1.Assessment of corticosteroids in prevention and treatment of neonatal bronchopulmonary dysplasia

There is a great controversy over the application of corticosteroids in preventing and treating bronchopulmonary dysplasia(BPD).Prenatal use of corticosteroids may reduce the occurrence of BPD.Intravenous corticosteroids within 1 week after birth can reduce the incidence of BPD.However,in light of the short and long-term adverse reactions,it is not recommended.For premature infants with BPD in high risk,clinicians should weigh the pros and cons and may apply a small dose,short course of corticosteroids for intervention in 1 week after birth.Early inhalation or instilling of corticosteroids may be an effective and safe way to reduce the occurrence of BPD.
2.Change of Thyroid Hormone and its Meaning in Patients with Chronic Heart Failure

Objective To investigate the clinical significance between congestive heart failure (CHF) and serum thyroxine. Methods 50 patients with CHF (7 cases of grade Ⅱ heart function,grade Ⅲ 19 cases,grade Ⅳ 24)and 30 normal persons wre selected. The serum triiodothyronine( T3 ), four-triiodothyronine( T4 ), free triiodothyronine (FT3), free triiodothyronine (FT4), thyroid stimulating hormone TSH were determined by radioimmunoassay. Results The T3, FT3 contens ( 1.95 ± 0.55) nmol/L, (2.20 ± 0.39 ) pmol/L in CHF group were lower than the control group (2.4 ± 0.50) nmol/L, ( 5.89 ± 0. 45 ) pmol/L ( t = 2. 415,2. 325, P < 0.05 ); The T3, FT3 contens ( 2.19 ± 0. 53 )nmol/L,(3.69 ± 0.57)pmol/L after treatment were significantly higher than that before treatment (t =2. 375,2. 405, P < 0.05 ); The T3, FT3 contens in cardiac function with stage Ⅳ ( 1.76 ± 0.7) nmol/L, ( 1.99 ± 0. 45 ) pmol/L were significantly lower than the control group( t = 2.245,2. 375, P < 0.05 ); Serum TT3, TT4 and cardiac function was negatively correlated ( r = -0.933, -0.383, all P < 0.05 ) , serum FT3, FT4,TSH and cardiac function were not correlated. Conclusion The changes of serum thyroid level could be used as an index of the severity of heart failure.
3.Clinical Analysis of 53 Cases of Postmenopausal Vaginal Bleeding Caused by Hormone Replacement Therapy

Objective To investigate the effects of HRT on endometrium of postmenopausal women.Methods 53 cases of postmenopausal vaginal bleeding(PMB),from Jan,1995 to Dec.1999 treated by HRT were selected to make a comparison with the other 60 cases of PMB without HRT during the corresponding period.Both groups had integrated ultrasonic and pathological data and getting information from succedent inspection.Results The rates of the endometrial carcinorma incidences that happen in the HRT group were non-HRT group were 5 66% and 8 33% respectively,which had little significant.While the occurring rates of inflammation were 1 89% and 16 67% respectively,a significant difference between the both existed here.Conclusions With HRT,the various symptoms of post menopause women can be efficiently improved.It is safe and effective in short term.But a close monitoring is absolutely necessary for its long term effect(5 years or more).
4.Bone marrow mesenchymal stem cells for repair of cartilage defects

BACKGROUND:Bone marrow mesenchymal stem cels are a kind of non-hematopoietic adult stem cels, mainly distributing in the bone marrow, with strong proliferation and differentiation potential, which has a promising clinical prospect. OBJECTIVE:To review the latest progress in growth factors and biological scaffolds to promote chondrogenic differentiation of bone marrow mesenchymal stem cels. METHODS:PubMed and CNKI databases were searched by the first author using key words of cartilage defects, tissue engineering, biological scaffolds, bone marrow mesenchymal stem cels, cytokines in English and in Chinese, respectively, to retrieve relevant articles published from 1990 to 2014. Literatures addressing the chondrogenic differentiation of bone marrow mesenchymal stem cels were included, and 51 articles were chosen for further analysis eventualy. RESULTS AND CONCLUSION:Bone marrow mesenchymal stem cels have the potential to differentiate into chondrocytes, which can be induced by many cytokines. Various biological scaffolds act as a carrier for chondrogenic differentiation of bone marrow mesenchymal stem cels. But there are stil lots of problems to be solved and in-depth explored clinicaly.
5.Chemotherapy combined with immunotherapy in treatment of middle and advanced stage non-small cell lung cancer

