1.Comparison of Contrast-enhanced Ultrasound, Contrast-enhanced CT and Contrast-enhanced MRI in the Diagnosis of Renal Space-occupying Lesions

Purpose To compare the value of contrast-enhanced ultrasound (CEUS), contrast-enhanced CT (CECT) and contrast-enhanced MRI (CEMRI) in the diagnosis of renal space-occupying lesions. Materials and Methods Seventy-eight patients whose 80 kidney space-occupying lesions were conifrmed by traditional ultrasound underwent CEUS examination. Among those patients, 39 patients also underwent CECT, 28 had CEMRI, and 5 had CECT and CEMRI examination together. Taken pathologic findings as gold standard, the value of conventional ultrasound, CEUS, CECT and CEMRI was compared in the diagnosis of renal space-occupying lesions. Results Among 80 renal masses, 57 lesions were malignant and the rest 23 lesions were benign. The diagnostic sensitivity, speciifcity, positive predictive value and negative predictive value were 93.0%, 69.6%, 88.3%, 80.0%for CEUS;those for CECT were 96.4%, 72.7%, 90.0%, 88.9%respectively;and for CEMRI were 86.4%, 66.7%, 90.5%, 57.1%respectively;and the diagnostic value among the three means showed no difference (P>0.05). Conclusion CEUS, CECT and CEMRI are all effective in the diagnosis of renal space-occupying lesions, We may select suitable examination means for patients according to their features. A combination of these techniques provides more information for the diagnosis of renal space-occupying lesions.

Discogenic low back pain is the common type of chronic low back pain. However,its mechanism has not been completely clarified. Considerable evidence shows that neurotrophins play an important role in discogenic low back pain. The paper summarizes the mechanism of neurotrophins on discogenic low back pain according to the pain transfer pathway of neurotrophins in intervertebral disc, dorsal horn ganglia and spinal trigeminal nucleus. Changing the pain transmission by regulating neurotrophins and its receptor will provide a new way for the treatment of discogenic low back pain.

4.Clinical study on skin needling plus heat-sensitive moxibustion for chronic facial paralysis

Objective:To observe the clinical efficacy of skin needling plus heat-sensitive moxibustion in treating chronic facial paralysis. Methods:Sixty patients with chronic facial paralysis were divided into an observation group and an acupuncture group by using the random number table, 30 in each group. Patients in the observation group were given skin needling plus heat-sensitive moxibustion; while patients in the acupuncture group were given conventional acupuncture. Results:Three treatment courses later, the total effective rate was 93.3% in the observation group versus 86.7% in the control group, and the difference was statistically significant (P<0.05). Conclusion:Skin needling plus heat-sensitive moxibustion can produce a more significant efficacy than conventional acupuncture in treating chronic facial paralysis.

Cervical spondylotic myelopathy (CSM) is a common cause of spinal cord dysfunction clinical disease. Surgery is the main therapeutic tool for CSM. However, there are obvious differences in clinical functional recovery after operation. For the past few years, the influence factors of prognosis in cervical spondylosis myelopathic has been widely concerned. Age, nerve function, course of desease, imaging findings,surgical method and related factors became the investigative point for prognosis of cervical spondylotic myelopathy. Present viewpoint showed that the older patient, preoperative worse nerve function, longer the course of disease would result in worse outcomes. Imaging examination maybe can indicate the prognosis, but the correlation is unclear. Selection of surgical method and approach should be based on the principles of sufficient decompression, stabilize the alignment of the cervical spine, keeping backward extension of cervical spine, maintain effective decompression, preventing complications. Therefore, the treatment of cervical spondylotic myelopathy should be on the basis of pathogenic condition and imaging examination at early stage and a suitable usrgical procedure should be performed to obtain a better prognosis.

8.Efficacy comparison of tissue selecting therapy stapler and procedure for prolapse and hemorrhoids in the treatment of severe hemorrhoids.

