1.Abnormal neural behaviors of neonatal rat due to hyperthermia and low dosage of cigarette exposure during pregnancy

BACKGROUND: Neural behavior is a sensitive parameter for predicting abnormal central nervous growth that is resulted from multiple factors. Therefore this study was designed to explore the influence of hypothermia and cigarette smoke exposure during pregnancy on neural behavioral growth of neonatal rats.OBJECTIVE: To study the influence of hypothermia and cigarette smoke exposure on neural behavioral growth of neonatal rats.DESIGN: Randomized case control study based on experimental animals. SETTING: Obstetric and Gynecological Department , Third Affiliated Hospital of Xinxiang Medical College; Cytobiological Department of Xinxiang Medical College.MATERIALS: Totally 270 healthy unpregnant SD rats were obtained, including 180 males with body mass of 190- 210 g, and 90 female rats with body mass of 225-275 g. Rats were raised with granulated feed and drinking water routinely in room of 25 ℃ with natural sunlight. Male and female rats in estrus were put in the same cage overnight in proportion of 2: 1, pregnancy was confirmed if vaginal bolt was observed the following morning. Totally 160 pregnant rats were obtained and randomly divided into 16 groups that were exposed to hyperthermia, cigarette smoke or their combination and by this way, earlier reflex, learning and memory of neonatal rats were observed after natural delivery,INTERVENTIONS: From the second day of pregnancy, rats in 37 ℃, 41 ℃,42 ℃ hyperthermia combining cigarette group received subcutaneous injection of 1 mL smoke hydrotrope every other day, while rats in 25 ℃ control group and 37 ℃, 41 ℃, 42 ℃ single hyperthemia groupwere given 2.0 mL double distilled water every day for totally 7 times. From the pregnancy of 10 days, pregnant rats were put into incubator, and rats anus temperature should be kept respectively at(37 ±0.5) ℃, (41 ±0.5)℃, (42 ±0.5)℃in the 37 ℃, 41 ℃, 42 ℃ of single hyperthemia groups and the temperature should last for 2.0 minutes; nats of 37 ℃, 41 ℃, 42 ℃hyperthemia combining cigarette group received the same treatment and were divided into four subgroups with the duration of 0. 5, 1.0, 1.5, 2.0 minutes respectively;rats in 25℃ control group were not exposed to hyperthermia. Rats were then singly raised from the pregnancy of 18 days for natural delivery. Pregnant days, number of neonatal rat and death rats were recorded, and visible malformation was observed. The average body masses of each cote of neonatal rats were measured every week. Time of eye-opening, ear-opening, hair-growing and teething were observed for assessing their physical growth. Reflex and sensory function of neonatal rats, such as planar tuning, spatial turning, visual orientation and auditory surprise was observed at 3, 7, 12, 16 days after birth respectively; The mean targeting time of each cote was used as reference for assessing their feflex. Y-lybirinth test was used for assessing learning and memory function of neonatal rats(30 days after birth).MAIN OUTCOME MEASURES: ① Main outcomes: Reflex and sensory, such as planar tuning, spatial turning, visual orientation and auditory surrise. ② Secondary outcomes: Influence of hyperthemia and cigarette exposure on learning memory function of neonatal rats.RESULTS: For hyperthermia group(41C and 42C), targeting time of neonatal rat physical growth(eye-opening, ear-opening, hair-growing and teething) and reflex sensory function(planar tuning, spatial turning, nvisual orientation and auditory surprise), as well as learning and memory(minute) were obviously delayed in contrast with those of the control group and 37 C group(χ2 = 11.34, P <0.01 ); meanwhile, longer hyperthermia exposure (1.5 minutes) combined with cigarette smoke had supplementary effect on the above neural behaviors, which was significantly different from that of single hyperthermia group(χ2=10.29, P<0.01).CONCLUSION: Abnormal neural behavior of neonatal rats, such as growing retardation, learning and memory obstacle could be induced by the combination of longer hyperthemia exposure and cigarette smoke.
2.Recent Research Situation of Heart Rate Variability-based Analysis of the Regulating Effect of Acupuncture on Autonomic Nervous Function

Traditional Chinese medicine considers that the final purpose of acupuncture treatment for disease is to harmonize yin and yang. Acupuncture redresses a balance between yin and yang by transmitting information to acupoints and then to corresponding visceral tissues and organs and the brain through the meridian system or the nervous system. In recent years, scholars at home thoroughly studied heart rate variability (HRV) analysis for acupuncture treatment of diseases and found that the regulating effect of acupuncture on visceral organs was closely related to autonomic nerves and acupuncture could restore a balance between sympathetic and parasympathetic nerves. Many scholars explored the effect of acupuncture on diseases related to autonomic nervous dysfunction by HRV frequency domain or time domain analysis and gained a certain achievements. This article summarizes recent years’ domestic studies on HRV-based analysis of acupuncture for diseases with autonomic nervous dysfunction and reviews the close relationship between autonomic nerves and diseases and the related mechanism of acupuncture action.
3.The endoscopic features of gastric low-grade intraepithelial neoplasia with carcinomatous transformation