Objective To study the efficacy of chemotherapy combined with immunotherapy in treatment of middle and advanced stage non-small cell lung cancer and improvement of patients'life quality.Methods 140 patients with middle and advanced stage non-small cell lung cancer treated from Mar .2012 to Mar.2014 were in-cluded and they were randomly divided into two groups:the observation group(70 cases)and the control group(70 cases).The observation group took chemotherapy combined with immunotherapy , while the control group took chemotherapy treatment alone .Patients'life quality , treatment effects and changes in immune indexes were co-mared between the two groups .Results The improvement rate of life quality of the observation group (90.0%) was significantly higher than that of the control group (58.6%), and the difference had satistical significance (P<0.05).The response rate of the observation group (88.6%) was significantly higher than that of the control group(70.0%), and the difference had statistical significance (P<0.05).The increasement rate of CD3 +, CD4 +, CD8 +cells, as well as CD4 +/CD8 +cells was significantly higher in the observation group than in the control group, and the difference had statistical significance (P<0.05).Conclusion Chemotherapy combined with immunotherapy in teatment of patients with middle and advanced non-small cell lung cancer has obvious effect, high safety, and high clinical value .
6.Effects of sMICA on receptors of NK cells in breast cancer

Objective To observe the expression of cytotoxicity and receptors on NK cells in breast cancer,and investigate the impact of soluble MICA(sohble MHC class Ⅰ-related molecules A,sMICA) on NK cells receptors expression and cytotoxicity.Methods ELISA was used to examine the sMICA in peripheral blood.The expressions of activated receptor(NKG2D),killer inhibitory receptor (KIR)(CD158b) and NK cells were identified by flow cytometry(FCM).Cytotoxicity of NK cells to breast cancer were tested by MTT.Results sMICA was (205.36±71.27)ng/L in breast cancer patients and 81.6 % samples were detected.There were positive correlations between sMICA levels with breast cancer stages.There were no difference of NK cells percentage between breast cancer and healthy person.The cytotoxicity of NK cells and expressions of NKG2D were obviously lower in breast cancer with sMICA(+) than in healthy person,but CD158b was higher in healthy person.After cultured with sMICA,NK cells cytotoxicity decreased from(76.2±6.7)% to(48.4±4.1)% and the expression of NKG2D reduced from(92.5±7.1)% to (62.5±6.4)%,but the the expression of CD158b increased from(10.6±3.2)% to (43.6±3.4)%.IL-15 up regulated the expression of NKG2D and NK cells cytotoxicity,but decreased the expression of CD158b by co-culturation with IL-15 and sMICA in sMICA+ patients with breast cancer.Conclusion sMICA reduced the expression of NKG2D and increased the expression of Kill,which lead to the down regulation of NK cells cytotoxicity.IL-15 can reverse this effect.
7.Requirements of human brain-derived neural stem cells for different storage, isolation and cultivation

BACKGROUND: Due to a new finding in the in vitro proliferation and differentiation of neural stem cells, neural stem cells are regarded as optimal materials for repairing and replacing the injured neural tissues. But how to enhance the proliferation of neural stem cells and how to induce neural stem cells to differentiate into certain phenotypes remain objects for discussing.OBJECTIVE: To investigate the frozen/nonfrozen storage techniques in isolation and cultivation of neural stem cells isolated from human brain and to discuss the conditions suitable for their differentiation.DESIGN: A single sample based on cells.SETTING: Urinology Institute of Jiangxi Medical University and Neurosurgery Department of Second Hospital Affiliated to Jiangxi Medical College.MATERIALS: From December 2003 to June 2004, this study was conducted at the Urinology Institute of Jiangxi Medical College. Brain tissues removed from sixteen-week-old embryos were studied.METHODS: After being digested with trypsin, cells were separated from the brains of embryos. They were cultured in the serum-free media, stimulated by basic fibroblast growth factor and epidermal growth factor and were induced to differentiate using serum. Immunofluorescent cytochemical staining technique was adopted to detect the expressions of the Nestin and the neuron-specific enolase(the marker for mature neural cells) in these cells. The influence of basic fibroblast growth factor, epidermal growth factor and serum on the proliferation and differentiation of the neural stem cells were investigated.MAIN OUTCOME MEASURES: The main outcome measurements in this tification of the Nestin and the neuron-specific enolase in these cells.bryo brain successfully. The embryo brain cells in primary culture were clear and round. After 3 days some cells aggregated to form neurospheres and some others suspended. Two weeks later, the neurospheres enlarged and a part of cells in it expanded into folds with strong reflection and proliferation. The continuous cultured cells inherited these characteristics and kept the morphological properties. The clones were cultured in the medium with serum. Twenty-four hours later, most of the cells were attached to the bottom of the plates. Cells were dissociated from the neurospheres and became irregular. Forty-eight hours later, most of the cells differentiated into many scattered patches of astrocytes with various shapes and neurites and many isolated neural stem cells were continuously cultured in vitro and they expressed Nestin, the marker of neural stem cells. After being cultured in medium with serum for 48 hours, the differentiated cells mainly expressed freeze-thaw cells were alive. Their living status had no significant difference compared to that of the fresh isolated cells.CONCLUSION: Serum-free culture medium, together with basic fibroblast growth factor and epidermal growth factor, enables the in vitro culture, proliferation and purification of the neural stem cells. The feasibility of the method is confirmed. Meanwhile, the comparison between the cultures of freeze-thaw cells and fresh isolated cells indicated that frozen storage can be used as a way to preserve the human embryo brain cells, which will be available for study after thaw.
8.Quantified walking and breathing exercises in the community rehabilitation of patients with chronic obstructive pulmonary disease

