3.Clinical study of anti-oxidant effects of vitamin E-modified membrane on hemodialysis patients

Objective To evaluate the anti-oxidant effects of vitamin E -modified membrane in hemodialysis patients.Methods In our four-week study,57 patients on maintenance hemodialysis were divided randomly into three groups.Group A (n=19) resorted to hemodialysis with VitE-modified membrane;Group B (n=19) resorted to hemodialysis with conventional dialyzers and oral VitE intake(400mg/d);Group C (n=19) resorted to hemodialysis with neither vitamin E-modified membrane nor vitamin E intake.Blood samples were collected prior to the first and thirteenth dialysis sessions,and were employed to determine the concentrations of malonylaldehyde (MDA),glutathione peroxidease (GSH-px),glutathione (GSH),superoxide dismutase (SOD) and Vitamin E in plasma and red blood cells (RBC),and advanced oxidative protein products (AOPP) in plasma.Results In group A,there were significant increase in VitE,GSH-px,GSH and SOD levels,and declined MDA and AOPP levels in plasma and in RBC(P0.05).Conclusion Our study suggests that VitE may have anti-oxidant effects on hemodialysis patients,and that VitE-modified membrane may have more advantages in anti-oxidative effect compared with.VitE oral administration.
4.A Meta-analysis about early results of small J-pouch versus straight anastomosis after low anterior resection for rectal cancer

5.Effects of the EL-1 Receptor Antagonist on Matrix Metalloproteinase 1 Expression in UV-irradiated Fibroblasts

Objective To study the effects of the IL-1 receptor antagonist (IL-1Ra) on matrix metalloproteinase 1 (MMP-1) expression in UV-irradiated fibroblasts. Methods Simulating the impact of environmental ultraviolet (UV) light on human skin, UVA-irradiated human fibroblasts were cultured in medium obtained from UVB-irradiated HaCaT cells. MMP-1 was detected by ELISA in the culture medium of fibroblasts. After treatment with IL-1Ra, the mRNA expression levels of C-Jun, C-Fos and GAPDH (internal control) of fibroblasts were measured by real-time fluorescent quantitative RT-PCR. Results Production of MMP-1 by UVA (10 J/cm2)-irradiated fibroblasts was increased in culture medium from UVB-irradiated HaCaT cells. The fibroblasts produced significantly higher levels of MMP-1 in culture medium from HaCaT cells treated without UVB than those with 15 mJ/cm2 UVB (t = 8.413,P= 0.014). However, IL-1Ra inhibited MMP-1 production of fibroblasts in a dose-dependent manner. Standard curves of real-time fluorescent quantitative RT-PCR showed a linear correlation between the copy number and the threshold cycle (Tc). Melting curves confirmed the specificity of PCR products. The original copy numbers of C-Jun and C-Fos as well as the ratios of the numbers to the GAPDH copy number showed that IL-1Ra inhibited the C-Jun mRNA expression of fibroblasts in a dose-dependent manner but had no significant effects on C-Fos mRNA expression. Conclusions The culture medium from UVB-irradiated HaCaT cells can promote MMP-1 production by UVA-irradiated fibroblasts. IL-1Ra reduces MMP-1 production via inhibition of C-Jun mRNA expression.

7.Investigation on critically ill patients with nosocomial infection

Objective To determine epidemiology and risk factors for nosocomial infections in severe patients.Methods The study included 434 patients (7394 patient-days)during a 2-year period (2010-2011).The incidence rate of infection,pathogenic bacteria,correlation factor of infected were analyzed.Results A total of 225 infections were identified in 113 patients(26.0%).The incidence and infection rates were 56.8 in 1000-patient days and 51.8%,respectively.The infections were pneumonia (40.9%),bloodstream (30.2%),urinary tract (23.6%) and surgical site infections (5.3%).Pseudomonas aeruginosa (22.6%),methicillin-resistant Staphylococcus aureus (22.2%) and Acinetobacter spp.(11.9%) were frequently isolated micro-organisms.Median length of stay with nosocomial infection and without were 13 days (Interquartile range,IQR,20) and 2 days (IQR,2),respectively (P < 0.01).In logistic regression analysis,mechanical ventilation[odds ratio(OR):16.35 ;95% confidence interval(CI):8.26 ~32.34;P <0.01),coma(OR:15.04;95% CI:3.41 ~66.33;P <0.01),trauma(OR:10.27 ;95% CI:2.34~45.01 ;P <0.01),nasogastric tube(OR:2.94 ;95% CI:1.47 ~ 5.90; P < 0.01),tracheotomy (OR:5.77 ;95% CI:1.10 ~30.20;P <0.05) and APACHE Ⅱ scores 10 ~ 19(OR:10.80;95% CI:1.10 ~ 106.01 ;P <0.05) were found to be significant risk faetors for nosocomial infection.Rate of nosocomial infection increased with the number of risk factors(P <0.01).Mortality rates were higher in infected patients than in non-infected patients(60.9 vs 22.1% ;P <0.01).Conclusion These data suggest that,in addition to underlying clinical conditions,some invasive procedures can be independent risk factors for nosocomial infection in ICU.

Colorectal cancer is one of the most common cancer.Despite the survival rate of patients with metastatic disease has been raised due to the combination therapy of chemotherapy and opetation,few longterm survivors exist.The detection of circulating tumor cells (CTC) is an evolving technology whose significance has been discussed for more than a century.The advence of modem technology has allowed for more reliable detection of CTCs,and recent studies have provided compelling evidence that CTCs can predict clinical outcome in metastatic colorectal cancer (mCRC).In this review,we will discuss the evidence for CTC significance,with particular focus on detection methods,novel markers,and clinical outcomes in CRC,and future challenges in testing them as a means to individualize the therapy of mCRC patients.
9.Changes of glucocorticoid-induced osteonecrosis of the femoral head under mechanics load

BACKGROUND:Femoral head necrosis induced by glucocorticoid is typical under mechanics load.OBJECTIVE:To create the rat models of the femoral head necrosis by long-term glucocorticoid treatment,and to Observe the changes of femoral head necrosis induced by glucocorticoid under mechanics load.METHODS:Wistar rats with 4-month old were randomly divided into the experimental and control groups.After the buttocks injection of dexamethasone 20mg/kg weakly for 8 weeks,rats in the experimental group were made to run on the running machine at a speed of 1 km/h to forme the hyper-weight model;at the same time those in the control group were with the normal physiological weight.Rats in both groups were sacrificed at weeks 2,4,6 and 8.The left legs were taken for the biomechanics test on the EnduraTEC ELF3200,the biggest vertical deformation and stiftness were calculated,the number of empty osteocyte lacunas was calculated,and the cumulative absorbance value was compared between two groups.RESULTS AND CONCLUSION:The femoral head presented with necrosis at 8 weeks in the experimental group.and the width of bone trabecula,and the stiffness of which were smaller than that of the control group.but the biggest vertical deformation was greater the concurrent control group(P<0.05).The Bcl-2 cumulative absorbance value of 2 groups had significant differences at 4 weeks(P<0.05),which was increased with time prolonged in the experimental group(P<0.05).The results demonstrated that:in glucocorticoid-induced osteonecrosis of the femoral head,mechanics Ioad induces the obviously osteocyte necrosis.more cartilage repair,and trabeculae rupture,which is considered as the direct reason of the necrosis of the femoral head The typical femoral head necrosis is presented.
