1.Etiological detective analysis in female urinary tract infection

Objective To discuss the disposition feature of pathogenic bacterium in female urinary tract infection(UTI) ,so as to elevate the level of clinical diagnosis and therapy. Methods To cultivate medistream urine,assess vaginal secretions in the aspects of bacteriology, mycoplasma, chlamydia, mycetes and parasite. All specimen collected from 129 female patients who chiefly complained irritation of urinary tract ,from January 2003 to December 2006. Results Pathogenic microorganisms that found in the 129 female patients with UTI are gram-negative bacteria( 53.49 % ), gram- positive bacteria ( 19.38 % ), mycoplasma ( 14.73 % ), mycetes (9.30 % ), chlamydia (4.65 % ),parasite(1.55% ). Among them, escherichia coli, klebsiella pneumoniae bacilli, enterococcus faecalis, staphylococcus aureus are common species. Sexually transmitted disease(27.91% ) include the infection of diplococcus gonorrhoeae,mycoplasma and ehoamydiae. Among them,non-gonococcal urethritis is common. Combined infection(17.38 % ). Infection combined with mycoplasma urealytium and other pathogen is the most, these patients are most young or middle aged. Conclusion Pathogen in female UTI is mostly gram-positive bacteria. STD and combined infection should be paid attention to by clinicians. We suggest female UTI patients take etiological test regularly,so as to elevate the level of clinical diagnosis and therapy.
2.Effect of ketamine on apoptosis and synaptophysin expression in hippocampal neural cell of SD infant rats

Objective To observe the effect of ketamine on apoptosis and synaptophysin expression in 7-day-old infant SD rat hippocampal neural cells. Methods Thirty SD rats of 7 days old were randomly assigned to receive the injection of 25 mg/kg ketamine (K_ 1 group)or 50 mg/kg (K_ 2 group) or 50 ml/kg normal saline (K_ 0 group). In 24 h after injecton, all rats were killed and the synaptophysin was tested by using immunhistochemistry and Western blotting, the apoptosis of neuronal cell by Tunel. Results In 24 h after injection of ketamine,the number of apoptotic neural cells increased,which of K_ 0 group was (5.3?1.7), K_ 1 group (9.5?4.2), K_ 2 group (23.4?7.6), and the grey values of synaptophysin by immunhistochemistry of K_ 0 group was (174.11?4.68), K_ 1 group (181.36?4.17), K_ 2 group (198.25?3.06), and the OD values of synaptophysin piece decreased, which of K_ 0 group (4 007?758), K_ 1 group (2 621?465), K_ 2 group (987?183). Conclusion In 24 h after treatment with ketamine, the expression of synaptophysin was decreased in hippocampl neural cells, the number of apoptotic neuronal cells were increased, which may be involved in neurotoxicity caused by ketamine.

4.Surgical Treatment of 38 Patients with Acetabular Fractures

Objective To investigate the clinical effect of surgical treatment of acetabular fractures. Meth-ods According to Letoumel and Judet typing, 17 cases of posterior wall fractures, 5 cases of transverse fractures, 3 cases of anterior wall fractures, 2 cases of anterior column fractures,5 cases of transverse and posterior wall fractures, 2 cases of posterior column and wall fractures, 2 cases of T-type fractures, 1 case of anterior and hemi-transverse fractures and 1 case of fractures of both column). Kocher Langenbeck (K-L) approach was applied in 30 cases, ilio-inguinal approach in 7, and combined approaches (K-L plus ilioinguinal) in 1. Results According to Matta evalua-tion, anatomical reduction(displace < 1 mm) was applied in 30 patients. Satisfactory reduction(displace 2~3 mm) was attained in 6 patients, unsatisfactory reduction(displace >3 mm) was in 2 patients. 38 patients were followed up for 6~48 months (mean: 25 months). The hip function was evaluated according to the modified Postel-D, Aubigne score and rated as excellence in 31 patients(81.5%), good in 5 patients(13.1%), fair in 1 patient(2.6%) and poor in 1 petient(2.6%). The excellent and good rate was 81.5%. Conclusions It is the important guarantee to elevate the therapeutic efficacy of acetabular fracture that shoud be correcdy analyze the fracture displacement and type, choose proper surgical approach, have good restitution implement and operate at the right moment. The quality of surgical reduction has a close correlation with the clinical results.
5.A case report of successful management of middle ear foreign body following hearing aid fitting with ossicular chains integrity.

A case of ear mold impression middle ear foreign body was reported. In the exploratory operation we found the ear mold impression surrounded the ossicular chains. During the process of removal the middle ear foreign body, the incudostapedial joint was protected from obvious external force and ossicular chains were kept integrity. Tympanoplasty was completed after management of foreign body. The patient recovered well postoperatively and was discharged as scheduled.

