[Objective]To search for a good way for complex total hip arthroplasty(THA) and hip reconstruction.[Method]From February 1995 to June 2007,THA was done 95 hips in 86 patients with good preoperative preparation,full evaluatibn and complete simulation the details occurring at operation.[Result]Eighty one patients were followed up from 6 months to 10 years(average 3.5 years).All patients were measured by Harris score.The preoperative Harris score in the patients ranged from 35 to 50(average 42.1).The postperative score ranged from 70 to 90(average 82.3).Leg length discrepancy ranged from 1 to 12 cm preoperative.(average 4 cm),but from 0 to 5 cm postperative(average 1.5 cm).Two with serious leg length discrepancy,one is 6 cm,the other is 12 cm,after the operation,one is 2 cm,the other is 5 cm.Two had poor incision healing,upper femur split occurred in 3,one with hip prothesis dislocate,nerve injury happened in 2 and two had venous embolism.[Conclusion]Anatomic structure in complex total hip arthroplasty changes so much that it is hard to rebuild and replace a normal hip anatomic structure.Excellent clinical outcome can be achieved with full preparation and evaluation before operation and careful management at opertation.

Objective　To explore the ideal effective treatment of liver trauma. Methods　The injury state, treating methods, effect and death causes of 210 patients with liver trauma were retrospectively analysed. Results　Of the patients, the liver trauma was in degrees Ⅳ Ⅴ Ⅵ in 88 cases (41.9%). Among 198 cases treated with operation, 39 underwent partial hepatectomy, 13 with gauze packing, 146 with debridement and suturing. 2 receieved inferior vena cava repairing, 4 liver atrery ligation. 19 patients died of serious injury, the death rate was 9% (19/210). Of the 19 cases, 12 (63.2%) died of great amount blood loss. Conclusions　The treatment of liver trauma should depend on the degree of injury. The death rate could be reduced by shortening the preoperative time and decreasing the amount of blood loss. Gauze packing can be applied as a temporary way to save the patients life.

Objective To analyze the clinical feature of thalassemia with pulmonary hypertension and investigate the influence of pulmonary hypertension on thalassemia.Methods From June 2007 to July 2011,clinical data of 19 patients with thalassemia complicated with pulmonary hypertension in TEDA International Cardiovascular Hospital of Tianjin Medical University and Affilated Hospital of Guilin Medical College were analyzed retrospectively.Twenty-two cases of thalassemia without pulmonary hypertension were as a control group,and follow-up time was set as 4 months to 24 months.Observed indicators included age,hemoglobin(Hb),lactate dehydrogenase (LDH),serum ferritin (SF),tricuspid regurgitation velocity (TRV) and left ventricular ejection fraction (LVEF).Results SF and TRV of the case group[(693.6 ± 234.6)μg/L and (2.6 ± 0.1)m/s] were significantly higher than those of the control [(209.5 ± 100.1) μg/L and (2.1 ± 0.3)m/s,all P ＜ 0.05]; but the differences between case and control group in Hb[(90.8 ± 10.7)g/L vs (89.3 ± 10.5)g/L],LDH[(320.9 ± 103.7)U/L vs (355.8 ± 140.3)U/L] and LVEF[(66.2 ± 7.1)％ vs (64.2 ± 4.7)％] were not statistically significant (all P ＞ 0.05).Logrank analysis showed that the prognosis was poor in patients with thalassemia complicated with pulmonary hypertension (x2 =4.95,P ＜ 0.05).Multiple regression analysis indicated that age and serum ferritin remained as predisposing risk factors for tricuspid regurgitation velocity,and serum ferritin had a greater impact on the velocity.Conclusion In patients with thalassemia complicated with pulmonary hypertension,the prognosis is poor; age and SF may be factors involved in the development of pulmonary hypertension.

Objective To establish a method of SYBR Green Ⅰ FQ-PCR for detecting the expression of miR-202 in peripheral blood mononuclear cells ( PBMC ) and analyze the expression of miR-202 and its clinical significance in MM.Methods Reverse transcription was performed with specific stemloop primer for miR-202,and then FQ-PCR was used to detected the expression of miR-202 in 21 MM patients and 20 healthy people.Data was presented as mean ± standard deviation ( (x) ± s ).Non-parametric Mann-Whitney test was used to analyze the difference between MM group and control group.In addition,1∶ 125 dilution of one test sample was detected by repeated 5 times,and the same sample was tested one time a day for 3 days,repeated 5 each time.The assessment of repeatability of measurements was done by calculating standard deviation and variation coefficient from threshold cycle (Ct).Results FQ-PCR detection of miR-202 in PBMC was amplified by the standard S-curve,with a single melting curve peak and no complex peak,which showed good specificity.The assay showed good reproducibility (intra-assay coefficient 1.2％ and inter-assay coefficient 3.2％ ) and high sensitivity ( 12.8 pmol/μ1).The expression of miR-202 in MM was 0.014 ( 0.007 - 0.221 ) and 1.844 ( 0.162 - 3.966 ) in normal controls.The expression level of miR-202 was significantly higher in MM than in normal controls (U =48.000,P ＜0.01 ).Conclusions FQ-PCR provide us a rapid,sensitive and specific method for detection of miR-202.The expression level of miR-202 is higher in MM than in normal controls.It is possible to play a role in MM progress and may be a useful marker to evaluate the development and treatment of MM.

Objective To explore the characteristics and biological functions of epigenetic treatment to DNMT1 expression in NSCLC cell lines. Methods Lung cancer cells were divided into 4 groups , and drugs with different concentrations. The effect of epigenetic treatment on NSCLC cell line was detected by CCK-8 and MTT. The effect of epigenetic treatment on the expression of DNMT1 in NSCLC cell line was detected by Western blot. Results CCK-8 and MTT assay showed that treatment with 5-Aza-CdR and MS-275 inhibited the proliferation of NSCLC cells. Western blot showed that treatment with 5-Aza-CdR and MS-275 inhibited the DNMT1 expression in NSCLC cells. Conclusion The expression of DNMT1 gene in the NSCLC cell lines may be suppressed effectively by epigenetic treatment , and the gene may inhibit the proliferation and growth of NSCLC cell lines.
