyzed by flow cytometry.Results The growth o f HL-60 cells was significantly inhibited by berberine in a time and dose-depe ndent manner.1,2,4,8 mg?L -1 berberine induced differentiation of HL-60 c ells: Morphological evaluation indicated that the berberine-treated cells exhibited granulation appearance; NBT reduction was significantly increase d; expression of CD11b increased obviously. Flow cytometry analysis showed that berberine arrested HL-60 cells in G 0/G 1 phase, the percent of the cells in S phase decreased.Conclusion Berberine can induce differentiatio n and growth inhibition of HL-60 cells
10.Expression, purification and characterization of catalytic domain of protein kinase D1 in baculovirus-insect cell expression system.

Protein kinase D (PKD) is a novel family of serine/threonine kinases and diacylglycerol (DAG) receptors and has been documented in a variety of cellular processes. To get high purity catalytic domain of PKD1 (PKD1-cat) for crystallography study, the GST-tagged PKD1-cat gene was cloned into a baculovirus transfer vector pFastBac1 (donor plasmid). When the recombinant plasmid was transformed into DH10Bac competent Escherichia coli, which contains a baculovirus shuttle vector (bacmid), transposition occurs to generate a recombinant bacmid containing the gene of interest (GST-PKD1-cat). The recombinant bacmid DNA was transfected into Spodoptera frugiperda Sf9 insect cells to generate recombinant baculovirus, which was then amplified through multiple rounds of infection in Sf9 cells. After that, Trichoplusia ni insect cells in suspension culture were infected with baculoviral stock at a multiplicity of infection (MOI) of 5 PFU/cell. SDS-PAGE and Western blotting analysis confirmed the detection of a 68 kDa protein by the glutathione S-transferase (GST) monoclonal antibody. The recombination protein was purified by Glutathione sepharose affinity chromatography and cleaved by PreScission Protease to remove GST tag, and a highly pure 42 kDa protein which was consistent with the molecular weight of the expected PKD1-cat protein was detected on SDS-PAGE. The activity of purified PKD1-cat protein was determined by in vitro PKD kinase assay. Our data showed that the kinase activity increased with the concentration of purified PKD1-cat protein. These results showed that the truncated recombinant PKD1-cat protein was highly active and pure, and could potentially be used for solving 3D structure of this protein by Nuclear Magnetic Resonance (NMR) or crystallography.
