1.Effect of Fufang Congrong Yizhi Capsule on myristoylated alanine-rich C-kinase substrate (MARCKS) mRNA level in hippocampus of old dementia rats

Objective:To observe the effect of Fufang Congrong Yizhi Capsule on myristoylated alanine-rich C-kinase substrate (MARCKS) mRNA level in hippocampus of old dementia rats induced by ?-amyloid(1-40). Methods: Animals were divided randomly into young group, old group, sham operation group, model group, western medicine group and Chinese medicine group. Model, western and Chinese medicine groups received ?-amyloid (1-40) intracerebroventrical injection to build dementia animal model, and then Chinese and westein medicine groups received Fufang Congrong Yizhi Capsule and donepezil for 14 days, respectively. To observe changes of behavior and MARCKS mRNA under the effect of Chinese and western medicine by Morris test and RT-PCR test. Results: Learning and memory function of Chinese and western medicine groups were better than that of model group. MARCKS mRNA of model group was increased significantly, that of Chinese medicine group was decreased compared with model group. Conclusion: Fufang Congrong Yizhi Capsule can reduce the level of MARCKS mRNA in hippocampus of old dementia rats caused by ?-amyloid(1-40), through which it can improve the learning and memory function of the dementia old rats.

Doctors of past dynasties already understood deficiency of kidney essence,phlegm and blood stasis were important for dementia.As the increasing recognition about Alzheimer disease and the offer of 'toxin damaging brain vessel' hypothesis,we thought ?-Amyloid,the pathology of Alzheimer disease,had the character of damaging brain and marrow,so it belonged to internal toxin,internal toxin damaging brain and marrow was also the pathology of the disease.Deficiency of kidney essence was the precondition,insufficient brain and marrow and internal toxin damaging brain and marrow were the two causes of brain and marrow consumption and spirit dysfunction,which were the pathology of Alzheimer disease in Chinese medicine.

AIM: The purpose of this study was to investigate the expression of myristoylated alanine-rich C kinase substrate (MARCKS) and p-MARCKS during cerebral ischemia. METHODS: The multi-cerebral infarction model was established by method of Kaneko. The stroke symptoms and signs scoring system were used to evaluate the animal model. Microscope and electronic microscope were used to watch the structure of rat brain. The expression of MARCKS and p-MARCKS was measured by Western blotting. The expression pattern of these proteins was further analyzed for their distribution at cellular level by immunohistochemistry staining of the tissues. 4.Development of the Chinese age norms of CES-D in urban area

Objective: To test applicability of the Center for Epidemiological Studies Depression Scale (CES-D) in different age groups in urban China, and to develop age norms. Methods: In this cross-sectional study, the CES-D was administrated to 16047 community participants with average age of (37.7±21.3) years (age ranged 11~100) in 21 provinces, who were as the norming sample. Criterion validity was tested in 349 psychiatric patients with average age of (32.0±12.1) years (age ranged 16~81) in 4 cities. A subsample (199 workers, 100 col-lege students, and 30 teachers in Beijing, Dongguan, and Baotou) was drawn from the national sample to provide 8 week interval test-retest reliability. Results: The Cronbach α was 0.90 for the scale, and 0.68~0.86 for its fac-tors. The 8 week interval test-retest correlation was 0.49 for the scale (P <0.01) and 0.39~0.51 for factors (P<0.01) . The result of confirmatory factor analysis supported the original 4-factor structure (RMSEA=0.057, CFI =0.976, GFI=0.948) . Patients scored higher than community sample [(21.72±13.39 ) vs.(13.24±10.33),P <0.01], and depression patients scored the highest [(27.82±14.42), P<0.01] . Age difference was signifi-cant. Age groups over 60-year-old scored higher than all the other age groups under 60-year-old (P<0.01). Objective To explore the reliability of handgrip strength test for evaluating mobility in patients with stable chronic obstructive pulmonary disease.Methods Sixty-one COPD patients in stable stage were measured for handgrip strength and 6-minute walking test(6MWT).The receiver operating characteristic curve(ROC) was calculated to determine the best cutoff points of handgrip strength.Results Handgrip strength was (33.72-±7.47) kgf,6MWD was (437.06±97.96) m,handgrip strength was moderately correlated with 6MWD (r=0.404,P=0.001).6MWD≥350 m was used to classify two groups,and there was significant difference between two groups(P<0.05).Area under the curv e was 0.722,and the best cutoff points was 32.8 kgf.Conclusion Handgrip strength test can be a useful tool to quickly identify mobility in patients with stable COPD.
6.Changes of MARCKS mRNA expression in rat hippocampus with acute multi-cerebral infarction

AIM: To observe the dynamic alteration of myristoylated alanine-rich C kinase substrate(MARCKS) mRNA expression in rat hippocampus with acute multi-cerebral infarction,and discuss the relationship between the alteration of hippocampus MARCKS gene and ischemia damage.METHODS: The acute multi-cerebral infarction model was established by method of Kaneko.Neurological function deficits were evaluated in the behavior test.The consequences of cerebral ischemic damage were examined by histopathological analyses.The MARCKS mRNA expression was measured by semi-quantitative PCR.RESULTS: The rats in acute multi-cerebral infarction group showed different level changes of neurological function deficits.The hippocampus damage of histopathology became significant 24h after ischemia.At the same time,the MARCKS mRNA expression was upregulated at the area of rats hippocampus during ischemia,and its overexpression started 1h after ischemia,and reached maximum7d after ischemia.CONCLUSION: MARCKS mRNA of rat hippocampus overexpresses during acute cerebral ischemia.This MARCKS mRNA overexpression is related with hippocampus ischemia damage.
7.Confrontation as a Mediator between Sense of Coherence and Self-management Behaviors among Elderly Patients with Coronary Heart Disease in North China.

