2.Short-term clinical effect of Zhenwu decoction combined with taxol and carboplatin (TC) in treatment of post-operative patients with ovarian cancer

Objective To observe and analyze the short-term clinical effect of Zhenwu decoction combined with taxol and carboplatin (TC) in treatment of post-operative patients with ovarian cancer.Methods The clinical data of eighty-six post-operative patients with ovarian cancer from June 2014 to June 2015 in our hospital were retrospective analyzed, and randomly divided into control group and treatment group, 43 cases in each group.The control group received TC treatment and treatment group received Zhenwu decoction on the basis of control group with two courses, 21d of one course. The clinical effect were compared between two groups post-treatment.Results After treatment, the total efficacy in treatment group was higher than that in control group (88.37%vs.65.11%;χ2 =6.515,P<0.05), the improvement rate of life quality was higher (72.09% vs.27.91%;χ2 =16.790,P<0.05), the stable rate of life quality had no statistical difference (23.26%vs.30.23%;χ2 =0.534,P>0.05), the reducing rate of life quality was lower (4.65%vs.41.86%;χ2 =10.030,P<0.05), the effect of Karnofsky performance score (KPS) was higher (93.02% vs.72.09%;χ2 =6.541, P<0.05), the complication rate was lower (4.65% vs.18.60%;χ2 =4.074,P <0.05).Conclusion The clinical effect of Zhenwu decoction combined with taxol and carboplatin (TC) in treatment of post-operative patients with ovarian cancer was satisfying, which could improve prognosis and improve survival rate.
3.Estimation of Shunt Severity of Patent Ductus Arteriosus by Color Doppler Flow Convergence Region

The left to right shunt rate(F)and the transverse seetional area of ductus arteriosus(A)were calculated by color Doppler flow convergence method in 20 patients with patent ductus arteriosus.The results indicated that the shunt rate(F)was correlated fairly with Qp-Qs,Qp/Qs(measured by pulsed wave Doppler),the pressure gradient(△P)and the diameter of left ventricle(DLV)(r=0.81,0.62,0.69 and 0.53;P<0.001,0.005,0.001 and 0.01,respectively).The transverse sectional area of ductus arteriosus(A)was also correlated closely with the diameter of ductus arteriosus(Dd)measured directly in two-dimensional view(r=0.81,P<0.001).The F value of group Qp/Qs≥2:1 was apparently higher than that of group Qp/Qs<2:1(P<0.005).The color Doppler flow convergence method can rapidly and exactly evaluate the left to right shunt severity of patent ductus arteriosus.
4.Assessment of the Severity of Eccentric Mitrai Regurgitation by Color Doppler Flow Convergence

The shape of regurgitant jets in 30 patients with eccentric mitral regurgitaltoll Showing flow convergence region(FCR)was observed.It was found that the shapes of the jets were allslender and close to left atrial wall.In comparison of the regurgitant volume calculated by pulsed Doppler and regurgitant flow rate by flow convergence with the regurgitant jet area and the ratio 0f the regurgitant area to left atrial area respectively,the results showed that there were poor correlations between them (r=0.59,0.56 and r=0.55,0.57)and good correlation between regurgitant volume determined by Pulsed Doppler and regurgitant flow rate by flow convergence(r=0. 96,P<0.01).When the regurgitant flow rates calculated by flow convergence were same,the regurgitartt jet area and the ratio of the regurgitant jet area to left atrial area of non-eccentric mitral regurgitation were obviously larger than those of eccentric mitral regurgitation.The results indicated that the regurgitant jet area and the ratio of the regurgitant jet area to left atrial area underestimated the severity of eccentric mitral regurgitation apparently and that flow convergence region could be used to assess the seVerItY 0f eccentric mitral regurgitation accurately.
5.Ureteroscopy Combined with ESWL for Ureteral Calculus:Report of 256 Cases