6.Research progress concerning knee-joint subchondral bone bruises

Recently,wide application of MRI in clinic accelerates researches into the etiology,diagnosis,treatment and prognosis of bone bruises.MRI manifestations of bone bruises include trabecular microfracture associated with hemorrhage,oedema of subchondral bone because of direct or indirect traumatic force and focal abnormalities in subchondral bone of the knee.This condition is also known as occult injury of the knee,because it is likely to be overlooked or misdiagnosed in hospital due to its diagnostic difficulty by plain radiographs and CT.If timely intervention is not given,it will be accompanied by chronic pain which seriously affects the daily life of the patients,even resulting in early degeneration.This article discusses the current research on bone bruises of the knee.
7.Changes of plaque microflora during experimental gingivitis

Objective:To observe the changes of plaque microflora during the experimental gingivitis and to analyze the relationship between the plaque microflora and the clinical parameters.Methods:11 young male subjects with healthy gingiva and without systemic disease were selected.Subgingival plaque samples(2 sites /person)were collected and then smeared by Gongo red at baseline(0 day),the 7th,14th,21st day(without oral hygiene)and 28th day(7 days after reestablishing oral hygiene)respectively during experimental gingivitis.At the same time the clinical parameters were recorded.The results of smear and the clinical parameters were analyzed.Results:The percentage of spirochete was the lowest at the baseline and increased during the experimental gingivitis,and then reached the highest level on the 21st day.The percentage of spirochete of the 21st day showed the significant different compared with that of baseline(P0.05)Conclusion:Spirochete is correlated to the development of the gingivitis.
8.Determination of Fructose in Radix Ophiopogonis and Extracts of Radix Ophiopogonis by HPLC-ELSD

Objective To determine the content of fructose in radix ophiopogonis cropped in Sichuan and Zhejian, and in extracts of radix ophiopogonis. Methods A HPLC-ELSD method was used for determining the content of fructose. Results The content of fructose in radix ophiopogonis cropped in Sichuan and Zhejiang were 11.32%~19.96% and 3.85%~10.19%, respectively. The contents of fructose in extracts of radix ophiopogonis cropped in Sichuan and Zhejiang were 23.48%~25.82% and 21.39%~ 23.29%, respectively. Conclusion The content of fructose in radix ophiopogonis cropped in Sichuan was higher than that of in Zhejiang. The content of fructose in extracts of radix ophiopogonis cropped in Sichuan was almost the same as that of in Zhejiang.
9.Calprotectin and total protein in gingival crevicular fluid during experimental gingivitis

Objective: To measure GCF calprotectin and total protein for evaluating whether the calprotectin could be a sensitive marker for the initial inflammation of gingiva. Methods: Eleven young male subjects with healthy gingiva, who had no systemic diseases, were selected for this study. GCF samples (4 teeth /person) were collected with strips of filter paper at baseline (0 day), on the 7 th, 14 th, 21 st days (without oral hygiene), and 28 th day (7 days after reestablishing oral hygiene) during experimental gingivitis. The amount of calprotectin was measured by ELISA. The amount of total protein was assessed with protein dye binding assay. Results: The amount of calprotectin increased during the experimental gingivitis, and reached the highest level on the 21 st day. After oral hygiene was reestablished it reduced to the level of baseline. The amount of total protein had the same tendency as calprotectin. Conclusion: The amount of calprotectin and the total protein in GCF can reflect the initial inflammation of gingiva.
10.Clinical effect of breviscapine combined with nerve growth factor in ischemic optic neuropathy

Objective To investigate clinical effect of breviscapine combined with nerve growth factor in treatment of ischemic optic neuropathy . Methods 80 patients with ischemic optic neuropathy from March 2014 to March 2016 in our hospital were selected and divided into the control group and observation group, 40 cases in each group.The control group was treated with nerve growth factor, hip injection 30ug, one times a day, while the observation group was treated with breviscapine combined with nerve growth factor, 80 mg breviscapine added to 500 mL glucose intravenous infusion, one times a day.Then the changes of visual acuity, visual field loss difference (LV), the average damage of vision (MD), the average vision light sensitivity ( MS ) , adverse reactions, curative effect were compared between two groups.Results After treatment, the total effective rate in the observation group was significantly higher than the control group, the difference was statistically significant (P<0.05), the visual acuity of the observation group was significantly higher than that of the control group, the difference was statistically significant (P<0.05), LV, MD was significantly lower than the control group, the difference was statistically significant (P<0.05), MS was significantly higher than the control group, the difference was statistically significant (P<0.05).There was no difference in adverse reactions between the two groups.Conclusion Breviscapine combined with nerve growth factor in the treatment of ischemic optic neuropathy can improve nerve function of patients , improve the therapeutic effect.
