1.Chief physician Lian Yu-lin's experience on treatment of cervical spondylosis by classification.

In clinical teaching practice of over 30 years, professor LIAN Yu-lin combines physiological, pathological and anatomic knowledge with TCM treatment based on syndrome differentiation, forming own unique theoretical system. This paper introduces LIAN's experience on treatment of cervical spondylosis in detail, and location of cervical vertebra three lines and determination of needling depth and reinforcing-reducing manipulation based on clinical manifestations of various types of cervical spondylosis, and combination of long needle with short needle, deep needling with superficially needling, stressing the main point and having a definite object. This is an effective pathway for treatment of cervical spondylosis.

2.Study on lipid-lowering traditional Chinese medicines based on pharmacophore technology and patent retrieval.

The combined application of statins that inhibit HMG-CoA reductase and fibrates that activate PPAR-α can produce a better lipid-lowering effect than the simple application, but with stronger adverse reactions at the same time. In the treatment of hyperlipidemia, the combined administration of TCMs and HMG-CoA reductase inhibitor in treating hyperlipidemia shows stable efficacy and less adverse reactions, and provides a new option for the combined application of drugs. In this article, the pharmacophore technology was used to search chemical components of TCMs, trace their source herbs, and determine the potential common TCMs that could activate PPAR-α. Because there is no hyperlipidemia-related medication reference in modern TCM classics, to ensure the high safety and efficacy of all selected TCMs, we selected TCMs that are proved to be combined with statins in the World Traditional/Natural Medicine Patent Database, analyzed corresponding drugs in pharmacophore results based on that, and finally obtained common TCMs that can be applied in PPAR-α and combined with statins. Specifically, the pharmacophore model was based on eight receptor-ligand complexes of PPAR-α. The Receptor-Ligand Pharmacophore Generation module in the DS program was used to build the model, optimize with the Screen Library module, and get the best sub-pharmacophore, which consisted of two hydrogen bond acceptor, three hydrophobic groups and 19 excluded volumes, with the identification effectiveness index value N of 2. 82 and the comprehensive evaluation index CAI value of 1. 84. The model was used to screen the TCMD database, hit 5,235 kinds of chemical components and 1 193 natural animals and plants, and finally determine 62 TCMs. Through patent retrieval, we found 38 TCMs; After comparing with the virtual screening results, we finally got seven TCMs.

3.The value of radiotherapy in patients with T1 and T2 breast cancer with one to three positive nodes after modified radical mastectomy

Background and Objective: The role of adjuvant radiotherapy to the regional nodes in women with T1-T2 breast cancer and one to three positive nodes is controversial.This study compared and analyzed the prognosis of patients with T1-T2 breast cancer with one to three positive nodes after modified radical mastectomy with or without postoperative radiotherapy.Methods: The cases of 434 women patients with T1 to T2breast cancer with one to three positive lymph nodes after modified radical mastectomy were reviewed,of which 196 patients received postoperative radiotherapy and 238 patients did not.The ipsilateral chest wall and supraclavicular fossa were irradiated with doses of 46-50 Gy in 23-25fractions.Results: For all patients,the 3-and 5-year rates of overall survival(OS)were 94.7% and 85.7% respectively,local control(LC)96.5% and95.6% respectively,and disease-free survival(DFS)89.3% and 82.3%respectively.The 3-and 5-year OS rates for patients without radiotherapy were 92.7% and 97.1% and for those with radiotherapy were 82.4% and89.2%,both with significant differences(P=0.039).The 3-and 5-year LC rates for patients without radiotherapy were 94.8% and 98.4% and for those with radiotherapy were 93.6% and 97.7%,again with significant differences(P=0.041).The 3-and 5-year DFS rates for patients without radiotherapy were 87.8% and 91.3% and for patients with radiotherapy were 78.5% and86.1%(P=0.047).Conclusion: Postoperative radiotherapy confers better rates of OS,LC,and DFS in patients with T1-T2 breast cancer with one to three positive nodes after modified radical mastectomy.
4.In situ arterialization of the great saphenous vein for the treatment of ischemic lower limbs, report of 88 cases

Objective To investigate the effects of in situ venous arterialization on extensive artery obliterans occlusion of the lower extremity. Methods Lumbar sympathetic ganglionectomy and one stage in situ arterialization of the great saphemous vein were performed in 104 ischemic limbs of 88 patients with extensive arterial occlusion. Results Eighty-two of 104 limbs were followed-up from 6 months to over 6 years. The intermittent claudication, night pain improved in all cases, with satisfactory wound healing and no swelling of the lower limbs. Conclusions Arterial blood flow through venous conduit improves and reconstructs the blood circulation of the ischemic limbs.
5.Comparative study on hepatoma cell line 7721 and human fetal hepatocytes for in vitro culture of hepatitis C virus

