1.Changes of thrombin-antithrombin complex and other variables in acute respiratory distress syndrome

Objective To measure D-Dimer(DD) ,thrombin-antithrombin complex(TAT), lasmin-antiplasrain complex(PAP) and protein C(PC) in ARDS,to find the clinical significance of them,discuss the objective base in the early diagnosis of ARDS in the lab, and offer the serologic bases in the treatment and prognosis of ARDS.Methods 105 patients of ARDS were selected as the study group, and 105 people were selected as the control group, all of whom were healthy with no thrombus diseases. The venous blood of everyone in both groups was sampied,in order to take a quantitative determination of plasma prothrombin time(PT), prothrombin time(TT), kadin partial thromboplastin time(APTT), the amount of blood platelet, DD, TAT, PAP, PC. Results Serum concentratation of D-D, TAT, PAP was significantly higher in patients in ARDS group than that in control group(P<0.01).Serum concentratation of PC was significantly lower in patients in ARDS group than that in control group (P<0.01). Conclusion Measuring the concentration of DD, TAT, PAP and PC was very important, which not only did good to the early diagnosis of ARDS,but also had a clinic value.
2.Application value of serum tumor markers in diagnosis of pancreatic adenocarcinoma in the elderly

Objective To explore the clinical value of serum tumor markers including CEA,AFP,CA199,CA125,CK19,CA242 and CA724 in diagnosis of pancreatic adenocarcinoma in the elderly.Methods Totally 158 elderly and 102 middle-aged patients with pancreatic adenocarcinoma were selected.Pancreatic adenocarcinoma in all patients were confirmed by pathological examination,cytological examination and CT.100 elderly and 100 middle-aged patients with benign diseases were selected as control groups.The levels of serum tumor markers were detected and their sensitivities and specificities were calculated and compared between groups.Results The levels of serum CEA,CA199,CA125,CK19,CA242 and CA724 were higher in elderly patients with pancreatic adenocarcinoma than in the elderly control group [(70.9±11.4) g/L vs.(1.6±0.4) g/L,(158.9±19.1)U/mlvs.(12.6±3.2)U/ml,(53.9±15.4)U/mlvs.(11.6±1.1)U/ml,(70.2±16.3)U/ml vs.(1.4±0.2)U/ml,(96.4±18.5)U/ml vs.(7.9±2.2)U/ml,(57.6±13.6)U/ml vs.(4.4± 0.6)U/ml,t=26.13,31.63,17.57,19.03,20.95,15.59,respectively,all P<0.05].The sensitivities of serum CEA,AFP,CA199,CA125,and CA242 were higher in the elderly patients than in the middle aged patients (61.72% vs.48.69%,19.56% vs.4.13%,80.31% vs.69.55%,70.21% vs.46.77%,75.24% vs.59.44%,x2=4.28,12.63,3.94,14.32,7.23,respectively,all P<0.05).The specificities of serum CEA,AFP,CA199,CA125,and CA242 had no significant differences between the elderly and middle aged patients (all P>0.05).Conclusions Serum tumor markers may provide effective reference for the clinical diagnosis of pancreatic adenocarcinoma in the elderly,which has important value for pancreatic adenocarcinoma control and reversal.
3.Pleuropulmonary blastoma:one case report

ObjectiveTo investigate the early diagnosis and treatment of pleuropulmonary blastoma in children. Meth-ods The progress of pleuropulmonary blastoma from type 1 to type 3 was retrospectively analyzed.Results The male infant had no obvious abnormality in chest X-ray at 18 days. At one year old, X-ray iflm of the chest showed cystic lesions near hilum of the left lung (about 5 cm × 4 cm × 4 cm). The congenital cystic adenomatoid malformation was considered and an operation resection was suggested. However, it had been rejected by his parents. At 3 years and 2 months old, chest X-ray and CT showed left lung had a solid cystic lesion, the volume was signiifcantly enlarged, the back side was solid and the upper part had a large gas cavity. Two months later, the patient had asthmatic suffocation. The chest CT showed the tumor was completely solid. Pleuropulmonary blastoma type 3 was considered. The patient received the conventional chemotherapy after operation excision, and was followed up for 1 year. No recurrence was observed after the treatment.Conclusions Type 1 pleuropulmonary blastoma and congenital cystic adenomatoid malformation are hard to be differentially diagnosed. Early diagnosis and treatment of type 1 pleuropulmo-nary blastoma can improve the prognosis.
4.The clinical study of autologous mononuclear bone marrow cell transplantation repair infarcted myocardium by intracoronary route