PURPOSE: Self-management is critical to improve health outcomes of elderly patients with coronary heart disease (CHD). Sense of coherence (SOC) is found to be linked with self-management behaviors. However, their deeper relationship is not clear. The purposes of this study were to investigate the association between SOC and self-management behaviors among elderly CHD patients in China, and whether confrontation mediates this association. METHODS: A cross-sectional design was used. A total of 275 elderly patients with CHD recruited from the cardiology department in a general hospital in North China were surveyed from October 2015 to April 2016. SOC, confrontation, and self-management behaviors were measured using the Chinese version of the SOC scale, subscale of Medical Coping Modes Questionnair—Confrontation, and the CHD self-management scale, respectively. Correlation analysis and path analysis were conducted to analyze the data. RESULTS: The mean (±standard deviation) scores of SOC, confrontation, and self-management behaviors were 62.20 (±9.61), 19.55 (±3.15), and 76.17 (±10.63), respectively. Correlation analysis showed that SOC, confrontation, and self-management behaviors were significantly correlated with each other. Path analysis indicated that SOC exerted a direct effect on self-management behaviors, whereas could affect self-management indirectly via confrontation. Bootstrap test result showed that confrontation played a mediating role (β = .20, p < .001) in the relationship between SOC and self-management behaviors. CONCLUSION: SOC was related to self-management behaviors, whereas confrontation mediated the effect of SOC on self-management behaviors. In practice, the role of confrontation coping should be valued when developing strategies to strengthen SOC and to improve self-management practice among elderly CHD patients.

8.Study on Infrared Thermal Imaging Characteristics of Chronic Insomnia with Heart-spleen Deficiency Syndrome

Objective To observe the body surface calorific value characteristics of Governor Vessel and Conception Vessel,triple energizer,and zang-fu organs chronic insomnia patients with heart-spleen deficiency syndrome through infrared thermal imaging technology.Methods Totally 150 patients with chronic insomnia with heart-spleen deficiency syndrome who were treated in the outpatient department of Shenzhen Hospital of Beijing University of Chinese Medicine(Longgang)from December 2021 to December 2022 were selected as the observation group.Another 50 healthy subjects were recruited as the normal control group.Infrared thermal imaging technology was used to collect and measure the temperature of surface areas of Governor Vessel and Conception Vessel,triple energizer,and zang-fu organs,and the calorific values were analyzed and compared.Results Compared with healthy subjects,chronic insomnia patients with heart-spleen deficiency syndrome had a statistically significant decrease in the temperature of the Governor Vessel(P<0.05);there were statistically significant decreased in the temperature of upper energizer and middle energizer(P<0.05);there were statistically significant decreased in the temperature of chest,apex,right chest,epigastric region,big abdomen,and the temperature of left and right flank increased(P<0.05).Conclusion There were significant differences in infrared thermography distribution characteristics of Governor Vessel and Conception Vessel,triple energizer,and zang-fu organs between patients with chronic insomnia with heart-spleen deficiency syndrome and healthy subjects,which can provide some reference for the clinical diagnosis and syndrome differentiation of chronic insomnia with heart-spleen deficiency syndrome.
9.Application and effect evaluation of multidisciplinary collaboration in patients with tophi during perioperative period

Objective:To explore the application and effect of multidisciplinary collaboration in patients with tophi during perioperative period.Methods:Forty-five patients undergoing tophaceous gout surgery in our hospital from May to October 2020 were selected as the control group and treated with routine treatment.From November 2020 to April 2021, 41 patients undergoing tophaceous gout surgery in our hospital who were treated with multidisciplinary collaboration management mode were included as the intervention group. Postoperative pain, blood uric acid level, hospitalization expenses, hospitalization days and patient satisfaction were compared between the two groups.Results:After implementing the multidisciplinary integration man-agement mode, the pain score of the patients at 4 hours, 1 day and 3 days after operation was lower than that of the control group [(3.6±1.0) vs (4.1±1.0), t=2.33, P=0.022; (2.5±0.8) vs (3.0±0.6), t=3.85, P<0.001; (1.8±0.5) vs (2.2±0.7), t=3.52, P<0.001], the serum uric acid level was significantly lower than that of the control group at 1 month and 3 months after operation [(355±58) vs (3928±39), t=3.50, P=0.001; (316±48) vs (366±47), t=4.84, P<0.001], the hospitalization days and hospitalization expenses were significantly decreased [(8.90±2.48) d vs (10.62±3.96) d, t=2.44, P=0.017; (1.00±0.13) ten thousand yuan vs (1.11±0.17) ten thousand yuan, t=3.34, P=0.001], and the patient satisfaction was markedly improved (97.6% vs 82.2%, χ2=3.87, P=0.049). Conclusion:The multi-disciplinary collaboration mode in patients with tophi during perioperative period can effectively reduce the postoperative pain, improve the quality of care, promote reha-bilitation, improve the outcome, and reduce the economic burden of patients, which is worthy of popularization and being applied in clinic.
