Objective To observe the curative effect of integrated traditional Chinese and Western medicine treatment in the elderly patients with vertebrobasilar insufficiency.Methods Totally 120 elderly patients with vertebrobasilar insufficiency were randomly divided into two groups:treatment group and control group(n=60,each).All the patients received Vinpocetine injection by intravenous drip,besides,the patients in treatment group received compound recipe Xueshuantong capsule.Results The total effective rate in treatment group was 95.0％,which was significantly higher than that in control group(78.3％)(P＜0.05).The plasm viscosity [(1.2±0.2) vs.(1.6±0.2) mPa/s]and fibrinogen [(3.1±0.6)vs.(4.4±0.5)g/L]were decreased after compound xueshuantong capsule treatment as compared with control group (P ＜ 0.05 ) Conclusions Combination of traditional Chinese and Western medicine has definite therapeutic effect on the old patients with vertebrobasilar insufficiency and can be utilized widely.

Objective To observe the role of calcitonin (CT) during the implantation. Methods Human endometrial epithelial cells were cultured. After stimulated with various concentrations of CT, intracellular calcium(Ca 2+)in the epithelial and stroma cells and pre-embryos were measured by the laser scanning confocal microscope. Resutls When stimulated with different concentrations of CT, mean fluorescence levels in the epithelial cells were similar to that of the control. However CT can improve intracellar Ca 2+ of preimplantation embryos and stroma cells in a dose-dependent manner and significantly higher than those of controls. When 10 nmol/L CT was added to the culture medium, the intracellular Ca 2+ concentration in 8-cell embryos rose immediately. Embryo exposure to CT was followed by a series of Ca 2+ bursts that persisted for at least 2 hours. No change in Ca 2+ was observed when culture medium alone was added to the embryos. Pre-loading embryos and stoma cells with the Ca 2+ chelator,prevented the increased fluorescence after CT addition. Conclusions CT play an important role during the procesess of implantation. It maybe improve intracellar of preembryos and accerate the development of preembryos.

Objective To investigate neurological recovery effect and mechanisms of blood activating and phlegm removing decoction in the treatment of patients with hypertensive cerebral hemorrhage.Methods 68 patients with hypertensive cerebral hemorrhage in our hospital were divided into two groups.34 patients of the control group were given conventional western medicine,and 34 patients of the observation group were taken conventional western medicine combined with blood activating and phlegm removing decoction.The clinical outcomes, hematoma volume change and neurological recovery were compared,and analyzed its mechanism of action.Results The effective rate of the observation group(94.12%) was significantly higher than 73.53%of the control group (χ2 =5.31,P<0.05). After 7d,14d,21d treatment,NIHSS scores of the observation group [(12.87 ±1.43) points,(8.55 ±1.11) points and (5.67 ±0.76) points] were significantly lower than those of the control group [(20.43 ±2.57) points, (17.66 ±2.08)points,(14.54 ±1.44)points],and the differences were statistically significant (t=14.99,22.53, 31.76,all P<0.05).After treatment,hematoma volume of the observation group[(6.44 ±0.54)mL]was significant-ly lower than before treatment[(16.30 ±1.90)mL]and after treatment in the control group [(12.11 ±1.44)mL] (t=29.11,21.50,all P<0.05).After treatment,ALD score of the observation group [(66.34 ±5.22)points] was significantly lower than before treatment[(27.53 ±2.20)points]and after treatment in the control group[(47.55 ± 3.33)points],and the differences were statistically significant(t=39.95,17.70,all P<0.05).Conclusion The effect of blood activating and phlegm removing decoction in the treatment of patients with hypertensive cerebral hemor-rhage is significant,can significantly improve neurologic impairment and daily living skills,has no obvious conflict of interest and high application value,worthy of promotion and application.

