1.The application of antibiotics in the peri-operative period of the general surgery

Objective To explore reasonable application of antibiotics in the peri-operative period of the general surgery.Methods 121 cases who were admitted from Nov. 1998 to Jun. 2000 were divided into three groups including deep-vein group, biliary culture group and drainage-rube group. The antibiotics were Cefradine, Cefuroxime, Cefoperazone, Ceftriaxone, Imipenem, Ciprofloxacin and Metronidazole. We recorded the dosages and applying-time of the antibiotics. We also carried out the culture of the blood, cut-secreta and drainage tube at the same time.Results The clinical doctors applied the antibiotics with excess of indications and applying-time. That brought about more and more drug-fast bacteria, even multiple drug-fast bacteria.Conclusion We must apply the antibiotics reasonable in the peri-operation.
2.The effect of basic periodontal therapy on blood glucose control in diabetes patient with periodontitis

Objective:To investigate the effect of basic periodontal therapy on levels of blood glucose,blood lipid,and tumor necrosis factor-alpha(TNF-?) among patients with periodontal diseases in insulin dependent diabetes patients.Methods:Forty insulin dependent diabetes patients with periodontitis were selected and divided into basic periodontal therapy group and control group.The periodontal clinic index,blood glucose,blood lipid,glycosylated hemoglobin(HbA1c),and serum TNF-? concentrations were assessed before treatment and 4 weeks,3 and 6months after basic periodontal therapy.Results:The HbA1c,TNF-?,total cholesterol(TC),tyiglycerides(TG) in basic periodontal therapy group were significant decreased after periodontal therapy,and meanwhile had significant difference compared with that in control group.Conclusion:The basic periodontal therapy can help to control the blood glucose level in insulin dependent diabetes mellitus with periodonitis.

4.The correlation between hypothyroidism incidence and thyroglobulin antibody, thyroid peroxidase antibody in Graves disease after 131Ⅰ therapy

ObjectiveTo investigate the correlation between serum levels ofthyroglobulin antibody (TGAb),thyroid peroxidase antibody (TPOAb) and hypothyroidism incidence in Graves disease after 131Ⅰ therapy.MethodsThree hundred and twenty-five patients with Graves disease whose TGAb and TPOAb were negative before treatment were selected.Serum levels of FT3,FT4,TSH,TGAb and TPOAb were measured at the 3rd,6th,12th and 18th month after treatment respectively.All cases were divided into positive group and negative group according to the serum levels of TGAb and TPOAb at the 18th month after 131Ⅰ treatment.The hypothyroidism incidences of two groups were compared.ResultsThere were 271 cases in negative group and 54 cases in positive group.The hypothyroidism incidence was 7.4%(24/325),and the incidence of negative group was 3.0% (8/271),while the incidence of positive group was 29.6% (16/54).There was significant difference between two groups(P < 0.05).ConclusionsHypothyroidism incidence of Graves disease after 131Ⅰ therapy has obvious correlation with the serum levels of TGAb and TPOAb.Dynamic observation of the serum levels of TGAb and TPOAb in Graves disease after 131Ⅰ therapy has important significance for clinical guiding and prognosis judgement.
5.Effects of growth hormone on circulating neutrophil activation in septic peritonitis rats induced by lipopolysaccharide

Objective To study the effect of growth hormone (GH) on circulating neutrophil activation in septic peritonitis rats induced by lipopolysaceharide.Methods Male Wistar rats were divided into six groups (n=7) in randomized manner:normal control,septic peritonitis,septic peritonitis plus GH (0.5 mg/kg),septic peritonitis plus GH (1.0mg/kg),septic peritonitis plus GH (2.0 mg/kg),GH alone (2.0 mg/kg).Septic peritonitis rats were induced by lipopolysaccharide.CD11b expression and an oxidative burst of neutrophils were detected by flow cytornetric analysis. Results Growth hormone significantly increased sepsis-induced expression of CD11b and oxidative burst of circulating neutrophils (P<0.01).Control animals treated with GH alone had CD11b expression and oxidative burst similar to that animals,treated with saline.Conclusion Treatment of GH may be harmful,instead of beneficial,on sepsis-induced organ injury.
6.Association of CYP3A5 and C5 polymorphisms with the individual dosage regimen of tacrolimus in Chinese liver transplantation recipients during the early post-transplantation period