Objective To observe the effects and feasibility of using walking and breathing exercises to help patients with chronic obstructive pulmonary disease in the community.Methods A hundred and one communitydwelling patients with moderate to severe chronic obstructive pulmonary disease were randomized into an experimental group (51 cases) and a control group (50 cases).The control group was given conventional pharmacotherapy,while the experimental group was given quantified walking (i.e.walking for a pre-set number of steps daily) and breathing exercises (15 minutes,3 sessions daily) to do at home for 48 weeks in addition to the conventional pharmacotherapy,.Both groups were evaluated before and after the 48 weeks of treatment using the 6-minute walk test (6MWT),a clinical COPD questionnaire,Medical Research Council (MRC) dyspnea scoring,a general anxiety/depression questionnaire and a pulmonary function test.In addition,days of hospitalization and incidence of acute exacerbation were also recorded and compared between the 2 groups.Results Among the 101 cases,92 (91.1%) completed the study (45 in the experimental group,47 in the control group,a difference which was not statistically significant).After treatment,the average 6MWT distance in the experimental group was significantly increased and significantly greater than that in the control group.No statistically significant intra-group or inter-group differences were observed with regard to the pulmonary function test or the MRC dyspnea scores.Intra-group comparison and inter-group comparison of the clinical COPD questionnaire scores and anxiety/depression scores did,however,reveal statistically significant differences.There was no significant difference in the average number of acute attacks or in the length of hospital stays.Conclusions Quantified walking and breathing exercises are effective and feasible therapy for patients with chronic obstructive pulmonary disease in the community.
9.Effects of seretide on lower airway bacterial colonization in stable phase of chronic obstructive pulmonary disease

Objective To explore the effects of seretide (salmeterol 50 μg and fluticasone propionate 500 μg) on lower airway bacterial colonization (LABC) in the patients with stable moderate-tosevere chronic obstructive pulmonary disease (COPD).Methods A total of 84 patients with moderate-tosevere COPD were divided into 2 groups,i.e.test (n =43) and control (n =41).The control group received basic treatment while the test group seretide 50/500 μg per inhalation twice daily for 2 years plus basic treatment.Induced sputum specimens were analyzed for quantitative bacteriology and bacterial culture.The pulmonary function key parameters and exacerbation frequency were compared between two groups at pre- and post-therapy.Results The test group has significantly a lower positive rate of bacterial culture in the lower respiratory tract than before(16.3% vs.34.9%,x2 =3.9091,P =0.0480).While in the control group,the positive rate of bacterial culture stayed almost the same as compared with that at pre-therapy (x2 =0.2253,P =0.6350).The test group had significantly decreased exacerbation frequency than the control group (1.9±2.1 vs.2.9 ±2.3,t =2.0364,P =0.0449);FEV1 of the control group decreased by (0.0885 ± 0.0186)L in 2 years and decreased by (0.0807 ± 0.0165)L in the test group.Significant differences existed in two groups (t =2.0355,P =0.0450).Conclusion The treatment of seretide can decrease LABC,slow decreasing trend of lung function and improve clinical symptoms in the patients with stable moderate-to-severe COPD.
10.The influence of smoking on lower airway bacterial colonization in stable phase of chronic obstructive pulmonary disease

Objective To observe the influence of smoking on lower airway bacterial colonization(LABC)in stable phase of chronic obstructive pulmonary disease(COPD).Methods 104 patients with moderate or severe COPD were divided into three groups:42 cases smoking continuously were divided into A group,36 cases quit smoking were B group,34 non-smoker were C group.Induced sputum were analyzed for quantitiative bacteriology and bacterial culture,pulmonary function was evaluated at begining and in two years later.Results Smokers has higher positive rate of LABC than non-smokers;patients in B group has significantly lower positive rate of bacterialculture in quit smoking 2 years late than before,and significantly lower than that in A group;bacterial colonization in A group and B group include Streptococcus pneumoniae,Haemophilus influenzae,Moraxella catarrhalis; patients in B group quit smoking 2 years late and patients in C group has significantly lower positive rate of Pseudomonas aeruginosa,significantly decreased exacerbation frequency and decline of FEV1 than patients in A group.Conclusion Smoking increase positive rate of LABC in patients with moderate or severe COPD.Smoking is an important factor in deterioration of lung function in COPD.there is positive significance to quit smoking in patients with COPD.
