Objective To observe the clinical curative effect of ankle chute tibia bone graft fusion treatment of late kaschin-beck disease.Methods Thirty-three patients with late kaschin-beek disease performed ankle arthrodesis,the ankle surgery in 5 cases,simple right ankle surgery in 13 eases and 15 cases of left ankle surgery.Intraoperative used tibial chute hollow screw internal fixation and bone graft plaster external fixation postoperatively.Followed up 0.5-6.0 years,compared the American orthopaedic food and ankle society (AOFAS) score before surgery to determine the efficacy of last follow-up,X ray film fusion situation assessment in the postoperative period.Results Thirty-three cases did not occur postoperative incision infection,vascular or nerve complications.Last follow-up,30 cases were no obvious discomfort,3 cases of mild ankle pain,accompanied by walking or standing for a long time after the foot heel had mild acid bilges feeling,did not affect the daily life.The AOFAS score before surgery was significantly lower than that last follow-up [(25.84 ± 16.81) scores vs.(87.32 ± 4.68) scores] (P ＜ 0.01).Clinical efficacy excellent in 25 cases,good in 5 cases,general in 2 cases,poor in 1 case.Follow-up X line piece was showed that in all cases to achieve bony fusion.Conclusion Using sliding kashin-beck terminal patients treated ankle bone graft fusion,simple operation,high bone graft fusion rate,less complications,curative effect is positive.

Optical coherence tomography (OCT)is thought to be the most effective tool for recognizing form of vul-nerable plaques and evaluating endothelium coverage after percutaneous coronary intervention (PCI)so far.OCT can be used to guide stent implantation,evaluate endothelium proliferation status in stent,explore causes for in-stent restenosis and detect stent late malapposition and in-stent thrombosis etc.

A master-slave electrical impedance tomography (EIT) system for body complex impedance based on digital signal processor (DSP) is designed in this paper. Used as the master computer, a PC performs the image reconstruction and display on time, while DSP carries out such tasks as electrode selection, data acquisition and data communication with the master. Based on DDS technique, the signal generator can create the sine wave signals, and it's amplitude, frequency and phase can be continuously adjusted. The designed feedback unit can improve the linearity and the stability of the voltage-controlled current source effectively. Undersampling technology and digital demodulation technology are used in this system in order to attain the information of real and imaginary components of the body complex impedance. Back projection method is adopted to reconstruct the image in this system. Experimental results show that this system improves the data acquisition speed and the image resolution.

Objective To expore the method of clinical application and the final result of pedicled flap supplied by radial osteo cutaneous branch of anterior interosseous artery Methods Basing on the anatomic basis that the osteo cutaneous and myocutaneous branches of the anterior interosseous artery have constant collateral anastomoses which can give forth a large size dorso radial flap on dorsum of the forearm Four cases of skin defect of first web had been reconstructed by this flap Results All 4 cases were successful with good functional and cosmetic results Conclusion Reconstruction with dorso radial flap of forearm is simple,reliable procedure which bring minimum trauma to the patient and is an ideal alternative operation

Objective To describe the method and review the outcome of reconstruction ofⅠandⅡdegree defect of thumb and finger by p artial transplantation of distal to e tissue from the great and /or second toe.Methods A series of 65cases with 71ⅠandⅡdegree thumb and finger defect were r econstructed.Of those,49were thumb defect,12index finger,5middle finger and 5ring fin ger.In 6cases,two digits were reconstructed simultaneously at th e same setting.For thumb reconstruction,the donor transplants were har-vested from great toe in 14cases,fro m 2nd toe in 35cases.All finger recon struction cases were taken from2nd toe.Results Postoperative arterial crisis occu rred in 10digits and venous crisis in4.All digits survived after operative and medication trea tment.Twenty digits underwent seco ndary plastic operation for better c os-metic effect.Forty-two digits in 38cases followed-up from 6months to 13years revealed good pulp pinch function.Sensation recovery to S3 + and above in 30cases(34digits)and two-point discrimination from 6-8mm in 30digits.The patients were satisfied with the appearance of the reconstructed thumb and fingers.There were no major complication in the don or toes.Conclusion The defect of distal segment and finger tip of thumb and /or finger can be reconstru cted by transplantation of partial t issues from the great,2nd or /and 3rd toe.The operation will not be influenced by anomaly of the blood vessels.Both ultimate function and cosmetic effect can meet the patientsdesire.[

