3.Effect of 6% hydroxyethyl starch 200/0.5 infusion before operation on postoperative immune function

Objective To investigate the effect of 6 % hydroxyethyl starch (HES) 200/0.5 infusion before operation on postoperative immne function in patients. Methods Forty ASA Ⅰ or Ⅱ patients of both sexes aged 21-58 yr weighing 47-79 kg were randomly divided into 2 groups ( n = 20 each): 6% HES 200/0.5 group (group H) and compound sodium acetate group (group A). 6% HES 200/0.5 10 ml/kg or compound sodium acetate solution was infused intravenously over 15 min before anesthesia induction. Anesthesia was induced with iv injection of propofol, fentanyl and vecuronium and maintained with target-controlled infusion of propofol and infusion of remifentanil. Venous blood samples were collected before operation and at 1 h, 1 day and 3 days after operation to detect the serum concentrations of IL-6, IL-8, TNF-α, IL-2, IL-10, IgG, IgA andIgM. Results Serum concentrations of IL-6, IL-8, TNF-α, II-2 and IL-10 were significantly higher after operation in the two groups, and serum concentrations of IgA and IgG were significantly lower after operation in group A than those before operation ( P < 0.05 or 0.01 ). Serum concentrations of IL-6, IL-8 and TNF-α were significantly lower, while serum concentrations of IL-2, IL-10, IgA and IgG were significantly higher after operation in group H than in group A ( P <0.05). Conclusion Preoperative infusion with 6% HES 200/0.5 can improve the immune function after operation in patients.
4.Investigation and research on needs of home nursing of adult inpatients

Objective To identify the needs of home nursing of adult inpatients, and analyze the re-lated factors of the needs in order to give direction and information for the home care nursing. Methods A self-designed questionnaire was used to investigate the needs of home nursing of 172 adult inpatients. Results Of the 172 patients, 56(32.6%) patients considered the home care was necessary, and 66(38.4%) patients in an indifferent attitude,50(29.0%) patients thought it unnecessary. The need was positively related with the comprehension of home care. The need for home care of patients from different departments, age groups and subjective judgement of self-health had significant difference. Conclusions Home care are necessary for some kind of adult inpatients. We should perfect adult home care system and improve nursing level of community.
5.CHANGE IN IL-12 EXPRESSION AND ITS ROLE IN IMMUNODISSONANCE IN MODS MICE

Objective To determine the dynamic change in IL-12 in MODS mice at different post-trauma time points and its relationship with immunodissonance. Methods 96 male C57BL/6 mice, 6-8 weeks old, were randomly divided into normal control, 3h, 24h, 3d, 7d and 12d post-trauma groups. MODS model were replicated by injection of zymosan into the peritoneal cavity of the mice. Spleen and blood were sampled at different time points. Pathological changes of spleen were observed by light microscope. Immunohistochemistry marker of IL-12p40 in spleen was produced by Powervision method. The levels of IL-12p40 and IL-12p70 in serum and homogenate of spleen were determined by enzyme-link immuno-sorbent assay,and T lymphocyte subsets were analyzed by flow cytometry, respectively. Results Atrophy of the white pulps, proliferation of monocytes and dendritic cells, infiltration of neutrophilic granulocytes in the red pulps could be seen in 12d post trauma group. During the "two hits" in early period (3-24h) and late period (12d) in MODS, the amount of IL-12p40 positive cells increased significantly in the spleen; the level of IL-12p40 increased, whereas the level of IL-12p70 decreased evidently, in serum and the homogenate of the spleen. In the early period, the amount of CD8 + T cells in peripheral blood increased, while the amount of CD4 + T cells reduced obviously in the late stage, thus CD4/CD8 ratio declined in both periods. Conclusions The results indicated that over expression of IL-12p40 by antigen presenting cells in immune organs was correlated with immunosuppression of MODS mice.

