1.Ethical Discussion About the Media's Disseminate of Medical Sanitation Information

The essay mainly states and analyse some important cases,such as the physician-patient relationship,the clinical application of new medicine and new technology etc.Starting from the misleading caused by the disseminating,which is not scientific of medical health by the media,the author analyizes the reasons from the angle of the medium ethics and the medical ethics, and explores the ethical priciples which should be followed by the media while disseminating of the medical health information,in addition to some factors that should be considered by the media when disseminating information,if there aer conflicts among these primciples.
2.Study on the HPLC Character Spectra of Lonicera fulvotomentosa

Objective Establishing a kind of analysis method about HPLC character spectra of native medicinal materials, Lonicera fulvotomentosa, in Guizhou province for supplying experiment basis to guide processing and quality control of Lonicera fulvotomentosa. Methods Making the Lonicera fulvotomentosa sample liquors with 50% carbinol solvent and vibrating 30 minutes by ultrasonic. Adopting high performance liquid chromatography RP-HPLC method, Luna C18 (2) (5 ?m, 250 mm ? 4.6 mm) chromatographic column, the volume flow 1.0 mL/min, the column temperature 30 ℃, the detection wavelength 238 nm, the mobile phase 0.2% phosphoric acid acetonitrile (A)-0.2% phosphoric acid solution (B), the gradient elution 0~15 min, A∶B=10%∶90%, 15~60 min, A∶B=10%∶90%→40%∶60%. Results Determined 9 common peak in character spectra of the Lonicera fulvotomentosa by detecting 10 group Lonicera fulvotomentosa medicine materials with referring to chlorogenic acid, the precision and stability and reiteration test RSD value all less than 3.0%. The similitude degree between the samples and the character spectras was more than 0.99 by evaluating ten batch of the samples with the similarity assessing soft, proving that the quality of the ten batch of the Lonicera fulvotomentosa medicine materials was stable and homogeneous. Conclusion The character spectra of the Lonicera fulvotomentosa was established to supply experiment basis for effectively controlling process, quality and standard of Lonicera fulvotomentosa.
3.Single-center experience of steroid-free immunosuppressive therapy after liver transplantation for hepatocellular carcinoma patients

Objective To evaluate the efficacy and safety of using basiliximab in place of a corticosteroid for immunosuppression following liver transplantation for hepatocellular carcinoma (HCC) in Chinese patients.Method The records of 178 patients with HCC who underwent orthotopic liver transplantation from January 2003 to December 2009 were retrospectively reviewed.All patients received immunosuppression therapy that contained either basiliximab (n =78) or steroids (n =100) in addition to tacrolimus and mycophenolate mofetil.Assessments included complications related to liver transplantation,occurrence of steroid side effects,recurrence of HCC,and patient and graft survival.Results A smaller proportion of patients receiving basiliximab than steroids experienced de novo diabetes (38.7% vs.91.0%,respectively) or long-term de novo diabetes mellitus (4.0% vs.30.3%,respectively) (both,P<0.0001).The median overall and disease free survival was similar between basiliximab (50.8 months and 19.6 months,respectively) and steroid treated patients (64.2 months and 23.8 months,respectively).The 5-year overall survival and disease free survival rate was also similar between the basiliximab (42.5% and 38.9%,respectively) and steroid (50.5% and 39.2%) groups (all,P>0.730).However,in patients who met the Milan criteria basiliximab was associated with greater 5 year overall survival rate than steroid therapy (88.9% vs.57.4%,respectively,P =0.022).Conclusion It revealed that the non-steroid treatment does not increase the incidence of acute rejection but also can decrease the incidence of de novo diabetes in the patients with HCC following liver transplantation and prolong the survival time of patients who met the Milan criteria.
4.The effect of basic periodontal therapy on blood glucose control in diabetes patient with periodontitis

Objective:To investigate the effect of basic periodontal therapy on levels of blood glucose,blood lipid,and tumor necrosis factor-alpha(TNF-?) among patients with periodontal diseases in insulin dependent diabetes patients.Methods:Forty insulin dependent diabetes patients with periodontitis were selected and divided into basic periodontal therapy group and control group.The periodontal clinic index,blood glucose,blood lipid,glycosylated hemoglobin(HbA1c),and serum TNF-? concentrations were assessed before treatment and 4 weeks,3 and 6months after basic periodontal therapy.Results:The HbA1c,TNF-?,total cholesterol(TC),tyiglycerides(TG) in basic periodontal therapy group were significant decreased after periodontal therapy,and meanwhile had significant difference compared with that in control group.Conclusion:The basic periodontal therapy can help to control the blood glucose level in insulin dependent diabetes mellitus with periodonitis.
5.Effects of growth hormone on circulating neutrophil activation in septic peritonitis rats induced by lipopolysaccharide