Objective To investigate the clinical value of ureteroscopy combined with ESWL for the treatment of ureteral stone.Methods From October 2005 to June 2007,a total of 256 patients with ureteral stones were treated in our hospital.The calculus in the middle and lower ureter were removed by ureteroscopy,while the stones in the upper ureter was treated by ESWL.For the cases that failed ureteroscopy or those who had residual stones after the treatment,ESWL was performed as a supplement.ResultsIn the patients with lower ureteral stones,the one-stage cure rate was 98.2%(167/170),while in those with middle and upper ureteral calculi,the rate was 89.2%(50/56) and 73.3%(22/30) respectively.Three patients developed perforation of the ureter during the surgery,and one had mucosal injury of the ureter.One case was converted to open surgery because of perforation of the ureter resulted from ureteral twist.Twelve patients were failed ureteroscopy or had residual stones,and received ESWL as the second-stage treatment,11(91.7%) of them were cured afterwards.Conclusions Ureteroscopy is effective for middle and lower ureteral stones.For the cases failed the operation or have residual calculi,ESWL is a good choice.

A large number of studies from both humans and animals experiments have offered a substantive body of evidence supporting that sleep contributes crucially to memory consolidation. The processes of memory consolidation in hippocampus and cortex during sleep was reviewed and the primary cellular and molecular mechanism were briefly introduced.
7.Low back pain following discectomy for lumbar disc herniation

[Objective]To determine whether discectomy resulted in improvement in low back pain(LBP)associated with lumbar disc herniation. [Method]Two hundred and one consecutive single-level lumbar discectomy were followed up,one hundred patients underwent microendoscopic discectomy(MED group) and 101 patients were treated with conventional open discectomy(COP group).Outcomes were assessed using Visual Analog Scale(VAS) and patient satisfaction.The changes in LBP were compared between the MED group and COP group.The effects of severity of low back pain before surgery on the improvement of back and leg pain were also determined.The mean duration of follow-up was 45 months.[Result]The mean VAS leg pain significantly decreased from 70.02?15.80 before operation to 7.73?12.13 at final follow-up.A significant improvement was also noted in LBP,the mean VAS LBP score was 43.61?26.18 before surgery and 16.89?14.73 at the time of final follow-up.The important improvement in LBP and leg pain was seen in 70.1%~78.1% and 97.0% patients respectively.Improvements in back pain and leg pain were correlated(r= 0.1656,P=0.0304).The rate of important improvement of low back pain was greater in MED group than in COP group.The rate of important improvement in LBP was similar in the patients of a preponderance of LBP,a preponderance leg pain and an equivalence of LBP and leg pain,the patients with a preponderance of LBP showed a lower rate of important improvement in leg pain than the others.[Conclusion]Discectomy for relief of sciatica of lumbar disc herniation provides effective relief of LBP.The MED is more likely effective as a surgery way to relief LBP.Furthermore,it certified that the severe LBP maybe a negatively predictive factor for the follow-up outcome of leg pain.
8.Evaluation of left ventricular global systolic function using velocity vector imaging in patients with coronary heart disease before and after percutaneous coronary intervention therapy

Objective To analyze left ventricular (LV) global systolic function in patients with coronary artery disease(CAD) and their changes after percutaneous coronary intervention(PCI) using velocity vector imaging(VVI). Methods Eighteen healthy adults and twenty two patients with CAD were enrolled in this study. Two-dimensional dynamic images of standard apical four-chambers, two-chambers and parasternal short-axis views at the level of mitral valve and apex were obtained in VVI condition. All patients were examined 1 day before PCI, 1 week, 1 month and 3 months after PCI respectively. LV rotation degree, rotation velocity at baseline and apex were measured using the off-line syngo US workplace software and LV twist, torsion were calculated as global motion condition. Results LV ejection fraction(LVEF) and peak torsion at myocardial infarction group were lower than those at normal control and myocardial ischemia group. After PCI, LVEF were gradually improved in both myocardial infarction group and myocardial ischemia group,while changes of LV apex rotation degree and velocity, peak twist and peak torsion between two groups were opposite: those parameters of the former increased 1 week or 1 month after PCI but decreased 3 months after PCI,while those of the latter decreased 1 week after PCI but increased gradually 1 month and 3 months after PCI. Conclusions LV twist and torsion can be evaluated exactly by VVI.Compared with LVEF, LV twist and torsion can preferably reflect the different characteristic of LV global systolic function after coronary artery recanalization in myocardial infarction group and myocardial ischemia group.
9.Relationship between Helicobacter pylori infection,expression of caudal-related homeobox transcription factor 2,mucin 2 proteins and gastric cancer