Objective To investigate the possibility、 efficacy、 safety of the repair of infarcted myocardium and improvement of cardiac function by intracoronary autologous mononuclear bone marrow cell (MBMC) transplantation in patients with AMI. Methods Thirty-eight patients with AMI were enrolled in this prospective study and divided into PCI+cell transplantation group and PCI only group. Baseline and follow up evaluations included complete clinical and laboratory evaluations, 2D echocardiogram, positron emission tomography (PET), 48 ECG monitoring. MBMC were harvested, isolated, washed, and resuspended in saline for infusion. 21 patients were transplanted with autologous MBMC via a balloon catheter placed into the infarct-related artery during balloon dilatation by highpressure infusion 14.6 days after AMI, which was performed 6-8 times for 2 minutes each and 7 patients were transplanted via non-selective infarct-related arteries by highpressure infusion. Results Surgery was safe. 3 patients had limited premature ventricular contractions during cell infusion for several seconds. 3 patients felt cold after 15-30 minutes infusing cell. 5.Arthroscopic treatment for the rheumatoid arthritis with solitary joint involvement

Objective One of the characteristics of rheumatoid arthritis was multiple joints involved symmetrically. The purpose was to study the method of early diagnosis and treatment for the atypical rheumatoid arthritis involving solitary joint. Methods From April 1997 to December 2002, 5 cases which were 1 male and 4 female patients with an average age of 36.8 years (range, 23 to 47 years), suffered from the swelling and pain of knee joint, and were diagnosed as early rheumatoid arthritis. The interval of the onset to surgical treatment ranged from 2 months to 3 years with a mean of 16.2 months. The patients were evaluated with serum test, joint fluids examination and radiological imaging. They were treated by the synovectomy under the arthroscopy, while the appearances of the joint under arthroscopy were recorded, and the synovial tissue was sent to the pathological examination. The improvements of the joints symptoms and the range of motion were assessed at final follow up. Results During the operation, the light green and little muddy joint fluids were found in all patients. ESR of the patients ranged from 29 to 51 mm/1h with an average of 32.2 mm/1h, all of which were higher than normal value, whereas, 3 cases had positive rheumatoid factor and 4 cases showed significant higher ?2-MG. The radiographic findings were osteoporosis in 3 cases, soft tissue swelling in 4 cases and narrowed joint space in 1 case. The synovium were proliferated and enlarged in dark violet, there were creeping pannus and necrosis fibrin deposition in the cartilage. All of the patients were diagnosed as rheumatoid arthritis confirmed by pathological changes of the synovium. The patients were followed up from 4 months to 5 years (mean, 18.7 months). Pain released in 5 cases, swelling reduced in 3 cases, the range of motion improved by 19 degrees. Unfortunately, all of the patients developed later into typical rheumatoid arthritis after the mean interval of 10.9 months (range, 3 to 25 months), the symptoms were consistent with the criteria of American Rheumatoid Association . Conclusion Solitary joint with rheumatoid arthritis is rare and may be the early status of rheumatoid arthritis. The specific appearance under the arthroscopy prompt the early diagnosis of the knee rheumatoid arthritis, and synovectomy by arthroscopy may be benificial to release the symptoms of rheumatoid arthritis.
6.Fingerprint chromatogram analysis of Mylabris

AIM: To establish the GC-fingerprint chromatogram of Mylabris. METHODS: Mylabris from Guizhou Province was used under the conditions of Agilent Technoligies DB-1 capillary column and the program 160℃—2℃/min—210℃(10min), and the GC fingerprint chromatogram of Mylabris was set up. RESULTS: The methodological evaluation showed that this method had a good reproducibility, common peaks' relative areas of different samples were different. CONCLUSION: This method is reliable to evaluate the quality of Mylabris.
7.Total hip replacement in patients with ankylosing spondylitis: problems and their solutions

