1.Institutional Setup and Financial Management Model of Institute for Drug Control of China under Super-Ministry System

OBJECTIVE:To establish a set of scientific,standard management model and rational,impartial financial system in Chinese institutes for drug control.METHODS:The types of the established institutions and the financial appropriation model of institutes for drug control in China were studied by using the methods of classification,comparison and analysis.RESULTS & CONCLUSIONS:Under super-ministry system,local management in drug control institutions at all levels is expected to be put into practice soon.It is advisable to optimize the organization and management model and financial appropriation management system such as by shifting from county level institution to district level institution and carrying out a wage system in drug control institutes in accordance with the payment level of civil servants,etc.
2.Analysis of 5 cases of the misdiagnosis of spontaneous rupture of spleen

Objective To explore the causes of the misdiagnosis of spontaneous rupture of spleen.Methods 8 cases of spontaneous rupture of spleen during 1986~1998 were reviewed retrospectively.Results 5 cases were misdiagnosed, with the misdiagnosing rate of 62.5%.Conclusion For unknown upper bellyache accompanied with unexplainable loss of blood and with no history of trauma,the patients should be examined by B ultrasound or abdominal puncture should be peformed.If there is pathological process of liver and spleen such as schistosoma and hepatitis or the excessive functioning of spleen,the more attention should be paid to the occurrence of spontaneous rupture of spleen.Furthermore,the increase of the belly pressure caused by satiation or hard labour helps to diagnose such case.
3.Civil Liability of Pharmaceutical Manufacturers and Medical Institution in Medication Misadventures

OBJECTIVE:To provide references for the accountability of civil liability induced by different causes.METHODS:Literature retrieval and case study were applied to analyze the civil liability of pharmaceutical manufacturers and medical institution in medication misadventures.RESULTS & CONCLUSIONS:At present it is difficult for patients who suffered from medication misadventures caused by drug quality defects,drug adverse reaction or medication errors to claim.The main reasons are imperfect relevant law system and the vacancy of compensation mechanism of drug-induced injury events.It is suggested to establish technology institution for evaluating medication misadventures,state relief and compensation system and encourage pharmaceutical manufacturers to buy products liability insurance.
5.Wilson’s disease in Asia

Wilson’s disease is an autosomal recessive disorder of copper metabolism. The resultant accumulation
of copper primarily damages the liver and brain, resulting in hepatic, neurological and psychiatric
symptoms. There have been many recent studies advancing the understanding of Wilson’s disease in
Asia. There are indications that the incidence of Wilson’s disease in parts of Asia may be relatively
high. Many genetic studies have identifi ed various hot spots in theATP7B gene in a variety of the Asian
populations. Screening of these hotspot mutations may thus be useful in confi rming the diagnosis.
Despite the advances in treatment, lack of familiarity by the health care profession resulting in late
diagnosis, and poor access to treatment particularly among those from the developing economies
remain areas of major concern.
6.Study on expression and correlation of Rac1 and E-cadherin protein in oral squamous cell carcinoma

Objective To investigate the expression levels of Rac1 and E-cadherin and their correlation in oral squamous cell carcinoma, and to explore their relations to clinical pathological parameters. Methods The expression levels of Rac1 and E-cadherin were detected in 22 samples of oral squamous cell carcinoma and 8 samples of para-carcinoma tissue by using immunohistochemistry technique (SP). The patients were divided into two groups according to the clinicopathological parameters. The differences of Rac1 and E-cadherin expression levels were analyzed between the groups. Results The positive expression rates of Rac1 were 1/8 and 77.3%(17/22) in para-carcinoma tissue and oral squamous cell carcinoma, respectively. The difference was statistically significant (P=0.003). The positive expression rates of E-cadherin were 8/8 and 31.8%(7/22), and there was a difference between them (P=0.002). The Rac1 protein expression rate was increased and E-cadherin was decreased in oral squamous cell carcinoma. There were no significant differences in Rac 1 and E-cadherin between different age and gender groups. There was significant difference in Rac1 expression between patients with metastasis and patients without metastasis (P=0.021). While there was no difference in E-cadherin expression between patients with metastasis and patients without metastasis. No correlation was found between Rac 1 and E-cadherin expressions in oral squamous cell carcinoma. Conclusion The high expression of Rac1 and low expression of E-cadherin may play an important role in the tumorigenesis and progression of oral squamous cell carcinoma.
7.The effect of human high-molecular-weight salivary mucin on the aggregation of streptococcus mutans.

