1.The changes of endothelial function of blood vessels in diabetes mellitus and essential hypertension

Objective To study the endothelium-dependent d il atative function (EDDF) in the patients with diabetes mellitus (DM) and essentia l hypertension (EH). Methods A total of 25 DM patients and 25 EH were chosen as the experimental group and 20 matched normals were chosen a s the control group. EDDF was assessed with B-mode ultrasonography for the chan ges in the diameter of the brachial artery during reactive hyperemia and after t aking in nitroglycerin. Results The changes of brachial art ery diameter during reactive hyperemia were slighter in DM and EH patients. The changes of branchial artery diameter after sublingual glyceryl trinitrate were s lighter in DM patients. Conclusion Endothelial dysfunction exist in the patients with DM; and endothelium-dependent vasodilatative dysfunc tion exist in the patients with EH.

Since 1988,our school has been enrolling in increasing numbers students in 7-year programs of clinical medicine and stomatology. During the period, we have accumulated some experience, which can be embodied in five major points. On this basis, we have, starting from this year, enrolled students working on doctoral degree in 8-year programs with candidates who have completed 4-year undergraduate programs of science and engineering, designed to further intensify reforms in medical education programs.
3.Prostate Cancer Bone Metastasis: Interaction Between Tumor Cells and Bone Microenvironment

Cancer metastasis is composed of a complex cascade that involves a variety of critical steps beginning with detachment from the primary tumor and ending with growth of tumor at a distant site, such as bone. The "seed-and-soil hypothesis" predicts that the bone microenvironment expresses factors through which attract a variety of cancer cells and promote the tumor development. The ending point of tumor development in bone is achieved through the bidirectional and dynamic interaction between tumor cells and the cells in their growth microenvironment. A variety of factors produced by the bone microenvironment, contribute to the pathogenesis of cancer skeletal metastasis. In this review, using prostate cancer (CaP) as an example, some of general mechanisms of cancer metastasis will be summarized. In addition, the current understanding of the interaction between tumor cells and the bone microenvironment will be addressed. Finally, the research directions in the near future will be suggested.
4.Evaluation of clinical effect of ZeGuiLongShuang capsules combined with naftopidil on benign prostatic heperplasia

Objective To study the curative effect of ZeGuiLongShuang capsules combined with naftopidil on benign prostatic hyperplasia(BPH).Methods 140 cases with BPH were randomly divided into treatment group and control group.The patients in treatment group were given ZeGuiLongShuang capsules(once 2 tablets,3 times a day) combined with naftopidil(once 25 mg,once a day before sleeping).The patients in control group were given only naftopidil(once 25 mg,once a day before sleeping).Internatianal-prognosic-scoring-system(IPSS),uroflowmetry-max(Qmax),Quality-of-life(QOL),residual urine and weight of prostate before and after treatment were detected.Results IPSS,Qmax,QOL and residual urine changed in two groups.IPSS,Qmax,QOL and residual urine in treatment group were more significant than those in control group(P
5.Surgical procedure and prognosis analysis for elderly stage Ⅰ NSCLC patients above 70 years old

Objective The purpose of this study is to evaluate Surgical Procedure and Prognosis for elderly stage 1NSCLC patients above 70 years old.Methods The patients who were stage Ⅰ non-small cell lung cancer from 2003 to 2007were enrolled ( n =71 ).The median age was 74 years ( ranged from 70 to 84 years).The median follow-up of patients was 30months( ranged from 2 to 81 months).Results The percentages of postoperative complications after sublobar resection and lobectomy patients were 36.4% and 46.9%,respectively.The period in hospital were 11.36 days and 12.24 days.The 3 year survival was 85.9% for patients undergoing sublobar resection and 78.8% for lobectomy.The 5 year survival was 56.4% and 56.9% respectively.No significant difference was observed between two types of surgical procedure in the elderly.Staging is the independent factor of prognosis.Conclusion Lobectomy is still the main therapy method for elderly stage Ⅰ NSCLC patients.Especially,for those who can undergo radical resection.But sublobar resection also appears to be a viable surgical treatment for patients with cardiopulmonary physiologic impairment.

7.Three cases of orthotopic heart transplantation surviving more than 10 years

Objective To summarize the experience of long-term survival of the patients under- going orthotopic heart transplantation.Methods Heart transplantation was performed on 2 cases of dilated cardiomyopathy and one case of Keshan disease.Before operation,pulmonary artery pressure was 42-53 mm Hg(5.60-7.07 kPa)and pulmonary vascular resistance 5.6-7.0 wood.The body weight difference between donors and receptors was 10%-15%.There were three same antigens in HLA zygosity experiment for all of three patients.Two cases were subjected to standard heart trans- plantation and one case to whole heart transplantation.All the atriums and big vessels were sutured by evting suture method.Cyclosporin A,azathioprine and corticosteroid were used to prevent patients from rejection.Results Survival time of 3 patients was 13 years and 10 months,12 years and 10 years and 3 months.Heart functions of three patients were NYHAⅠand all of 3 patients are living and working commonly.Six,3 and 1 rejection(s)occurred in 3 patients respectively and cured by appro- priate treatment.Electrocardiogram revealed that case 1 and case 2 had two P waves and case 3 sinus rhythm.Ultrasonic cardiogram showed that in case 1 and case 2,the left and right atriums were enlarged and tricuspid valve had slight backstreaming,and in case 3,all of the cardiac chambers were normal and had no backstreaming of tricuspid valve and mitral valve.No abnormal findings were found in 3 cases by 4-9 times of coronary arteriongraphy.Conclusion The important factors for the patients' long-term survival after heart transplantation include the choice of appropriate donors and acceptors, protection of donors' hearts,selection of appropriate operations and suture methods,rational use of im- munosuppressants and prevention of cardiac allograft vasculopathy.
8.Clinic outcome of gefitinib as the first line treatment in elderly patients with lung adenocarcinoma

Objective To evaluate the efficacy and adverse effects of gefitinib as the first line treatment in elderly patients with lung adenocarcinoma.Methods 81 elderly patients of previously untreated advanced lung adenocarcinoma,who were non-smokers and unsuitable for chemotherapy,received gefitinib treatment until disease progression or intolerable toxicities occurred.The curative effect performance status of improvement and adverse effects were observed.Results All of the patients were evaluable.Partial response rate and stable disease rate of gefifinib were 25.9 % (21/81) and 48.1% (39/81),respectively.55.5 % (45/81)of patients had performance status improved after treatment.Conclusion Gefitinib has curative effect and is well tolerated in the treatment of elderly patients with previously untreated advanced lung adenocarcinoma.

10.Relationship between PMI and ATR-FTIR Spectral Changes in Swine Costal Cartilages and Ribs.

OBJECTIVE:
To analyze postmortem chemical changes in Landrace costal cartilages and ribs using attenuated total reflection-Fourier transform infrared (ATR-FTIR) spectroscopy, and to provide a novel technique for estimation of postmortem interval (PMI).
METHODS:
The swines were sacrificed by hemorrhage and their costal cartilages and ribs were kept in 20 degrees C. The chemical analysis of the costal cartilages and ribs were performed using ATR-FTIR every 72 h. The correlation between the certain spectral parameters and PMI was also analyzed. The time-dependent changes of costal cartilages were more significant than ribs.
RESULTS:
There were no obvious changes for the main absorbance bands position, and some absorbance band ratios showed time-dependent changes and significant correlations with the PMI.
CONCLUSION
ATR-FTIR has the ability to analyze postmortem chemical changes of the swine costal cartilages and ribs, and it can be a new method to estimate PMI based on spectroscopy.