Objective To investigate the effects of sericin pretreatment on the expression of extracellular matrix(ECM) associated protein in diabetic nephropathy(DN) rats' kidney.Methods Sixty six male SD rats were randomly divided into 3 groups(n=12):normal control group,DN model group and sericine pretreatment group.DN rats model in model group and sericine pretreatment group were established by intraperitoneally injection of streptozotocin(STZ).Blood glucose≥16.7 mmol/L was taken as the standard of successful modelization.The rats in sericine pretreatment group were lavaged with sericine(2.4 g·kg~(-1)·d~(-1)) for 35 days before injecting STZ.The enzymic method was used to measure the blood glucose.Type Ⅳ collagen(cⅣ)and laminin(LN)content in the serum were detected by ELBA.The expression of transforming growth factor-β_1,(TGF-β_1)and tissue inhibitors of maprix metalloproteinase-1(TMP-1) protein in the kidney was observed by immunohistochemical staining.The expression of Smad 3 protein in the kidney was detected by Western blot.Results Compared with normal control rats,the blood glucose,cⅣ and LN content in the serum,TGF-β_1,TIMP-1 and Smad 3 expression in the kidney of the model group rats increased obviously(P<0.01).The blood glucose,cⅣ and LN content in the serum,TGF-β_1,TMP-1 and Smad3 expression in the kidney of rats in sericine pretreatment group were significantly lower than those of the rats in model group(P<0.01).Conclusion Sericin pretreatment can inhibit the activation of TGF-β/Smad 3 signal pathway in the kidney of DN rats,and prevent the decrease of MMPs activity induced by up-regulation of TIMP-1.So sericin can prevent accumulation of ECM and glomerulosclerosis during DN,and has satisfactory apotropaic effects on the development of DN.

Objective To evaluate the value of echocardiography in investigation the failure of interventional therapy of VSDs and to increase the successful rate. Methods 15 cases with failure of VSD closure through interventional approach were undertaken measurement of major parameters of the defect on left ventriculography and then followed by Philips 5500 color Doppler US for repeated multi-direction tangential measuring of the size, morphologic change, relation with peripheral structure and individual valvular regurgitation. Results (1) Marginal membranous VSD 10 cases; membranance aneurysm 8 cases, ≥2 outlets 6 cases. Width of basal part of membranous aneurysm was 8 ～ 18 (10 ?2) mm with depth of 3 ～ 10 (6 ? 2) mm, distance from aortic valve was 0 ～ 6 (2 ? 1) mm and from tricuspid valve was 2 ～ 5 (2 ?1) mm; associated with slight tricuspid valvular regurgitation. Aortic valvular regurgitation 5 cases showed slight to mild amount and without membranous aneurysm occurred in 2 cases. (2) Intracristal VSD 5 cases showed the size of defect as 6 ～ 10 (7 ? 1) mm, with distances of 2 ～ 3 (1 ? 0.8) mm from pulmonary valve and 1 ～ 2 (1 ? 0.6) mm from aortic valve associated with slight regurgitation in 4 cases, slight tricuspid valvular regurgitation in 5 cases and aortic valvular prolapse in 4 cases. Conclusions Many factors can influence the direction tangential continuous scanning should be taken for accurate localization and measurement of VSD in order to select the very sight occludor providing high successful rate of occlusion and long term efficacy.