Objective To evaluate the efficacy of total hip replacement in patients with ankylosing spondylitis(AS) and to solve the problem often encountered during surgery. Methods One hundred and twelve hips of 98 patients with AS undergone total hip replacement were reviewed. The mean duration of follow up was 4.2 years(range, 1.5 to 8 years). The pre and postoperatively comparative study on pain, range of motion, correction of deformity and total function evaluation were conducted. Results Pain relief obtained in all but 4 hips. Mean range of motion was improved from 37.5? to 69.5? and the flexion deformity of the involved hip was corrected from mean 38.2? to 5.7?, all of the patients could take care of daily living by themselves. Complications included penetrating fracture of proximal femur in 4, sciatic nerve injury in 3, early dislocation of the hip in 2, disassociation of the acetabular component in 2 and sinking of the femoral stem in 23. Conclusion Total hip replacement is a very important and effective treatment of choice for patients with AS. The modified combination of anterolateral and lateral approach used by the authors facilitated release of contracture of the soft tissue anterior to the hip joint and correction of flexsion deformity of the hip. In case of multiple joint involvement, the surgery should be performed on the most severely involved one and the next surgery, if required, should be scheduled later within 3 to 6 months.
8.Research on Information System of Mobile Hospital Based on Shelter

Objective To research a set of information system to improve the ability of rescue on mobile hospital. Methods The industry server and shelter client were used for establishing the network system of medical information, which used telemedicine in satellite communication, analyzed hardware topology and software demand. The measures of increasing safety and reliability were provided. Results The medical information was transmitted by network to share; the transmitting speed of remote diagnosis was 768Kb/s and dynamic image in 25 frame per second. Conclusion The level of the hospital informationization was improved for designing the IMS in modular thought.
9.Dorsal double locking compression plate for treatment of stretched unstable distal radial fractures

Objective To evaluate the outcome of dorsal double locking compression plate (LCP) in treatment of stretched unstable distal radial fractures.Methods Fourteen cases of stretched unstable distal radial fractures were treated by reduction and internal fixation using straight or L-shaped anatomic LCP via dorsal approach.Wrist functional exercise was conducted immediately after operation.Follow up was made after operation to assess motion pain,functional score of wrist,and complication incidence at postoperative 12 weeks,24 weeks,and 1 year.Results Follow-up was lasted for 5-24 months.At postoperative 12 weeks,24 weeks and 1 year,mean visual analogue scale (VAS) was (1.88 ±0.26) points,(0.87 ± 0.14) points and (0.37 ± 0.06) points respectively and wrist functional score (Gartland-Werley score) was (6.45 ± 1.72) points,(2.73 ± 0.52) points and (2.10 ± 0.31) points respectively.According to Garfland-Werley score in the latest follow-up,the results were excellent in 10 cases,good in two,and fair in two.Besides,one case was combined with myotenositis of extensor pollicis longus muscle tendon and another case with myotenositis of extensor tendon.Conclusion Dorsal double LCP is one of the effective methods for stretched unstable distal radial fractures that can reconstruct anatomical structure of the wrist efficiently and attain satisfactory functional recovery,with no obvious pain.
10.Study on HPLC fingerprint of Dangguijisheng Injection

AIM: To establish the fingerprint chromatogram of Dangguijisheng Injection (Radix Angelicae Sinensis, Herba Visci) METHODS: HPLC with Zorbax SB-C 18 ( 4.6mm? 250mm,5-Micro) column was used, A phase (mechanol∶tetrahydrofuran=85∶15) and B phase ( 0.5% acetic acid (gradient elution)) were adopted as a mobile phase, respectively, and detection wavelength set at 270 nm. RESULTS: 18 peaks constitued the HPLC fingerprint of Compound Dangguijisheng Injection. CONCLUSION:The method is simple and accurate with a good reproducibility and can be used as a quality control for Dangguijisheng Injection.