Objective To investigate the effect of CYP3A5 and C5 gene polymorphisms on the concentration/dose ratio of tacrolimus in Chinese liver transplant patients during the early posttransplantation period.Methods A total of 100 adult patients who underwent primary liver transplantation (LT) were enrolled.Tacrolimus dosage and trough blood concentration were detemined at first week after liver transplantation.Two single-nucleotide polymorphisms were genotyped and analyzed in both donor and recipient groups.The relationship between gene polymorphisms and tacrolimus concentration/dose ratio (C/ D ratio) was analyzed.Results The distribution of allele A in C5 rs17611 was 63.5 % among donors and 58.5% among recipients.For CYP3A5,the rs776746 allele G represented the major alleles in both donors and recipients (71% and 72%,respectively).The tacrolimus C/D ratio of recipients carrying allele AA in C5 rs17611 was significantly higher than that of recipients carrying the C5 rs17611 allele G.Both donor and recipient CYP3A5 rs776746 polymorphisms were highly correlated with the tacrolimus C/D ratio at first week after liver transplantation.No linkage disequilibrium between CYP3A5 rs776746 and C5 rs17611 polymorphisms was found (D'max =0.392,r2max =0.034).Recipient CYP3A5 rs776746 allele A,C5 rs17611 genotype AA,and donor CYP3A5 rs776746 allele A were associated with rapid tacrolimus metabolism.With increasing number of these alldes,tacrolimus C/D ratio was reduced during the one week after transplantation.Conclusion Recipient C5 rs17611 polymorphism is a new genetic locus that influences tacrolimus metabolism in patients after OLT during the early post-transplantation periocd.
7.Comparison analysis of clinical features of varicella in pregnant women and outcomes of mothers and their newborns

Objective To investigate the clinical features of varicella and the prognosis in pregnant women and their newborns .Methods Clinical data of pregnant women with varicella zoster virus (VZV) infection (n= 25) and their newborns hospitalized in Beijing Ditan Hospital from 1st Jan .2008 to 31st Dec .2014 were retrospectively analyzed and randomly compared to non‐pregnant women with VZV infection (n=50) .Clinical features and prognosis of varicella in pregnant women and their infants were analyzed .Chi‐square test was used for categorical data and t test was used for quantitative data .Results Time to rash scab of varicella in pregnant women was longer than non‐pregnant women ([10 .1 ± 2 .1] d vs [5 .6 ± 1 .4] d ,t=10 .941 ,P<0 .05) .The rate of bacterial infection in pregnant women was higher than non‐pregnant varicella women (72 .0% [18/25] vs 32 .0% [16/50] ,χ2 = 10 .761 , P < 0 .05) , with statistical significance .Among 25 cases of varicella pregnant women ,the pregnancy complications were observed in 3 cases of diabetes ,2 cases of premature rupture of membranes ,5 cases of anemia and 1 case of oligoamnios .Seven cases out of 25 pregnant women underwent parturition during fever and varicella period ,and 3 cases (12 .0% ) were complicated with intrapartum hemorrhage . Twenty five varicella pregnant women were all cured after antiviral and supportive treatment and gave birth to their babies ,with no abortion ,stillbirth or birth defects .No congenital varicella was observed in newborns .Of the 25 infants ,4 developed (16 .0% ) varicella within 2 weeks after birth and they were all born from mothers who developed varicella around delivery time .The clinical features of neonatal varicella presented with classic rash with no fever .The time to rash scab was longer (11 .0 ± 2 .1) d and antibody test for VZV was negative .All neonates were cured after antiviral and immunoglobulin treatment .Conclusions Longer duration of skin rash scab and higher rate of bacterial infections are the features of varicella in pregnant women .Intrapartum hemorrhage occurrs more commonly in pregnant women with varicella onset around delivery time .Varicella occurring during mid‐pregnancy may not increase the risk of neonatal birth defects after treatment .The newborns whose mothers with varicella onset during perinatal time especially around delivery time may suffer from varicella .The prognosis of neonatal period varicella is good after treatment .
8.Related factors about microcirulatory injury following elective PCI for elder patients with stable angina

