1.Clinical Observation of Vascular Headache Treated by Acupuncture plus Needle-Embedding Method

Purpose: In order to explore the clinical effect in acupuncture treatment of vascular headache. Methods: 93 cases of vascular headache were randomly auocated into acupuncture group (n= 30), treated by puncturing Fengchi (GB 20), Ashi point and Sanyinjiao (SP 6) on the sick side, acupuncture plus needle-embedding group (n= 36), treated by basic acupuncture treatment plus needle-embedding method in Taiyang (Ex-HN 5) and Xuanzhong (GB 39) on the sick side, and control group (n = 27), treated with oral administration of 5 mg Sibelium,twice a day, for observation of the changes in the clinical symptoms before and after treatments in the patients of three groups. Results: After treatment, the clinical symptoms in the patients of three groups were relieved or disappeared. The recent therapeutic effect was better in acupuncture plus needle-embedding group than in the control group (P< 0.05), and there was no significant difference (P> 0.05) between the acupuncture plus needle-embedding group and acupuncture group and there was no significant difference (P > 0.05) between acupuncture group and control group, either. The long-term therapeutic effect was better in the acupuncture plus needle-embedding group than in the control group (P< 0.01), in the acupuncture group than in the control group (P < 0.05), and there was no significant difference between the acupuncture plus needle-embedding group and acupuncture group. Conclusion: Acupuncture treatment of vascular headache offers a better recent and long-term therapeutic effect, and acupuncture plus needle-embedding method is much better in the therapeutic effect.

3.Chiral Separation and Identification of D,L-Histidine Based on Nanochannels Membrane Coupling with Surface Enhanced Raman Scattering Spectroscopy

Gold nanochannels were prepared using Al2 O3 nanotubules membrane as the carrier and modified with chitosan by a classical N-(3-dimethylaminopropyl)-N-ethyl carbodiimide ( EDC)/N-hydroxysuccinimide ( NHS ) coupling reaction. The nanochannels were characterized by field emission scanning electron microscopy ( FESEM) , cyclic voltammetry and AC impedance method. The Au nanochannels modified with chitosan showed a chiral environment and can be used to separate histidine enantiomer. The effects of pore size and solution pH on the separation efficiency of histidine were investigated. To increase the detection sensitivity of D-, L-histidine, Ag nanoparticles were used to enhance the surface enhanced Raman scattering ( SERS) activity. The results showed that the chitosan-modified gold nanochannels can be used to separate chiral histidine based on this unique selective nanochannel membrane. L-Histidine and D-histidine were respectively detected by SERS at wavelengths of 1000 and 1590 cm-1 . The results showed that L-histidine and D-histidine were separated well in the mixture containing 200 μL of histidine, 100 μL of colloidal Ag and 100 μL of 80 mmol/L NaCl ( pH=7 . 59 ) with a separation efficiency of 4 . 91 .
4.Endothelial progenitor cells homing to the orthotopic implanted liver tumor of nude mice.

This study investigated the "homing" phenomenon in hepatocellular carcinoma (HCC). The "homing" specificity of endothelial progenitor cells (EPC) by establishing an orthotopic implantation model in nude mice. EPCs harvested from the marrow cells were separated by density gradient centrifugation. Fluorescence microscope, flow cytometry (FCM) and double fluorescence staining with FITC-UEA-I and DiI-ac-LDL, were employed to identify the cells. 4',6-diamidino-2-phenylindole (DAPI) labelling and real-time PCR were used for detecting the expression of CD133 and chemokines to trace and observe the distribution of EPCs. Our results showed that the distribution rate of EPCs was obviously higher than that in other important organs and the negative control group. Detection of CD133 and chemokines yielded similar results in difference tissues. Our experiment confirmed that the chemotaxis of EPCs does exist in HCC. Moreover, HIF-1α, SDF-1 and VEGF might play important roles in the "homing" of EPCs in HCC. EPCs might be a potential candidate for targeting vector of HCC for gene therapy.
5.Isolation of cultured endothelial progenitor cells in vitro from PBMCs and CD133(+) enriched cells.