To investigate the clinical improvement of limping gait in patients with ankylotic hips and limb length discrepancy.Methods:From 1996 to 2005,12 patients with ankylotic hips and limb length discrepancy were treated by distraction osteogenesis with a mono-lateral external fixator and an intramedullary nail.The limb length discrepancy was 6.20-12.50 (median 8.45) cm.Limping gait was classified according to the recommendations of the American Academy of Orthopedic Surgeons/Hip Society and scored according to Harris:no limping scored 11 points,mild limping scored 8 points,moderate limping scored 5 points,while severe limping scored 0 points.Limping gait was severe in all patients pre-operatively and the total score was 0.Results:All patients were followed up for 30.00-46.00 (median 38.55) months,and all reported improvement in limping gait.The gain in length was 6.00-12.50 (median 8.20) cm,and the mean residual limb length discrepancy was 0-0.50 (median 0.20) cm.The total treatment time was 41.00-82.00 (median 61.50) weeks,the lengthening time was 14.00-38.00 (median 29.55) weeks.At the last follow-up,10patients had mild limping gait and 2 had moderate limping gait; the total score was 90.00.The median score was 7.50 (P25 was 8.00,P75 was 8.00).According to Wilcoxon signed rank test,the post-operative limping gait scores were significantly higher than pre-operative (P=0.001 ).Conclusion:Femoral lengthening can improve the limping gait significantly in ankylotic hips and limb length discrepancy.

BACKGROUND: The blocking treatment can improve the clinic symptom of facial spasm. But the site, depth and the angle of puncturing point are very difficult to be defined, which will seriously infect the curative effect.OBJECTIVE: To study the applied anatomy of the related structures of facial muscle for blocking the facial nerve, and to provide anatomical bases for accurate puncturing point and preventing complications.DESIGN: An observation study based on cadavers and making the mimic puncture and measuring the correlative structures with anatomical method.SETTING: Department of anatomy in a medical college.PARTICIPANTS: Totally 28 adult male cadavers(56 sides) were used and the correlative index were measured.METHODS: The blocking point was located at the convergent point of the interior edge of cartilage of the external acoustic canal, the anterior fringe of mastoid processes and the posterior fringe of mandible ramus, the needle must be thrust vertically to the median sagittal plane. The puncturing needle stopped until it was barred. A longitudinal incision was made from the puncturing point to mandibula angle, and skin, superficial fascia were cut separately. Then carotid superficial fascia was cut from the posterior fringe of parotidean. The facial nerve trunk and the posterior auricular artery were preserved in site. A blunt isolation was made along its incision. Jugular glomus was appeared. Then the correlative index were measured and dealt with statistics method.MAIN OUTCOME MEASURES: The puncturing point, angle and depth,the distance between facial nerve trunk and puncturing point, the position relationship between facial nerve and puncturing needle and the distance relationship between facial nerve and principal structures adjacent to it.RESULTS: The blocking point was located at the convergent point of the interior edge of the external acoustic canal, the anterior fringe of mastoid processes and the posterior fringe of mandible ramus, the needle must be thrust vertically to the median sagittal plane. On the left side, the puncturing depth was (19.91 ± 0.09) mm, On the right side, the puncturing depth was(19.28±0. 10) mm.CONCLUSION: The experimental study could provide accurate puncturing point, angle and depth for blocking the facial nerve and bring the blocking treatment into full play.

Objective: To a evaluate the effectiveness of comprehensive nutrition interventions among primary school students in Yiwu City, so as to provide insights into malnutrition control among children. Methods: Grade 3 to 5 students were sampled from four primary schools in Yiwu City using a random cluster sampling method and randomly assigned into the intervention group and the control group. Students in the intervention group received comprehensive interventions, including nutritional health education, promotion of physical activities, nutritional meal support and creation of a nutritional campus, while students in the control group were given no interventions. The awareness of nutritional health knowledge, dietary behaviors and nutritional status were compared in students between the two groups prior to interventions and one year following interventions, and the effectiveness of interventions was evaluated using generalized estimating equations. Results: Totally 879 students were enrolled. There were 440 students in the intervention group, including 243 males and 197 females, with a male to female ratio of 1︰0.81 and a mean age of (10.47±0.99) years; and there were 439 students in the control group, including 244 males and 195 females, with a male to female ratio of 1︰0.80 and a mean age of (10.35±1.02) years. Following comprehensive interventions, the awareness of “type of food”, “seven nutriments from food”, “eating at least 12 types of food daily”, “less than 6 g of daily salt intake”, “food composition in nutritional breakfast”, “nutritional labels of pure milk”, “no less than 60 min of daily exercise duration” and “too fat or too thin may threaten health” and the increase in the proportion of 3 and more types of food in breakfast were significantly higher among students in the intervention group than in the control group (all P<0.05); however, there was no statistical difference in the proportion of normal nutrition between the two groups (P>0.05). Conclusion The comprehensive nutritional interventions may effectively increase the awareness of nutrition health knowledge and improve dietary behaviors among primary school students.