Objective To study the similarities and differences on in vitro replication and expression of hepatitis C virus (HCV) between human fetal hepatocytes (HFH) and 7721 cell line. Methods Human fetal hepatocytes and a hepatoma cell line 7721 were incubated with a serum from hepatitis C patient. After incubation, the presence of HCV RNA, the expression of HCV NS3 antigens in cells and/or supernatant were examined by RT-PCR, in situ hybridization and immunohistochemistry, respectively. Results It was found that: ①The intracellular HCV RNA was first detected on d 2~3 post-incubation and then could be intermittently detected in cells and/or supernatant subsequently (HCV RNA could be detected in 7721 cells during a period of at least 66 days. In HFH, HCV RNA could be detected up to 25 days after incubation); ②HCV-NS3 antigen could be expressed in infected cells; ③Minus-strand RNA of HCV was mainly located within cytoplasm by in situ hybridization. Conclusion The results suggest that both the fetal hpatocytes and the hepatoma cell line 7721 are susceptible to HCV, and especially 7721 cell line can stably support HCV replication in vitro and may be used as the target cell for long-term cultures of HCV.
6.Clinical analysis of pregnancy and postpartum cardiovascular complications in Marfan syndrome

Objective To investigate the management of pregnancy and cardiovascular complications in women with Marian syndrome(MFS).Methods From October 1994 to September 2006, 30 patients with MFS undergoing cardiovascular surgery were studied retrospectively.Results In the labor of 46 offsprings given birth by 30 women,5 cases(11%)were performed elective cesarean section because of the existence of aortic complication,and 12(26%)were diagnosed as MFS.The gestation in two patients was terminated due to deterioration of aortic abnormalities during their third trimester,and they received surgical treatment with Bcntall procedure.Two developed acute aortic dissection during labor and post delivery respectively.With the manipulation of anticoagulation peripartum,one who had the implantation with mechanical prosthesis went through pregnancy and delivery uneventfully.The average duration between delivery and cardiovascular surgery was(15?9)years.Conclusions Vaginal delivery can be done safely in patients with the MFS who do not have or have mild cardiovascular system abnormalities,aortic dissection,or other important cardiac abnormalities,cesarean section should be the preferred method of delivery.Women with MFS are at increased risk for dissection and congestive heart failure during pregnancy and should be counseled before pregnancy about these risks,as well as the inheritance of the condition.

9.Effect of professional nursing teams direction on the doctors-nurses coordination to perform emergency procedures

Objective To explore the effect of professional nursing teams direction on the training of doctors-nurses coordination to perform emergency procedures,so as to improve professional theory knowledge of nurses.Methods Thirty-four nurses were trained with doctors-nurses coordination to perform emergency procedures,and then nurses were test for emergency theory and operation.The score of emergency theory and operation before and after professional nursing teams direction were observed and compared.Results All nurses completed the training.There were statistically significant differences in the scores of emergency theory (80.38 ± 4.34 vs 87.03 ± 4.50,t =10.32,P < 0.01) and operation (79.12 ± 4.50 vs 90.38 ± 4.97,t =17.28,P < 0.01) before and after professional nursing teams direction among nurses.Survey results suggested that all nurses thought that training have improved their abilities of emergency response,97.1% nurses belived that training have fostered the team cooperation spirit,and 94.1% nurses considered that training have improved abilities of analyzing and solving problems and the professional abilities of understanding.Conclusions The method that professional nursing teams direction in training of doctors-nurses coordination to perform emergency procedures is superior to traditional method,and more efficient and worth popularizing.
10.Expression of endo-beta-mannanase gene from Trichoderma reesei in Pichia pastoris.

Complete mannanase gene with two introns was cloned from Trichoderrna reesei by PCR. The two introns were then removed by overlap extension PCR. The gene encoding the mature mannanase protein was inserted into the expression vector pPIC9K, downstream of a alpha-factor signal peptide sequence. The resultant recombinant vector was named pM242. After linearized with Sac I , pM242 was transformed to Pichia pastoris GS115 by electroporation. After screening, the recombinant strain Gpmf25 that expresses the secretory protein at high level was obtained. The activity of the recombinant mannanase reached 12.5 IU/mL. Optimum pH and temperature for the recombinant enzyme were 5.0 and 80 degrees C, respectively. The enzyme was stable at pH 5.0-6.0 and maintained over 50% of original activity after incubation at 70 degrees C for 30 min.