ObjectiveTo display the hot research points in stroke awareness, emergency treatment and evolution. Methods Papers on stroke awareness and emergency treatment were retrieved from Web of Science.Their knowledge atlas was plotted by key word frequency analysis and co-word analysis.Results The number of papers on stroke awareness and emergency treatment has been increasing year by year, and stroke awareness and emergency treat-ment have been concerned more and more since 1996 , indicating that the situation is still severe in stroke aware-ness and emergency treatment and no breakthrough has been made in this field.ConclusionStress should be laid on acute ischemic stroke, especially tPA, in study of stroke awareness and emergency treatment Research should be transformed into combined prevention and control from treatment .
7.Therapy and prognosis in diabetic SARS patients

To compare the therapy, manifestations (such as body temperature, chest X-ray image and immunologic indexes, etc.) and prognosis in severe acute respiratory syndrome (SARS) patients with and without diabetes mellitus. The results suggested that rational use of glucocorticoid is important in curing SARS, and the recovery of cellular immune function and pulmonary lesion is slower in the diabetic SARS patients.
8.Relationship between membrane phospholipids and CD14 expression of macrophage stimulated by LPS

Objective To investigate the effect of cellular membrane phospholipids on CD14 protein expression of macrophage stimulated by LPS. Methods Changes of CD14 protein expression and CD14 mRNA of macrophages stimulated by LPS in vitro were determined by Western blotting and RT PCR. Effects of membrane phospholipids on CD14 protein expression were also detected. Results After stimulation by LPS, CD14 expression increased at 1 h, reached the peak value at 5 h and decreased to the normal level at 8 h but CD14 mRNA reached the peak value at 3 h and decreased at 5 h. The levels of phospholipids and membrane fluidity decreased at 5 h but CD14 protein expression increased after LPS stimulation. After pretreatment with liposomes, membrane phospholipid microenvironment improved and CD14 protein expression decreased. Conclusion LPS can up regulate CD14 protein expression, which might be regulated at least at the transcriptional level of the CD14 gene. Changes of membrane phospholipid microenvironment may be an important reason for the up regulation of CD14 induced by LPS.
9.Correlation analyses of serum levels of homocysteine and hypersensitivity C-reactive protein in elderly patients with cerebral infarction

Objective To observe the serum levels of homocysteine (Hcy) and hypersensitivity C-reactive protein(hs-CRP) in elderly patients with cerebral infarction and to investigate their relationship and clinical significance by National Institutes of Health Stroke Scale (NIHSS).Methods The serum levels of Hcy and hs-CRP were by enzymatic cycling method and scattering turbidimetry in the elderly patients ( 116 cases with cerebral infarction and 100 cases of healthy control).Those 116 cases with cerebral infarction were divided into three groups by the degree of NIHSS.The three groups were compared with each other.Results The levels of serum Hcy and hs-CRP in elderly patients with cerebral infarction were significantly higher than that of healthy control group ( t =6.97,P <0.01 ; t =14.96,P <0.01 ).There has significant difference among those three groups with cerebral infarction by comparing with each other( F =23.49,P <0.05; F =28.19,P <0.05).A positive correlation was found between Hcy and degree of NIHSS( r=0.54,P <0.05),and between hs-CRPand degree of NIHSS( r =0.58,P <0.05).Conclusions Serum levels of Hcy and hs-CRP are correlated with the occurrence of cerebral infarction and its severity.There has positive clinical significance to evaluate the effect of cerebral infarction by measuring the serum levels of Hcy and hs-CRP dynamic.
10.CHANGE OF COX-1/2 EXPRESSION IN BRAIN AFTER SPARED NERVE INJURY-INDUCED NEUROPATHIC PAIN AND ANALGESIC EFFECTS OF COX INHIBITORS WITH DIFFERENT SELECTIVITY

To observe the expression of cyclooxygenase (COX)-1 and COX-2 in brain after spared nerve injury (SNI) and compare the analgesic effects of COX inhibitors with different selectivity. Radioimmunoassay, RT-PCR and Western blotting techniques were used to evaluate the change of brain COX expression at different time points( before SNI, 1 h, 12 h, 1 d, 3 d, 7 d, 14 d, 30 d and 60 d after SNI); By exploring hot plate test, we observed the reacting time of animals after injection of saline, NS-398, SC-560 and indomethacin at different time points. The results showed that: ( 1 ) The expression of brain COX-1 didn't increase significantly until 14 d after SNI, while that of COX-2 increased significantly and rapidly after SNI and reached peak at the time point of 1 d ( all P <0.05 ); (2) NS-398 showed significant analgesic effect on neuropathic pain after SNI at the early phase ( P < 0.05 ), but didn't persist for over 30 d; ( 3 ) Indomethacin and SC-560 didn't show significant analgesic effects until 14 d. These results suggest that brain COX-1 is involved in the late phase of neuropathic pain and may play a role in the persistence of pain, while brain COX-2 is involved in the early phase of neuropathic pain and may play a role in the pain origination.