Objective To study the effect of growth hormone (GH) on circulating neutrophil activation in septic peritonitis rats induced by lipopolysaceharide.Methods Male Wistar rats were divided into six groups (n=7) in randomized manner:normal control,septic peritonitis,septic peritonitis plus GH (0.5 mg/kg),septic peritonitis plus GH (1.0mg/kg),septic peritonitis plus GH (2.0 mg/kg),GH alone (2.0 mg/kg).Septic peritonitis rats were induced by lipopolysaccharide.CD11b expression and an oxidative burst of neutrophils were detected by flow cytornetric analysis. Results Growth hormone significantly increased sepsis-induced expression of CD11b and oxidative burst of circulating neutrophils (P<0.01).Control animals treated with GH alone had CD11b expression and oxidative burst similar to that animals,treated with saline.Conclusion Treatment of GH may be harmful,instead of beneficial,on sepsis-induced organ injury.
6.Content Determination of Dauricine in Menispermi Rhizoma from Different Producing Areas by HPLC

Objective To establish an HPLC method to determine the contents of dauricine in Menispermi Rhizoma from different producing areas. Methods C18 was set as chromatographic column filler, with acetonitrile-water-triethylamine (45:55:0.1) as the mobile phase, 284 nm as the ultraviolet wavelength detection, 1 mL/min as the flow rate, 30 ℃ as the column temperature. HPLC chromatograms of eight different batches of Menispermi Rhizoma were established. Results HPLC testing conditions of Menispermi Rhizoma was established. Within 20-100 μg/mL, there was a good linear relationship between the injection volume of the reference substance and the peak area (r=0.9995). The average recovery of dauricine was 100.30%, RSD=1.000%. The contents of dauricine in Menispermi Rhizoma from different producing areas were different. Conclusion The HPLC method is with sensitivity, accuracy, precision, good reproducibility and simple operation, which can be used as detection method to determine the content of dauricine in Menispermi Rhizoma.
7.The imaging findings of unexpected splenic autotransplantation after splenectomy

Objective To study the imaging findings of unexpected splenic autotransplantation after splenectomy and to improve diagnostic accuracy of splenic autotransplantation.Methods The findings of 10 patients with splenic autotransplantation confirmed by histology in our hospital were retrospectively reviewed.In 7 patients CT and plain and dynamic enhanced MRI scanning were carried out,and in 2 of them 99mTc-DRBC scanning were also done.In 3 patients,plain and dynamic enhanced CT scannings were done.Results (1) Multiple lesions were detected in 7 patients and a single lesion in 3 patients.The masses were round and oval.The nodules were in the splenic fossa in 9 patients,in the pancreatic tail in 4 patients,in the right liver in 2 patients and in other of parts of the abdominal cavity in 2 patients.These nodules varied in size and 94.6% showed a maximum diameter of less than 3 cm; (2) The findings on CT and MR:all the nodules were homogeneous with soft tissue density.There was no cystic degeneration,necrosis and calcification.In one patient with a nodule in the pancreatic tail,there was a slightly short T1 and short T2 signals.Other nodules showed long T1 and long T2 signals.All the signals from the nodules were homogeneous and their outlines were clear.In a patient with a nodule in the right liver,the blood supply came from the abdominal aorta.There was a surrounding thin layer of low-density ring which showed long T1 and long T2 signals.The enhanced features on CT and MR were similar.The nodules showed homogeneous or inhomogeneous enhancement in the arterial phase,with continuous homogeneous enhancement in the portal venous phase,with an obvious decline in the delayed phase;(3) The findings of 18 F-FDG PET:The nodules had obvious increase in FDG uptake.Conclusions In patients with a history of splenic trauma or splenectomy,abdominal nodules with multiple,homogeneous density or signal,clear outline,enhanced features similar to spleen,splenic autotransplantation should be considered.99mTc-DRBC scanning is helpful in the diagnosis of splenic autotransplantation.
8.Clinical analysis of partial cystectomy combined with chemotherapy in treating localized muscle invasive bladder cancer (a report of 53 cases)