Objective To evaluate the effectiveness among computed tomography-guided aspiration.minimally invasive microsurgery and conventional craniotomy on patients with intracerebral hemorrhage and their quality of life separately.Then to study the cost-effectiveness of the 3 surgical procedures. Methods One hundred and five patients with intracerebral hemorrhages were randomized into 3 groups:conventional group,stereotactic group and key-hole group.Karnofsky Performance Status Scale was examined 3 months after operation,and the cost of hospitahzation was calculated separately,then the cost-effectiveness was compared using cost-utility analysis. Results Costs of 3 procedures were 9741 yuan,7957 yuan and 13256 yuan separately,and Karnofsky Performance Status Scale were 59.7,63.7 and 50.3 separately.When self-care case was achieved in all conventional group,the stereotactic group and key-hole group need to remedy 51 eases and 10 cases separately.and the total cost was 496819 yuan for stereotactic group and 79575 yuan for key-hole group. Conclusions Minimally invasive microsurgery is optimal treatment for intracerebral hemorrhage.

Objective: To expatiate the application of int er ventional treatment in emergency management of life-threatening bleeding follow ing maxillofacial and jugular trauma. Method : Three cases of ma xillofacial and jugular serious injury with life threatening bleeding were treat ed by external carotid arteriography and embolization at branch of external car otid artery. Result:External carotid arteriography and emboliza tion could effectively stop life-threatening bleeding of maxillofacial and jugu lar serious injury. Preoperation arteriography could define diagnosis and direc t the treatment. 2 cases survived after treatment and 1 died of blood lose durin g treatment. Conclusion:External carotid arteriography and embo lization is effective in the treatment of maxillofacial and jugular serious inj ury with life-threatening bleeding.

Objective To introduce the improvement of the flap from the dorsal site of the index in the application of treatment in the thumb web contracture. Methods Apply improved first dorsal metacarpal flap for 111 cases of patients of the thumb web contracture releasing. The original island or tongue-shaped flaps was modified to be the local flap transfer to the thumb web. Surgical method simplified. Before thumb web contracture was released, the flap was taken from ulnar aspect to radial aspect on the dorsum of the index. The flap was transferred to cover the soft tissue defect on the first web space following the contracture releasing. Flap donor area was closed by split-thickness skin graft. Results One hundred and eleven cases of flaps survived. After the 3-18 months (mean follow-up of 8.5 months), the local appearance, thumb function recovered well for 94.6 percent. Conclusion Improved first dorsal metacarpal flap is effective methods in thumb web contracture releasing.

ObjectiveTo investigate the technique and clinical results of curved osteotomy under the metatarsal head's cartilage for improving reconstructed metacarpophalangeal joint function of fingers with second toe transplantation. MethodsThere were total 21 cases with 21 digits. During second toe transplantation with the metatarsophalangeal joint, the bottom of the second metatarsal head was incised. Then a curved osteotomy were carried out on about 5.0 mm under the metatarsal head's cartilage until the passive range of motion could be 90°.Longitudinal or cross-Kirschner wires were used to fix the joint.Finally,the conventional methods were used to reconstruct the blood supply, movement and nerves. ResultsAll 21 digits of the 21 cases survived uneventfully.The follow-up time was ranged from 6 to 24 months.Their average passive range of motion of the reconstructed metacarpophalangeal joint was 75°,ranging from 65° to 85°.The average active range of motion was 65°, ranging from 45° to 80°. Postoperative X-ray revealed fracture healed well without joint degeneration. ConclusionCurved osteotomy under the metatarsal head's cartilage is an effective way to improve active and passive activities function of the reconstructed metacarpophalangeal joint with second toe transplantation.

Objective To explore the inflnence of autologous peripheral blood hematopoietic stem cell transplantation (auto-HSCT) for lymphoma process on the quality of patients blood coagulation.Methods Plasma samples were collected before the conditioning and on day 0,7,14,28,35 follow auto-HSCT from 20 patients.The following parameters were measured:prothrombin time (PT),activated partial thromboplastin time (APTT),fibrinogen (FIB) and D-dimer(D-Di).Results Compared with the values before conditioning,a significant rise in the FIB values was detected on day 7 and day 14 after auto-HSCT,no essentially change in the FIB values was detected on day 21 and day 28 and day 35 after auto-HSCT.Other coagulation parameters investigated(PT.APTT.D-Di) remained essentially unchanged.All of the patients not concurrent thrombotic disease.Conclusion Lymphoma patients with autologous peripheral blood hematopoietic stem cell transplantation appear abnormality in coagulant function.Clinical doctors should concern.
