1.Practice and Exploration for the Teaching of New Drug Clinical Research

OBJECTIVE:To Provide reference for improving teaching levels of New Drug Clinical Research and training for pharmaceutical research talents. METHODS:The experiment teaching in 2013 graduate student of the Fourth Military Medical Uni-versity was taken as a pilot,different teaching methods were introduced in various chapters of New Drug Clinical Research,and ap-praisal system was perfected. RESULTS & CONCLUSIONS:Some appropriate methods are established,for example,conductive teaching,problem-based learning,spot-teaching and bilingual-teaching. The appraisal system including usual performance and re-search proposal is established,it has improved enthusiasm and initiative of students,promoted teaching quality,obtained certain achievements,which provides reference for the training methods on the cultivation of innovative and professional pharmaceutical tal-ents.
2.Observation on Efficacy of Compound Glycyrrhizin in Treating Chronic Hepatitis B Complicated with Active Cirrhosis

OBJECTI VE: To observe the long termtherapeutic effect of compound glycyrrhizinin treating active cirrhosisfollowing chronic hepatitis B.METHODS:58patients with active cirrhosis received compound glycyrrhizin60ml intra-venously once a day for18consecutive months.The changes in clinical symptoms,signs,biochemical indicators,virologyindices after treat ment were observed.RESULTS:35cases of active cirrhosis remained stable after treat ment,ascites disap-pearedin5cases,and17patients returnedto work.The quantitated decreasein HBV-DNA was greater than103copies/ml,HBeAg negative rates reached33.3%.CONCLUSION:Long-termtreat ment with compound glycyrrhizin for active cirrhosisfollowing hepatitis B cani mprove liver function,prevent the worsening of patients’condition,andi mprove life quality.
3.Resting heart rate and target organs damage in senile metabolic syndrome

Objective To investigate the relationship between resting heart rate (RHR) and target organs damage (TOD) in senile metabolic syndrome(SMS). Methods With case-control study,215 elderly patients with MS were divided into 3 groups according to the level of RHR [RHR1 group: RHR <70 beats/ minute (bpm) (n=55);RHR2group: 70 bpm ≤RHR<80 bpm [n=87);RHR3 group: RHR≥80 bpm (n=73).Mean-while ,60 healthy senile persons were taken as normal control group. For all objects researched, electrocardiography, echocardiography,carotid ultrasonography, creatinine clearance rate (Ccr) and quantitative assay for 24 hours' urina-ry microalbuminuria (MAU) were performed. Part of SMS patients was examined by coronary angiography and com-puter tomography (CT) of head. Results ① Compared with normal control group, RHR groups showed higher lev-els of carotid intima-medial thickness (IMT), carotid arterial diameter (CAD), left ventricular mass index (LVMI) and MAU, lower levels of left ventricular ejection fraction (LVEF) and Ccr (P<0.05 or 0.01). With RHR increas-ing among RHR groups,IMT,CAD,LVMI,MAU and the score of coronary artery stenosis increased,and LVEF and Cer decreased (P<0.05 or 0.01). ② The levels of IMT, CAD, LVMI and MAU were positively correlated to RHR (r=0.34,0.25,0.62, and 0.57, respectively, P<0.05 or 0.01). However, the levels of LVEF and Ccr were nega-tively correlated to RHR (r=-0.60,-0.52 respectively,P<0.01). ③ Logistic multivariate analysis showed that RHR, pulse pressure (PP), systolic blood pressure (SBP) and diastolic blood pressure (DBP) influenced TOD,such as myocardial hypertrophy,coronary heart disease,heart failure, cerebral stroke and renal damage in dif-ferent degrees, among them, PP and RHR played more important roles than those of other factors. Conclusion RHR may be an independent risk factors of TOD in senile MS. It is indicated that RHR regulation is important for the de-velopment of senile MS.