Objective To investigate microvascular injury-related factors post-percutaneous coronary intervention (PCI). Methods Seventy-two elderly patients with stable angina , who underwent PCI from February 2009 to February 2014, were enrolled in this study. The index of microvascular resistance (IMR) was assessed and the clinical data were collected. The correlation between general clinical data and the IMR value before and after PCI were analyzed, and regression analysis was conducted on the relevant factors with postoperative microcirculation after PCI. Results After PCI, FFR, CFR, CK-MB, troponin were higher than those before PCI, with significant differences (P < 0.05). Linear regression analysis result showed that glycated hemoglobin, the degree of stenosis after PCI were positively correlated with IMR, but ejection fraction and CFR before PCI were negatively correlated with IMR. Conclusion Clinically, diabetes, severe vascular stenosis, low ejection fraction, low CFR before PCI in patients with unstable angina may likely occur microcirculation after PCI.
9.Analysis of related risk factors for bypass graft occlusion in elderly patients after coronary artery bypass grafting

Objective To explore the related risk factors for bypass graft occlusion in patients within 1 year after coronary artery bypass grafting,in order to provide the basis for the prevention of postoperative bypass graft occlusion.Methods Clinical data of 197 cases treated with coronary artery bypass grafting were collected,and the incidence rate of bypass graft occlusion and its related risk factors were analyzed.Results Graft occlusion occurred in 28 (14.2%) of 197 patients.The incidence rate of bypass graft occlusion were much higher in patients with BMI ≥24 kg/m2,smoking,hypertension,hyperlipemia,diabetes,peripheral vascular diseases and cerebrovascular disease than in patients with BMI <24 kg/m2,non-smoking,patients without hypertension,hyperlipemia,diabetes,peripheral vascular diseases or cerebrovascular disease (20.0% vs.9.8%,22.4% vs.10.8%,21.2% vs.3.8%,31.1% vs.9.2%,23.4% vs.9.8%,25.5% vs.10.7%,26.3% vs.11.3%,x2 =4.106,4.534,11.735,13.658,6.615,6.486,5.656,respectively,P=0.043,0.033,0.001,<0.001,0.010,0.011,0.017).Smoking,diabetes,hyperlipidemia,peripheral vascular diseases and cerebrovascular disease were the independent risk factors for bypass graft occlusion.Conclusions Many risk factors are related with short-term postoperative bypass graft occlusion in patients with coronary artery bypass grafting.The corresponding control measures should be conducted to decrease the risk of postoperative bypass graft occlusion,aiming directly at the risk factors,especially the independent risk factors.
10.Pyodermatitis-pyostomatitis vegetans: a case report

A 58-year-old female was admitted to the hospital for a 3-month history of erythema,pustules and vegetating plaques on the lips and scalp,as well as in intertriginous areas.She had a 2-year history of ulcerative colitis,which had been aggravated before the development of skin lesions.Skin examination demonstrated diffuse thickening of both lips covered with multiple granule-to mung bean-sized confluent pustules.Yellow crusts were observed at the lip margins.A 1.5 cm × 2 cm vegetating plaque surrounded by anular pustules was found in the left buccal mucosa.There were multiple irregularly-sized vegetating plaques with erosions and yellow crusts on the scalp,neck,as well as in the periumbilical and left inguinal and axillary region,and annular vesicles and pustules were observed in the center and margin of these plaques.The lesions subsided leaving hyperpigmented macules.There were four fresh pustules in the right axillary region.Histological examination of a biopsy specimen taken from the lower lip margin revealed mild hyperplasia of and neutrophil microabscess in the epidermis,as well as a dense inflammatory infiltrate composed mostly of neutrophils and eosinophils with microabscess formation in the dermis.Direct immunofluorescence examination of the normal-appearing skin next to the lesions was negative.The percentage of peripheral blood eosinophil was slightly elevated.Bacterial and fungal cultures of pustular contents were negative.A diagnosis of pyodermatitis-pyostomatitis vegetans was made.
