Objective: To explore serum levels of lipoprotein associated phospholipase A2 ( LP-PLA2) in patients with acute coronary syndrome (ACS) who is smoking, to further clarify the impact of smoking on LP-PLA2 and the relationship with coronary artery disease (CAD). Methods: A total of 170 patients who received coronary angiography (CAG) because of chest pain in our hospital from 2015-01 to 2015-12 were enrolled. According to CAG results and smoking history, the patients were divided into 2 groups:Control group, n=70 healthy subjects including 2 subgroups as Smoking control subgroup, n=31 and Non-smoking control subgroup, n=39; ACS group, n=100 patients including 2 subgroups as Smoking ACS and Non-smoking ACS subgroups, n=50 in each subgroup. Blood levels of LP-PLA2 and C-reactive protein (CRP) were detected and compared among different groups. Results: Serum levels of LP-PLA2 in Smoking control subgroup was higher than Non-smoking control subgroup, P=0.018; LP-PLA2 level in Smoking ACS subgroup was higher than Non-smoking ACS subgroup,P=0.027; LP-PLA2 level in ACS group was higher than Control group,P=0.000. Serum levels of LP-PLA2 was positively related to CRP (r=0.724,P<0.01). Conclusion: Blood levels of LP-PLA2 and CRP were increased in ACS patients suggesting both of them might be involved in CAD occurrence; LP-PLA2 level was even higher in smokers implying this is could be one of the reasons for smokers were more likely to suffer from CAD and aggravate CAD progress.

Objective To study the residual gallbladder stones and the relationship between the onset of acute pancreatitis and its surgical treatment.Methods Retrospective analysis was made in 13 patients with residual gallbladder admitted tO the hospital from June 1998 to December 2007,of which 8 cases of residual gallbladder stones and acute pancreatitis were invostigated.Thirteen cases of patients had underovent cholecystectomy 2 to 9 years before,the first symptoms occurred before 1 month to 8 years.Other remnants of the gallbladder were diagnosed by B-ultrasound and CT,magnetic resonance cholangiopancreatography (MRCP).Results Two cases were confirmedas pigment stones;6 cases as cholesterol stones(size0.2 cm ～0.5 cm,an average of 3.2 cm);1 cases as anonlalous pancre-aticobiliary junction(anomalous pancreaticobiliary ductal union,APBDU);5 cases as anomal;8 cases(61.5%)as episode of acute pancreatitis.Au patients underwent resection of residual gallbladder,with line choledochal cyst excision and biliary-enteric Roux-en-Y internal drainage in 1 case.choledocholithotomy T-tube drainage in 3 cases.Postoperative follow-up of residual gallstone was done in 8 cases,no pancreatitis attack in 6 cases,pancreatitis attack in 2 cases.Significant difference was found in onset of acute pancreatitis (P=0.019,P<0.05).Five patients without stones in patients with postoperative all none pancreatitis attack.Conclusion Patients with residual gallbladder stones are prone to acute pancreatitis,and cholecystectomy for patients with residual treatment Can reduce the pancreatitis attack.

Autoimmune pancreatitis is a chronic autoimmune-related pancreatitis.It is characterized by a high globulin, IgG~4 increased, anti-carbonic anhydrase antibody and anti-lactoferrin antibodies exist, diffuse swelling of the pancreas, a significant lymphocyte infiltration accompanied by fibrosis, multiple clinical manifestations , and sensitive to the hormone treatment.

Objective To investigate the clinical efficacy of gemcitabine and cisplatin chemotherapy combined with CT guided microwave ablation used in treatment of advanced squamous cell lung cancer. Methods 76 patients with lung squamous carcinoma in our hospital from April 2013 to July 2015 were selected,and randomly divided into treatment group and control group,38 cases in each groups. The control group were treated with gemcitabine and cisplatin chemotherapy. The patients in the treatment group in the control group based on the combined CT guided ablation under microwave, after the end of treatment to evaluate the efficacy of the two groups of patients, observe two groups of patients with adverse reactions, and were followed up for 1-2 years. Results The total effective rate of treatment group was significantly higher than the control group (P<0.05); pneumothorax treatment group after microwave ablation in patients with 2 cases, 3 cases of hemoptysis, 2 cases of fever, recovered after symptomatic treatment. The adverse reactions of the two groups due to chemotherapy caused by comparison of the rate of no significant difference; the treatment group of patients with PFS was significantly longer than the control group, the 1 year survival rate and 2 year survival rate was significantly higher than the control group(P<0.05). Conclusion Gemcitabine combined with cisplatin chemotherapy regimen combined with CT guided microwave ablation is effective in the treatment of advanced squamous cell lung cancer, and is worthy of clinical promotion.