5.Treatment of gynecomastia with a combined method of liposuction and direction areolar incision

Objective To explore the clinical curative effect of combined liposuction and direction areolar incision on gynecomastia. Methods Fourty-three patients,aged 14-36 years,were involved in this study,including fat type (n=15) and mixed fat and gland type (n =28).The hypertrophic mammary tissues were incised through a small incision (1.0-1.5 cm) after the tumescent liposuction.And tumescent liposuction technique was used again to make sure of the transition-layer more smooth.The incision was in compression bandage after surgery area and stitches taken out after 7 days.Results All the patients with gynecomastia were treated.The nipple-areola complex were all alive.The scar of incision was not obvious.Follow-up was done for 3 years,showing that configuration of the breasts was good.Conclusions The surgical procedure is simply,less haemorrhage and quick recovery,with good aesthetic effects,which deserves wide application.
6.Moving cupping at Hechelu combined with rubbing method for depression of diabetes mellitus.

OBJECTIVETo compare the efficacy between moving cupping at Hechelu combined with rubbing method and western medication for depression of diabetes mellitus (DM).
METHODSTwo hundred and sixteen patients were randomly divided into an observation group and a control group, 108 cases in each group. Patients in the observation group were treated with moving cupping at Hechelu combined with rubbing method, once every: other day; six treatments were considered as one course, and totally two courses were given with an interval of: 4 days between courses. Patients in the control group were treated with oral administration of fluoxetine hydrochloride capsules, once a day for consecutive 4 weeks. The Hamilton depression scale (HAMD), self-rating depression scale (SDS) and TCM symptom score were measured before treatment, after the treatment and in follow-up visit one and a half months after treatment. The fasting blood glucose was tested before and after treatment. The glycosylated hemoglobin (HbA1c) was tested in the follow-up visit.
RESULTSThe total effective rate was 90.9% (90/99) in the observation group, which was superior to 73.7% (70/95) in the control group (P < 0.05). After the treatment, HAMD, SDS and TCM symptom scores were all reduced apparently in the observation group and the control group (all P < 0.05). After the treatment and the follow-up visit, the TCM symptom score in the observation group was lower than that in the control group (P < 0.05). The levels of HbA1c and GLU were stable in the observation group, and were decreased compared with those before treatment; but the difference between the, two groups was not significant (P > 0.05).
CONCLUSIONThe Hechelu theory-based TCM treatment has better: efficacy for depression of diabetes mellitus than fluoxetine hydrochloride capsule, which has less adverse effects.

7.The treatment of aortic dissection with stent-graft

Objective To summarise the clinical experience on the treatment of aortic dissection with stent-graft. Methods From June 2005 to November 2008, 117 patients with aortic dissection underwent surgery with stented elephant trunk (CRONUS stent-graft)which was implanted through the aortic arch under deep hypothermic circulatory arrest, including Stanford A in 50 patients who underwent Bentall operation and total arch grafting with stented elephant trunk operation in 28, Wheat operation and total arch grafting with stented elephant trunk operation in 8, ascending aorta and total arch replacement with stented elephant trunk operation in 14 and Stanford B in 67 patients who all underwent stented elephant trunk operation. Results The location of the entry was in the ascending aorta in 35 patients, the aortic arch in 4, the proximal descending aorta in 78. Cardiopulmonary bypass time was ( 159 ±31 ) min, average cross clamp time was (95 ±23) min, and average selected cerebral perfusion and lowerbody arrest time was (27 ±8) min. The in-hospital mortality was 2.6% (3/117). One patient died of the hemorrhage shock, one died of multi-organ failure postoperatively and the third died of infective endocarditis. The complications included transient neurologic deficit in 11(9.4%), cerebrovascular accident in 6 (5. 1%, cerebral embolism in 2 and cerebral hemorrhage in 4, of which 5 cases were discharged from hospital. One case died of multiorgan failure 18 days after discharge owing to his family' s giving up treatment), hemorrhage of anastomose which was treated by reoperation in 5,hoarseness in 2, renal failure in 1. One case died of the cerebral hemorrhage after 4 months due to inappropriate anticoagulation during following up. Conclusion The treatment of the aortic dissection with stent-graft which was implanted into the descending aorta through aorta arch is safe, effective and feasible, which will be helpful to vascular reconstruction by accurately close subintimal rip and can streamline surgery with minimally invasive and less complications.
8.Total arch replacement combined with stented elephant trunk implantation for DeBakey Ⅰ aortic dissection

