1.Changes of glucocorticoid-induced osteonecrosis of the femoral head under mechanics load

BACKGROUND:Femoral head necrosis induced by glucocorticoid is typical under mechanics load.OBJECTIVE:To create the rat models of the femoral head necrosis by long-term glucocorticoid treatment,and to Observe the changes of femoral head necrosis induced by glucocorticoid under mechanics load.METHODS:Wistar rats with 4-month old were randomly divided into the experimental and control groups.After the buttocks injection of dexamethasone 20mg/kg weakly for 8 weeks,rats in the experimental group were made to run on the running machine at a speed of 1 km/h to forme the hyper-weight model;at the same time those in the control group were with the normal physiological weight.Rats in both groups were sacrificed at weeks 2,4,6 and 8.The left legs were taken for the biomechanics test on the EnduraTEC ELF3200,the biggest vertical deformation and stiftness were calculated,the number of empty osteocyte lacunas was calculated,and the cumulative absorbance value was compared between two groups.RESULTS AND CONCLUSION:The femoral head presented with necrosis at 8 weeks in the experimental group.and the width of bone trabecula,and the stiffness of which were smaller than that of the control group.but the biggest vertical deformation was greater the concurrent control group(P<0.05).The Bcl-2 cumulative absorbance value of 2 groups had significant differences at 4 weeks(P<0.05),which was increased with time prolonged in the experimental group(P<0.05).The results demonstrated that:in glucocorticoid-induced osteonecrosis of the femoral head,mechanics Ioad induces the obviously osteocyte necrosis.more cartilage repair,and trabeculae rupture,which is considered as the direct reason of the necrosis of the femoral head The typical femoral head necrosis is presented.
2.Laparoscopic versus Open Cholecystectomy in Aged Patients in Plateau Region:A Clinical Comparison of Cardiopulmonary Complications

Objective To evaluate the safety of laparoscopic cholecystectomy's in aged patients in plateau region.Method From January 2005 to June 2006,a total of 64 local aged gallbladder disease patients were included,in which 32 cases received open cholecystectomy(OC)and 32 cases received laparoscopic cholecystectomy(LC).Postoperative cardiopulmonary complications were compared.Results In the LC group,1 case of acute pneumonedema,1 case of chronic right heart failure and 1 case of pulmonary infection occurred;and 3 cases of left heart failure accompanied with pneumonedema,2 cases of right heart failure and 5 cases of pulmonary infection occurred in the OC group.The incidence of cardiopulmonary complications of the LC group was significantly lower than that of the OC group(9% vs 31%,?2=4.730,P=0.030).Conclusions In the circumstances of high altitude,low atmospheric pressure and low oxygen partial pressure,LC is safer than OC for aged patients in regard to postoperative complications.

Objectives:We investigated and studied the incidence of depression in the wake of cerebral infarction Methods:Self-rating Depression Scale (SDS) and Hamilton Depression Scale (HAND) were evaluated and tested among 210 patients of cerebral infarction, and at the same time a contrast was made among 50 normal persons At the patients were divided into two groups, one suffered from depression and the other not suffered from depression Then they were compared Results:The depressive mood of the patients of cerebral infarction was obviously higher than those of normal group In this thesis, the incidence of cerebral infarction with depression is 58% The incidence of this disease was higher among the patients with depression history, females and those who lived apart from one's family Conclusions:The incidence of cerebral infarction with depression is comparatively high, so it should be taken seriously by the clinical doctors

