Objective To evaluate the cardiovascular stiffness and its coupling of patients with hypertension by ultrasound. Methods Fifty patients with essential hypertension and 30 age- and gendermatched subjects without hypertension, diabetes and other cardiovascular diseases as control group were enrolled in this study. The parameters of structure and function of left ventricle, blood flow were measured by echocardiography. The blood pressure and carotid-femoral pulse wave velocity (CFPWV) were also measured. The derived corresponding parameter:end-systolic pressure(ESP),effective arterial elastance (Ea),end-diastolic (Ed),end-systolic ventricular elastance (Ees) and Ea/ Ees were calculated respectively. Results Ees was correlated positively with ejection fraction (r =0.378,P =0.005),while Ea was correlated positively with CFPWV( r =0.289,P <0.001).Ed was correlated negatively with e/a ( r =- 0.333,P =0.027).Posterior wall of left ventricle was correlated positively with Ed and Ea( r =0.388,P =0.016; r =0.336,P =0.026).Ea and Ed in patients with hypertension were significantly higher than those in control group( P <0.05),but there was no significant difference of Ees and Ea/ Ees between two groups( P > 0.05).Conclusions Arterial stiffness is associated with ventricular stiffness, and their matching relation can be applied to evaluate ventricular-arterial coupling.

The personnel training mode of school-enterprise cooperation, interaction of study and work is the inevitable trend of senior vocational education reform and development at present. The Changsha Social College cooperates closely with rehabilitation agencies and implements the personnel training mode of the “love to help people with disabilities, interaction of study and work, integration of courses and certificate” for training professionals of rehabilitation therapy, has gained some experience and achievements.

Objective:To compare diagnostic effect between coronary CT angiography (CTA) and intravascular ultra-sound (IVUS) for coronary calcified lesion .Methods:A total of 50 patients ,who were diagnosed as coronary heart disease (CHD) or suspected CHD in our hospital ,were selected .All patients received coronary CTA and IVUS ex-amination .Characteristics of coronary artery plaques and CT values of different types of plaques were observed , coronary CTA diagnostic effect was evaluated according IVUS as control .Results:There were no significant differ-ence between two methods in plaque area ,plaque burden and lumen area;CT value of calcified plaques was signifi-cantly higher than those of fibrous plaques and fatty plaques [ (725.68 ± 68.42) Hu vs .(58.12 ± 7.05) Hu ,(60.21 ± 6.78) Hu ,P<0.01 both];Sensitivity ,specificity ,accuracy ,positive predictive value and negative predictive val-ue of coronary CTA for diagnosing calcified plaques were 92. 86% ,92. 86% ,92. 86% ,81. 25% and 97. 50% respec-tively .Conclusion:Coronary CT angiography possesses higher diagnostic effect for qualitative and quantitative de-tection of coronary calcified lesions ,which can be regarded as a preferred screening method .

Journal of Chinese Physician 2016;18(6):884-887
Objective To investigate the clinical effect of proximal femoral nail antirotation (PFNA) and dynamic hip screw (DHS) in treating senile intertrochanteric fractures. Methods From February 2006 to December 2014,111 cases of senile intertrochanteric fractures treated with PFNA and DHS at our institution were retrospectively analyzed.Fifty-nine cases were treated with PFNA (average age 77.4 years);and fifty-two cases were treated with DHS (average age 76.1 years). The outcome measures collected for statistical analysis on following aspect:surgical time, blood loss in operation, blood transfused, rate of patients transfused, time to partial weight beating, hospital stay, healing time of fracture, orthopedic complications, reoperation rate and post-operation hip function. The Harris Hip Score was used for functional evaluation. Results One hundred and eleven patients were followed up for 10 to 24 months (average 17.2 months). There were no significant differences between two groups with regard to the functional outcome at 1 year, hospital stay, orthopedic complications and reoperation rate (P > 0.05).There were significant differences between the PFNA group and DHS group with regard to the surgical time [(60.7 ±9.9)min vs (97.5 ± 20.5) min],the blood loss in operation [(169.2 ± 82.1) ml vs (428.8 ± 126.O) ml],per patient concentrated red blood cells transfused [(0.7 ± 0.9) U vs (1.2 ± 1.3) U],blood transfusion rate (35.6% vs 55.8%) and time to partial weight bearing [(12.9 ± 10.3)d vs (47.0 ± 15.5)d] (P <0.01).Conclusions PFNA is an effective method for the treatment of senile intertrochanteric fracture with the advantages of simple operative procedure, minimally invasion, stable fixation and fewer complications.

Objective To investigate the effect of the rehabilitative therapy on functional recovery of patients with cervical cord injury. Methods 92 cases with spinal cord injury(SCI),including cervical, thoracic and lumbar cord injury, accepted normal rehabilitation. They were assessed with the Barthel Index and Functional Independence Measure(FIM)before and after treatment. Results After rehabilitation of 60~426 d,the scores of the Barthel Index and FIM were significantly improved(P<0.001)in all the patients, but it was the worst in the cervical cord injured ones, and the best in lumbar ones(P<0.05).Conclusion Rehabilitation can promote the functional recovery of patients with cervical spinal cord injury, but it is not as effective as on the thoracic and lumbar cord injury.

Objective To observe the time course of the changes in nitric oxide (NO) content and the activity of NO synthase (NOS) during hippocampal long term potentiation (LTP) in vitro . Methods The production and maintenance of LTP were detected by using extracelluar electrophysiological recording. NO content and NOS activity were determined by biochemical reaction. The expression of NOS mRNA was detected by in situ hybridization. Results Conditioning stimulation for 10 min induced LTP production and significant increases in NO content, NOS activity and expression of NOS mRNA. However, at 60 min after conditioning stimulation, LTP remained stably but NO content and NOS activity returned to the pre conditioning stimulation level. Moreover, the NOS mRNA was overexpressed at the early stage of LTP production. Conclusion Significant NO changes may occur at the early stage of LTP formation.

Objective To observe the effects of NMDA receptors and nitric oxide (NO) on neural cell adhesion molecule(NCAM) synthesis during hippocampal long term potentiation (LTP) induction in vitro . Methods LTP induction and maintenance were tested by using extracelluar electrophysiological recording. The synthesis of NCAM protein was detected by Western blotting. Results Application of conditioned stimulation induced LTP and a rise in NCAM at 10 min. NCAM protein level continued to rise while LTP remained stably at 60 min. The NMDA receptor inhibitor AP 5 and the NO synthase inhibitor N nitro arginine inhibited the LTP induction and the increase in the NCAM synthesis. Conclusion The changes in NCAM synthesis during hippocampal LTP induction in vitro may be involved in NMDA receptors and NO.