BACKGROUND:Osteoperiosteal decortication can treat the bone nonunion effectively, promote healing of fracture, but its action principle and key operation are not very clear, and there is less research addressing the postoperative treatment. OBJECTIVE:To review research progress of osteoperiosteal decortication for treating nonunion. METHODS: China National Knowledge Infrastructure, Wanfang database and PubMed database were used to search the related articles. The retrieval key words were “osteoperiosteal decortication, stripping, bone nonunion, bone disunion, bone delayed union”. Osteoperiosteal decortications combined with other fixation methods to repair the healing of bone nonunion were compared and analyzed. Bone implantation and implant management were summarized during repair. RESULTS AND CONCLUTION:105 literatures were retrieved by computer. After primarily screening by reading titles and abstract, irrelevant and repeated literatures were excluded. Finaly, 45 articles were summarized. Action principle, indications and operating points of osteoperiosteal decortications for bone nonunion were summarized. Osteoperiosteal decortications combined with other different fixation methods for the healing of bone nonunion were compared so as to better serve the clinic. Simultaneously, the paper also explores the combination with bone implant and management of implant during repair. Osteoperiosteal decortication with its simplicity and effectiveness has been respected in the field of orthopedics. However, for special patients, such as patients combined with destructive bone disease, osteoporosis, nonunion of hand and foot short bones, the treatment with osteoperiosteal decortications needs further researches.

Objective To investigate the effect of nesfatin-1 (NSF-1) on T-type Ca2+ channel currents in adult mouse dorsal root ganglion (DRG) neurons and possible signal transduction mechanisms involved.Methods We measured the expression of melanocortin 4 receptors(MC4-R)in mouse DRG by using western blotting analysis.The whole-cell patch clamp technique was used to record the effects of NSF 1 on T-type Ca2+ channel currents in small DRG neurons and several ligands were experimented to further clarify relevant signaling pathways.Results MC4-Rs were abundantly expressed in DRG neurons.NSF-1 enhanced T-type calcium channel currents in a dose-dependent manner in small DRG neurons in mice.NSF-1 mediated increment of T-type calcium channel currents was blocked by the MC4-R antagonist HS024,phosphokinase C antagonists GF109203X,and chelerythrine chloride,while the blockade of phospohokinase A PKI 6-22 elicited no such effects.Conclusions NSF-1 can enhance T type calcium channel currents in small DRG neurons through an MC4-R-dependent PKC signaling pathway.

Objective To evaluate the effects of angiostatin on vascular endothelial growth factor expression in human tongue squamous cell carcinoma cells.Methods Subcutaneous xenografts of human tongue squamous cell carcinoma(Tca8113)cell line in BALB/c nude mice were employed and eighteen mice were randomly divided into three groups(6 mice each):the PBS group injected with PBS,the AS group injected with 250 ng purified angiostatin,and the AS group injected with 30 ?g purified angiostatin.Reagents were injected intraperitoneally twice daily and tumor volume was calculated twice weekly for 21 d,then animals were sacrificed and each tumor specimen was divided into two parts after surgical resection.The first part was submitted to both immunocytochemistry for VEGF,VEGFR1,VEGFR2 and CD34,and TUNEL apoptosis assay.The second part was prepared for semi-quantitative RT-PCR analysis of VEGF,VEGFR1 and VEGFR2 mRNA abundance.Results There was an increase of apoptotic index(AI)(P