Objective To evaluate the safety and effectiveness of partial cystectomy combined with chemotherapy in treating localized muscle invasive bladder cancer.Methods The patients admitted to localized muscle invasive bladder cancer from October 2011 to December 2015 were screened according to the strict inclusion criteria,and the final 53 patients were enrolled in the study,37 males and 16 females,aged 36 to 85 (63.72 ± 13.31)years.Tumor diameter 1.5-4.0 (2.52 ±0.56)cm,the number of≤ 3 and the edge of the tumor from the ureteral opening > 2 cm.All pathological types are urothelial carcinoma.All patients were examined by MRI clinical stage T2a-T2b period.A total of 53 patients with muscle invasive bladder cancer who refuse or older,merge the heavier medical disease,difficult to tolerate radical cystectomy.Using partial cystectomy,intraoperative electrosurgical excision to deep muscle,parallel to the tumor bed frozen to confirm negative margins,postoperatively systemic chemotherapy of gemcitabine combined cisplatin and bladder perfusion chemotherapy of hydroxycamptothecin.Results Patients were successfully opcrated,and no margin positive or incision planting cases.The operating time is 55-110 min and average (79.11 ± 17.25)min.There was no serious complication in operation and postoperative,pathological diagnosis was invasive epithelial cell carcinoma of the urinary tract,pathology classification:low level 41 cases,and high level 12 cases.Follow up of 12 to 62 months,an average of (27.11 ± 12.22)months,21 cases of recurrence,12 patients had superficial recurrence in the bladder who was treated successfully by transurethral resection of bladder tumor,5 cases of multiple myometrial invasion who was treated by radical cystectomy (3 cases died of bladder cancer after the surgery in 8,12,and 18 months later),4 cases of myometrial invasion with recurrence of pelvic lymph node metastasis or even distant metastasis to give up treatment (all died within six months of bladder cancer).The first tumor recurrence time 3-18 months,11 months and the median time.Conclusions Our data indicate that partial cystectomy combined with chemotherapy is a safe and effective in treating localized muscle invasive bladder cancer,with less trauma,shorter operative time and higher safety,so as to provide an alternative treatment for patients with advanced age,high risk or who can not be implemented transurethral resection of bladder tumor or who refused to undergo radical cystectomy.
9.Clinical Observation of Warm Needling plus Herbal Medicine for Ocular Myasthenia Gravis

Objective To compare the therapeutic efficacy between warm needling and Pyridostigmine bromide in treating ocular myasthenia gravis, for seeking the optimal treatment method. Method Seventy patients with ocular myasthenia gravis were randomized into a treatment group intervened by warm needling and control group intervened by Pyridostingmine bromide, 35 in each group; meanwhile, fortified Yi Qi Tang decoction was given to both groups. The observation lasted 6 weeks and a 3-month follow-up was conducted. The clinical absolute and relative scoring system for myasthenia gravis, and the Chinese medicine syndrome scale for spleen-qi deficiency were adopted for quantitative comparison and efficacy evaluation. Result The total effective rate was 85.7% in the treatment group, higher than that in the control group. Conclusion Warm needling plus herbal medicine is effective in treating ocular myasthenia gravis.
10.CT-guided radiofrequency ablation for the treatment of osteoid osteoma in femoral neck:preliminary experience in 6 cases

Objective To evaluate the feasibility, safety and efficacy of CT-guided radiofrequency ablation in treating osteoid osteoma located at femoral neck. Methods Six patients with osteoid osteomas in the femoral neck received CT-guided percutaneous radiofrequency ablation. In all patients the main complaint was pain at the hip, and the course of disease varied from one month to 2 years, with an average of 8 months. Under spinal anesthesia the surgery was performed. With the help of CT guidance , a 3.5 to 4.0 mm coaxial drill system was inserted into the nidus, and an osseous access was established, then the bone biopsy needle was used to obtain specimens for pathological examination. Subsequently, a 1.5 to 2.0 cm active tip was introduced through a non-cooled radiofrequency needle into the nidus. Radiofrequency ablation was performed with the therapeutic temperature of 90℃, lasting for 6 minutes. The pain visual analogue scale (VAS) was used to evaluate the clinical effectiveness. The postoperative MRI findings were compared with the preoperative ones. Results Three days after the treatment, different degrees of pain relief was obtained in all patients, and all patients could get out of bed and walked around in one week. Postoperative VSA was significantly decreased (P<0.01). No severe complications occurred during and after the procedure. And no recurrence was seen during the follow-up period. Conclusion For the treatment of osteoid osteoma located at femoral neck, CT-guided radiofrequency ablation is a safe and effective minimally invasive treatment with fewer complications and satisfactory clinical results.