Two isolation methods for sorting of endothelial progenitor cells (EPCs): from peripheral blood mononuclear cells (PBMCs) and CD133(+) enriched cells were compared, by defining the cell morphology, phenotype, reproductive activities and function in vitro, to provide a reference for clinical application of EPCs. PBMCs from healthy subjects were used either directly for cell culture or for CD133(+) sorting. The two groups of cells were cultured in complete medium 199 (M199) for 7 to 14 days and the phenotypes of EPCs were analyzed by FACS. The proliferation of differentiated EPCs was studied by MTT assay, and the VEGF concentration was measured using an ELISA kit. ECM gel experiment and migration assay were performed in vivo. The results showed that PBMCs produced more colony-forming units (CFU) than CD133(+) enriched cells from the same volume of blood (P<0.01). From day 7 to 14, the two groups showed decreased expression of hematopoietic stem cell markers and increased level of endothelial markers, but CD144(+) cells in CD133(+) group were less than in PBMCs group (P<0.01). PBMCs group secreted more VEGF than CD133(+) group on the day 7 (P<0.01). As compared with CD133(+) group, PBMCs group had more potent potential of proliferation and vascularization in vitro. It was concluded that CD133(+) sorted cells showed a lower capacity of differentiation, secretion, proliferation and vascularization in vitro, suggesting that CD133-negative cells may be a preferential way to get EPCs for clinical therapy.
6.THE TECHNOLOGICAL CONDITIONS OF PRODUCING 2-KETO-LGULONIC ACID FROM D-GLUCOSE BY DIRECT FERMENTATION

2-Keto-L-gulonic acid (2-KLG), the precurcor of L-ascorbic acid synthesis, was prepared directly from D-glucose by tandem fermentation. In the first step fermentation Erwinia sp. SCB 247 translated D-glucose to 2,5-diketo-D-gluconate (2,5-DKG), which accumenlated 180 mg 2,5-DKG per ml in the broth. In the second step fermentation Corynebacterium sp. SCB 3058 reduced 2,5-DKG to 2-KLG, which accumulated 35 mg 2-KLG per ml in the broth. This reductive fermentation was obtained under aerobic conditions by adding a hydrogen donor such as glucose.The average yield of five batches fermentation was 56.3 mol%, from D-glucose to 2-KLG in 10 L fermentor.
7.Late-course hypofractionated stereotactic radiotherapy for locally advanced non small cell lung cancer

Objective To evaluate the effect of late course hypofractionated stereotactic radiotherapy (LCHSRT) on locally advanced non small cell lung cancer. Methods From October 1997 to June 1999, 106 patients with locally advanced non small cell lung cancer were randomized into conventional radiotherapy group (CRT) and LCHSRT group. Ninety one patients with complete data were analyzed including 43 patients in CRT group and 48 patients in LCHSRT group. In CRT group, 18 patients were staged Ⅲa and 25 Ⅲb; whereas 19 patients were staged Ⅲa and 29 Ⅲb in LCHSRT group. The initial median dose of 43.5?Gy was given in CRT group to the primary tumor, ipsilateral hilum and mediastinum using AP PA fields with 1.8 2.0?Gy per fraction, then 21.7?Gy was boosted to the residual primary and metastatic regional lymph nodes to the median total dose of 65.2?Gy. In LCHSRT group, after the first 44.4?Gy to the primary tumor, ipsilateral hilar region and mediastinum by AP PA fields with a dose fraction of 1.8 2.0?Gy, noncoplanar arc hypofractionated stereotactic radiotherapy was delivered to the residual primary and metastatic lymph nodes to the total dose of (22.8?5.5)?Gy with 4 7?Gy per fraction treated every other day. Results The radiation pneumonitis rates were 11.6% and 14.6% in CRT group and LCHSRT group, respectively. The radiation pulmonary fibrosis rates diagnosed by CT scan 6 months after radiotherapy were 48.8% and 66.7% in CRT group and LCHSRT group, respectively. 8.Evaluation of short axis systolic function of single left ventricles using velocity vector imaging

Objective To investigate the short axis systolic function of children with single-left ventricle(LV),and to appraise the clinical value of velocity vector imaging(VVI) on assessing it.Methods The study group consisted of 14 patients with single-LV.The control group consisited of 14 age-matched normal children.VVI was used to analyse the circumferential and radial strain and strain rate of regional single-LV at the level of papillary muscle.Results Compared with values in control group,single-LV circumferential strain values were significantly lower in anterior septum,posterior septum,anterior wall,lateral wall and inferior wall(all P <0.05).Single-LV circumferential strain rate values were significantly lower in anterior septum,posterior septum,anterior wall,lateral wall and posterior wall compared with controls(all P <0.05). In control group,circumferential strain and strain rate were higher in inferior septum and anterior septum compared with other segments( P <0.05).Single-LV radial strain values were significantly lower in all segments compared with values in control (P < 0.05).In control group,radial strain and strain rate of papillary muscle level showed no significantly difference( P >0.05).Conclusions Circumferential and radial systolic ventricular function are impaired in children with single-LV.VVI can beused as a quantitative tool in evaluating the short axis systolic function of single-LV.
9.Topical tacalcitol and 308 nm excimer laser phototherapy in patients with vitiligo

