1.Analysis of influence factors of hospital knowledge team performance based on WSR

Objective To understand the various factors affecting hospital knowledge team performance.Methods Constructing the theoretical model of factors affecting team performance based on the WSR,adopting a self-designed scale to carry out questionnaire investigation,and using the method of statistical analysis to validate the rationality of the model. Results Hospital knowledge team performance is influenced by physical factors (4.38 ),Shi-Li factors (4.5 1 ),and Ren-Li factors (4.59),which Ren-Li factors influence is greater.Conclusion The WSR methodology provides a convenient and efficient way for understanding the determinants of hospital knowledge team performance.
2.Research on the performance structure of hospital knowledge team

Objective To establish the performance structure model of hospital knowledge team. Methods Proposing the hypothesized model on the basis of theoretical research,adopting a self-designed scale to carry out questionnaire investigation,using the method of statistical analysis to validate the fitting degree of the model.Results The performance structure of hospital knowledge team is composed of six dimensions,including patient service,quality management,medical quality,work efficiency,team growth,and team members feeling.The six dimensions can explain 76.92% of the variance.Conclusion With its special of hospital knowledge team performance,the performance structure is different from the existing research results.

4.Dosimetric study in intensity-modulated radiotherapy with dissimilar position for cervical cancer

Objective To analyze the difference of irradiation dose and volume of organs at risk (OAR) particularly in small intestine between supine position and prone position on intensity-roodulated radiotherapy(IMRT) for cervical cancer. Methods 11 patients with Ⅱ_B-Ⅲ_B cervical cancer were scanned with supine position and prone position by CT.The CT images were transported to TPS,then target volumes were delineated and the IMRT plans were designed respectively.The prescribed dose was 95%PTV receiving 45 Gy in 23 fractions of 2 Gy.The exposure volumes of the OAR at different position and different dose levels in the dose volume histograms (DVH) were compared and analyzed.Results When tlle dose di8tributions met to the clinic request,the exposure volumes of small intestine at prone position were redueed than that at supine position in dose range 46-30 Gy(P<0.05),but this phenomenon Was not distinct in low dose range(< 20 Gy)(P>0.05).The exposure volumes of bladder, rectum, femur head and spine cord were no obviously differences at dissimilar position. Conclusion IMRT of cervical cancer should adopt prone position.because their small intestine will be protected better.
5.Clinical efficacy of facial masses caused by exogenous growth factor injections

Objective To explore the reasons and the best treatment options of facial mass complicated after exogenous growth factor injections.Methods 17 cases of postoperative complications caused by exogenous growth factor injections were retrospectively analyzed.15 patients with chin masses received mass resection and repairment with submental fat and fascial flap.Facial masses of the other 2 patients were located in cheek.Simple cheek mass resection was usually useful.All the patients had scar treatment.The postoperative satisfactory survey was investigated after 6 months.Results The incisions of 17 patients were healed well.An analysis through color Doppler ultrasound examination showed no residual or recurrent mass within half a year follow-up.One of the 17 patients had facial asymmetry and needed further treatment.Others were satisfied with the facial appearance of treatment.Conclusions The masses resulted from injection of exogenous growth factor should be cut off timely and reasonable reconstruction through fat and fascial flap and scar treatment are helpful to restore the original appearance of patients.
6.Comparative analysis of cultured endothelial progenitor cells in vitro from PBMCs and enriched CD133~+ cells

AIM: To compare the methods of two currently employed isolation methods for endothelial progenitor cells (EPCs): from total peripheral blood mononuclear cells (PBMCs) and from enriched CD133~+ cells, by defining the cell morphology, phenotype, reproductive activities and function in vitro, providing a reference for clinic application. METHODS: PBMCs from the healthy subjects were used for CD133~+ sorting or not. The two groups of isolated cells were suspended in complete medium M199 for 7 d to 14 d. EPCs phenotype were characterized by FACS. The proliferation of differentiated EPCs was studied by MTT assay, and VEGF concentration was measured using an ELISA kit. Matrigel experiment and migration assay were imitated vascularization in vivo. RESULTS: PBMCs produced more colony-forming units (CFU) than CD133~+ cells from the same volume of blood (P<0.01). From 7 d to 14 d, the two groups show decreased expression of hematopoietic stem cell markers and increased level of endothelial markers, but CD144~+ cells in CD133~+ group were lower than those in PBMCs groups (P<0.01). Cells in PBMCs group secreted more VEGF than that in CD133~+ group on 7 d (P<0.01). Compared to CD133~+ group, PBMCs group showed more potential of proliferation and vascularization in vitro. CONCLUSION: CD133~+ sorted cells show a lower capacity of differentiation, secretion, proliferation and vascularization in vitro, which is unable to differentiate to mature endothelial cells, indicating that it's not a preferential way to obtain EPCs for clinic therapy.
7.Analysis on misdiagnosis of insulinoma