Objective:To study the protective effect of glucagon-like peptide-1 (GLP-1) on the inflammatory damage induced by high glucose(HG) in placental trophoblasts HTR8/SVneo and its molecular mechanism.Methods:Trophoblasts HTR8/SVneo cells were cultured and divided into low glucose(LG) group treated with low glucose (5 mmol/L), HG group treated with high glucose (25 mmol/L), GLP-1 group treated with high glucose combined with GLP-1, miR-137+ GLP-1 group treated with high glucose combined with GLP-1 after the transfection of miR-137 mimic, miR-137 mimic group transfected with miR-137 mimic, negative control (NC) mimic group transfected with NC mimic, and miR-137 inhibitor group transfected with miR-137 inhibitor, NC inhibitor group transfected with NC inhibitor. Apoptotic rate, expression of miR-137 and IL-6 were measured.Results:The apoptotic rate and the expression levels of miR-137 and IL-6 in HG group were significantly higher than those in LG group. The apoptotic rate and the expression levels of miR-137 and IL-6 in GLP-1 group were significantly lower than those in HG group. The apoptotic rate and the expression levels of miR-137 and IL-6 in miR-137+ GLP-1 group were significantly higher than those in GLP-1 group. The apoptotic rate and the expression level of IL-6 in miR-137 mimic group were significantly higher than those of NC mimic group, the apoptotic rate and the expression level of IL-6 in miR-137 inhibitor group were significantly lower than those in the NC inhibitor group.Conclusion:GLP-1 is able to alleviate the inflammation injury of HTR8/SVneo induced by high glucose through the miR-137/IL-6 pathway.

Objective To investigate the clinical efficacy of uterine artery embolization (UAE) for the treatment of cesarean scar pregnancy compared with gelatin sponge strip and gelatin sponge particles as an embolic agent.Methods A retrospective study was performed of 94 consecutive patients with a diagnosis of cesarean scar pregnancy, and they were divided into two groups randomly.The embolic agent in experimental group was gelatin sponge strip and gelatin sponge particles were used in control group.All the cases underwent curettage operation after UAE.The mean amount of bleeding loss during curettage operation, abdominal pain degree after UAE procedure, UAE costs and time of recovery to the normal level of blood β-human chorionic gonadotropin (β-HCG) were compared between two groups.Results The mean amount of bleeding loss during curettage operation and time of recovery to the normal level of blood β-HCG were no statistical differences compared with the two groups, but the abdominal pain degree after UAE procedure and UAE costs in experimental group were lower than in control group, there were significant statistical differences compared with the two groups.Conclusion Gelatin sponge strip as an embolic agent in the UAE procedure of cesarean scar pregnancy could reduce post-operative pain and the cost, which is worthy of clinical promotion.
9.Correlation between plasma levels of urotensin Ⅱ and severity of liver fibrosis in CCl4 rats

Objective To examine the correlation between urotensin Ⅱ (UⅡ) concentration and the severity of liver fibrosis. Methods Liver fibrosis model was induced in rats by intraperitoneal administration of CCl4. At 4,6,8 weeks, the rats were sacrificed, and the hepatic tissue hydroxyproline (HYP) content was measured. Hepatic tissue specimens were histopathologically evaluated according to a fibrosis scoring system. Plasma UⅡ levels were measured by ELISA method. Results Plasma UⅡ gradually increased with the increase of duration of CCL4, UⅡ concentration correlated to liver fibrosis ( R2 = 0.875, P < 0.05) and hepatic HYP( R2 = 0.65, P <0.05). Conclusion UⅡ was involved in the pathogenesis of liver fibrosis.
10.OBSERVATION OF BLOOD SUPPLY OF METATARSUSES AND PHALANGEALS IN FETUSES AND INFANTS

The blood supply of metatarsuses and pha-langeals in 30 cases of fetuses and infants was ob-served by microangiographic methods. The resultsdemonstrated that diaphyses and cartilages of bothends of metatarsuses and phalangeals gradually in-creased in the fetal period and appeared nourishingartery and cartilage canal. Both ends of metatar-suses and phalangeals gradually decrea.ed in the. infant period and cartilage canals gradually disap-peared also. These implied that the appearance ofcartilage canals was related to the size of cartilageblocks. Cartilage Canals were temporary bloodsystem of fetal cartilage. Characteristics of bloodsupply of metatarsuses and phalangeals: 1. Asym-metrical distribution of blood supply on metatar-suses and phalangeal, that is the blood supply ofepiphyseal end is richer than one of non--epiphy-seal end, is adapted with the difference of growthability at both ends. 2. There is distribution ofcartilage canals at the epiphyseal end of the distalphalanx, and it is adapted with having growth a-bility at the proximal end. These show that bloodsupply of metatarsuses and phalangeals is closelyrelated to their growth and development.