OBJECTIVETo compare the efficacy and complication of tissue selecting therapy stapler (TST) and procedure for prolapse and hemorrhoids (PPH) in the treatment of severe hemorrhoids.
METHODSClinical data of 542 cases of severe hemorrhoids undergoing TST (258 cases) or PPH (284 cases) in The First Affiliated Hospital of Fujian Medical University from November 2010 to January 2012 were analyzed retrospectively. Operative parameters, efficacy and complication 3 months after operation were assessed and compared.
RESULTSNo significant difference in cure rate between TST and PPH (96.5% vs. 95.4%) was found, while the operation time and hospital stay after operation in TST group were significantly shorter urgency [(20.6±4.7) vs. (26.4±6.3) min, (2.9±0.5) vs. (3.5±0.7) d, both P<0.05]. Incidences of postoperative pain, bleeding, anal urgency and urinary retention in TST group were significantly lower than those in PPH group (all P<0.01). No anal stenosis was observed in TST group, and 5 cases developed anal stenosis in PPH group (P<0.05). Hemorrhoid recurrence did not differ significantly between the two groups.
CONCLUSIONSThe efficacy of TST and PPH is comparable for severe hemorrhoids patients, while TST is associated with faster postoperative recovery and less complications.

9.Surgical treatment of primary pulmonary low-grade malignant carcinoma.

BACKGROUNDTo arrive at a rational diagnosis and effective surgical treatment of primary pulmonary low-grade malignant carcinoma.
METHODSEighty-nine patients with primary pulmonary low-grade malignant carcinoma received surgical treatment from 1956 to 2000.
RESULTSOut of the 89 patients, 54 were bronchial carcinoid, 18 were mucoepidermoid cancer, and 17 were adenoid cystic carcinoma. The operative procedures included resection of tracheal carcinoma in 3, lobectomy in 49, sleeve lobectomy in 14, total pneumonectomy in 14, and wedge pneumonectomy in 6. All patients were complaint free. The 5-year survival rates of bronchial carcinoid and adenoid cystic carcinoma were 91.3% and 70.6% respectively, and no patients with bronchial mucoepidermoid cancer died until now.
CONCLUSIONSBronchial carcinoid, adenoid cystic carcinoma and mucoepidermoid cancer are a series of primary pulmonary carcinoma of low malignant potential, whose predilection sites are main or lobe bronchi, and clinical symptoms are repeated infection of the lung or intermittent hemoptysis. The diagnosis depends on radiography or CT scan of chest and bronchofiberscopy. The operation is most important, and whose procedure is lobectomy or sleeve lobectomy mainly.
10.Efficient expression and purification of human beta-defensin-2 in E.coli.

OBJECTIVETo demonstrate the possibility of high-level expression of bioactive human beta-defensin-2 (hBD2) in E.coli, and to purify the recombinant hBD2.
METHODSDNA fragment containing mature hBD2 coding region (smhBD2-cDNA) was amplified by PCR, multiple copies of smhBD2-cDNA were linked using Bgl II and BamH I enzymes, pET32-nsmHBD2-cDNA with 1, 2, 4, or 8 copies of smhBD2-cDNA was constructed. The soluble and insoluble hBD2 proteins were separated and analyzed by SDS-PAGE analysis. The soluble protein underwent a separation process containing affinity chromatography, enterokinase digestion and ion exchange chromatography to get the recombinant hBD2 peptide. The bioactivity of recombinant hBD2 was examined by bacteria-inhibition tests in liquid culture.
RESULTThe plasmids pET32-nsmHBD2-cDNA with 1, 2, 4 copies of smhBD2-cDNA were constructed and the expressed soluble protein accounted for 52 %, 48 %, and 31 % respectively. The plasmids with 8 copies expressed mainly insoluble protein with few in soluble form. The growth of E.coli K12D31 was dramatically suppressed with a inhibition rate of 90 %, when the final concentration of recombinant hBD2 reached between 0.4 to 0.5 mug/ml.
CONCLUSIONFusion expression of human beta-defensin-2 with multiple joined genes in E.coli could increase the expression of hBD2.