Objective:To develop an HPLC-DAD method for the simultaneous determination of seven active constituents chloro-genic acid, 3,5-dicaffeoylquinic acid,linarin, luteolin-7-O-β-D-glucoside, apigenin-7-O-β-D-glucoside , luteolin and acacetin in the different parts of Dandrantherna morifolium( Ramat. ) Tzvel. Methods:The chromatographic separation was performed on a Diamonsil C18 column (200 mm × 4. 6 mm,5 μm) with 0. 2% phosphoric acid-acetonitrile as the mobile phase with gradient elution at the flow rate of 1 ml·min-1 . The detection wavelength was set at 348 nm,and the column temperature was maintained at 30 ℃. Results:Chlorogenic acid, 3,5-dicaffeoylquinic acid, linarin, luteolin-7-O-β-D-glucoside, apigenin-7-O-β-D-glucoside , luteolin and acacetin was linear within the range of 0.014-0.560 μg(r =0.999 5) ,0.031-1.260 μg(r =0.999 6), 0.022-0.900 μg(r =0.999 8), 0. 036-1. 440 μg(r=0. 999 6),0. 029-1. 160 μg (r=0. 999 7) , 0. 005 5-0. 220 0 μg (r=0. 999 8) and 0. 006 5-0. 260 0 μg (r=0. 999 9), respectively. The average recovery was 97. 91%, 97. 91%, 97. 55%, 96. 43%, 96. 44%, 97. 07% and 97. 48%, and RSDs was 1. 24%, 1. 29%, 1. 48%, 1. 83%, 0. 67%, 1. 88% and 1. 80%, respectively. Conclusion: The method is convenient, accurate and reproducible in the quality control of the multicomponents in different parts of Dandrantherna morifolium( Ramat. ) Tzvel.

Objective To investigate the relationship between the appearance of donor cells in urine and acute rejection and the clinical implication. Methods Eighty renal transplantation patients were observed, in which the donors were male and the recipients were female, or HLA DR antigen were mismatched (30 cases were at perioperative period, 20 cases were subjected to acute rejection, 30 cases had stable renal function). Urine samples were collected regularly. PCR and PCR SSP were applied to detect DYZ 1 (special gene fragment of Y chromosome) and DRB 1of HLA DR antigen respectively after DNA were obtained.Results Perioperative period group: donor cells in urine were detected in all the patients 24?h after operation. With the development of disease, the intensity of donor DNA expression in urine was decreased generally. 30 days later, donor cells in urine disappeared only in 3 cases of 30 cases, and acute rejection happened in 8 cases of the rest 27 cases. Acute rejection group: donor cells in urine were detected in 18 cases (90%); 2 weeks following anti rejection therapy, donor cells in urine were negative only in 3 cases, still positive in the other 15 cases, and the intensity of donor DNA expression in urine was decreased generally during the treatment. Donor cells in urine were negative in 16 cases ( 88.9% ) after treatment for 3 months. Stable renal function group: DYZ 1 or HLA DRB1 was positive in 2 cases ( 6.7% ), negative in 28 cases ( 93.3% ). Conclusion PCR and PCR SSP were used to detect DNA of donor cells in urine, which would be a new method to diagnose acute rejection of renal transplantation, but would not exactly fit for those happened in early stage. The intensity change of donor DNA expression in urine represented the recovery of renal transplantation, which provided the possibility to evaluate renal allograft rejection quantitatively at the same time.