Objective To compare the efficacy and safety of excimer laser 308 nm phototherapy alone and the combination of excimer laser 308 nm and topical application of vitamine D3 alanogue tacalcitol in the treatment of vitiligo. Methods Seventy-eight patients with vitiligo were enrolled in the single-blind clinical trial, treated with excimer laser 308 nm. The lesions were devided into two groups: patients in the experimental group were instructed to use tacalcitol ointment and the control group were applied with placebo ointment. The lesions were evaluated once per month and photos taken for analyses of clinical effects. Results The results in different locations were compared, the effective rates of the experimental group in cephalofacial site, trunk and limbs were 93.51%, 84.16 % and 42.35 %, respectively. The effective rates of control group in opposite and adjacent sites were 90.9 %, 77.45 % and 34.15 %, respectively (P < 0.05). The comparison of results in different types of lesions indicated that the effective rate of the experimental group in vitiligo vulgaris and segmental vitiligo were 73.81% and 84.00 %, respectively. The effective rate of control group in vulgaris and segmental vitiligo were 86.8 %, 77.45 % and 34.15 %, respectively (P <0.05 ). The comparison of results in radiation times and doses of phototherapy showed that the radiation time and dose on the time of initial pigment regeneration were (16. 15 ± 3.22)times and (4.40 ± 5.03)J/cm2 in the experimental group, while ( 18.56 ± 3.50) times and ( 6.60 ± 1.01 ) J/cm2 ( P < 0.05 ) in the control group, the time and dose on the time of apparent effect were ( 20. 36 ± 1.50 ) times and ( 7.50 ± 3.54 ) J/cm2 in the experimental group, and (21.68 ± 2.40) times and( 8.80 ± 9.24)J/cm2 (P < 0.05 ) in the control group. Conclusion Application of tacalcitol ointment in combination with twice-weekly 308 nm excimer laser light phototherapy is an effective alternative treatment for patients with generalized vitiligo.
10.Effects of APRV-BIPAP ventilation On lung recruitment/open maneuvers in piglets with acute lung injury

Objective To study the effects of APBV (airway pressure release ventilation) / BIPAP(bipha-sic positive airway pressure) on lung recmitment/open maneuvers in piglets with acute lung injury. Method The model of acute lung injury (ALI) was induced by E. coll. intraperitoneal injection in piglets. Based APRV/BI-PAP model, the different pressure combinations (Phigh/Plow) of RMs increased gradually, such as RMI(30/15),RM2(35/20), RM3(40/25), RM4(45/30),RM5(50/35), RM6(55/40), RM7(60/45) cmH2O. The effects of stepwise RMs were studied by computed tomography (CT) at iaspiratory phase. Meantime the oxygen index (PaO2/FiO2), hemodynamic parameter and mean pressure of airway (Pmean) were continuously observed. The piglets were killed when RiMs finished and pulmonary pathological examination were done routinely by optical microscope. Data was analyzed by self-contrast method, using SPSS 11.5 software package. Results were expressed as mean ± standard deviation (x±s). Multiple comparisons were made with One-way ANOVA. Pearson correlative analysis was used to describe the relativity of PaO2/FiO2 and the collapsed alveolar area. Changes were considered as statistically significant if P value was less than 0.05. Results Eight piglets with ALl model were successfully made and all of them showed different degree of alveolar collapse under chest CT scan. During RMs their PaO2/FiO2 increased obviously (P<0.05) were decreased obviously (P<0.05) too, specially after RM2 finished (P<0. 05). But the alveolar over-inflatian could be found in some non-diseased area. The heart rate (HR) increased and mean artery blood pressure (MAP) decreased significantly while the pressure combinations (Phigh/Plow) of RMs were added gradual]y ( P<0.05). Meantime the Pmean and Ppeak inspiratory pressure (PIP) of airway and central venous pressure (CVP) were increased significantly ( P<0.05). But when RMs were finished,all of these indexes were hack to the levels of pre-RMs. Even there were no barowaumas happened, such as pneumothorax and pneumomedistinum, the alveolar overdistention and interruption of the alveolar separation still could be seen by pathologic examination. Conclusions RMs could be done well by APRV/BIPAP. Phigh/Plow (35/20cmH2O) would be the best pressure combination with more efficacy of RMs and less influence on hemodynamics,airway pressure indexes and others. When the effect of RMs was satisfied enough, setting Phigh/Plow to 30/15cmH2O for 20 mitt may maintain the good efficacy of RMs.