Objective This study aimed to evaluate the potential predictive factors for carcinomatous transformation of gastric low-grade intraepithelial neoplasia (LGIEN) on the basis of endoscopic features.Methods The study involved 312 gastric LGIENs that were histologically confirmed by endoscopic forceps biopsy (EFB) between May 2004 and September 2010,and then removed by endoscopic mucosal resection or endoscopic submucosal dissection were enrolled in this study.According to EMR/ESD postoperative pathological findings,they were divided into LGIEN group and the HGIEN group,and compared their endoscopic characteristics.Results There were not significant different between the two group,as the mean age、gender the diameter of the lesions、lesion sites and surface nodularity.The diameter of the lesions was 14.6 ± 8.2 mm in the LGIEN group and 22.0 ± 0.55 mm in the HGIEN group (P < 0.05).42 of 69 gastric adenomas (60.9%)larger than 20 mm in diameter showed HGIEN (P < 0.05).Hyperemia and mucosal ulceration were significant differences in the two groups.Conclusion The LGIEN lesions with these endoscopic characteristics,such as size (> 2 cm),surface hyperemia,ulceration should be considered for EMR/ESD.
4.Efficacy of Saccharomyces boulardii in treatment of candida esophagitis

The efficacy and safety of Saccharomyces boulardii in treatment of candida esophagitis was investigated by a single center, prospective, open label and non-inferiority trial.Eighty nine patients with candida esophagitis attending in Shanxi Provincial People′s Hospital during April 2015 to March 2016 were randomly divided into Saccharomyces boulardii group (45 cases) and nystatin group (44 cases), and oral Saccharomyces boulardii powder and Nystatin tablets were given to two groups respectively.The curative effect was evaluated by gastroscopy after the treatment.The results showed that the cure rate of Saccharomyces boulardii group and nystatin group was 68.9%(31/45)and 63.6%(28/44), and the Saccharomyces boulardii was noi inferior to nystatin (x2=3.027, P<0.05).Chi-square test showed that there was no significant difference in effective rate between two groups [91.1%(41/45)vs.93.2%(41/44), x2<0.001, P>0.05].The incidence of adverse reactions in boulardii group was significantly lower than that in nystatin group [2.2%(1/45) vs.22.7%(10/44), x2=8.636, P<0.05].The study suggests that the cure rate of Saccharomyces boulardii is not inferior to nystatin in treatment of candida esophagitis, while the incidence of adverse reactions of Saccharomyces boulardii is lower.
5.Clinical significance and change with age of cervical vertebral uncinate process and transverse process of teenagers

Objective To explore the uncinate-transverse hole spacing developmental patterns and aging characteristics by means of studying the teenagers aging from 6~20 years old,and provide the basis for image analysis of the treatment and prevention of cervical spondy-losis. Method the 66 adolescents without trauma nor problems of nervous system were selected to do thin spiral CT scan(0. 625 ~1. 25 mm) ,the range was C1 ~T1 . The original data in the form of DICOM are analyzed according to gender and age. Results There was no sig-nificant difference in gender and different sides. Overall,the physical development of human increases gradually with age and has some signifi-cant differences. Conclusion There exists a close relationship between cervical hook-transverse process and the incidence of vertebral artery type and nerve root type cervical spondylosis.
6.Laparoscopy with simultaneous ERCP for cholecystolithiasis combined with choledocholithiasis

ObjectiveTo investigate the feasibility of laparoscopy with simultaneous ERCP for cholecystolithiasis combined with common bile duct stones.MethodsThe clinical data of 150 cases with combined choledocholithiasis and cholecystolithiasis who underwent laparoscopy and simultaneous ERCP from November 2009 to March 2012 were analyzed retrospectively.Under general anesthesia,laparoscopic cholecystectomy (LC) was performed and followed by ERCP.Common bile duct stones were removed by ERCP and endoscopic nasobiliary drainage (ENBD)was placed.Therapeutic effect and complications were observed.ResultsLC and ERCP were successfully completed in all patients.There was no bleeding,perforation,severe pancreatitis or other severe complications occurred in any patient during and after operation.Postoperative gastrointestinal function recovery time and hospitalization were short.During follow-up,no residual or recurrent stones were found by abdominal ultrasound examination.Conclusion Simultaneous ERCP and laparoscopy is effective and safe treatment with less invasiveness and quick recovery for cholecystolithiasis combined with choledocholithiasis.
7.Short-term acute rejection incidence of recipients under the steroid-free immunosuppressive therapy after liver transplantation