Retrospectively from February, 2006 to June, 2009 the clinical characteristics of 14 misdiagnosed cases of insulinoma in the First Affiliated Hospital of Zhengzhou University were analysed. The rate of misdiagnosis was 38% ( 14/37). The rate of IRI/G≤0. 3 was 33%. Insulinoma has often been misdiagnosised when IRI/G≤0. 3. Hence a diagnosis of insulinoma can not be excluded with this IRI/G rate.
8.Expression of mRNA and protein of ZNF217 and EF1α and their significance in pathological scar

Objective To study the expression of ZNF217 and EF1α gene in the pathological scars and to investigate role and probable mechanism in the pathogenesis of abnormal scar.Methods Quantitative real-time PCR and Western blot were performed to detect the expression and distribution of mRNA and protein of ZNF217 and EF1α in hypertrophic scar (10 cases),keloid (10 cases),normal scar (10 cases),and normal skin (10 cases),and statistics was used to analyze the data.Results The expression of ZNF217 mRNA and protein in the normal skin,normal scar,hypertrophic scar and keloid were 1.46±0.397,1.45±0.265,4.49±0.999,5.47±0.808; 0.276±0.0211,0.299±0.0150,0.743t0.0509 and 0.747±0.0377,respectively.The expression of EF1α mRNA and prorein in the normal skin,normal scar,hypertrophic scar,and keloid were 1.47±0.469,1.47±0.218,5.10±1.68,5.74±1.92; 0.505±0.0371,0.518±0.0153,0.780±0.0369 and 0.792±0.0290,respectively.The positive rate of mRNA and protein of ZNF217 and EF1α was not statistically different between the hypertrophic scar and keloid (P>0.05),while they were all remarkably significant in comparison between normal scar and abnormal scar (P<0.01).In pathological scar mRNA and protein of ZNF217 and EF1α showed a strong positive correlation.Conclusions The expression of ZNF217 and EF1α is increased in pathological scar.Therefore,ZNF217 and EF1α overexpression may play an important role in the proliferation of fibroblasts and in the pathogenesis of pathological scar.
9.Inhibitory mechanism of exenatide on the apoptosis of endothelial cells induced by high glucose

After human umbilical vein endothelial cells (HUVECs) were incubated with various concentrations of exenatide for 24,48,and 72 h under the conditions of5.5 and 33 mmol/L glucose,the cell viability was detected by MTF assay.The apoptosis rate of endothelial cells was measured by flow cytometry.Protein kinase B (Akt) phosphorylation,Bcl-2,and Bax protein expression were detected by Western blotting.The results showed that the cell viability was decreased after HUVECs were incubated with high glucose for48 and 72 h (P<0.01).Treatment with 1,10,and 100 nmol/L exenatide for48 h significantly increased the cell viability in the presence of high glucose,in a dose-dependent manner (P<0.01).High glucose inhibited Akt phosphorylation and Bcl-2 protein expression,stimulated Bax protein expression,with decreased Bcl-2/Bax ratio and increased apoptosis ratio of cells.After treatment with exenatide,the cell apoptosis reduced,Akt phosphorylation and Bcl-2 protein expression increased,and Bax protein expression decreased,with increased Bcl-2/Bax ratio (P < 0.01).These effects of exenatide on endothelial cell were abrogated by an inhibitor of phosphotylinosital 3 kinase (PI3K) LY294002 (P < 0.01),suggesting that the exenatide may regulate Bcl-2/Bax protein expression and inhibit endothelial cell apoptosis induced by high glucose through PI3k/Akt signal pathway,and thus may protect endothelial cells.
10.Release Mechanism and Preparation of Liposome Gel of Total Alkaloids of Sophoras Flavescentis Radix

Objective To optimize the formulation and process of liposome gel of total alkaloids of Sophoras Flavescentis Radix; To prepare the liposome gel of total alkaloids of Sophoras Flavescentis Radix and study its release mechanism.Methods Matrine liposomes was prepared by using film dispersion method; With entrapment efficiency and medicine loading as indexes, acid dye colorimetric method was used for the determination of matrine content in liposomes. Orthogonal design was used to optimize the formulation and the optimal formulation of liposomes was selected. Poloxamer-407 was set as the substrate preparation of matrine liposome gel. The transdermal rate of medicine gel and medicine liposome gel was investigated.Results Obtained through formulation and technology optimization of liposomes formation uniform, particle size was in the range of 100 nm to 400 nm, entrapment 74%, loading 26%. Preparation of liposome gel was transparent semisolid. In vitro results showed, cumulative release dose of matrine hydrogel was 6.34 mg/cm2 within 48 h; cumulative release doses of liposome gel of total alkaloids of Sophoras Flavescentis Radix was 6.97 mg/cm2 within 48 h; cumulative volume, steady-state penetration rate through skin and 48 h volume in the skin of the latter were significantly improved compared with that of the former. Conclusion Optimum preparation is reliable and practical. Liposome gel of total alkaloids of Sophoras Flavescentis Radix made by the preparation is with high quality, which can effectively delay the medicine release rate, increase the volume of medicine in human body.