0bjective: To observe the effects of parotid saliva(PARS), extraparotid saliva(EPS) and purified high molecular weight mucin(MG1) on the aggregation of streptococcus mutans . Methods: PARS, EPS and MG1 were prepared routinely and applied in the cultures of S. mutans Inbritt, S.mutans LM and S.sobrinus OMZ 176 respectively for 2 h. The aggregation of the bacteria was measured spectrophotometrically.Results: The aggregation (%) of S.mutans Inbritt, S.mutans LM and S.sobronus OMZ 176 induced by EPS was 32.80 , 57.87 and 35.46 , that by MGI 25.68, 32.77 and 24.16 , that by PARS 13.52, 22.23 and 16.00, respectively. Conclusion: The effect of MGI on the aggregation of streptoccus mutans is weaker than that of EPS and stronger than that of PARS. The aggregation may be primarily induced by mucins.
8.Changes of Surfactant Protein A and D in Bronchoalveolar Lavage Fluid in Human Fetal Lungs during Various Gestational Ages

Objective To investigate the metabolic rules of surfactant protein A and D(SP- A,SP- D )in bronchoalveolar lavage fluid(BALF)in human fetal lungs during gestational ages. Methods BALF with 30 mL saline was performed on clinically collected human fetus with induction of labor by water- bag Their BALF was respectively retrieved [total retrieval rate(85. 6% ? 13 1)% ]for analysis of protein content. The BALF SP - A and SP - D from fetus of various gestational ages or newboms were detected by RPHA and ELISA. Results The total protein in BALF gradually increased since 10th week to newborn peak during lung development. And SP - A and SP D were respectively updated from(0.34 ?0.07 ) ,(0.05?0.01) ng/L to newborn climax[ (6 42 ? 0 36),(1.22 ? 0 13)ng/L] .Conclusions The protein in BALF gradually increases with fetal growth and lung development. SP-A and SP- D may reach prenatal climax and become the main indicator of newborn lung maturity.
9.Application of high-quality nursing care in patients with advanced pancreatic cancer

Objective To discuss the clinical value of high-quality nursing care applicated in patients with advanced pancreatic cancer.Methods 70 patients with pancreatic cancer were randomly divided into the observation group and the control group,35 cases in each group.The control group was given routine nursing measures.The obser-vation group received high-quality nursing care on the basis of routine nursing measures.The hospitalization time, satisfaction degree,treatment compliance and mentality were observed.Results The hospitalization time of the obser-vation group was (33.2 ±6.5)days,which was significantly shorter than (44.5 ±10.3)days of the control group(t=5.269,P<0.05).The satisfaction degree of the observation group was 97.14%,which was significantly lower than 77.12%of the control group (χ2 =8.254, P <0.05 ) .The treatment compliance of the observation group was 94.29%,which was significantly lower than 71.43% of the control group(χ2 =8.323,P<0.05).The SAS,SDS scores of the observation group were (39.71 ±5.22)points,(41.36 ±5.41)points,respectively,which were signifi-cantly lower than (48.98 ±5.56)points,(47.72 ±5.76)points of the control group(t =9.336,8.544,all P<0.05).Conclusion High-quality nursing care can significantly improve the postoperative anxiety and depression, and improve the quality of life of patients with advanced pancreatic cancer.
10.Regulation of estrogen, interleukin-6 and interleukin-8 in ovarian cancer cells

Objective To discover the reciprocal regulation and its molecular mechanism of estro-gen, IL-6 and IL-8 in ovarian cancer cells. Methods Based on our previous studies, the effect of 17β-estradiol (E2) on the expression levels of IL-6, IL-8 and their respective receptors was investigated. Mean-while, the effect of IL-6/IL-8 on estrogen receptor (ER) expression and estrogen-dependent transcriptional activation was analyzed. Gene expression profile analysis revealed that CAOV-3 and OVCAR-3 cells, which express ER, IL-6 and IL-8 receptors, were suitable models for this study. Results We found that E2 not only enhanced IL-6/IL-8 secretion via NF-κB signaling pathway, but also modulated IL-6 and IL-8 receptors expression. Tamoxifen (Txf), an ER antagonist, completely abolished E2-stimulated IL-6/IL-8 expression. On the other hand, in the absence of estrogen, both cytokines increased ERα expression, decreased ERβ ex-pression, and activated estrogen-dependent transcriptional activation, which was completely blocked by Txf. Pretreatment of OVCAR-3 with p38 MAPK, MEK1/2 or ErbB2 MAPK inhihitors, respectively, IL-6-media-ted ER activation was blocked, while IL-8-indueed ER activation was blocked by Src inhibitor. Conclusion These data suggest that estrogen, IL-6 and IL-8 may form a mutual amplifying signaling which contributes to the growth and development of ovarian carcinoma.