Objective To investigate the short-term acute rejection incidence of the recipients under the steroid-free immunosuppressive therapy after liver transplantation. Methods This retrospective study included 186 patients who were divided into two groups by random number table.The patients in no steroid group (the study group, n =94) received tacrolimus (Tac) with mycophemolate mofetil (MMF) or cyclosporine with MMF,and those in the steroid group (the control group,n =92) received the aforementioned immunosuppressive therapy combined with steroids.The acute rejection incidence was analyzed during six months post-transplantation.Results There was no significant difference in the gender,age,indication for transplantation,Child-Pugh score,MELD score,operating time,bleeding and transfusion volume during the operation,warm ischemia time and cold ischemia time between the two groups (P>0.05).Liver biopsy was done on 9 cases of each group.The acute rejection incidence had no significant difference between the study group and the control group (5/94 vs 4/92,5.3% vs 4.4%,P>0.05).Conclusion The steroid-free immunosuppressive therapy after liver transplantation did not increase the short term acute rejection incidence.
8.ImmuKnowTM cellular immune functional assay in the diagnosis of infection after liver transplantation

Objective To evaluate the applied value of functional immunity measured by the ImmunKnow assay in the diagnosis of post-transplant infection in Chinese 1iver recipients.Methods Thirty-eight normal adults and 68 adult liver transplant recipients were under investigation.Whole blood samples from either normal volunteers(each sample for one person)or the liver recipients(one or more samples for one person)were collected freshly and cultured within 6h.The CD4+T cells were selected and their ATP value was assayed the next day.The liver recipients were grouped in stable status(n=52)or infection(n=64)according tO their clinieal manifestation.Results The average ATP value in the recipients with infection after liver transplantation was 165.7±100 μg/L,significantly lower(P<0.05)than that in stable recipients(309±126 μg/L)or normal volunteers (292±83 μg/L).The low ATP levels in post-transplant recipients had fair good correlation to infection clinically(RR=0.5021,P<0.01).Infectious risk was high when ATP value was less than 165μg/L(OR=11,95%CI 3.9-32.2,P<0.01).Specificity and sensitivity of low ATP value in post-transplant infection were 86.53%and 73.81% respectively,Conclusion ImmuKnow assay provides a new tool in monitoring immune status in post-transplant recipients,and call helpfully predict and diagnose clinical infection.
9.Radiographic anatomy features of teenagers’ upper-middle thoracic vertebrae

BACKGROUND:The conditions of teenager and adult skeletons have great differences. Thus, it is dangerous to place screw on teenagers using the methods for adults.
OBJECTIVE:To observe the anatomical features of teenagers’ upper-middle thoracic vertebra images, and to provide experimental basis for pedicle nailing and pedicle-rib head unit nailing in teenagers’ upper-middle thoracic vertebrae.
METHODS:We chose 30 teenagers (13 to 18 years old) without spine lesions, and then they were scanned (0.625-1.25 mm) from T 1 to T 12 segments using CT. The original data were loaded into a three dimensional software in DICOM format for measuring related indicators. At last, we analyzed the data according to different segments.
RESULTS AND CONCLUSION:The trend between the width of pedicle and pedicle-head unit was similar, showing a“V”shape. The minimum of width was located in the T 4 , and there were significant differences between the corresponding vertebral bodies (P<0.05). The trend between pedicle nail road length and pedicle-rib head unit nail road length was similar, and the trend between them was gradual y increasing. The minimum of length was located in T 1 , and the maximum of length was located at T 10 , and there were significant differences between the corresponding vertebral bodies (P<0.05). The trend between the pedicle transverse angle and pedicle rib-head unit transverse angle was similar. The trend from T1 to T10 was on a decline, and there were significant differences in corresponding vertebrae (P<0.05). The trend of curves between pedicle sagittal angle and pedicle rib-head unit sagittal angle was similar, showing a wavy shape, and there were significant differences in corresponding vertebrae (P<0.05). These results confirmed that the same segmental pedicle rib-head unit can provide a safer nailing place than the pedicle relatively.
10.Pharmacokinetics of genistein in urine of healthy volunteers.

In order to study the excretion of genistein (GEN) capsule, an estrogen drugs, in human, 30 healthy volunteers were selected and orally administered 50, 100, and 300 mg genistein in an parallel study. Genistein were determined in urine by LC-MS/MS and glucuronidated genistein (GENG) were indirectly determined with enzymatic hydrolysis in urine by LC-MS/MS, and the pharmacokinetic parameters were analyzed by DAS software (ver 2.0). The result showed that the concentrations of genistein in human urine were less than 1% of the GENG, and the cumulative excretion of GEN in 48 h were 0.037, 0.134, and 0.142 mg, separately, and the urinary excretion percentage were only 0.07%, 0.13%, and 0.05%, separately. But the cumulative excretion of GENG in 48 h was 5.3, 13.8, and 15.4 mg, separately, and the urinary excretion percentage were 10.6%, 13.8%, and 5.1%, separately, and the max urinary excretive rate was 0.4, 1.0, and 1.4 mg x h(-1), separately (tmax were 6 h). Studies showed that part of drug excreted through kidney in a form of GENG in human, and the cumulative urinary excretion and the maximum excretion rate of GENG showed a proportional increase conditioned with the dose in the range of 50-100 mg, but showed non-linear increase feature in 300 mg.