Objective To study the significance of Helicobacter pylori (Hp) infection and the expression of caudal-related homeobox transcription factor 2 (CDX2),mucin 2 (MUC2) proteins in gastric cancer,and elucidate the prognostic significance of CDX2,MUC2 proteins in gastric cancer.Methods Ninety cases of gastric cancer were available for the study.Hp was studied by Warthin-Starry bacterium stain and CDX2 and MUC2 proteins were studied by immunohistochemical stain.Results There was no Hp infection and no expression of CDX2 and MUC2 proteins in normal gastric mucosa.The positive rate in the cases of intestinal-type gastric cancer was 67.86%(38/56) for Hp,75.00% (42/56) for CDX2,and 66.07%(37/56) for MUC2,respectively,and it was higher than that in the cases of diffuse-type gastric cancer,which was 35.29%(12/34),41.18%(14/34) and 38.24%(13/34) respectively (P<0.01 or < 0.05).In intestinaltype gastric cancer,the positive rate of CDX2 and MUC2 proteins was also higher in Hp positive cases than that in Hp negative cases,and it was the same as CDX2 protein in diffuse-type gastric cancer.Higher positive rates of CDX2 and MUC2 proteins were observed in the cases of early gastric cancer than those in the cases of advanced gastric cancer (P < 0.01 or < 0.05),also it was observed in the cases of no lymph node metastasis than in the cases of lymph node metastasis (P < 0.01 or < 0.05).Moreover,the positive rate of CDX2 protein in the cases of stage Ⅰ~Ⅱwas higher than that in the cases of stage Ⅲ~Ⅳ (P < 0.05).Conclusions Hp infection and the expression of CDX2 and MUC2 proteins are related with gastric cancer,and has close relationship with intestinal-type gastric cancer.The expressions of CDX2 and MUC2 proteins may inhibit the infiltration and metastasis of gastric cancer.
10.Effects of DHEA Combined with Meloxicam Tablets on Therapeutic Efficacy of Osteoarthritis and Biochemi-cal Indicator

OBJECTIVE:To explore the effects of dehydroepiandrosterone(DHEA)combined with Meloxicam tablets on thera-peutic efficacy of osteoarthritis and biochemical indicator. METHODS:104 patients with osteoarthritis were randomly divided into ex-perimental group and control group with 52 cases in each group. Control group only received Meloxicam tablets,7.5 mg/time,bid;ex-perimental group was additionally given DHEA,25 mg/time,qd,on the basis of control group. The treatment course of 2 groups was four weeks. The clinical efficacy,VAS score before and after treatment,joint function score,levels of IL-1β and TNF-α,cartilage thickness,synovial thickness and score of liquid aggregation of joint cavity were compared between 2 groups. RESULTS:The total ef-fective rate of experimental group(86.54%)was significantly higher than that of control group(69.23%),with statistical significance (P<0.05);compared with before treatment,VAS scores of 2 groups decreased significantly after treatment,while joint function scores increased significantly,with statistical significance(P<0.05). VAS score of experimental group was significantly lower than that of control group,while joint function score was significantly higher than control group,with statistical significance(P<0.05). Af-ter treatment,the serum levels of IL-1βand TNF-αin 2 groups significantly decreased,with statistical significance(P<0.05);those of experimental group were significantly lower than those of control group,with statistical significance(P<0.05);there was no statis-tical significance in the cartilage thickness between 2 groups before and after treatment(P>0.05). Compared with before treatment, the thickness of the synovial membrane and the score of liquid aggregation of joint cavity significantly decreased after treatment,with statistical significance(P<0.05);the thickness of synovial membrane and the score of liquid aggregation of joint cavity in experimen-tal group were significantly lower than in control group,with statistical significance(P<0.05). CONCLUSIONS:DHEA combined with Meloxicam tablets is significant in the treatment of osteoarthritis,can significantly relief the pain and decrease the levels of serum IL-1βand TNF-α.