Objective To investigate the effect of gene silencing of Y-box binding protein-1(YB-1) by RNA interference on the proliferation and migration in prostate cancer cells lines PC-3 cells .Methods YB-1 siRNAs(pGenesil-1-YB-1-1 and pGenesil-1-YB-1-2) were synthesized and transfected into cloned into the the PC-3 cells by liposome .The expressions of YB-1 were measure by RT-PCR and Western blotting .The proliferation and migration were respectively detected by MTT and Transwell method . Results ThemRNA and protein expressions of YB-1 were significantly decreased by pGenesil-1-YB-1-1 and pGenesil-1-YB-1-2 (P<0 .05) ,compared with the control group ,the inhibition ratio of mRNA expression was 36 .23% and 39 .42% respectively and the inhibition ratio of protein expression was 41 .56% and 55 .33% respectively .The proliferation and migration were significantly decreased by pGenesil-1-YB-1-1 and pGenesil-1-YB-1-2(P<0 .05) .Conclusion YB-1 gene silencing by RNA interference inhibits the proliferation and migration in prostate cancer cells lines PC-3 cells .

Objective To explore the relationship between uric acid (UA) level and cognitive function in elderly patients with Parkinson,s disease (PD) and analyze the cognition related factors.Methods The clinical data of 60 elderly PD cases in our hospital from 2001 to 2009 were retrospectively analyzed. The 60 healthy people receiving medical examination in our hospital and matched by gender and age, were as control group. The information including gender, age, illness duration, Hoehn & Yahr stage (H-Y stage), serum UA level and Mini-Mental State Examination (MMSE) scale were recorded. Results The serum UA level was significantly lower in PD group than in control group [(262±53) μmol/L vs. (332±45) μmol/L, t=-6.724, P＜0.001]. In PD group, the serum UA level was slightly higher in males than in females [(271 ±48) μmol/L vs.(254±39) μmol/L, t=3. 282, P=0. 058]. The serum UA level was significantly lower in male PD patients than in male controls [(353± 62) μmol/L, t=- 5. 625, P＜0. 001], and was lower in female PD patients than in female controls [( 294 ± 59) μmol/L, t = - 4. 721, P = 0. 012]. There were no significant differences in serum UA level among different H-Y stage subgroups (P＞0. 05), but the serum UA level was lower in different H-Y stage subgroups than in control group (F=22. 039, P＜0. 01 ). There was no correlation between the UA level and the illness duration (r=0. 961, P＞0.05).The MMSE score had significant difference between elderly PD group and control group (t= -3. 168,P＜0. 001). In PD patients, the MMSE score was positively correlated with serum UA level (r=0. 789, P= 0. 000), and was negatively correlated with H-Y stage (r= - 0. 577, P = 0. 019 ), age (r= -0. 333, P=0. 034), but was not correlated with illness duration (r= -0. 333, P=0. 207) and BMI (t=- 0. 410, P= 0. 115). Conclusions The level of serum UA is lower in elderly patients with PD than in normal controls. There is correlation between the serum UA level and cognitive impairment. Lower serum UA level predicts worse cognitive scores.

Objective To assess the influencing factors of perioperativc delirium in the elderly with hip fracture and to define strategies for prevention and management. Methods DSM-Ⅳ was adopted to diagnosis of perioperative delirium in the elderly with hip fracture, The data of 148 elderly patients with hip fracture were analyzed retrospectively and classified according to the clinical characteristics. The differences were analyzed statistically between the delirious group and the control group. Results There were 26 (17.6%) cases diagnosed with delirium in the perioperative period, including 22 cases with postoperative delirium, and the delirious symptom lasted 3 days in average. For the type of delirium, there were 15 cases with mania type, 4 cases with inhibitory type and 7 cases with mixed type. Compared with patients in control group, patients with perioperative delirium were older [(79.67±7.7) years old vs. (75.5±7.0)years old, t=2.668, P=0.008)]. Five cases who had previous delirium occurred perioperative delirium, and five patients who received external fixator showed no perioperative delirium. Conclusions Perioperative delirium is a heterogeneous symptom on the basis of brain aging and degeneration. It affects the prognosis of the elderly patients with hip fracture and should be viewed as a separate entity, which can be prevented and treated through comprehensive management.