Objective It is controversial to the surgical treatment of the DeBakey I aortic dissection.The purpose of the study was to summarize the clinical experience on total arch replacement combined with implantation of stented elephant trunk into the descending aorta for DeBakey I aortic dissection.Methods From June 2005 to March 2008,41 consecutive patients with acute(in 31)or chronic(in 10)DeBakey I aortic dissection underwent total arch replacement combined with implantation of stented elephant trunk into the descending aorta.The mean age was 57(27~76)years.Thirty-two patients were male.The procedure was performed under deep hypothermic circulatory arrest and selected cerebral perfusion.The stented elephant trunk was implanted through the aortic arch under deep hypothermic circulatory arrest.The stented elephant trunk was an 8~10 cm long self-expandable graft.Concomitant procedures included Bentall operation and total arch replacement in 24,Wheat operation and total arch replacement in 6,ascending aorta and total arch replacement in 11.Computed tomography was performed in every patient before discharge to evaluate the postoperative outcomes of the procedure.Results Cardiopulmonary bypass time was(168±32)min.The cross clamp time was(109±24)min and selective cerebral perfusion and the lowerbody circulatory arrest time wsa(31±11)min.The in-hospital mortality was 4.9%(2/41).One patient died of hemorrhagic shock and another died of multi-organ failure postoperatively.Fourteen cases suffered complications and 12 cases were cured.One case died of the cerebral hemorrhage after 4 months due to inappropriate anticoagulation during follow up.Conclusion Ascending aorta and total aortic arch replacement combined with implantation of stented trunk into descending aorta is a safe,effective and feasible way in closing the residual false lumen of the descending aorta with a low morbidity and mortality.This procedure might contribute to a better long-term outcomes of DeBakey I aortic dissection.
9.Experimental study of the stachyose, an extract of Chinese herbal medicine, in rats functional bowel disorder induced by stomach irritation with cool water

Objective To investigate the regulatory effects of stachyose through oral administration on the facilitating feces excretion function and its relationship to the intestinal flora in stomach irritation with cool water-induced functional bowel disorder rats. Methodes Stomach irritation with cool water -induced constipation-predominant irritable bowel syndrome model rats were administrated by i.g. with stachyose at intestine were determined within 12 hours on the 7th and 14th day after stachyose administration, and intestine flora was measured on the 14th day after Stachyose administration. Results The feces granules and the moisture contests of feces and the efficiency of the small intestine were increased in stachyose treated C-IBS rats, and the number of intestinal flora was significantly higher than that in the control C-IBS rats [ (38.43±4.57) grain VS (32. 21 ± 3.43) grain, F = 3.892, P < 0.05; (47. 88 ± 3.43)% VS (43. 18 ±6.85)%, F =6.724, P <0.01;(1.04±0. 11)radian VS (0. 88 ±0.08)radian, F =4.965, P <0.05;(10.77 ±0.44)1g(CFU/g) VS (9.85 ±0.43)1g(CFU/g), F =7.613, P <0.01;(10.96±0.35)lg(CFU/g) VS (9. 84 ±0. 35)1g(CFU/g), F = 10. 413, P <0. 01 ]. Conclusion The results indicated that the stachyose,an extract of Chinese herbal medicine, had the facilitating feces excretion function and modulation functionon intestinal flora in stomach irritation with cool water-induced functional bowel disorder rats.
10.Role of Microglia Polarization in Alzheimer's Disease and Association to Electroacupuncture (review)

There are two phenotypes of microglia, M1 and M2. Microglia in M2 polarization may associate with the phagorytosis of be-ta-amyloid and inhibition of Tau hyperphosphorylation, as well as in other pathology. Electroacupuncture can impact the phenotypes of mi-croglia, which may play a role in the treatment for Alzheimer's disease.
